he Access Team has been developed to offer a flexible and responsive single point of contact to all Hospice services. The overarching aim is to support people to get to the right service at the right time.

We provide timely access to advice and support for new patients, families and supporters, as well as professionals across health and social care. The Access team are a reactive service, providing an urgent same day response to people already known to our community team.
