Chairman’s message

Patrick Foster Chairman of Trustees

It has been a great honour to take over as chairman of St Clare during an exciting phase in the Hospice’s history, a time during which longheld dreams have come to fruition and we have seen great change, not only in the Hospice building itself, but also through the development of our services.

In particular, the launch of our new Hospice at Home service was a truly momentous day, enabling us to extend the skilled, compassionate and dignified care already available to those who we look after in our Inpatient Unit to families in their own homes. Ensuring

excellent patient experiences, and, more importantly, providing a choice for patients of where they wish to be cared for, is truly what hospice care is all about.

The completion of a major refurbishment to the Hospice, providing modern and uplifting facilities all centred on the needs of patients and their loved ones, has also been wonderful to see. It is a true testament to the creativity and vision of a dedicated leadership team and the commitment of staff to ensure the Hospice is able to provide the best possible end-of-life journey for everyone who needs our care and support.

Finally, I must pay tribute to my predecessor Michael Chapman, who retired at our AGM having served as a trustee for the maximum nine-year term. During that time, Michael devoted his hard work and expertise to the Hospice, playing a pivotal role in developing the Hospice we see today. There is no doubt that St Clare is stronger due to his professional input and guiding hand.

About St Clare St Clare Hospice is a charity providing specialist care for people in West Essex and East Herts border who have a life-limiting illness that is no longer responding to curative treatment. This can include chronic respiratory, neurological and cardiac conditions as well as cancer.

We care for patients and their families, providing physical, emotional, social, psychological and spiritual support, ensuring we respect and encourage individuality, dignity and self-esteem.

Our care and support extends to relatives and friends both during their lovedone’s illness and through bereavement, ensuring they have timely access to skilled, compassionate and sensitive care in a place of their choice.

Summary of our services

Day Therapy Our Day Therapy services offer people the opportunity to engage in therapeutic activities, from creative workshops to complementary therapy, which are designed to support and promote independence and normality.

Community Team The St Clare Specialist Palliative Care Nurses visit patients out in the community to provide support, information and advice, working alongside GPs and District Nurses to plan, deliver and respond to the changing needs of families at home.

Hospice at Home Our Hospice at Home team provides specialist handson palliative care in patients’ own homes to those approaching the end of life or when families need additional support.

Inpatient Care Our multidisciplinary Inpatient team provide high quality specialist care in beautiful surroundings designed to aid comfort and support independence.

Care is provided in an environment that is homely and inviting, allowing people to be together with privacy and dignity.

Bereavement & Family Support Support services for our patients and their loved ones include social care and specialist advice to help families cope with practical and emotional matters, as well as chaplaincy for spiritual support and a bereavement counselling service.

When St Clare Hospice was established almost 25 years ago, it was the vision of our founders to be able to offer a complete end-of-life care service, combining expert professional clinical care and emotional and practical support to every local person who needs it, not only here at the Hospice but in people’s own homes.

That passion to provide the best specialist palliative care has remained at the heart of all St Clare has achieved in the years since, and I have been immensely proud of the huge steps forward our organisation has taken in the past 12 months, as we celebrated the long-awaited

launch of Hospice at Home and the completion of a major refurbishment.

We believe everyone’s journey towards the end of life should be dignified, comfortable and free from worry and distress, and it has been wonderful to see the very real difference these important new service developments have made to the lives of those we care for.

Quality is at the forefront of all we do here at St Clare, in every aspect of our work, and the changes in the past year will ensure the Hospice remains in a strong position to face the future challenges we know healthcare, and indeed specialist palliative care, will bring. Key to this is

working in partnership with our healthcare and social care colleagues, sharing knowledge, experience and resources to continue delivering the best quality services for the benefit of local people.

Most importantly, we continue to actively engage with those who receive our care and support, gaining an enormous amount of positive and constructive feedback on how we can shape and expand future services to ensure patients and their families get the right care, at the right time, in the right place.

Patient & Family Services Day Therapy The number of patients coming to Day Therapy continued to rise, with our team providing extra support to those facing a life-limiting illness, helping them to better manage their symptoms, gain confidence and independence and maximise their quality of life.

More people attended our outpatient clinics, allowing them to receive the correct support to cope with their condition at home, while our wide range of therapeutic groups, such as our neurological and Brighter Living sessions, continued to benefit patients, relatives and carers, enabling our team to ensure individualised care.

The Day Therapy department was central to the ‘Space to Breathe’ refurbishment, with patients enjoying the use of modern, purposedesigned rooms for complementary therapy and counselling.

The positive feedback from those who use these spaces on a daily basis has been wonderful to hear.

In line with all healthcare organisations across West Essex, the Hospice adopted the use of Advance Care Plans to ensure patients’ final wishes are respected, even if they can no longer speak for themselves. The important documents allow patients to express the type of care they wish to receive, and where they wish to be cared for, enabling our staff to tailor care to their individual needs and ensure St Clare services continue to be centred round each patient.

Bill’s story

“I was struggling to get up the stairs and it was suggested an extra banister might be helpful and within three days it was fitted,” he said. “I didn’t realise something so simple would be so helpful, but that’s one of the great things about St Clare, if you need help with something, it’s done.”

When you are diagnosed with an incurable illness, even the smallest of things can suddenly become an uphill battle. But being able to help Day Therapy patient Bill Quigg with something as simple as having an extra stair banister installed at home has made a huge difference to his day-to-day life.

The former black cab driver said he was at first reluctant to come to the Hospice as his only knowledge of hospice care was of end of life, but attending the 12-week Day Therapy course and our Breathlessness and Brighter Living groups have given him the reassurance and confidence to face the road ahead.

The dad-of-four has chronic obstructive pulmonary disease, which affects the lungs and means he often struggles to catch his breath, and started attending Day Therapy at the Hospice after being recommended by the cardiology team at St Margaret’s Hospital in Epping.

“My first reaction was that I wasn’t ill enough because I thought hospices were just for people at the end of life; I had no idea of all the other support on offer,” he said. “But by the end of my first visit I was already looking forward to coming back.

“The whole experience has been very positive, everyone here is kind and considerate and to be able to discuss your illness with people with similar problems is a great comfort. Being under the St Clare umbrella has transformed my life and given me an invaluable support network, I don’t know why I ever felt anxious or worried about coming.”

Making people feel special and cared for when they are at a difficult time in their lives is something our Day Therapy team is passionate about, which is why they organised two popular ‘All About You’ pamper days to treat our patients and their carers to some extra TLC. Supported by local beauty therapists, reflexologists, colour experts and masseurs, a range of different therapies and treatments were offered, from reiki and reflexology to manicures and pedicures.

Patient & Family Services Community Team As part of our commitment to ensure people with lifelimiting illnesses have timely access to skilled, sensitive and compassionate care in a place of their choice, we recruited two additional St Clare Specialist Palliative Care Nurses (SPCNs), increasing our Community Team from four to six and enabling us to introduce a 7-day service.

Our SPCNs assess and initiate treatment, offering specialist advice and support to patients, their families and carers as well as guidance for other health and social care colleagues, such as GPs and District Nurses. Together we can provide holistic care that meets the individual needs of each patient.

Hospice at Home In January, we officially launched our new Hospice at Home service, realising a long-held dream to be able to deliver the same exceptional care provided in our Inpatient Unit directly in patients’ homes.

As more and more people choose to be cared for and die at home, the need for efficient and effective community services has never been greater. The first of its kind in West Essex, Hospice at Home has transformed the way we are able to provide end-of-life care, providing families with hands-on nursing care and support in the final 14

Delivered by our team of specially trained Nursing Assistants, supported by a Hospice at Home co-ordinator and administrator, the service is already making a huge difference to those we care for, helping to avoid unnecessary admissions to hospital and enabling people to spend their last days at home even when their symptoms are difficult to manage.

Hospice at Home, which covers Harlow, Epping Forest and Uttlesford, operates seven days a week and is available to anyone over 18 who is registered with a West Essex GP.

The team made a total of 145 home visits and 61 telephone calls Peggy’s story St Clare patient Terry Inkersole was adamant that he would not spend his final days in hospital, and as one of the first patients to be supported by our new Hospice at Home service, his family were able to honour his dying wishes and care for him at home, where he died peacefully not long after he and wife Peggy had celebrated their 50th wedding anniversary.

She added: “Terry was so much more relaxed being at home. Our 14-year-old grandson came round to see him and laid on the bed for hours just chatting to him, which could never have happened at the hospital. The St Clare staff worked so hard to keep him peaceful.

I would have been delighted to have paid for his care but no, we were told everything was given freely, which just astonished me.”

“It was a terrible, terrible blow to lose him from my life but I have really happy memories of our last few days together at home thanks to the support we both received from the St Clare Hospice at Home team,” said Peggy.

“Terry was fiercely determined to stay at home, it’s where he’d always lived, and the St Clare nurses were just wonderful. So kind and knowledgeable, I knew Terry was in safe hands. They kept him as comfortable as possible and spent lots of time talking to us about what was going to happen, helping us not to be afraid of the future. It was such a relief to be able to hand over his care to someone I knew I could trust, right from the start.”

Patient & Family Services Inpatient Unit Our eight-bed ensuite Inpatient Unit continued to provide high quality specialist palliative care delivered by a complete multidisciplinary team. A wide range of support and clinical training has been implemented to ensure staff remain skilled and supported to deliver excellent individualised care to our patients and their loved ones in comfortable surroundings.

Our care extends beyond just making our patients feel comfortable, with staff often going the extra mile to make their time with us as positive an experience as possible, and helping make special memories for their families to treasure after their loved ones have passed away. In the lead up to Christmas, 20-year-old cancer sufferer Charlotte James spent several weeks in our Inpatient Unit and as a surprise we arranged a special video message from pop star Olly Murs. Charlotte’s mum Denise said: “We watched it with her cousins on the telly in Charlotte’s room, she was absolutely thrilled. St Clare organised it off their own back, it’s just amazing that they put themselves out to do something like that for her. The staff really go above and beyond.”

Making our patients more comfortable by helping alleviate symptoms such as pain, nausea and seizures has been possible thanks to funding to allow us to update our stock of syringe drivers. The small batterypowered digital devices deliver a continuous dose of medication, helping avoid repeated painful injections.

Our bereavement service saw new clients including five young people under the age of 18 Bereavement & Family Support Our Bereavement and Family Support service is an important part of the ongoing care provided by the Hospice throughout a patient’s journey with us as we strive to support each and every family even after their loved one has died.

Over the past year, the service has seen a small increase in the number of families being supported and remains responsive to people’s needs. As part of the development of this service, we successfully recruited a social worker to the team as well as an experienced consultant in psychosocial palliative care two days a week who will help guide further growth and development, including enhanced bereavement care.

St Clare’s chaplaincy service has continued to develop to ensure it is multi-faith and available to everyone, from patients and their relatives to St Clare staff and volunteers.

Our chaplain plays a lead role in the organisation of our annual Light up a Life services, which bring relatives and friends together to remember a lost loved one at Christmas time. Due to the popularity of the services, extra ceremonies were offered over the 2013 festive period, with nine held across the Hospice catchment area.

Patient & Family Services ‘Space to Breathe’ refurbishment A major refurbishment to provide inspiring, modern and therapeutic spaces for patients and families was completed in March, complementing previous renovation work in our Inpatient Unit.

The ‘Space to Breathe’ project, which was funded through a £472,098 capital grant from the Department of Health, has transformed the Day Therapy, Outpatients and family facilities into bright, revitalising areas which enhance the Hospice environment.

The name for the project was inspired by the wife of a patient, who described the physical and emotional support their family received from St Clare while her husband was attending Day Therapy sessions as giving them breathing space.

The Hospice was given a clean bill of health by the Care Quality Commission when the independent regulator carried out an unannounced inspection in November 2013. The CQC team looked at the personal care records of patients, checked how people are cared for at each stage of their treatment, and spoke with patients and relatives about the Hospice’s services, awarding full marks for meeting essential standards of quality and safety in all five key areas that were scrutinised: Care and welfare of people who use the service

Awareness & Access Raising the profile of the Hospice remains one of our biggest challenges as we strive to change attitudes towards death and dying and overcome some of the barriers that can prevent people from benefitting from our life changing care.

Challenging the common misconceptions surrounding hospice care is a key part of our communications and marketing strategy as we continued to work hard to increase awareness of the work we do providing expert care and understanding for people at any stage of a life-limiting illness, a journey that can span months and years, not just days and weeks.

We regularly post St Clare news and updates on our Facebook and Twitter pages, using social media as an important tool to communicate with and engage St Clare supporters.

TV horticulturalist Christine Walkden and Harlow MP Robert Halfon became the newest Patrons of St Clare, helping to promote our work and raise our profile in the community and beyond. As longstanding supporters of the Hospice, we were delighted they both agreed to bring their wealth of expertise and experience to the prestigious role, acting as important ambassadors for the charity.

Our clinical team run a successful education programme for other healthcare professionals to enhance patient and family care in the community and are regularly asked to speak at external events and conferences highlighting the work undertaken at the Hospice. As a widely respected expert in the field of palliative care, our Medical Director Dr John Zeppetella was invited to speak at international medical conferences in Florence and Cannes as well as at the 17th annual Congress of the European CanCer Organisation in Amsterdam, exchanging ideas and sharing experiences with doctors, nurses and pharmacists from across the globe.

The addition of a communications and marketing assistant to the St Clare communications team to promote Hospice events and services has seen increased press coverage in the local, regional and national media, helping enhance publicity celebrating the work of the Hospice.

This has included an article in women’s magazine Your Weekly marking Hospice Care Week, a feature on ITV Anglia News as part of our People’s Millions campaign and a radio campaign on Heart FM to promote our annual Midnight Walk.

Supporting national awareness initiatives, such as Hospice Care Week and Dying Matters, is an important way for the Hospice to join voices with our charity partners in a bid to further raise the profile of hospice care across the UK. During Hospice Care Week in October, St Clare ran a campaign entitled ‘We’re So Much More’ with a series of five roadshows in Bishop’s Stortford, Loughton, Dunmow, Epping and Saffron Walden involving nurses, volunteers and fundraisers highlighting the ways in which staff go the extra mile to care for our patients.

Our Staff Providing for the health, welfare, support and development of our staff is a crucial aspect of the St Clare ethos as we strive to develop staff engagement and satisfaction, improve employee retention and ensure the Hospice is a preferred employer of choice.

We believe our staff are an asset to the Hospice and as such should be appropriately trained and supported, enabling them to be knowledgeable and confident in the workplace. As part of

this, we offer a free 24-hour confidential Employee Assistance helpline and also offer the opportunity for external counselling and occupational health checks. Quarterly breakfast meetings take place for all staff to attend, giving colleagues from across all departments the opportunity to meet, support each other and ask questions, whilst at the same time enjoying and participating in a range of presentation topics focused around service delivery and development.

St Clare is committed to investing in the development of our workforce, offering staff a wide range of opportunities for training and development and promoting an open culture so as to ensure knowledge and skills gained from courses, events and seminars are shared within our organisation. As an organisation, we provide study leave support and sponsorship in areas such as specialist clinical degrees, financial qualifications and general training needs.

During 2013/14, Senior Staff Nurse Alison Plumb completed her BSc (Hons) degree in Palliative Care at Anglia Ruskin University while our Day Therapy Manager Emily Stowe started her Masters degree in Clinical Leadership in Palliative Care, Oncology and End of Life Care at the University of Southampton.

A monthly news e-bulletin is also published for all staff so everyone can keep up to date with the latest developments and ideas.

Our Volunteers As a very busy Hospice service, it would not be possible to function without the commitment and support of an enthusiastic team of more than 530 volunteers.

Our volunteers are responsive, flexible and add diversity, working alongside staff in a complementary way and engaging in all aspects of the Hospice, from retail to fundraising. Without this collaboration, the Hospice would be unable to sustain its services or develop the high level of care for patients and their loved ones.

Throughout the year, St Clare volunteers gave 100,000 hours of their time to support our work and are worth more than £1.3million a year to the charity.

That is how much we estimate it would cost if we had to pay for all the ways that our volunteers help us.

As part of National Volunteers’ Week in June 2013, a series of ‘thank you’ events were held to show our volunteers how much they are appreciated, while two were chosen to represent the Hospice at a Royal Garden Party at Buckingham Palace.

In January 2014, a new team of evening volunteers were introduced to offer an extra pair of hands to our clinical team in the Inpatient Unit.

Answering telephone calls, meeting and greeting visitors and preparing snacks are just some of their duties, allowing our nurses to dedicate more time to meeting the needs of our patients and their families.

As part of the development of our bereavement service, a team of specially trained volunteers have been recruited to offer bereavement befriending, providing compassionate support to those grieving the loss of a loved one.

Following the introduction of our new Hospice at Home service, we successfully recruited the first team of St Clare Community Friends.

These specially trained volunteers provide support, friendship and practical help to patients and families in all aspects of everyday living, such as walking the dog or light household chores.

Fundraising Many people do not realise that the wide range of skilled and compassionate care, comfort and support delivered by St Clare Hospice is free of charge to those who need it. Only 30% of our ÂŁ3.2million annual running costs are funded by the NHS, which means that as a charity, for every ÂŁ1 we spend, we have to raise 70p through donations and other voluntary sources.

It means we have to raise significant amounts of money each and every year to sustain the breadth of palliative care

services that are available to those facing the end of their lives, and it is true to say that without the help and support of our local community we would not be able to continue doing what we do best, caring for local families at a critical time.

It is the job of the St Clare fundraising team to ensure funding streams are maximised and resources used effectively, and they continually explore ever more innovative ways of generating the regular, reliable income.

To provide all our clinical services at the Hospice for one day costs more than ÂŁ7,500, and in January we launched a new initiative offering major supporters the opportunity to provide 24 hours of care, support and therapy for our patients. Donors can choose any day of the year to sponsor and are invited to spend time at the Hospice to see the impact their contribution is making to the lives of those who use our services.

New supporter groups were established in Waltham Abbey, Dunmow and Clavering, joining our existing groups in Harlow, Epping, Ongar and Stansted.

The seven St Clare Hospice shops had a good trading year, with a net contribution of £510,000 compared to £493,000 for the previous year.

Retail highlights included the completion of a planned refurbishment of our Epping outlet, which resulted in a notable increase in sales income. Gift Aid sign-ups also rose, with income increasing by 32% and continuing to be significantly higher per shop than national charity stores.

Weston Homes held its most successful golf day for the Hospice, raising an amazing £50,500 at its 19th annual event.

Thousands of Hospice supporters picked up the phone to vote for St Clare as part of the People’s Millions competition in conjunction with ITV, helping us win £50,000 of Big Lottery funding for our ‘Space to Breathe’ family garden to transform a little used part of our grounds into a tranquil outdoor haven for patients and visitors to enjoy. We were overwhelmed by the response to our appeal, with our community rallying to poll 12,403 votes – beating our nearest rival by nearly 11,000 votes.

A new Lottery Manager was appointed to raise the profile of the St Clare Lottery, review prize structure, oversee a canvassing team and boost player numbers. About 6,500 people play the St Clare Lottery a week, helping raise more than £16,000 each month.

Financial information During the 2013/14 financial year, total expenditure for the Hospice was £3,182k. Income generated was £4,035k, which included a one-off capital grant from the Department of Health for our ‘Space to Breathe’ refurbishment project. Excluding this grant, a financial surplus of £381k was achieved, which will enable us to continue developing our services caring for patients and their families in the new financial year.

Financial Highlights • The Hospice continued to invest in developing and improving our services for patients and families, in particular with the launch of a new Hospice at Home service. Total patient care expenditure was £2,637k, up 11% on the previous year.

• A one-off capital grant of £472k was received from the Department of Health for our ‘Space to Breathe’ refurbishment project.

• We continued to be well supported by our generous local community, with donations, events and other fundraising activities continuing to grow, increasing by 8% during the year. This resulted in an encouraging increase in several key income streams, including individual giving and appeals.

• Profits generated from our weekly Lottery and our seven shops increased slightly, overall showing growth of 4%.

• Legacy income almost doubled from the previous year. We were fortunate to receive £571k from people who remembered St Clare in their Wills.

• NHS income was £970k, which represented 30% of our expenditure. This was in line with 2012/13.

Leave a gift in your Will

Our specialist palliative care services are free of charge to those who need them but cost us ÂŁ3.2million a year to provide, with only 30% of our spending covered by NHS income. We are therefore extremely grateful to our local community for the support we receive, helping us raise money each year so we can continue being there for local families when they need us the most.
