Welcome to the July Edition of the Syneron – Candela Industry Newsletter Welcome once again to the Syneron – Candela e-newsletter, this month is our fourth edition. Syneron – Candela team members Martyn Roe, Adrian Myrick and Gary Barker (Husband of Michaela Barker UK General Manager) in cooperation with The Consulting Room™ Team Ron Myers, Danielle Lowe, Dan Huxley and Danny Large will be taking part in a joint charity bike ride. The 140 mile Coast to Coast (C2C) cycling challenge on 31st August & 1st September 2012 to raise money for Chester Childbirth Appeal. The C2C 2012 takes two days, riding through the Northern Lakes and over the Pennines. From the start in Whitehaven on the East Coast, the team will cycle over Whinlatter Pass to Keswick then on to Penrith and then overnight in Alston at the 77 mile halfway point. Bright and early the next day they will negotiate four big hill climbs as they travel through Stanhope, Consett and on to Newcastle and Tynemouth where they will finish by dipping the front wheels of their bikes in the sea – and no doubt themselves! Team member Dan Huxley chose the Chester Childbirth Trust as their nominated charity because: “Last year my baby daughter became ill. After taking her into hospital with a temperature of over 42 degrees she was diagnosed with pneumonia. She couldn't breathe without oxygen, wouldn't eat anything and was hardly moving. After 4 days of drips, oxygen masks and a mother that never left her side she started to get better. I will never forget how helpful the nurses, doctors and staff were and without the Chester Childbirth Appeal Molly's stay would have been a lot worse.” The Chester Childbirth Appeal has supported the Maternity Unit at the Countess of Chester NHS Hospital since its inception in 1991, providing facilities and amenities over and above those afforded by the NHS. Only with the help of fund-raisers, volunteers and the great generosity of our supporters of the Childbirth Appeal have we been able to raise money to finance these improvements. We are assured that training schedules are running to plan and all are gearing up to get fit for the challenge, with the ultimate obstacle – how not to get a sore bottom being hotly debated! Please give generously and help our team to raise lots of money and hopefully reach our target.

Journal of Aesthetic Nursing, Clinical Excellence in Medical Aesthetics On 18th July The Journal of Aesthetic Nursing held their inaugural conference ‘Clinical Excellence in Medical Aesthetics’ at America Square Conference Centre, London. Delegates celebrated the successful launch of the new monthly title Journal of Aesthetic Nursing which provided an exciting platform for nurses to enhance their skills and engaged in lively discussion and debate with their peers. Highlights of the conference included an exciting keynote address on the future of aesthetic nursing and regulation, a clinical overview of sclerotherapy from highly experienced aesthetic nurse Emma Davies, two fiery and interactive debates on raising standards in practice, and a live vote on whether a mandatory qualification is needed in aesthetic medicine. The British Sychronised Swimming Team Sponsored by Motif™ Countdown to the Olympics Syneron - Candela’s new Motif laser hair removal system is helping the British Synchronised Swimming athletes be a success in the pool. Keeping the team silky smooth, the girls have awarded Motif a gold standard in hair removal. Dose Response of Combination Photo Rejuvenation Using Intense Pulsed Light–Activated Photodynamic Therapy and Radiofrequency Energy Jeffrey A. Hall, MD; Pamela J. Keller, RN; Gregory S. Keller, MD Background and Objective: To report the results of a dose-response study using a novel photorejuvenation regimen consisting of intense pulsed light–activated photodynamic therapy and Radiofrequency energy in patients with Fitzpatrick skin types I through III. Methods: A combination intense pulsed light and radiofrequency device (Syneron Medical Ltd, Yokneam, Israel) was used in combination with the topical photosensitizer aminolevulinic acid (ALA) (Levulan Kerastick; DUSA Pharmaceuticals Inc, Wilmington, Mass). Duplicate titration trials were performed on the dorsal forearm skin of 3 patients with Fitzpatrick skin types I through III. Multiple treatments, with varying times of application of ALA (30 minutes and 1, 2, and 3 hours) and intense pulsed light fluence (24-30 J/cm2), were completed to determine a minimal erythema dose and a maximal tolerated dose based on epidermal reaction (Erythema and crusting) and patient discomfort. Radiofrequency energy levels were constant in all treatment groups. Results: The ALA application time of 1 to 2 hours for all skin types and fluence levels of 26 to 28 and 24 to 26 J/cm2 for Fitzpatrick skin types I/II and III, respectively, were determined to be the minimal erythema dose. The ALA application times of 2 to 3 hours and fluence levels of 28 to 30 and 26 to 28 J/cm2 for skin types I/II and III, respectively, were determined to be the maximal tolerated dose and resulted in severe erythema and crusting. The presence of severe erythema and discomfort precluded longer ALA exposure and higher fluence levels. No epithelial breakdown was observed at any treatment levels. Conclusion: The dose-response results defining the minimal erythema dose and the maximal tolerated dose of intense pulsed light–activated photodynamic therapy and radiofrequency energy have led to the development of 2 photorejuvenation protocols based on skin type and severity of photodamage. Post-Pregnancy Body Contouring Using a Combined Radiofrequency, Infrared Light and Tissue Manipulation Device Marc L. Winter, M.D. Background: Non-invasive body contouring is an increasingly popular aesthetic application. Previous data support the efficacy of combined radiofrequency, infrared and skin manipulation for cellulite treatment. Objective: To evaluate the performance of a high-power device (50W as opposed to 25W) combining these energies for reshaping and improvement of skin texture/laxity in postpartum women. Methods: Twenty women received five weekly treatments to the abdomen, buttocks and thighs with the VelaShape™ system. We followed up each patient’s weight and nutritional habits. Outcome was assessed using reproducible circumference measurements, digital photography, the physician’s scores of cellulite and improvement as well as patient satisfaction. Safety was evaluated by recording subjects’ comfort and tolerance. Results: The overall mean circumferences reduction was 5.4󲐀󲐀0.7cm (p󲐀󲐀0.001). Significant (p󲐀󲐀0.02) improvement in skin laxity and tightening was noted by both the physician and patients. Treatments were well tolerated with no major safety concerns (one purpura, one mild burn). Conclusion: The enhanced capabilities of the evaluated system enabled significant results in fewer and shorter sessions without compromising patients’ safety or comfort. These data suggest that postpartum reshaping via circumferential reduction and skin laxity improvement can be effectively and safely achieved using a high-energy combination of radiofrequency, infrared and mechanical manipulation.

elure’s Natural Formulation Provides Effective Topical Lightening Solution for All Skin Types By Lori Brightman, M.D. Patients seeking skin lightening treatment are often frustrated by skin irritation resulting from topical medicines or the need for multiple, uncomfortable chemical peel or laser procedures. These adverse effects have patients seeking efficacious options without the discomfort and risks. As a board certified dermatologist with The Laser &Skin Surgery Centre of New York (New York, N.Y.), I find many of my patients have experienced irritation or the need for long-term use with hydroquinone- based products. Which is why my clinic now offers elure™ Advanced Skin Lightening Technology as an alternate treatment. The elure topical product line is based on the proprietary enzyme Melanozyme™ – Syneron – Candela’s patented version of the naturally occurring enzyme lignin peroxidase (LIP). The product works by breaking down the durable melanin compound within the skin. While other skin lightening treatments work to block melanin biosynthesis, preventing its formation, elure’s active ingredient reduces melanin in the upper layers of the epidermis. I find elure most useful for patients who have diffuse photodamage. It gives a more even tone and a brighter appearance to the skin; I also recommend elure for use with melasma patients. Many have responded to elure alone. For others, I use elure in conjunction with other treatments, targeting the melanin in two different ways. For example, if a patient can tolerate it, I may prescribe hydroquinone in the evening and elure in the morning to reduce existing pigment. Another attractive feature of elure Advanced Skin Lightening products is the fast-acting results. I find that several of my patients notice quicker reduction of pigment with elure in comparison to other topicals. Also, with other, medicated topicals, many patients need to take breaks in between treatments; adding to the amount of time before positive results are observed. With easy, home-based application, the elure treatment protocol begins with the Advanced Facial Wash, followed by application of the Melanozyme from the patient’s choice of either the Advanced Lightening Lotion or Advanced Lightening Night Cream. Lastly, the Activator is applied to initiate the skin lightening process. After one hour, the product can either be removed or allowed to remain on the skin without fear of irritation. On average, when applied twice a day, results are seen within one month. When treatment is discontinued, results are generally sustained for three weeks.

I use elure on all skin types including Fitzpatrick skin types IIIV and have not encountered, a difference in rate of response or percentage of patients with different skin types responding. Furthermore, I have not experienced any episodes of irritation. Irritation and, although rare, ochronosis, are issues associated with extended hydroquinone use. I have actually seen two patients referred to me after very long-term use of hydroquinone with the ochronosis diagnosis. It is sad to see how this affects the patient’s self esteem. They do not want to show their face. I like the fact that I do not have such a risk with elure. The majority of my patients have been quite satisfied with their results. Particularly those who’ve had no response to other treatments or who found other skin lighteners too irritating. They are happy to see improvement without skin irritation issues.

After graduating cum laude from Boston College, Dr. Brightman received her masters in medical science and medical degree from Boston University Medical School where she was elected to Alpha Omega Alpha, the national medical honors society. She was also the recipient of the Bertha Curtis Humanitarian Award. Dr. Brightman completed her residency training at Boston University /Tufts New England Medical Center combined program. Dr. Brightman continued her training in Skin Oncology at Boston University as well as Mohs micrographic surgery and procedural dermatology at the Laser & Skin Surgery Center of New York. Dr. Brightman regularly lectures nationally and publishes widely about advances in cutaneous oncology, laser surgery and cosmetic dermatology. Her current research involves the use of laser and light devices in the treatment of medical and cosmetic conditions. Her service to the community includes lectures and skin cancer screenings for many community based programs as well as major corporations. Dr. Brightman is a member of the American Academy of Dermatology, American Society for Lasers in Medicine & Surgery, American Society for Dermatologic Surgery, and the American Medical Association.
