Syneron Candela Newsletter // Edition 18

Welcome

First of all, I hope that your start to 2015 has been a happy and prosperous one. 2014 was an exciting year with new treatments for the VelaShape lll including cellulite and facial protocols, the addition of the fungal nail treatment for our Gentle laser devices and then the launch of the new PicoWay device at CCR in October. To start the New Year we have been working closely with Charlotte and her team at True Grace PR to secure even more great press coverage for the brand. Our first major event of the year was a hosted press event, to get the consumer press excited about our body treatments. This exciting day took place at the prestigious PHI clinic, Harley Street. Already producing positive consumer press stories, this event will help drive clients to your clinic. We revealed the results of our body beautiful survey, in which we asked 1000 British women to reveal exactly what they think and feel about their bodies. Read these results, as well as more about the press event, later on in this issue. We are really pleased for UltraShape® to be recognised as a breakthrough In non-invasive abdominal fat reduction thanks to its painless and effective ultrasound technology, by New-Beauty Magazine. Wishing you all the very best for 2015!

UltraShape® Wins NewBeauty Magazine Beauty Choice Award For Best Body-Contouring Treatment UltraShape Is Recognised As A Breakthrough In Non-Invasive Abdominal Fat Reduction Thanks To Its Painless And Effective Ultrasound Technology. NewBeauty Magazine awarded UltraShape® the “Best BodyContouring Treatment” in its annual 2014 Beauty Choice Awards. As a winner in the coveted “Beauty Game Changers” category, UltraShape is recognised for its innovative ultrasound-based treatment that painlessly and permanently destroys fat in the abdominal area where fat is most resistant to diet and exercise. Featured alongside an elite list of

outstanding aesthetic products, NewBeauty notes that UltraShape was chosen by a panel of thousands of readers, editors and beauty industry experts for its ability to deliver real, noticeable and highly customisable results “without surgery or hours spent in the gym”. “As the number of satisfied UltraShape users continues to grow, we are honoured to earn recognition from NewBeauty Magazine, further validating our leading position in the body shaping space”, said Dr. Shimon Eckhouse, Chairman of the Board of Directors for Syneron. “UltraShape represents the next generation in body-shaping with proven clinical outcomes and a superior safety profile, underscoring Syneron’s ongoing commitment to advancing treatment outcomes through technological innovation”. “What sets UltraShape apart from other body shaping solutions is its fast outcomes with results visible in often as little as 2 weeks post first treatment,” says Amit Meridor, Chief Executive Officer of Syneron. “With UltraShape’s customisable treatments, for the very first time physicians are able to truly sculpt instead of bulk heat or freeze fat. UltraShape is a painless solution for those who cannot get rid of stubborn fat resistant to diet and exercise”. The UltraShape System is the first and only non-invasive body shaping treatment that uses pulsed focused

ultrasound energy to precisely target subcutaneous fat, while keeping the surrounding tissue, vasculature, nerves and muscles intact. Unlike other body shaping technologies that use heat or freezing techniques, UltraShape uses a pure mechanical effect to destroy fat cells without inducing thermal damage. This unique feature of UltraShape technology results in a safe and comfortable treatment experience with no visible signs of treatment and no downtime. The newest fat-blasting treatment on the block, UltraShape, with over 300,000 procedures performed worldwide, was cleared by the United States Food and Drug Administration (FDA) 510(k) for non-invasive reduction of abdominal circumference via fat cell destruction in April of last year. About NewBeauty® NewBeauty® magazine is the definitive authority on all things beauty. What the Press had to say... “What a great event. Top speakers who explained the benefits of the latest treatments brilliantly – and plenty of impressive before and after pictures, too.” Alice Hart-Davis, Beauty Journalist

“Thought the event was amazing! Dead interesting and useful. I’ve just included the stretch marks treatment in my round-up of ‘less-invasive versions of surgery/older nasty treatment’ piece.” Lynne Hyland, Beauty Editor Notebook mag, Sunday Mirror “Tattoo removal is a key issue which I wrote about a few year’s ago for the FT, but would be great to address in the Times Cosmetic Procedures I’m writing. I also found the information on stretch marks and skin laxity of great interest and the survey stats are really helpful.” Beatrice Aiden, Beauty Journalist “The new stretch mark treatment is particularly interesting to me and cellulite always a good one. I have covered off Vbeam a few times but not for scarring. I also like the idea of the treatments being suitable for all skin types.” Trish Leslie, Beauty Journalist: Bella, At Home etc “Great event at PHI Clinic. We understand first hand how much goes into organising an event like this and the ones that Charlotte and her team at TG organise are always well organised and attended - we’ll always continue to attend these as they always offer up valuable information on the cosmetic and beauty industry. “ Carl Frascina, Harley St Journal Edition 18 // syneron candela newsletter // 8

Mechanisms of Action The UltraShape V3 works by emitting acoustic waves of focused ultrasonic energy that converge into a confined focal volume targeting only subcutaneous fat at a controlled depth. Unlike traditional ultrasound technology, UltraShape V3’s energy transmits pulsed ultrasound, allowing control over temperature elevation and enabling fat destruction to occur instantly, selectively and mechanically (non-thermal).

The UltraShape Transducer: The heart of the UltraShape handpiece is a silver coated half dome shaped ceramic filled with medium matched to the human body impedance. When electrical voltage is applied to the silver coating, an electric field is created within the ceramics causing alteration in the ceramic thickness. This in turn creates pressure waves within the medium – these pressure waves are interpreted as sound waves and create the ultrasonic energy. (Figure 1)

(Figure 1) Focused Ultrasound: The rounded shape of the ceramic delivers low energy at the surface but provides a concentrated intensity of energy where the ultrasound waves converge at the focus. This focused energy allows targeting of defined tissues at a controlled depth, while leaving adjacent structures, such as skin, blood vessels, nerves, and muscles, unharmed (Figure 2). Pulsed Mode: The UltraShape transducer delivers the energy in bursts; this unique feature allows the generated heat to dissipate before the next pulse begins. This minimises the temperature rise (0.50 C) within and around the targeted tissues. The energy delivered is transformed into mechanical stresses that remain within the focus and exist only for the duration of energy delivery. This allows the targeting of tissues that are most susceptible to mechanical disruption, while more resistant structures remain intact. (Figure 2) Fat Clearance: The most common concern regarding non-invasive fat reduction is what happens to the destroyed fat cells. In clinical studies that have been conducted by Teitelbaum et al. (2007), the blood laboratory tests concluded that there were no clinically significant changes in lipid profile. During the UltraShape procedure, the membranes of the fat cells are disrupted. The fat cell content, primarily composed of triglycerides, is dispersed into the interstitial fluid among the cells and then transported by the body’s natural processes to the liver. The liver makes no distinction between fat coming from the procedure and fat originating from consumed food. Both are processed via the body’s natural mechanisms, the lymphatic, venous, and immune systems. Fat metabolites are processed in the liver in the same manner as fat originating from digested food.

Peer Reviewed Paper No.1 ABSTRACT: Body Contouring by Non-Invasive Transdermal Focused Ultrasound Moreno-Moraga et al, Lasers Surg Med. 2007 Apr; 39(4): 315-23 BACKGROUND & OBJECTIVES: The risks of currently available invasive procedures in body contouring motivate a need for safer, non-invasive technologies for improving the appearance of body silhouette. A new device has been developed that uses focused therapeutic ultrasound to reduce adipose tissue non-invasively. The aim of this study was to assess the efficacy and safety of a novel non-invasive focused ultrasound system (UltraShape Ltd, Tel Aviv, Israel) in reducing localised fat deposits to improve body contours. METHODS: A prospective study was conducted on 30 healthy patients. All patients underwent three treatments, at 1-month intervals, and were followed for 1 month after the last treatment. Areas treated were the abdomen, inner and outer thighs, flanks, inner knees, and breasts (males only). No other body contouring procedure was used during the study. Efficacy was determined by change in fat thickness, assessed by ultrasound measurements, and by circumference measurements. Weight change

was monitored to assess whether reduction in fat thickness or circumference was dependent on or independent of weight loss. Safety was determined by clinical findings, assays of serum triglycerides, and liver ultrasound evaluation for the presence of steatosis. RESULTS: All patients showed significant reduction in subcutaneous fat thickness within the treated area. The mean reduction in fat thickness after three treatments was 2.28+/-0.80 cm. Circumference was reduced by a mean of 3.95+/-1.99 cm. Weight was unchanged during the treatment and follow-up period. No adverse effects were observed. Before After

Abdomen before treatment and after treatment Before After

Outer thighs before treatment and after treatment CONCLUSION: This study shows the efficacy and safety of focused ultrasound, using the UltraShape contour I, as a non-invasive transdermal method for reducing unwanted fat deposits in the body. Multiple treatments combined with appropriate patient and treatment area selection can produce dramatic improvements in body contour.

Peer Reviewed Paper No.2 ABSTRACT: Safety and efficacy of UltraShape Contour I treatments to improve the appearance of body contours: multiple treatments in shorter intervals.

significant. Circumference reduction on day 112 positively correlated with patients’ subjective satisfaction scores.

Ascher B. Aesthet Surg J. 2010 Mar; 30 (2): 21724. BACKGROUND: The UltraShape Contour I System (CE 0344; UltraShape Ltd., Yoqneam, Israel) is a noninvasive fat reduction and body contouring system currently approved for use outside the United States that utilises focused ultrasound to selectively disrupt adipocytes. OBJECTIVE: To evaluate the clinical safety and efficacy of the Contour I system when the intervals between treatments are shortened. METHODS: Twenty-five healthy Caucasian women were selected from the patient population at two clinics in Paris, France, and received three 30- to 90-minute Contour I treatments in the abdominal region at two-week intervals. Safety parameters evaluated included adverse events, local skin reaction, and pain. Efficacy parameters evaluated included treatment area circumference, body weight, and comparison of before and after photos. Untreated thigh areas served as an internal control. Subjects were followed for 84 days after the last treatment (day 112).

36-year-old female before the first treatment (A, C) and after the third treatment (B, D). 7.5-cm reduction in waist circumference. CONCLUSION: Our data showed that successive Contour I treatments at two-week intervals were safe and tolerable.

RESULTS: No adverse events occurred. The majority of subjects (n = 23; approximately 90%) reported no pain. Mean midline circumference (2 cm below midline) was reduced by 2.47 cm (P < .001) on day 14 after the first Contour I treatment, 3.51 cm (P < .001) on day 56, and 3.58 cm (P < .001) on day 112. Peak midline circumference reduction was 3.12 cm on day 112. Most patients (n = 14; 63%) reported a positive change in body contour. Mean thigh circumference (the control area) was unchanged; the relative change between treated and untreated areas of the abdomen was significantly different at all time points. Circumference and weight reduction were significantly correlated (r = 0.42-0.71) at all time points; mean weight decrease was not statistically
