New tool helps reduce brain surgery risks Huntington neurosurgeon touts procedure’s success By Giselle Barkley

When Jean Noschese’s left hand started to go numb, she didn’t expect her doctor’s visit to lead to brain surgery at Huntington Hospital, where she met Dr. Robert Kerr, a neurosurgeon who had a new way of operating on the brain. On Oct. 16, 2013 Noschese experienced a head-on collision while driving in Hauppauge. The car accident left her in need of several surgeries, including ones to repair her rotator cuff and replace her hip. But it was when she started losing sensation in her left hand, in 2014, that she went to a specialist. Noschese, who initially wondered if her issue with her hand was related to her accident, was rushed to the hospital after her hand specialist thought Noschese was experiencing a stroke. But instead of a stroke, the doctors found a three-by-four-centimeter lesion on the right side of her brain that caused paralysis on the left side of her body. Her lesion wasn’t caused by the accident, but from Noschese’s breast cancer that had

metastasized to her brain. Noschese was diagnosed with breast cancer in 2005. Doctors wanted to perform brain surgery the following day, on Dec. 22, 2014, according to Noschese, “It’s overwhelming to hear that you need brain surgery,” she said. But Kerr, who met with Noschese when she entered the hospital, reassured her and reviewed the procedure with her. A new brain-mapping technique, using the Synaptive Medical’s BrightMatter Plan product, involves several new tools, including a highly engineered tube that splits brain tissue fibers and allows neurosurgeons to access difficult and deep parts of the brain easily. The procedure also utilizes a fiber optic, high definition telescope that creates a “cone of visualization” that allows surgeons to clearly view all planes of the brain they are working on. The technique also features a procedure called the myriad, which uses a blunt suction device that peels off tumors from dangerous or sensitive areas without damaging surrounding areas in

the brain. “Traditionally, surgeries for deeper regions actually involve destroying a certain amount of tissue to get to the target area,” Kerr said. According to Kerr, in traditional brain surgeries, metal retractors are used to create a pathway so surgeons can access

Kerr said the issue with this technique is that, regardless of how careful a surgeon is, he or she may still push on these retractors, which widens the pathway the surgeon created from surface of the

As a result, the patient is left with a hole in part of the brain, which means the patient will take longer to recover from the surgery. Stony Brook Medicine Neurosurgeon Dr. David Chesler said Synaptive Medical’s BrightMatter Plan procedure is only appropriate under certain circumstances. “Tumors that come right to the surface, where they’re easily approachable, I don’t think there’s any benefit to using this technique, because the tumor is right there,” Chesler said. Chesler took a course for the procedure about two years ago. While he thinks the technique is beneficial, he does not think it is a be-all and end-all procedure for brain surgery. While the procedure is minimally invasive, may decrease the chance of injuring the patient during the operation and allows surgeons to approach lesions or blood clots, Chesler said there are some downsides to the technique. He said that the technology of this technique is not new, but simply creates a new system that makes it easier for surgeons to implement. Additionally, minimally invasive procedures double or triple the length of an operation, depending on the surgeon and the nature of the surgery. Surgeons who may not be very experienced may leave parts of lesions or tumors and blood clots behind because of limited visualization. Surgeons can only see what is at the end of the tube, which is around the diameter of a dime. Kerr said this procedure will decrease patients’ recovery time. Patients are usually tired following the surgery and require extensive amounts of rest before they are discharged from the hospital. Noschese, however, was alert and speaking two hours after the surgery. Not only did she regain sensation in her hand, but she was also able to grab and hold onto a variety of objects. Chesler, like Kerr, said patients who receive traditional surgeries for deep-seated lesions or blood clots can do well. He said his patients’ deficits were more related to the “structures involved with the tumor,” as opposed to the approach used. Chesler has seen both good and bad outcomes from this technique According to Kerr, few surgeons are using this technique. “Neurosurgeons are skeptics and slow adopters and I think that’s appropriate,” Kerr said, explaining why more surgeons may stick to traditional brain surgery practices. While Chesler said the procedure should be used for the right case and with an experienced surgeon, he said staff are looking to adopt this technique at Stony Brook University Hospital. Chesler, who does both pediatric and adult neurosurgery, said he is simply looking for the right case. Recently he hasn’t come across a case that calls for the technique. Doctors must demonstrate the procedure and illustrate its benefits when introducing the technique to the hospital. Hospitals need to invest in the procedure for a surgeon to officially implement it. Although Chesler said there are other systems that surgeons can use to reach a similar goal, Kerr said the technique is a glimpse into the future of this type of surgery. “I think this technology ref lects the future of neurosurgery and accessing deep-seated lesions in a kinder, gentler, more precise way,” Kerr said. “I think this is representing a future paradigm shift in the way that brain surgery is done, and I think that we will see many more adopting this in a very short period of time.”

He said the residents yelling this are people living in singledwelling homes occupied by five different families. “We have to go after the overcrowded houses,” McGoldrick said. “It’s not fair

to the police officers and fire firefighters. One of the biggest problems is how housing is handled in this town.” As members of the audience agreed housing is a town issue, not a police one, the tone changed toward a desire to see a change in leadership in Huntington Town. Councilwoman Susan Berland (D) and Councilman Gene Cook (R) were both present at the meeting, as well as Huntington Town Board candidate Jennifer Thompson, a member of the Northport-East Northport school board. Despite the criticism throughout the night, the 2nd Precinct deputy inspector defended the department’s work. “We’re covering all our sectors, we’ve been doing it for years,” he said.

The Suffolk County Police Department arrested a Staten Island man early Friday morning after officers allegedly found him stealing 800 pounds of cooking oil from two local restaurants. Police said the officers, Daniel Denig and John McAleavey of the 2nd Precinct, were patrolling in Huntington at 6 a.m. when they spotted a man stealing cooking oil from a holding container behind the New York Avenue businesses, New York Pizza and New China Restaurant. The two restaurants, in a strip of stores off New York Avenue just west of Lowndes Avenue, put their used cooking oil into that holding container, police said. The container is owned

by Newark-based biofuel recycling company Darling Ingredients Inc.. Officers arrested the suspect, 36-year-old Joskey Henry, and charged him with petit larceny. The suspect is a resident of a neighborhood in northeastern Staten Island, near the Verrazano-Narrows Bridge. The defendant’s attorney didn’t immediately return a call seeking comment on Monday afternoon. A police department spokesperson said in a phone interview Friday that Henry’s vehicle was “impounded for evidence,” but the exact details of the vehicle’s connection to the crime were not immediately clear. The Suffolk County Police Department said it is on the hunt for the suspects, who allegedly damaged a door and roof panel on a building at the Centerport museum on Little Neck Road on Friday afternoon. Police said after a witness noticed

the vandals, the men fled in a red boat that had been parked on the adjacent beach. Officers from the SCPD’s 2nd Precinct Crime Section and Suffolk County Crime Stoppers are asking for the public’s help to identify and locate the men, who are wanted for criminal mischief. Anyone with information is asked to call Crime Stoppers anonymously at 800-220TIPS. There is a cash reward of up to $5,000 for information that leads to an arrest.

Woman can receive free mammograms, sonograms and breast biopsies at Huntington Hospital’s Women’s Center and the Charles and Helen Reichert Imaging Center at Huntington with the help of a new grant. On Friday, Aug. 7, Pink Aid, an organization that aims to help women receive and survive breast cancer treatment, gave the Dolan Family Health Center a one-year, $25,000 grant.

According to Joseph Volavka, senior administrative director of the Dolan Family Health Center, around 23 percent of the center’s patients pay out of pocket for their regular appointment. The purpose of the grant is to encourage women who may not have health insurance to receive free breast screenings, which can be costly. Most patients usually have enough money to pay for their regular appointments, so the grant gives more women the opportunity to get additional health care than they would otherwise

receive due to financial limitations. “We are very grateful for this grant, which will help so many women to get the medical care that they need, and

Commack School District mismanaged cash: Audit By PHil Corso

A state audit cracked down on the Commack Union Free School District, accusing officials of mishandling funds and costing taxpayers. The audit, which was released Aug. 5, said Commack school administrators needed to do a better job overseeing the budgeting process after the district overestimated expenditures in its adopted budgets and did not use surplus cash to finance operations. The audit also found the district did not maintain a “complete and adequate” record of its fuel inventory to safeguard and account for its fuel. “From 2011-12 through 2013-14, total actual revenues exceeded expenditures by as much as $3.7 million,” Comptroller Tom DiNapoli said in the audit, and while the district had a $24 million fund balance, it only used $1.8 million to offset taxes. “Had district officials used more realistic budget estimates, they could have avoided the accumulation of excess fund balance and possibly reduced the real property tax levy.” The report also found that discrepancies in the fuel inventory records were not investigated. According to DiNapoli, Commack’s head groundskeeper performed a monthly reconciliation of district fuel purchase and use records with the actual fuel on hand but never acted

on discrepancies, even though anything left unresolved within 48 hours must be reported to the state Department of Environmental Conservation. In response, Commack Superintendent Donald James said the district had “varying fiscal philosophies” but cited a list of changes it would be implementing moving forward. As for the comptroller’s remarks on Commack’s financial condition, James kept it short and sweet. “The district will review the expenditure budget areas and the variables affecting such areas discussed in the audit report in depth to assure reasonable estimates are presented,” he said in a statement. District spokeswoman Brenda Lentsch said the district saves money through strong budgeting practices and all of its savings are returned to the taxpayers the following year. “We go to great efforts not to spend the money the residents of this community entrust to us,” she said in a statement. “Further, the district returns every dollar not spent in the budget to the taxpayers to keep the tax levy as low as possible, and to continue to offer the multitude of programs and services that Commack is known for, and the community expects.” On the subject of fuel inventory records, James had a lot more to say. “The district has taken great care and effort to develop and implement new

procedures to ensure that fuel supplies are adequately safeguarded, accounted for and protected against risk of loss or unidentified leakage,” he said in a response outlined within the audit. Moving forward, James said the district would record, monitor and reconcile its fuel inventory via a senior account clerk and install video surveillance systems to monitor the area of the 2,500-gallon underground fuel tank and pump. DiNapoli’s audit set out to evaluate the district’s overall financial condition and fuel inventory, specifically between July

1, 2013, and Nov. 30, 2014. The comptroller extended the scope of his audit back to July 1, 2011, however, to provide better perspective and background. DiNapoli recommended the district develop procedures to ensure it adopts more reasonable budgets — to avoid raising more real property taxes than necessary — and use more of its surplus funds to support future budgets and reduce the burden on taxpayers. He also recommended the district adopt written policies to ensure fuel is periodically measured and to report discrepancies promptly.

Huntington High School student earns honors

She didn’t see it coming. Huntington High School senior Katie Reilly was recognized with a special award for her academic and athletic achievements at the Young Leaders annual dinner at Crestwood Manor in Fort Salonga last Thursday night. Proceeds from this year’s Young Leaders dinner are dedicated to the John Bosco Memorial Scholarship Fund. A member of Huntington’s Class of 2012, John Bosco was struck by a car and killed in January, 2015, while crossing a street in Florida during a vacation with his family. A participant in the Young Leaders club, Bosco played lacrosse for the Blue Devils. A fine student, he was studying at SUNY College at Oneonta at the time of his death. Young Leaders co-founder Kevin Thorbourne told a crowd of 125 that Bosco was a unique individual, with the ability to quickly win over people of all ages and rally them behind a good cause. He noted his academic interests, including chemistry, and drew some parallels with Reilly, who will be attending Princ-

eton University as a sophomore to play NCAA Division I lacrosse for the Tigers. Thorbourne listed some of Reilly’s attributes and accomplishments, including working as a summer camp counselor, before calling the teenager to the dais and presenting her with a lacrosse stick in honor of Bosco. Verbally committed to Princeton, Reilly is pursuing formal admission to the Ivy League school early this fall. She was an All-County player and high scorer for Huntington’s girls’ soccer team last fall. On the lacrosse field, Reilly led Long Island in assists as a sophomore with 85 and Suffolk in total points with 134. A two-time All-County player, she notched 34 goals and 53 assists last spring as a junior. The Suffolk League IV basketball Rookie of the Year as a freshman, Reilly is expected to start as point guard for the Blue Devils this winter. “Katie has made good use of her time at Huntington High School and is a wonderful role model for the children of the community,” Thorbourne said.

Promoting water safety The Huntington YMCA recently opened its doors to Kim and Stew Leonard Jr. as they read their children’s book “Stewie the Duck Learns to Swim” to over 300 campers, ages 3 to 8. The YMCA of Long Island partnered with Stew Leonard III Children’s Charities in an effort to continue to further spread awareness about water safety and healthy living. The YMCA of Long Island and Stew Leonard III Children’s Charities share a commitment to keeping children safe around water. The foundation’s mascot, Stewie the Duck, made a special guest appearance, giving hugs, high-fives and stickers to campers. In 1989, Kim and Stew Leonard Jr. tragically lost their 21-month-old son to a drowning accident. The following year, they founded Stew Leonard III Children’s Charity and have since raised over $2 million to go toward water safety education for children and parents. The very first Long Island Stew Leonard’s location is set to open in Farmingdale in early 2016.

Northport student earns scholarship A local student will continue his education this fall with the help of Walt Whitman Shops and Simon Youth Foundation. Gavin Sands of Northport has received a Simon Youth Foundation Community Scholarship valued at $1,500. The scholarship is awarded in partnership between Walt Whitman Shops and Simon Youth Foundation, a national nonprofit organization dedicated to providing educational and career development opportunities for youth. Walt Whitman Shops Director of Marketing and Business Development Kathleen Herrmann and Assistant Director of Marketing and Business Development Tyler Andrews presented the oversized check and gifts to Sands during a special ceremony held at Roosevelt Field. Sands exemplified himself through academic excellence, leadership skills and participation in community activities. During his high school years, during which he was home schooled, Sands maintained a perfect 4.0 grade point average and completed 51 dual college credits. He has logged more than 700 community service hours, aiding the homeless in New York City and traveling to Belize to help build a home for a family, and received the Gold Presidential Service Award each year

Tyler Gerbavsits has used his summer vacation to sharpen his golf play. Already one of the best young players on the links in Suffolk County, the Huntington sophomore is gearing up and looking ready for the fall season with the Blue Devils. Gerbavsits is a member of a talented group of Huntington golfers. At the Long Island Golf Association’s Junior Championship at the Garden City Country Club, the athlete was joined by teammate and state tournament qualifier Jim Smoot. The duo both posted first-round scores of eight over par 78, with Gerbavsits finishing second in the boys’ division, which was based on one day’s play. The twin rounds of 78 qualified the two Huntington stars for the junior division match play championship the following day. Finishing tied for seventh in the standings, Gerbavsits and Smoot drew each other in the first round of match play. The pair battled it out through the front nine, with Gerbavsits being one-up after four holes and then Smoot turning it around and taking a two-up lead after seven holes. However, Gerbavsits won the eighth and ninth holes, bringing the match to even after the first nine holes. The back nine was, again, a battle. Gerbavsits took a one-up lead after 11, but Smoot again responded by bringing it back to allsquare after the 12th hole. Smoot took a oneup lead after the 13th hole. Gerbavsits won the 15th, bringing the match back to all even. Both Huntington golfers made par on 16 before Smoot won the 17th hole with a par on the par-three, giving him the advantage with a one-up lead going to the 18th hole.

With both of them making par on the final hole, Smoot won the match one-up, posting a score of 77 to Gerbavsits’ 78. At the Long Island Amateur Championship, hosted by the LIGA at Southward Ho Country Club in Bay Shore, Gerbavsits was among 141 players competing for 31 qualifying spots on the first day. With plenty on the line, Gerbavsits posted a round of one-under par 70, one of the lowest scores of any qualifier. The cut was made at 74. The Huntington sophomore recorded an eagle on the par four 15th hole, where he holed out from 130 yards with a nine iron. Gerbavsits also tallied four birdies, eight pars and five bogeys over the round. Match play for the 32 participants followed later in the week, with Gerbavsits facing Cold Spring Harbor graduate Steven Tanen, a college senior and a member of McDaniel College’s golf team. Gerbavsits lost on the 17th hole after a great match, shooting a 75 to Tanen’s 73.

Fishing still feasible in late summer months AngeLo PeLuSo

What connection does the Latin phrase dies caniculares have with summer fishing on the North Shore of Long Island? Literally translated the idiom means dog days. The Romans linked hot summer weather to Sirius, the brightest star in the constellation Canis Major, or large dog. Contemporary humans link “canicule” to dogs sleeping in the shade on hot days, beach and pool time, barbecues and a period of stagnation in the fishing season, but the dog days of July actually bring some great fishing throughout the Long Island Sound and the North Shore of Long Island. Anglers may lament during mid to late summer that they can’t wait for the fall run to begin in the Long Island Sound, but while those anglers are looking to the future for quality fishing, others are cashing in on the summer bounty that the North Shore of Long Island provides. If you give up on fishing in the Sound when the mercury rises, you may miss out on some of the largest bass and bluefish, which has said to have been caught in July and can continue to hang around into the late summer months. I once even enjoyed a very special fly and light tackle surprise: an advance guard of early Atlantic bonito. And the best part was that I had that fabulous fishing all to myself for almost a week. There can be a lot of fishing variety to

appeal to even the most ardent anglers. July entertained, and August continues to host open seasons for fluke, striped bass, bluefish, sea bass, scup and the occasional weakfish. There was a time when sizeable tiderunner weaks could be found mixed in with hordes of July bluefish and bass. While the extreme western areas of the North Shore of Long Island that heated up first in the spring may experience somewhat of a daytime slowdown in fishing activity, changing tactics to early morning and evening tides can prove to be the key to unlocking the secrets of summer bass fishing.

As one moves farther east along the North Shore toward the central and eastern regions of the Sound, you will begin to encounter some unique hydrodynamics that are favorable for supporting good fishing throughout the summer months. Deeper, cooler water combined with a solid dispersing of oxygen throughout the water works to keep these central and eastern areas of the Sound suitable for sustaining baitfish and game fish. As a matter of fact, some of the areas within this part of the Sound are so oxygen-rich that clams are transplanted from lower water quality areas farther west so that the filter feeding bivalves can cleanse themselves

before being shipped off to market. So far, the 2015 season has shaped up as a decent one. Striped bass settled in late, but have maintained an extended presence in the Sound. While the peak of early season fishing has passed, and August will continue to see summer feeding and behavioral patterns emerge in full swing. Daytime fishing will have its share of rewarding surprises as will the magical times around dawn and dusk. If your bass fishing is being done from a boat, make sure to spend some time prowling the deeper water off rock promontories, the drop-offs around shoals and around the depths of the offshore lobster pots. Most all the areas around the navigation buoys from Eatons Neck out to Rocky Point will hold fish during periods of the tide cycles, as will most areas in between. Light tackle jigging and bucktailing these areas will yield fine results.

Going underground after the big storm

ood news. We are finally off the generator and back onto the electricity supplied routinely, but not to us these last nine days, by PSEG. Why have we been suffering while others around us have had electricity all this time? The answer has to do with supply and demand. Because there has been so much demand for contractors and carpenters as a result of the recent storm, we have had to wait our turn.

When the huge, stately tree that lived next to our driveway was snapped off at its roots by what the National Weather Service termed as straight-line wind — but not a tornado — it collapsed across our parking lot. The falling tree pulled down the lines and the tube that served as a conduit for the electrical wires on the rear of the building, too. In effect it snapped the tube in two as if it were a matchstick. The electric company was not going to restore the lines that had been mounted outside the building until we were sure of the permanence of the wooden construction of our rear wall. And that required the approval of a contractor. So we were hung up, waiting for help that wasn’t hurrying our way. Yes, we were kept in business by a big generator, powering our computers, Internet and phone lines but decidedly not our air conditioning. That required more energy than our generator could deliver. Perhaps you didn’t notice, those of you reading this, that there were a lot of beastly hot

days during the last nine. We noticed. It was like the historically miserable sweatshop, I imagine. We kept going but it wasn’t pretty. We would still be waiting but for a happy alternative. Yes, it required the approval of others, but that was fairly quickly forthcoming. We decided to trench the distance from the electric lines on the road, across the parking lot and into the basement. By doing so, we were able to avoid refastening the power lines to the back of our building. Now we are no longer in a hurry for a contractor. More satisfying, too, is putting the electric lines underground, something we have been editorializing about for most of our almost-40 years of publishing. We can still lose power in future storms if lines are broken somewhere on the roads leading to our building, but not ever again if the problem is within our property. In order to trench our way from the roadside electric lines to our building, we first had to get approval from PSEG, certifying

that there were no other lines underground that we might be cutting into in the process. They call such on-site evaluation “marking,” and you have undoubtedly seen differently colored painted arrows and drawings on the roads that indicate where utility lines are or are not to go.

It took the better part of a day to do the trenching, lay in the lines, cover them with dirt and blacktop the ditch. PSEG was then summoned, and to their credit they arrived with three trucks that evening and approved of the entire job. We are set now to power on.

Beyond the bright screens There’s been a lot of hubbub about the 21st century classroom, where interactive whiteboards replace chalk, and pencils and pens are subbed out for iPads and Chromebooks. Even our own governor has incentivized such reforms at our schools. But let’s push pause and ask: Is all this technology helping or hurting? And what benefits are we missing out on in the real world beyond the bright screens? This week, a Huntington Station company, Teq, announced it had partnered with Canadian company SMART Technologies — yes, the creators of the famous SMART Board that is a staple of today’s classrooms — to be the sole distributor of Smart products for grades K to 12 in New York. That’s a big deal and we applaud Teq’s success. The educational tech company,

already projecting sales of $50 million this year, anticipates the partnership will boost its revenues 20 percent. That’s not just chump change, and it’s a good deal for Long Island’s economy. Yet how much of our new technologies are really needed for learning and how much are we just advancing for the sake of advancing? It feels like a lot of the new software and hardware is needed only to keep today’s student boredom at bay, as many kids are so used to having tech products in the home that they will not concentrate on paper. A culture of distraction is one of the greatest setbacks of today’s overly technological society. We understand that it benefits our students to be familiar with today’s gadgets, so they will be prepared for tomorrow’s success. But it also benefits children to

know what it feels like to hold a real book in their hands, to solve a difficult math problem using a pencil and loose-leaf notebook, to be able to tell time without a digital display, to play outside instead of staring at their phones.
