Malnutrition Awareness Week Highlights TMF As malnutrition awareness week draws attention to an area that most people would identify with poorer countries, the incidence of malnutrition in the elderly in New Zealand is of concern. Nutrition deserves special attention as people reach older age and is essential for good health. Healthy ageing is associated with physiological, cognitive, social and lifestyle changes that influence dietary intakes and nutritional status. One of the best options for treating malnutrition is to enhance normal eating and drinking along the lines of a food first approach that encourages eating frequent, small, high energy protein meals and snacks. We focus this month on malnutrition as a cause and consequence of ill health. The ability

to eat and eat well and the incidence and impact of malnutrition in older people is of particular concern inside aged care facilities. The consequences of malnutrition are many, physiological, biochemical and psychological. They include reduced immunity, delayed wound healing and decreased muscle strength, which in turn have detrimental effects on recovery and movement. The mental and social impact of malnutrition is also significant in this vulnerable group with changes in mood, attitude, self-esteem and reduced socialisation. Texture modified foods have changed a lot in recent years, with improvements in nutrition, texture and visual appeal. Along with aids to eating that give seniors more assistance with independent eating, this area of care is improving

Matt Orr, Ryman Food Services Training and Development Chef, has been working on some recipes for the new menu that launched late September. “We are really excited about partnering with Food Nation – their products both taste good and deliver on the nutrition requirements which is important for our residents. “They give us more options for vegetarian, vegan and dairy-free dishes, but we think residents who are looking to eat less meat will like them too. “Food Nation’s products such as Magic Minces, are versatile and packed with flavour. We are currently developing ideas in our test kitchen for our spring menu,” said Orr. Miranda Burdon, the team at Food Nation, including her sister and fellow director Josie Lambert, use a lot of fresh New Zealand ingredients and really celebrate the plant ingredients as the stars of their products. This transparency and the fact they are in well-known formats like minces, patties and balls means that consumers will find them easy to use in meals they are already cooking. Nutrition and balanced food choices are particularly important when it comes to the aged care sector. The healthy food packages, based on customer requests, were designed to be well balanced with relatively low fat, high fibre, low sugar and include protein and carbohydrate

levels that are both filling and support a healthy balanced diet. "We’re proud Ryman Healthcare has started with one of our company’s first products, Magic Mince. The name came about to reflect the fact they can be cooked like mince and used like mince but are made entirely of plants - it has to be magic," said Burdon. There are two types of Magic Mince so far, mushroom and hemp and Mexican corn and capsicum varieties. These and other products are produced in the Auckland commercial kitchen at St John's from ingredients generally sourced from around New Zealand. “You can see the ingredients in the products when you cook them with visible pieces of corn, capsicum, onions, leeks and mushrooms. Delivering on this expectation of showcasing the ingredients as well as supporting dietary requirements are all key to providing a great-tasting food solution that actually could be used as a meat alternative,” said Burdon. Burdon is quick to point out though, these products are a celebration of plants, not fake meat and are about providing people with a choice. “Our products are for omnivores and carnivores, vegans, vegetarians and flexitarians – we think of them as ‘Food for All’.” Ryman keeps investing in its food delivery options and has recently recruited three regional ‘Lead Chefs’ to ensure food and menu changes flow through as smoothly as possible into Ryman kitchens.

LEGALISING THE DEVIL'S LETTUCE

With the General Election already here, an independent survey of 1,300 Kiwis shows the referendum on legalising the personal use of cannabis is on a knife-edge. When asked to make a choice between supporting or opposing the Cannabis Legalisation and Control Bill, 49.5 percent of all respondents said they were in favour, 49.5 percent were against, and one percent gave no response. Commissioned by New Zealand’s largest medicinal cannabis company, Helius Therapeutics, the latest Horizon Research survey marks the seventh in a series of comprehensive surveys tracking nationwide opinion on cannabis law reform. “For nearly two years we’ve tracked public opinion, and this is an incredible result. It’s increasingly clear that it will come down to voter registration and election turnout, particularly if younger adults lift their intention to vote,” says Paul Manning, Chief Executive of Helius Therapeutics. For the third time running, survey respondents were forced to take a position to better emulate the stark yes/no choice people will have when they vote. The referendum options and the information provided to respondents were the Electoral Commissions. When respondents were initially given an ‘I’m not sure’ option, 12

percent chose it, 44 percent supported the bill and 41 percent opposed it, with males more strongly supportive. This shows that when later forced to take a yes/no position, of those unsure, slightly more opted for no. Survey respondents were also asked if they were both registered to vote and 100 percent likely to vote. From those that were, 48.4 percent supported the bill and 50.8 percent were against, meaning committed voters at the time of the poll would have defeated the bill by a slender majority of around 67,900 votes. For those who are either not registered or less than 100 percent likely to vote, 53 percent supported the bill, with 45 percent against. “This could be the closest vote since 1919 when alcohol prohibition was defeated by just 10,362 votes. As this survey shows, cannabis is already widely accessible, and so next month’s decision is fundamentally about who we want to control it: Government or the gangs. “Wider regulation of cannabis isn’t perfect, but I believe it offers some improvements on the status quo. A regulated environment for cannabis would see the development of locally-owned businesses, delivering jobs and tax revenue for healthcare services – a welcomed addition for New Zealand’s post-COVID economy.”

artin’s day to day goes from book talks to the box office, charity to cheerleading. Nothing is out of her depth. In her role, she supports residents in the six villages with social and entertainment activities – the ‘Manager of Fun’ title is quite literal in this sense. She designs and executes at least one big regional event a year, and recently, Martin taught and trained the Papamoa Beach Village Pom Poms into a new cheerleading routine. Along with this, Martin gives acting lessons, books entertainment, talks, organises fashion shows, basically everything and anything you have ever dreamt of in terms of entertainment. No one is left feeling bored on her watch. When running larger events like fashion shows, she finds a way to rope in her loving family to help her out with the logistics, lighting and much more. Her favourite part of her role is the fact she gets to write, direct and act. Anything she can do that’s related to putting on a show will get her buzzing. “I love working with residents on the stage, and it’s so rewarding to see them finding a new passion in theatre craft,” said Martin. Martin has been working in this role for six years, and one of her most recent highlights is that she completed a post lockdown comedy show called “O, Corona” – it was just what

everyone needed after lockdown. Undoubtedly, Martin loves to write and produce plays and, it runs in her family. Her family has been involved in theatre their whole lives. With her daughter being an awardwinning singer and theatre director and two of her sons are performers. One of her son's was the youngest person ever to run tech for a theatre stage show in Tauranga, he was just nine at the time. “My background and passion are events of all kinds; I previously ran my own company specialising in Mind Body and Spirit events. I’ve toured all over New Zealand with a show called ‘Mediums Live’ and worked with famous New Zealand medium, Kelvin Cruikshank.” One of the most rewarding experiences Martin has had with Metlifecare is when she was photographing residents for a calendar to raise money for cancer charities for men. It was titled ‘Calendar Blokes’ and featured the more ‘daring’ men in the village doing activities ‘naked’. “It was so much fun to do, and we raised more than $4,000 for charity.”

AGED CARE IN AUSTRALIA STILL UNDER FIRE To date more than 4000+ aged care residents and staff have been infected with the coronavirus in Australia and with 657+ deaths, aged care facilities in Australia, and worldwide are facing some shocking home truths. The Australian Quality and Safety Commission has visited just 30 of the 220 homes with Coronavirus outbreaks, yet tough action has only been taken against 20 aged care homes with outbreaks by placing sanctions on most of them for a period of six months. Those in the industry in Australia however question the transparency of the decision-making process. The correlation between the number of residents and staff that have been infected by the virus and the data that has been used to issue notices is blurred at best. It has been reported that the regulator said it went on site to just six of the 20 facilities to issue sanctions after information from federal and state departments of health and first responders. Each day, since the start of the COVID-19 pandemic, Victorian authorities have published the worst of the clusters. Nine of the

ten facilities named have been sanctioned including homes at St Basils and Epping Gardens. Drawing heavy criticism however, is a home from aged care provider Bluecross at Ruckers Hill, which recorded 131 cases and 12 deaths yet was not sanctioned. The regulators assessor had determined that the facility was taking the necessary action to remediate and there was no immediate risk to residents. Bluecross is one of the Australia’s biggest aged care providers, with 23 residential care facilities. Ten of its facilities have had coronavirus outbreaks. More than 280 residents and staff have been infected with the virus and 45 residents have died. Even though Bluecross had a large number of infections and deaths, Janet Anderson, Aged Care Quality and Safety Commissioner, referred to the fact that the commission had issued Bluecross with a notice regarding failings of standards on September 5. Anderson said she was “giving them an opportunity to respond” and “an undertaking to remedy” the situation. On the commission’s website all

but one of the Bluecross facilities have been given scores of 100 percent for quality and safety over the past two years. While no one foresaw the COVID-19 pandemic nor did anyone worldwide take pre-emptive action against the possibility of a virus sweeping through residential care facilities and the threat to life

that it would be. While care homes across the world scramble to find ways to shore up PPE stocks, strengthen infection control protocols, improve staffing and training, innovation and new structures of care to ensure the safety and wellbeing of the most vulnerable are yet to emerge.

CODEINE MOVE WELCOMED

THUMBS UP FROM RESIDENT FOCUS GROUP It was a thumbs up for the delicious carrot cake and bite-sized ginger slice, and a thumbs down for the crumbly Vietnamese rolls. The café focus group at Bupa Parkstone Retirement Village in Ilam, Christchurch provided their feedback on the potential cabinet food for a new village café experience. The group enjoyed tasting both savoury and sweet items including muffins, macarons and wraps. Harvey Bray, who has been a resident at Bupa Parkstone Retirement Village for five years, says he’s looking for food that’s easy to eat, wholesome, and nutritious. “It’s really important that we have our say and are part of this after all this café will be at our home and we are the village. A lot of people I talk to about the possibility of retirement village

living are frightened they’ll lose their independence and that’s just not true, we even have a say on our own café,” said Bray. Other feedback from the group was around making sure portion sizes aren’t too large, choices for their grandchildren when they come to visit, such as the lollie cake they tried. There were also comments on ensuring the café will have gluten-free, and vegan options too. Jan Summerell, Bupa National Hospitality Services Manager says the village residents are really excited about the opportunity to give feedback on a new café experience. “We want to provide a café that not only our residents want to visit, but also their families and the local Ilam community. coffee, a Christchurch based roastery.” The focus group have named the café as Café Corfe (pronounced corfay), and it’s scheduled to open in mid-October, three days a week – Friday to Sunday from 10 am till 4 pm.

PHARMACIES UNDER PRESSURE & OUT OF POCKET Guild calls on the government to give pharmacies access to promised financial support. The Pharmacy Guild of New Zealand (the Guild) has significant concerns over community pharmacy’s ability to simply and quickly access the government’s $18 million financial support package. The Guild is calling for urgent government action to support under-pressure pharmacies around New Zealand struggling to cope with the high cost of responding to Covid-19, by directly allocating the already announced $18 million financial support package across all community pharmacies. “Ministers in June set aside urgent funding for the sector

but we understand so far no money has gone to cash-strapped pharmacies because the process to access funding developed by District Health Boards is far too complicated. This is completely at odds with financial support measures the government has introduced for others in the health sector, who all received their money automatically and quickly,” Guild Chief Executive Andrew Gaudin says. “The government required pharmacies to stay open to provide an essential service to communities throughout the lockdown earlier this year and pharmacies were hit with extra costs as a result. These costs should be met given

the important role performed. “We believe Ministers fully intended for and want the money made available to pharmacies really struggling with increased costs due to Covid-19. “We have grave concerns that while health officials keep giving Ministers poor and uncosted advice, pharmacies will have to cut staff, cut hours and cut services offered to their communities, with some pharmacies ultimately forced to close. Unfortunately, we have already started to see this happen. “Pharmacies are under pressure, out of pocket and disillusioned, and left wondering what they have to do to get the support they desperately need” said Gaudin.

Medsafe, which made the decision to reschedule all codeine and codeinecontaining medicines last year, has confirmed that all codeine and codeinecontaining medicines will become prescription only from 5 November 2020, a decision that has been welcomed by the aged care sector. AFT Pharmaceuticals managing director Dr Hartley Atkinson said: “We are pleased Medsafe has set a firm date for the rescheduling of codeine. We expect the move, which is aimed at reducing codeine overdoses and poisonings, will increase demand for alternative pain relief treatments including, AFT’s patented paracetamol and ibuprofen combination treatment Maxigesic.” “In the 12 months after Australia rescheduled codeine-containing medicines in 2018, sales of Maxigesic tablets increased by more than 50 percent. While it is unclear whether the New Zealand market will respond in a similar way, we expect the move to assist the growth of Maxigesic in New Zealand in the current financial year.”

Innovation In Breakfast Offerings

Breakfast is one of those meals that quickly reflects the ebb and flow of trends. Flavour profiles, superfood additives, changes with portion control packaging, all have an impact on breakfast offerings.

ever have these changes been more apparent than in COVID times. With isolation, an everpresent worry for those caring for vulnerable residents during lockdowns, having breakfast together rather than in-room became a treat that could be looked forward to.

So, what is trending? Researchers are talking comfort and control while aged care facilities are reeling from the changes that COVID-19 pandemic

has brought to the sector; this is something that facilities should look at. As residents reel from being isolated from their families, they look to control at least what they are eating. For providers, it is a case of making sure that residents maintain their nutrition for health and energy while still providing for residents for whom food memories are significant. Health and wellness are always top of mind but expect a big spike as we enter 2021 as suppliers endeavour to provide innovation in the aged care space. Probiotics, superfoods, microbiome, active ageing, nootropics and protein alternatives will all feature through 2021. Plant proteins are also coming through as meat alternatives and are surprisingly tasty and nourishing. High plant protein diets are linked to many health benefits, with studies suggesting vegetarians tend to have lower cholesterol and lower blood pressure levels. Cannabis was trending in 2019 and has continued this year. Hemp products are still entering the market, from hemp oils to flour, baked items to granolas to crackers - hemp has inspired supplier innovation this year. A good source of fibre and protein, hemp can be added to granolas, as an oil it can be drizzled on oats – hemp has been seen in a wide variety of products so far and expect more in 2021. Honey is booming – with many new players entering the market. Given its health properties, it is not unexpected, and flavour infusions are becoming more common. Single-use packaging is becoming more common as suppliers address the sector's concerns over infection controls. Single-serve portion control cereals, fruits, butters, jams and spreads means less handling with the resident able to self-serve. Innovation in the packaging area is something that the sector has been in great need of, from lips on bowls to facilitate the transfer of food from bowl to mouth, push plates where those with tremor can push food against an edge, through to sustainable straw options. All are providing solutions across the variety of resident needs.

New Bottle For Anchor Anchor is set to launch New Zealand's first plant-based milk bottle, which is 100 percent kerbside recyclable and aligns with Fonterra's commitment to have all packaging reusable, recyclable or compostable by 2025.

The sugarcane is natural, renewable and sustainably sourced, and is an alternative to bottles made from non-renewable sources like fossil fuels. Also, sugarcane captures CO2 from the atmosphere as it grows, resulting in a bottle that also has a low carbon footprint. Finding a lab that handles this virus is difficult, as it requires a high-security level and these labs are both rare and extremely busy these days. As long as this virus is considered dangerous, it will not be available for commercial testing of specific product applications. But with these tests, it will be possible to compare commercial tests to the results on the SARS-CoV-2 virus. “On one hand we were convinced even before the tests were back. But now we have it on paper, and that has an enormous significance for our partners and end-users. People all over the world discard or wash things every day out of the concern that the virus might be present in the textile. This takes a completely unnecessary toll on the environment”, explained Ulrika Björk, CEO of Polygiene. The purpose of treating textiles with ViralOff is not to protect the wearer from infection or make any other health claim. It is a treatment to protect the garment. TTAF GREAT NEWS FOR AGED CARE SECTOR The Government recently announced a $320m Targeted Training and Apprenticeship Fund (TTAF), as part of a training package to support training in the health and wellbeing sector.

T

his development is very good news for the aged care sector, and Careerforce Chief Executive, Jane Wenman, welcomes the announcement. “The health and wellbeing sectors continue to face workforce shortage challenges, brought about by an ageing and increasingly diverse population, and an associated ageing workforce in many areas. This is being exacerbated by COVID-19 related immigration restrictions. Any Government funding that will support new people into the sector, and remove any barriers to workforce development is welcomed.” The fund effectively removes enrolment costs for eligible programmes in targeted areas for learners and employers, through to December 2022. The fund is targeted towards industry skill needs where demand from employers for these skills will continue to be strong, or is expected to grow, during New Zealand’s recovery period from the impacts of COVID-19. The aged care sector has been included as a targeted area, with “care for elderly” specifically highlighted.

Malnutrition Awareness Week

This month is about raising awareness about the risk of malnutrition amongst older adults. Information about the importance of encouraging seniors to eat enough and to ensure that what they are eating is nutritious.

M

alnutrition develops when a person doesn't consume enough calories, protein, vitamins, minerals and/or other nutrients. The lack of which can have a significant impact on an older person's overall health. A cross-sectional study, conducted in the Waitemata District Health Board region between July 2014 and September 2015, (involving 234 participants over the age of 65 years, with an average age of 84 years) found close to half (46.6

percent) of the participants were identified as at risk of malnutrition and just over a quarter (26.9 percent) were malnourished. The study identified issues with swallowing, low body mass index (BMI), low muscle strength and poor cognition. Conditions which commonly increase a person's risk of malnutrition. Milk is an excellent food for improving both protein and calories. Frequently it is the basis of advice for enhancing protein and calorie intake for people with a poor appetite weight loss and high calorie needs. The carbohydrate content of milk might appear to be predominantly 'simple', but as lactose milk, it has a low GI together with milk's protein content. This results in a slow rise in blood sugar and one which lasts for longer. It is still the effect if other sugars, e.g. fruit sugar, table sugar are added into milk-based foods such as smoothies, custard, and yoghurts, for example. Dietitians recommend older people at risk of malnutrition or who are malnourished making their porridge with milk, eating snacks between meals, eating ice cream or custard with their canned fruit, drinking a latte or hot chocolate. This approach covers the 'food first' approach, which Pharmac requires before people are eligible for nutrition supplements.

Plant-based eating has become a mainstream movement. Globally retail sales of plant-based foods have experienced strong growth. Meat and dairy continue to drive plant-based sales. Leading drivers of plant-based sales continue to be meat and dairy alternatives, including milk, cheese, yoghurts and ice cream. Sales of meat and dairy alternatives continue to be the leading drivers in plant-based sales, including new brands in the milk, cheese, yoghurt and ice cream categories experiencing the most rapid growth. Expect to see more movement in the spreads, dips and sauces as artisanal producers focus on NPD in this category.
