Saint Apollonia, Patroness of Dentistry

S

ince the early middle ages, the legend of Saint Apollonia has been regarded by the Roman Catholic Church as a source of comfort and alleviation for sufferers of toothache. During the eighteenth century, the patronage of this Saint was extended to the dental profession. For example, the “toothbreakers” of Turin always carried out their extractions “con adjute di Sancta Apollonia.”

T

he main source of the historical background to the legend is a letter written by Bishop Dionysius of Alexandria describing the persecutions of the Christians in that city in the year A.D. 249. The bishop makes a special reference to the behavior of the virgin Apollonia who refused to renounce her faith at the order of the magistrate. For this defiance, her teeth were knocked out, her jaws were broken, and she was threatened finally with the stake. Apollonia appeared to consent, but as soon as she was released from her chains, she cast herself into flames to die a martyr’s death. The Roman Martyrology records February 9, her birth anniversary as her feastday.

Congratulations, too, to the organizers and the new set of officers of this annual event. The Department of Health commends your organization for its continuous efforts to provide quality health care services and seek improvement of oral health and wellbeing of all Filipinos, particularly the poor. Some charity works from your end like health education, free community dental services and feeding programs are very welcome gestures as these will already go a long way in helping us realize our dream of accessible quality healthcare for our people. Message My warmest greetings to each and everyone of you! It has been a tiring yet worthwhile experience for the officers of FCDFI. Among this years harvest are The Outstanding Affiliate Society Award, Meritorious Achievement Award and Exemplary Achievement Award in Community Service for Manila Chinatown Charity Clinic, given during the Awards Night of Philippine Dental Association. The long planned seminar on Basic Life Support has become a reality and is one of the accomplishment this years’ officers take pride of. Conducted by topnotch American Heart Association certified instructors who are also emergency medicine practitioners. The Seminar is aptly titled “ Hey, Hey, Hey! Are You Ok?”. Part of this years’ pioneer project that no other chapter or affiliate has gone is being voice of the voiceless, giving importance to the welfare of animals. FCDFI joined the Animal Welfare March at Quezon City Memorial Circle and the 6-Legged Run at Mall of Asia. All of these would not have been possible without the great teamwork of all the officers as well as the advices of past presidents. I would also like to thank our partners-in-service for continuing the advocacy of FCDFI in providing dental service to financially challenged members of our society. And also to the members of FCDFI for supporting the various programs, namely the dental missions and The Fil-Chi Sports Team, who continue to bring pride to the foundation. Much gratitude also for the members of FCDFI who’s thirst for knowledge is quenched by the quarterly seminar that we dish out. Proceeds of which benefits the successful scholarship program the benefits deserving 3rd and 4th year dental proper students, as well as other worthwhile projects of the foundation. FCDFI Milestones... 1978

A group of young dynamic, hardworking, and civic-minded dental students from the University of the East were meeting regularly. From their deep passion and vision, had conceived and shaped the future Foundation.

1980 Around 12 young dentists met at a Chinese restaurant in Ongpin to draft and organize a

dental society to be known as the Filipino-Chinese Dental Foundation, Inc.

1981 The Filipino-Chinese Dental Foundation, Inc. celebrated its 1

st

Annual Convention. This

was held at the Century Park Hotel in Manila.

1983 FCDFI was finally recognized as an affiliate society of the Philippine Dental Association

from which membership steadily swelled in number.

1984

Filipino-Chinese Dental Foundation, Inc. then presented exemplary continuing education programs and actively participated in all PDA activities.

1986 The scholarship program was formed. This project became one of the hallmarks and

the “crowning glory” of FCDFI. The main goal of which was to help deserving indigent dental students.

1987 FCDFI amassed recognitions especially in the PDA bowling tournament. 1988 FCDFI won awards in the PDA’s Healthy Smile Contest and Sports Fest. 1990

The maiden issue of the FCDFI newsletter, the forerunner of the Primer, was released. It was also the year when the hard plastic ID cards were first issued to the members.

1991 The official FCDFI pin was given to officers and as an incentive was also given to the

members who completed the seminars of Fil-Chi. During its annual convention, an unprecedented influx of delegates attended the convention and the hotel had to set up additional tables in the foyer area.

1992

It was noted to have invited Senator Joseph Estrada.

1993 The Foundation received the first Most Outstanding Affiliate President award from PDA. 1994

The Foundation bagged in the championship in the PDA bowling tournament for the nth, which proves that Fil-Chi was “the team to beat”. The Best Affiliate Award was given by the PDA.

1995 The program on health education started. The Association instigated the recognition of

the affiliate societies, including Fil-Chi, in the House of Delegates, the legislative body of the PDA.

1996

The Foundation organized a movie premiere wherein the proceeds went to FCDFI Mission Fund and an allocation was provided for the Building and Welfare Trust, the “brainchild projects” for the year. It was also the year wherein the FCDFI President and Board of Directors paid a courtesy call to the Republic of the Philippine President Fidel V. Ramos and lauded the efforts of the Foundation.

1997 The Civic Action Team was recognized as a more visible group of Fil-Chi and the association received a Meritorious Award from the PDA.

1998

A Wet Clinic on Apicoectomy was performed during the Annual Convention, was feedin and viewed on wide screen. Received award during PDA’s Award night as the Most Outstanding Award.

1999

Emphasis was given on the importance of oral health education by conducting lectures on Preventive Dentistry to private and public elementary schools and some private offices. The Foundation made a significant endeavor in the protection of the environment by way of tree planting at Plaza Dilao, Paco, Manila. Awards received from PDA are the Most Outstanding Affiliate Chapter and the Most Outstanding Affiliate President.

2000

The Welfare and Trust Fund for the active and life members of the FCDFI was implemented. Awards received from PDA are the Most Outstanding Affiliate Chapter and the Most Outstanding Affiliate President.

2001 Received the First Runner-up in the Most Outstanding Affiliate Chapter Award from PDA. 2002 Received the Most Outstanding Affiliate Society Runner-up from PDA. 2003

The Foundation embarked on an Oral Health Education Program for Pre-schools around Metro Manila. Awards received from PDA are the Most Outstanding Affiliate Chapter and the Most Outstanding Affiliate President.

2004 Was able to come up with Book-Giving and Anti-hepatitis B Vaccination from PDA. 2005

First to form sisterhood with another affiliate, Philippine Academy of General Dentistry (PAGD), undertaking joint projects such as Christmas Gift Giving, Feeding Program, and Special Seminar. Awarded PDA Most Outstanding Affiliate Society and President, and Merit Award.

2006

The very first foreign venue special seminar in Singapore was organized and interisland dental missions under the sisterhood program with Cebu Dental Society was undertaken. Awards received from PDA include: Most Outstanding Affiliate Society and President, Achievement Awards Dental College Scholarship Program, and Best Report Presentation Award.

2007 Breakthrough in Community Service: Environmental Cleanliness & Preservation Awareness Campaign activities. PDA recipient of the following: Most Outstanding Affiliate Society and President, Outstanding in Community Service, Presidential Distinction Award, and Overall Champion on On-the-Spot Poster Making Contest, making it the Official Billboard of the 5th National Dental Health 2009

2008 Longest term (1½ years) due to changes on PDA fiscal year to calendar year. “Libreng

Pustiso Project” in Bulacan & Binondo were accomplished; non-stop out of town Community Dental Outreach Program was made for the indigent people. Awards gathered were: Most Outstanding Affiliate Group Alay Parada 5.0 NDHM, 1st Place & Best in Talent Healthy Kid Smile. In sports, the Foundation bagged the Over-all Team Championship in Bowling tournament, 3rd Place PDA Ladies Billiard Tournament, Champion of the PDA Badminton Tournament Mixed Level A & B Women Division, 2nd Place Mixed Level C, Champion Ladies Double Level B, 2nd Place Ladies Double Level A, Champion Men’s Double Level B. Received from PDA: Most Outstanding Affiliate President, Most Outstanding Affiliate Society, Special Awards for Exemplary Implementation of Scholarship Program and Achievement Awards Community Service.

2010 The official FCDFI website, www.filchidental.com and Facebook page were launched.

The first undertaking of cleft lip and palate and speech rehabilitation project dubbed as “Fashion for Smile” was carried out. Contributed 1,300 imported hardbound books to fellow PDA chapters for distribution to their area of locality. Awards received from PDA: “Runner-Up Most Outstanding PDA Affiliate” and “Runner-Up Most Outstanding PDA Affiliate President.”

2011

The FCDFI privilege card acknowledged by both dental and non-dental establishments was first introduced. Unbroken projects are the community outreach programs helping 174 Barangays, 5000 of which is a populace of the less fortunate, reaching ethnic groups and our brethrens at the Bureau of Jail Management Penology in San Pablo City. Awards received are: Most Outstanding PDA Affiliate and President - General Affiliate Division, and First runner up PDA MTV Contest.

2012

Extended term (1½ years) following changes in PDA calendar. A recipient of a Presidential Award and a Special Award for Best in Community Service from PDA. A significant endeavor in the service for its members include the morphing of the FCDFI MEMBERSHIP ID into a discount privilege card... as reflected by the remarkable increase in its number of active members. Also awarded by PDA as the Most Outstanding First Runner Up both for General Affiliate and Affiliate President. Another notable outstanding Professional Recognition received was during the radio guesting of President Carolyn G. Torres-Basa for “Heard on Thursdays” show on July 5, 2012 in Monster Radio RX 93.1 with the topic: “Who’s Afraid of the Dentists? ”.

2013

Bagging the Outstanding Affiliate Society Award, Meritorious Achievement Award, and Exemplary Achievement Award in Community Service for Manila Chinatown Charity Clinic capped the usual amazing achievements of The Fil-Chi Sports Team -- bagging major awards in Women’s Darts, Bowling, and Badminton. Being true to its theme “Making a Difference In This Fast-Paced World”, FCDFI embarked on which not one chapter and affiliate has gone… to start awareness on animal welfare. Among the baby steps taken were joining the Animal Welfare March at Quezon City Memorial Circle, 6-Legged Run at Mall of Asia. Among the last and most significant milestone is making the long planned Basic Life Support Seminar — aptly titled “Hey, Hey, Hey! Are You Ok?”, which was conducted by certified American Heart Association instructors who are also emergency medicine practitioners.

This presentation will introduce you to the principles of Gnathological dentistry and the benefits of a Canine Guided Occlusion. It will increase your understanding of the basics of a functional occlusion and the para-functional forces that can lead to treatment failures. I will also share with you some examples that will help you identify and address these destructive forces to help you prevent failures in all phases of dentistry from single to multiple restorations, periodontics, orthodontics, endodontics and especially, TMD pain management.

Has a permanent resident status in the Philippines; has shared his experiences with various dental groups here in the Philippines since 2009 in Functional Occlusion and TMD Pain Management Speaker at the 2011 and 2013 PDA and Endodontic Society of the Philippines July 2013 Weekly volunteer TMD consultant for the ENT Resident Program at Makati Med Member of the Balintawak Chapter of Rotary

Born, raised and attended high school in Honolulu, Hawaii Attended college in Oregon for his pre-dental requirements and received his DMD from the Oregon Health Science Centre, in Portland, Oregon, USA Holds dental licenses in Hawaii, Oregon and California and a lifetime member of the American and Hawaii Dental Association

Presently... Conducts study group classes in Manila and a university based postgraduate course with Davao Medical Foundation Dental College in basic functional occlusion concepts in relationship to all phases to dentistry and TMD pain management and Bite splint therapy.

Background… Since 1974, as General Practitioner focusing on “Full mouth reconstruction with fixed and removable restorations” he has followed the principles of Gnathology, which embraces the philosophy of Canine Guided Function and how it applies to all phases of dentistry in as well as for TMD pain. -also has background in functional orthodontics to Invisalign and straight wire techniques

Most of all, he enjoys his new permanent home in the Philippines sharing his knowledge and clinical experiences with his fellow dental and medical professionals.

TITLE: Standard of Care for Oral Surgery Patients Abstract... Dental treatment especially oral surgery should be based on standards of care. The different aspects of surgical care from patient evaluation to treatment and patient recall is governed by evidence based practice founded on guidelines. Esthetic treatment and rehabilitation of the anterior dentition through direct and indirect veneers have been used extensively in restorative dentistry and prosthodontics with proven clinical success. With the development of different materials like zirconium silicate (indirect composite), feldspathic ceramics, reinforced lithium disilicate ceramics (pressed ceramics), zirconium, Dentists have the choice of selecting the best material for their patients’ esthetic needs. Traditional veneer preparations are now being modified either minimally invasive or extended defect-oriented preparation designs as well. Choosing correctly on the materials and preparation design when restoring the anterior esthetic zone is crucial to achieving a conservative and long-lasting treatment.

CONSTITUTION AND BY-LAWS of the FILIPINO-CHINESE DENTAL FOUNDATION, INC. Preamble The ideal of service to humanity as a worthy enterprise is a concept ingrained in the dental profession. As such, we dentists throughout the Philippines, holding and believing firmly in these principles as well as the spirit of mutual understanding, fellowship, cooperation; the encouragement and enhancement of professional standards by promoting scientific researches, progressive continuing dental education programs and adherence to the Regulatory Code of Dental Practice in the Philippines; do hereby pledge to form, constitute and promulgate this Constitution and By-Laws. Article I – NAME The name of this Organization shall be Filipino – Chinese Dental Foundation, Inc. hereinafter referred to as the F. C. D. F. I. or the Association. Article II – HEADQUARTERS

Active Member - Dentist duly registered, practicing and who has paid his membership fee for the current year. Inactive Member - Dentist duly registered and practicing who has been an active member in the past but has not yet paid his membership fee for the current year. Honorary Member - Any person of any citizenship who has made an outstanding contribution in the dental science may be elected as an Honorary Member upon the recommendation, evaluation and approval by the Board of Officers and the Council of Presidents. Life Member - The membership is given automatically to all past presidents and any member who has continuously been an active member for at least twenty (20) years.

Any member of the Association whose dues for the current year has been fully paid shall be considered a member in good standing.

3 Privileges Active Member - All such members shall be entitled to attend all scientific functions; be eligible to any elective or appointive position; and be qualified to vote. b. Inactive Member - All such members shall be entitled to attend all scientific functions sponsored by the Association. c. Honorary Member - All such members shall receive a Certificate of Honorary Membership and be entitled to attend all scientific functions sponsored by the Association. d. Life Member - All such members shall receive a Certificate of Life Membership and be entitled to all privileges of an active member in good standing; shall further act as an adviser to the Association if and when consulted or duly appointed by the incumbent administration; and shall be exempted from payment of membership dues. a.

All members except the Honorary and Life Members shall pay an annual membership fee of Two Hundred Fifty Pesos (PHP 250) on or before the annual convention, wherein 20% will go to the welfare trust fund. All elective, appointive and ex-officio officers shall constitute the Board of Officers with the President as Chairman and with all having the right to participate in discussion and the power to vote. Article VII - ELECTIONS AND TENURE OF OFFICE

Special elections shall be held once an elective officer has been relieved or a position has been declared vacant, on a date and place to be decided upon by the Board of Officers. If such a vacancy is declared for the position of Director, the candidate who garnered the next highest number of votes in the previous election shall be declared successor without any special election.

Qualifications and Eligibility a.

Only members who have been active for three (3) consecutive years and in good standing both of this Association and the Philippine Dental Association may be nominated and elected or appointed to any position in the Association. b. No absent member may be nominated and elected provided that the member has written his intention to run for an elective position and has a valid reason for his absence. c. No member may be elected to more than one position in the Association. d. Nominees for President-Elect and Vice Presidents must have been a member of the Board of Officers for at least two (2) years during the past five (5) years prior to his nomination. e. Honorary and Life Members cannot hold any elective position. All Past Presidents cannot run for the position of President-Elect within three (3) years after the termination of their tenure in office. f. Nomination for the elective officers must be submitted by at least twenty ( 20 ) members of the Association g. Election of Officers shall be by secret ballot and/or viva voce with each voting member entitled to only one vote. h. All election protests shall be referred to and decided upon by the Committee on Elections. Appeals may however be directed to the Council of F.C.D.F..I. Presidents. There shall be a constitutional body called the Committee on Elections or COMELEC composed of three (3) members from the Council of Presidents appointed by the incumbent President.

The members of COMELEC shall hold office for a term of three (3) years or until their successors shall have been duly chosen and qualified, provided that in the initial term, the Chairman should serve for one (1) year; the second member for two ( 2 ) years; and the third member for three (3) years; and provided further that thereafter the most senior member shall serve as Chairman.

The COMELEC shall conduct the election of officers by prescribing the necessary rules and regulations and shall be empowered to administer and enforce the same; it shall likewise maintain impartiality and none of its members must be identified for or against any candidate nor should any of its members hold or aspire for any elective positions while in office. All members are obliged to attend all scheduled meetings; to pay all membership fees and dues promptly; to faithfully abide by the Regulatory Code of Dental Practice in the Philippines; and to adhere to the provisions of this Constitution and By-Laws and all other policies that may be approved hereafter.

Officers a.

President - The President shall deliver the opening remarks in the annual meeting as well as preside in all meetings of the Association and of the Board of Officers; shall serve as the official representative of the Association in its dealings with the mother organization (PDA), government, civic, business, and other professional organizations for the purpose of advancing the policies and objectives of the Association;

shall create other committees and appoint members for emergencies and purposes not otherwise provided for in the Constitution and By-Laws; shall countersign all disbursements and expenditures approved by the Board of Officers and make an official report of the activities of the Association in its annual meeting; shall submit within thirty (30) days after assumption to office an annual budget for the activities of the Association; shall perform such duties and functions as may be provided for in the Constitution and ByLaws. b. President-Elect - The President-Elect shall succeed the President on the following year of his election; shall serve as a member of the Board of Directors; shall assist and advice the President when requested; and shall be the General Chairman of the Annual Convention of the Association. c. Vice President - All Vice Presidents: Executive Vice President, First (1st) Vice President and Second (2nd) Vice President shall assist the President when requested; shall carry out the policies of the Association and such other duties and functions that the President may assign to them; and the most senior Executive Vice President among them shall assume the Office of the President in case of the latter’s incapacitation due to resignation, illness, impeachment or other valid causes and shall only cover the unexpired term of the President or until such time that the said incapacitation shall have ceased to exist or until such time when special elections can be held. d. Secretary - The Secretary shall attend all meetings and keep all the records of the Association; shall take charge of all the correspondences, communications and issue notices of all the meetings and shall prepare written reports of all the affairs of the various committees as directed by the President or the Board of Officers; and shall turnover all the records of the Association to his successor. e. Treasurer - The Treasurer shall be the custodian of all money, properties, securities, funds and deeds belonging to the Association as directed by the Board of Officers; shall countersign all checks and disbursements with the President to pay the Association’s obligation; shall submit a financial report when so requested by the President or the Board of Officers; and at the expiration of his term of office, shall turnover immediately to his successor all pertinent matters and records thereto appertaining, making certain that half of the available cash funds are turned over to the Chairman of the Council of F.C.D.F.I. Presidents and other half to his successor. f. Auditor - The Auditor shall perform such duties necessary and expedient in auditing the money, properties, securities, funds and deeds and other assets of the Association and such other duties as may be assigned to him by the President or Board of Officers. g. Directors - All Directors shall attend all meetings of the Association and be appointed by the President to any position necessary for the implementation of the policies and projects of the Association. h. Public Relations Officers - All Public Relations Officers (P.R.O.) shall take charge of all public information and propaganda activities of the Association through the media in order to promote the proper and effective implementation of all the programs of the Association. Article X - MEETINGS Section

The Association shall hold an annual convention within the First (1st) Quarter of the PDA Fiscal Year and Three (3) quarterly scientific meetings at a time and place to be agreed upon by the Board of Officers unless constraint by Force Majeure.

Any unexcused absences by any member of the Board of Officers for three (3) consecutive meetings would mean automatic resignation on the part of the member involved.

The President or any five (5) members of the Board of Officers may call a special meeting whenever it is deemed necessary.

A simple majority of the members of the Board of Officers shall constitute a quorum.

All notices of meetings shall be handled directly by the Secretary and persons concerned are to be notified at least seven (7) days prior to such meetings as to date, time and place.

Committee on Ways and Means - To take charge of all the business affairs of the Association in collaboration with the President, Treasurer and Auditor. g. Committee on Complaints and Grievances - To receive and evaluate complaints and grievances and to recommend to the Board of Officers measures to be taken to solve these complaints and grievances. The Council of F.C.D.F.I. Presidents shall be an autonomous body composed of the incumbent President, the President-Elect and all past Presidents.

The Council shall elect from among themselves a Board of Governors, who shall administer the affairs of the Council, composed of the following : Chairman, Vice Chairman, Treasurer, and Auditor. The Secretary of the Board of Governors shall be the President-Elect. The incumbent President cannot be elected to the position of Chairman of the Board of Governors.

Term of Office a.

The Members of the Board of Governors shall serve for two (2) years except for the Secretary who shall serve for one (1) year.

Duties and Functions a. The Council shall be the advisory and judiciary arm of the Association. b. The Council shall exercise powers related to the adjudication of matters presented before it by the Board of Officers or by any member of good standing, c. The Council shall interpret, when requested or if the occasion arises, provision in the Constitution and By-Laws. d. The Council may investigate any alleged violation of the Regulatory Code of Dental Practice in the Philippines after the same shall have passed the corresponding committee of the Association, and shall impose the necessary disciplinary action upon any errant officers and members, and likewise refer the matter to the Philippine Dental Association of the Board of Dentistry for further action. e. The Council, through the Chairman, shall act as an adviser to the incumbent President during board meetings or whenever such advice is requested. f. The Council, through the Board of Governors, shall conduct all the activities of the Association with regards to scholarship offered to dental students by the F.C.D.F.I. The Board of Governors shall be tasked with the evaluation and selection of the F.C.D.F.I. scholars. Any member of the Council may be appointed by the Chairman to assist in the Scholarship Program of the Association. g. The Council, through the Chairman, shall collect from the outgoing Treasurer of the Association, duly certified by the outgoing President, half of the available cash funds for deposit to a reputable bank in the name of F.C.D.F.I. Trust Fund. This Fund shall be utilized by the Association in the funding of the F.C.D.F.I. Scholarships and other projects to promote the functions of the Council and the Association. Any disbursements shall be subject to the approval of the two-thirds (2/3) majority of the Council members in a meeting. The Signatories to the bank account of the Trust Fund shall be any two (2) of the following : Chairman of the Council, the Vice Chairman, and the Council Treasurer.

Meeting and Quorum a.

The Council shall meet twice (2x) a year, once in June and another in December, or as often as the Chairman may decide. A simple majority of the members of the Council shall be sufficient to constitute a quorum for a meeting to be called. b. All members of the Council can vote except for the President-Elect who is the Secretary of the Council. The Chairman can vote only in case of a tie. Article XIII - IMPEACHMENT AND EXPULSION

Any member, upon the recommendation of the Board of Officers, may be expelled, if after due process, if registration has been revoked by the Board of Dentistry or has committed an offense involving moral turpitude. The Board of Officers may create an Investigating Committee to conduct the investigation and give all parties the opportunity to be heard. Officers

Any officer of the Association may be impeached for a cause, upon the recommendation of the Board of Officers and after having been given due process. A vote of three-fourths (3/4) of the total members of the said Board is needed for impeachment. Article XIV - AMENDMENTS

Any article of this Constitution and By-Laws may be amended, repealed or altered by a two-thirds (2/3) vote of the members of the Board of Officers subject to ratification by the General Assembly or by Postal Referendum.

Dental students applying for this scholarship grant should possess the following qualifications :

Total income of the parents during the immediate past year should not exceed Five Hundred Thousand Pesos (Php 500,000).

Official Transcript of Record reflecting the performance of the student for the first two (2) or three (3) years of Dental Proper. 3. Endorsement Letter from the Dean of the dental school where the student is enrolled. 4. Endorsement Letter from the Clinical Chairman and one (1) faculty member of the dental school. 5. Must not be a recipient of any form of Scholarship Program that would coincide with this grant. 6. The applicant must be available for interview. 40

BUILDING AND WELFARE TRUST FUND PROVISIONS AND REQUIREMENTS of the FILIPINO-CHINESE DENTAL FOUNDATION, INC. Welfare Benefits for Active and Life Members : FIRE : Benefit can only be given with a single occurrence of fire in either member’s residence or office within one (1) fiscal year of the officer’s term. Eligibility for Benefits : a. PHP 10,000 will be given to the disclosed beneficiary or member who has been active and in good standing for the past three (3) consecutive years. b. PHP 5,000 will be given to the disclosed beneficiary or member who has been active and in good standing for the past two (2) consecutive years. c. PHP 3,000 will be given to the disclosed beneficiary or member who has been active and in good standing for the past year and maintains his/ her membership up to the present year. The check should only be made payable to the name of the member. Provisions and Requirements : a. The fire should not be intentional. b. The fire must be accidental in nature and a clearance certificate must be obtained from the Local Fire Department Arson Squad. c. Members must show a proof of residence/ office by providing us a copy of Meralco/ Manila Water District/ Rental receipt in his/ her name. d. If fire occurred in his/ her residence/ office and only a portion of the residence/ office was burnt, benefit will be given partially depending upon the extent of the damaged structures. e. Full payment will be given on total loss. f. Must provide a satisfactory proof of membership, may it be his/ her payment receipts during the years required and certified by the Chairman of the Membership Committee. Note :

If both spouses are members, benefits will be given for both the husband and wife. Benefits are only given to Active and Life Members and are non-applicable for Honorary Members.

DEATH : Only the member’s legal spouse/ legitimate children or should he/ she be single, then the parents will be the beneficiary. The qualified beneficiary may only claim once by any of the above-mentioned. In case of dispute regarding the legitimacy of the claimant, the decision of the Chairman is deemed to be final. Eligibility for Benefits : a. PHP 10,000 will be given to the disclosed beneficiary or member who has been active and in good standing for the past three (3) consecutive years. b. DISMEMBERMENT / DISABLEMENT : I. Total Dismemberment / Disablement : Complete loss or dysfunction of both eyesights or extremities, either arms or legs or any impairment which may hinder his/ her ability to practice dentistry. Eligibility for Benefits : a. A sum of PHP 10,000 will be paid if he/ she has been an active member of good standing for the past three (3) consecutive years. b. A sum of PHP 5,000 will be paid if he/ she has been an active member of good standing for the past two (2) consecutive years. c. A sum of PHP 3,000 will be paid if he/ she has been an active member of good standing for the past year and maintains his/ her membership up to the present year. II. Partial Dismemberment / Disablement : If the ability to practice dentistry is found to be a hindrance for the practitioner then he/ she may claim total disability. There is no partial disability / disablement provision. The check should only be made payable to the name of the member. Provisions and Requirements : a. It should be accidental in nature and not an intentional incident. b. The eyesight or extremities should be non-functional. c. A medical doctor’s certificate is required. Benefits are only given to Active and Life Members and are non-applicable for Honorary Members. Note : The finances will come from the funded account being held by the Chairman of the Committee on Building and Welfare Trust Fund. However, it must not exceed PHP 50,000 per term year of the President of the Foundation to avoid depletion of the existing fund.
