Since mid-March, when we all went into the COVID-19 period, local businesses have seen a huge decrease in sales, and while they’re working hard to maintain their companies, the hidden story is that with the loss in revenue, and our huge appetite for buying online, local sales taxes have plummeted, which isn’t good for anyone. Local sales tax is what our cities and towns rely on to provide the services we all expect and take for granted: police, fire, parks and recreation, road maintenance, hiking and biking trails, waste collection, and more. When taxes decrease, it affects these services and more, and this affects each of us and the way of life we’ve become accustomed to. We’re talking about MILLIONS and MILLIONS of dollars in tax revenue that our cities are losing. Make no mistake about it, this impacts each and every one of us, and we need to take action. What action can we take? It’s simple. BUY FROM LOCAL BUSINESSES! The holidays are the perfect time for us to band together, to help ourselves and the beautiful, locally owned places where we live. Purchases like these not only help with taxes, but they keep our friends and neighbors employed—and positively employed people spend more locally. Of course, there will be items that you may need to go online for; however, did you know that when you buy from Walmart or Target online but pick the order up at your local store, the tax dollars stay here? We’re all in this together, and your buying decisions this holiday season are more important than ever.

As of press time, the winning team hadn’t been announced. Folsom native and rising country star Colton Graves released his debut EP, Smile for Me, with LA-based production company Extreme Music. American Public Works Association Sacramento Chapter (sacramento. apwa.net) selected Folsom’s Oak Parkway Trail Undercrossing—which provides a seamless and safe route for trail users to transition between the Oak Parkway and Johnny Cash trails—as its 2020 Project of the Year in the “Parks & Trails” category. El Dorado Union High School District received the Energy Stewardship Award from Cenergistic, a nationwide sustainability company, after reducing their energy use by 18% in 18 months and saving over $400,000 in utilities.

Though shut down for a time, the Gallery at 48 Natoma has reopened with welcome arms. Free, self-guided tours on a variety of days are being offered for individuals and groups of up to 10 people, but a reservation is required. “The gallery is following current public health guidelines; all visitors and staff are required to wear a face covering while inside the building and maintain social distancing at all times,” shares Director Cindy Abraham. 7 Tips for a StressFree Season
hanksgiving is over, and Christmas is right around the corner. While it’s a joyful time of year, it’s also stressful, as we try to make time for family and friends, find the perfect present, and be present in each moment. We turned to local experts to explain why the holidays can feel overwhelming, as well as share tips on how to manage stress during the most wonderful time of the year. Gretta Smith, yoga teacher and owner of Ohana Moon Yoga, says the holidays tend to be a stressful time because we defy the natural rhythms of nature. “Winter is a season of hibernation with fewer hours of sunlight and more hours of night, which should be devoted to more sleep,” she says. “People fight these primal instincts with caffeine and overstimulation, including overeating, overspending, and overexerting. A lack of sleep plus overstimulation equals stress.” A lot of the pressures we feel around the holidays stem from our own internal expectations. “We put a lot of pressure on ourselves to do it all,” says Joy Arnold, a 500 E-RYT yoga teacher, reiki master, and holistic health coach. “We need to let go of the image we have in our heads of what the ‘perfect’ holiday is and what we ‘should’ be doing during this time. Instead, we need to allow ourselves to be in the moment and enjoy the true meaning of the holidays: spending time with those you love and appreciating all we have.” Here are seven suggestions on ways to minimize holiday stress and put the focus back on feeling grateful and enjoying each moment.

“Stress starts in the mind; however, we feel it everywhere,” Smith says. “Yoga is a practice that helps connect mind, body, and spirit. When we go into active and passive yoga poses to feel sensation in the body with conscious breath, we’re able to quiet the busy, loud, anxious mind and relieve stress. As we close out an especially stressful year, I recommend practicing yin or gentle yoga to help ground your nervous system and calm your mind. The combination of stillness and slow, mindful breathing helps relieve physical, mental, and emotional tension.” Amitis Pourarian, owner of THE STUDIO Martial Arts & Fitness, says exercise is a huge stress reducer, as it provides a break from having to deal with issues and an outlet for stress. “What’s more, there are endorphins released in the brain as a by-product of exercise, which uplift our mood and our sense of wellbeing,” she shares.

“Make a budget and stay within it,” Arnold recommends. “No one enjoys giving or can relish the holiday season when they know they’ve overspent and will have to face it come January. The gift of giving is not about a dollar amount; it’s the intention and energy behind what we give.” Homemade presents never go out of style. Who wouldn’t be grateful to open a framed family picture, recipe book filled with time-tested dishes, or a calendar of birthdays with photos?
“Take time for yourself during the holidays to break off and do things that ground you and bring you joy,” Pourarian says. “Creating space to be of service to others who are less fortunate is a great stress reliever.” Turn to page 22 for a list of ways you can be of service this season.
“Take some quiet time to choose a word or sentence for the holiday experience you want and let it drive your commitment to a stress-free holiday,” suggests Jenifer Novak Landers, a professional certified coach at Fully Expressed Potential. “For example, my word is ‘resilience.’ I visualize myself in ease and joy, being fully present to the magic of every single holiday moment. When things get messy or off track, (because they WILL!) I can return to my word, resilience, and remember to allow myself to practice it, rather than

pushing on my stress or fear, which only makes it worse.”

“When we’re stressed, our breath tends to be shallow, erratic, and in the chest. This actually causes the body to release more stressor hormones, ultimately causing us to be more stressed,” Arnold says. “No matter where you are, try this. Close your eyes, sit up tall, relax your shoulders, take a deep breath in through your nose and sigh it out your mouth, emptying your lungs completely. Now, bring awareness to the belly, begin to sip the air in through your nose to a count of five, pause the breath, then exhale through the nose slowly to a count of five.”
“Being aware of what thoughts or stories you’re telling yourself about the holidays will give you the option to choose a different thought—or to at least think yourself into a neutral place,” Novak Landers says. “You can shift how you feel by choosing thoughts that are more positive. In your mind, change an anxious story into a relaxed one. When the feeling of stress creeps in, ask yourself: ‘What do I need right now to feel loving about myself?’ Or, ‘How can I be loving to ME right now?’ And then do it. You’ll notice a big shift in your energy. For example, old thought: I won’t have time or energy for everything I want to do. New thought: Even though my time is short, I’m excited to be fully present with all I can do. This is also a great opportunity to schedule follow up time with people in 2021.” As you navigate the extra stresses of COVID-19 during the holidays, remember this. “Even though the negativity of surrounding the pandemic completely changes this year's holiday season, choose to look for love in the smallest, simplest things, especially within yourself. Love is something you can focus on.”
“It can be 10 minutes in the morning, 10 minutes at night, or both! Better yet, make it 30 minutes,” Arnold says. “What you do during this time is your choice, so long as it helps you to feel at ease, calm, and grounded. Maybe you go for a run, jump on your yoga mat, or sip a cup of coffee outside and just be. The important thing is that you take the time to nourish and recharge yourself. Then, you’ll have the energy and stamina to be present for family, friends, and all the hustle and bustle of the season.”

A
trip to the emergency department can be stressful, whether you’re there for yourself or accompanying a loved one. There’s a stigma of long wait times associated with ERs and it’s understandable to want to get in and out as fast as possible. With this in mind, the Marshall Medical Center Emergency Department staff has a few tips to help you through the process. • Arrive with your insurance card, medical history, list of medications, allergies and names of doctors who’ve provided you care in the past. If possible, provide a list and timeline of your symptoms to help the staff understand what brought you to the point of needing emergency care. Your health history is the most significant tool the staff has to help make the most accurate diagnosis.

• Bring a health partner with you. A health partner can help give the medical history if you’re too ill, take notes, ask questions and help you follow discharge instructions. They can also be your ride home.

• Use any wait time to make phone calls to coordinate a ride home if you’ll need one, ask a friend to care for pets or pick up children from school or day care and let your doctor’s office know you’re in the emergency department. |

• It’s easy to let healthcare professionals drive the conversation, especially in such a tense, busy environment. But make sure you speak up and advocate for yourself— tell the doctor or nurse everything— even if it’s embarrassing—and make sure to ask all your questions before they discharge you. • Before you use the restroom, ask the staff if they think they’ll need a urine sample. This simple step could save you time and several bottles of water later in your visit.

• When you’re discharged, ask for copies of any tests or results so you can share with your primary care provider. • Do not leave until you fully understand any test results, diagnoses and discharge instructions. The amount of time you spend waiting in the Emergency Department will vary based on what you’re there for, how many other patients are there and the severity of the cases. Those with risks to life or limb are attended to first. Rest assured the doctors, nurses and support staff are doing everything they can to move things along quickly.

How Smoking or Vaping Impacts Your Risk of COVID-19
The dangers of smoking and vaping are well documented. But how, specifically, do these habits change your risk of contracting COVID-19? And if you do become ill, how will your experience differ from that of a non-smoker or non-vaper? Ravi Chopra, MD, of Marshall Pulmonology has answers to common questions. Q: If I smoke or vape, am I more likely to catch COVID-19? A: Not surprisingly, studies have shown that those who smoke or vape are at a higher risk for catching COVID-19. In fact, people who vape are five to seven times more likely to catch it than those who don’t smoke or vape. This higher risk is due to an increased number of receptors in the lungs where the virus attaches, which happens when lung tissue is exposed to nicotine. Q: If I do come down with COVID-19, will it be worse for me than for a person who doesn’t smoke or vape? A: Once a person who smokes or vapes contracts COVID-19, the respiratory symptoms will be more severe and the damage to the lungs greater than in non-smokers. They’ll have more complications and will take longer to recover. Lung tissue is very fragile; membranes are thin enough for
oxygen molecules to pass through into the blood stream. Some of the symptoms of COVID-19 can cause scarring in the lungs, which is worse for those who smoke or vape. This scarring can cause coughing, shortness of breath and reduce oxygenation of the blood, requiring the use of supplemental oxygen. While some of this scarring will heal over time, for some the need for supplemental oxygen could be lifelong. Q: Since I’ve already done the damage to my lungs, will quitting now provide any benefits? A: If you smoke or vape, you may think the damage has already been done and there’s no use quitting now. But with every cigarette smoked, the cycle of inflammation in the lungs restarts. It’s never too late to quit smoking or vaping and see the immediate and long term health benefits it brings. If you need help to quit smoking or vaping, there are lots of free resources online. Over-thecounter aids such as nicotine gums, lozenges and patches will ease the cravings as you try to change your habits. I
t’s Tuesday afternoon at Upper Room Dining Hall in Placerville. Jim, a local man who is currently without a home or shelter, has come for a hot meal before he faces a night out in the cold. What’s different about this visit is that Shaunda Crane, RN, from Marshall Medical Center is there. Shaunda helps clients in the Upper Room with wound care assistance, a common condition for many homeless. Jim has an infected wound and appreciates instruction on how to care for himself. During their conversation, Jim is able to open up about some of his other health concerns. He’s also diabetic and is having trouble controlling his blood sugar on the street, which lands him in the ER on occasion for treatment. Shaunda is able to help him with this before he takes his dinner to go. She hopes to see him next week to check in with
who meet certain requirements are provided with temporary housing at a motel in Pollock Pines. Phase three is where people work on obtaining basic necessities in order to find a permanent home and employment, such as a driver’s license, social security card, food stamps, and a cell phone. The program is goal-oriented, with each person provided a check list or care plan form to keep track of their tasks and to know what comes next. The program also teaches life skills in addition to addressing health issues that may be thwarting the ability to obtain safe, stable housing. It’s been so successful that other California counties look to El Dorado County as an example for their own programs.
This weekly check in at Upper Room is how Marshall’s Care Coordination for Vulnerable Populations program started. Conversations open up opportunities to discuss health issues and other challenges. Working closely with El Dorado County’s Homeless Outreach Team (HOT) and other assistance organizations, Shaunda expanded outreach efforts to include HOT’s Trailer Days, where people can get haircuts, laundry services, food, clothes and more, as well as regular check ins throughout the county to provide medical support. It encompasses many vulnerable populations, including unsheltered, elderly, at-risk women and the Latino community. When the pandemic hit, Martin Entwistle, Executive Director of Population Health at Marshall, saw a need to help find
services for the county’s most vulnerable. The effort dovetailed with Governor Gavin Newsom’s Project Roomkey, an executive order mandating California Health and Human Services Agency and Office of Emergency Services to make hotels available as temporary residences to quarantine and treat individuals who have tested positive for COVID-19. Many local organizations and agencies, including Health and Human Services, the El Dorado County Sheriff and Placerville Police Departments, the Public Health Department, and Community Health Center, as well as Snowline Hospice, Barton Hospital, Veteran’s Services and more are working together to coordinate the Navigation Program, a three- phased program to help address the needs of the homeless community. Phase one of the program is to address acute, immediate medical needs including wound care, mental health and addiction, blood sugar and blood pressure management. Phase two is Project Roomkey, where individuals

Creating a culture of partnership with organizations has earned Marshall national recognition for civic leadership in addition to patient outcomes and value of care.

In a related effort, Marshall has established the Multi-Visit Patient (MVP) Program to address utilization of health services in areas such as the Emergency Department (ED). This is a prime example of health services that are misused and over-used by the unsheltered, who tend to use the ED for primary care. Due to the stigma that surrounds homelessness, many are hesitant to visit a primary care provider out of fear of judgement so they wait until they’re very sick and end up in the ED. The goal of the MVP Program is to treat chronic health problems so they are more manageable and reduce some of the burden on the ED.

needs. At the top of the list of needs was the ability for providers to care for patients virtually, allowing for patient appointments from the safety of home. Thanks to many generous donors, Marshall Foundation for Community Health was able to procure 70 iPads for physician use, making telehealth for Marshall possible in a few short weeks. “Since the deployment of the iPads, telehealth has been widely accepted and embraced by patients. Virtual visits are vital to ensuring that our patients, especially those with chronic or acute issues, are treated in a timely manner. ICU patients are also able to communicate with their families, alleviating their anxiety and isolation,”

Screening by the Numbers
It’s easy these days to let routine healthcare lapse. Especially during these times, it’s important to keep up with your regular health screenings. Regular screenings lead to earlier detection of medical issues including certain types of cancer as well as chronic health issues like diabetes and heart conditions. Earlier detection in turn results in to better outcomes and easier disease management. Many screenings are age specific. Here’s a guide to help you determine what annual screenings you’ll need and when.
Beginning at Ages 18 – 39
• For women – a pap smear every three years starting at age 21
Beginning at Ages 40 – 64 • Fasting blood sugar levels – to check for signs of diabetes if you’re obese • Colonoscopy – at age 50 and every three to ten years thereafter • For women – mammograms every other year starting at age 40 - 50 (earlier if you have a family history of breast cancer) • For men – annual prostate screenings beginning at age 50 (earlier if you are high-risk) if you choose
Age 65 and older • Bone density study – starting at age 65 every two to five years, or age 60 if you’re high-risk • Pneumococcal and shingles vaccines • Height and weight – shrinking could be a sign of osteoporosis Many of these and other screenings can be done during your annual physical with a Marshall doctor. During this visit, your doctor will also likely order routine blood work to screen for other common issues like vitamin deficiencies and cancer markers. Q: WHAT ROLE DOES MENTAL HEALTH PLAY WHEN STARTING A WEIGHT LOSS JOURNEY? A: It’s not breaking news that many people start and stop weight loss programs or exercise routines over and over throughout their lifetime. Does this mean they were never successful or couldn’t stay focused? Of course not. Did life get in the way and health goals get put on the back burner? Most likely. Perhaps it’s time for a mental health check. I teach our patients, “What gets measured, gets managed.” In other words, write down the negative obstacles keeping you from your goals, how you can overcome them, and what positive changes you can make to start your journey right and be successful. Consider things such as setting reasonable goals, time-management, and not being so hard on yourself. Continue measuring your successes and setbacks throughout your journey. andy Peters came home late one night a year or so ago. Lisa, his wife and business partner, somewhat dismayed, asked him where he had been. Before he could volunteer his excuse, Lisa fired back at him. “You’ve been driving up and down Vernon Street looking for a vacancy again, haven’t you?” And so it is for the restauranteurs/caterers—not out carousing after hours but searching for the perfect location to open their newest endeavor: 105 Noshery. And find it they did, not so coincidentally, at 105 Vernon Street where the Peters and their crew are bringing locally sourced food and beverages—alongside a wealth of expertise—to the region. I arrived a few minutes early for my 7 p.m. reservation and was seated at the bar. Indoor

seating was at a premium, especially during opening week and with occupancy limitations, but I’m always comfortable sitting at a bar— especially one that has over a dozen local beers on tap, an impressive cocktail menu, and a well-thought-out wine list. To start, I ordered the cast iron skillet-seared garlic shrimp, featuring crumb-coated prawns bathed in a buttery broth. Dunking the accompanying toast in said sauce made the appetizer even more enjoyable. Next up was the 16-ounce bone-in New York steak. Cooked perfectly medium-rare—as were all of the proteins I sampled in three visits— alongside Brussels sprouts sautéed with bourbon and bacon, the entrée could’ve benefited from a bit of pan sauce, but the lardons combined with the sprouts still made the dish a standout. Save the best for last? Yes, definitely. The aptly named “Cedar Plank Salmon, with the Fear of Becoming Boring, the Flavors Change Weekly” was literally a mouthful—one of the best (and biggest) salmon dishes I’ve had in recent memory. On this visit, the “weekly flavor” was lemon, butter, and garlic. The chef, once again, executed the medium-rare preparation to perfection. Paired with sautéed garlic spinach (I added a squeeze of lemon) made for a winning combination. 105 Noshery is a welcome addition to the Vernon Street dining scene, which has gained momentum over the past few years, and is a great place to gather with friends morning (they also serve breakfast!), noon, or night. Chocolate Fish Coffee Roasters // Strange name for a coffee shop, right, Chocolate Fish? Here’s the explanation. “A chocolate fish is a fish-shaped confection indigenous to New Zealand given out as a ‘thank you’ for a good deed.” The upshot of the unique name is that the owners, Edie and Andy Baker—who relocated here from New Zealand over a decade ago—brought the Kiwi culture with them in their brewing style and philosophy. After scanning the beverage menu—which includes down under favorites like a perfectly poured flat white and “fluffy”—I ordered my standard Mocha (coffee purists, please don’t hate), which was to my standards—not too sweet with full java flavor. And, of particular importance to many, it had the requisite presentation of latte art. Since I arrived during weekend lunch hour, the pastry case had already been pillaged, so I moved my attention to the menu board and had a eureka moment: Smashed Avo Toast with extra virgin olive oil, lemon, salt and pepper, and “the works” (thinly sliced radish, micro basil, and dukkah (an Egyptian spice blend of nuts, cumin, and coriander). I also ordered the locally roasted Almond Butter Toast with extra virgin olive oil, flaked salt, and “the works” (banana, honey from The Bee Box, and ground cocoa nibs). All of their toasts—designed specifically for Chocolate Fish by chef Kevin O’Connor—start with EVOO and are prepared in-house from Sacramento-sourced Check out ingredients—including fresh-made sourdough from Bella Bru Café and seasonal preserves from The Chocolate Fish's Good Stuff (on their Jam Toast). In another bowl, mix butter and sugar at medium speed until creamy. Add yolks and mix until incorporated. Add dry mix and milk. Cover with plastic and chill in refrigerator for 20-30 minutes. Preheat oven to 325 and cover a sheet pan with parchment paper. With a rolling pin, roll the dough on a lightly floured surface until you have a thickness of 1/8”. Cut 2” circles or any desired shape, and place on sheet pan. Bake for about 10 minutes. Note: Cookies should be pale in color and not golden brown. Cool completely. Spread about two teaspoons of dulce de leche on the flat part of the cookie. Sandwich together with another cookie, roll the sides on the shredded coconut or chopped nuts, and dust with powdered sugar.

In a bowl with a stand mixer (or using a hand mixer), combine the butter and sugar on medium-high until light and fluffy, 2-3 minutes. Add the egg and peppermint extract. Beat until combined, about 1 minute, scraping the side of the bowl as needed. With the mixer on low, slowly add the flour mixture and stir until just incorporated. Stir in the chocolate chips. Scoop a heaping tablespoon of dough and roll into a ball. Slightly flatten it and press the top into crushed candy cane bits. Repeat with remaining dough and place on prepared cookie sheet 1 ½ inches apart. Bake 10-12 minutes, until edges are firm and the center is still soft. Let cool on the cookie sheets 5-10 minutes, then transfer to a wire rack to cool completely. *The flour can be substituted with a glutenfree flour mix, almond flour, or coconut flour.

3/4 cup butter 1 cup sugar 1 egg ¼ cup molasses 2 cups flour 2 tsp. baking soda 1 tsp. ground cinnamon ½ tsp. ground cloves Pinch of nutmeg ½ tsp. salt ½ tsp. ginger
Cream the butter and sugar together. Add egg and molasses. Combine the dry ingredients and blend into the molasses mixture. Preheat oven to 350. Portion dough into desired cookie size and bake for 1012 minutes.
Cream together the butter, sugar, salt, vanilla, and peppermint extract for about 5 minutes on low and 10 minutes on medium, scraping the sides of the bowl as needed. Add the egg and mix for about 1 minute on low and 3 minutes on medium. In a separate bowl, sift togehter the flour, cornstarch, baking soda, and baking powder. Add the dry ingredients to the butter mixture and stir together, being careful not to overmix. Separate your dough into 2 equal parts, put them in separate mixing bowls, and stir in the red food coloring to one of the bowls. Keep both bowls covered with a bread towel or baking paper. Preheat oven to 375. Roll the dough into portioned balls, about the size of a walnut or tablespoon, depending on how big you want your candy canes to be. Next, roll them into ropes, keeping the dough you aren’t using covered. Place 1 red and 1 white rope side by side; press together lightly and twist until it has a candy cane shape. Place on a baking sheet 2 inches apart. Bake on the middle rack for about 12 minutes, turning the tray halfway through. Let cookies cool on baking sheet. *If you don’t like the flavor of peppermint, vanilla or almond extract works, too.

Cream together the butter and sugar on mediumhigh until light and fluffy, about 2-3 minutes. Add eggs and chai concentrate and mix on medium-high until emulsified with the butter and sugar. Next, add flour, baking powder, and sea salt. Using a small scoop or spoon, make 1-inch balls and place on a parchment-lined baking sheet. When all the dough has been scooped, place trays in the refrigerator and chill for about 30 minutes. Preheat oven to 325. Smash cookies with palm evenly and bake for 12 minutes. Cool to room temperature. Make frosting. Cream together the cream cheese and butter. With the mixer on low, add the powdered sugar. Gently pour in the chai concentrate and mix on low until fully incorporated. Using a pastry bag fitted with a large star pastry tip, pipe the frosting onto each cookie. Sprinkle with a little cinnamon and sugar. Keep finished cookies refrigerated.
