1: Equipment Installs, Exchanges, Removals & Other Service Requests All Citavo Coffee & House Recipe Beverage Requests for new equipment install(s), additional equipment request(s), equipment removal, shipping of parts, equipment exchange(s), must be submitted electronically on the Sysco Arizona Beverage Equipment Request Form. Please see the sample form contained in this packet. This form needs to be submitted electronically to BOTH Kevin Barko & Ted Pearson. All sections of the Beverage Request Form, with the exception of the Equipment Order Information and the Approval Sections need to be filled out completely. The Beverage Equipment Request Form is for your records, in addition serves as the official work order authorizing S&D Coffee & Tea to complete the requested service on behalf of Sysco Arizona. Once the form has been completed, please submit it directly to BOTH Kevin Barko and Ted Pearson for processing.
2: Requests for equipment installs, exchanges and additional equipment are reviewed by Senior Management for approval If the request has been declined, you will either receive a phone call or email explaining the reason why your customer’s request was declined. If approved, the Beverage Form is processed with S&D Coffee & Tea by Kevin Barko and Ted Pearson and you will receive a conformation that the request has been approved and processed.
3: If new, equipment is ordered, the request for install, exchange or additional equipment requests can take up to two weeks from the time the Beverage Equipment Request Form was approved and processed with S&D Coffee & Tea This process depends on whether or not the requested equipment is stocked and available for shipment from Wilber Curtis or Bunn Equipment or if the requested equipment is available in our inventory. Once the new equipment has been received and/or when refurbished equipment is available for install, the Scheduling Department will contact your customer to schedule said requested service directly with your customer.
