LIVING A HEALTHY LIFE WITH CHRONIC CONDITIONS Are you living with multiple chronic conditions? The Chronic Disease SelfManagement Program is a six-session interactive education program that runs in many locations across BC. The program gives participants the skills, knowledge and confidence to manage their chronic conditions. T opics include: healthy eating; starting and maintaining exercise; pain, fatigue and stress management; communicating with your healthcare provider; managing medications; and planning and problem-solving. Participants take an active role in the program by setting weekly goals. Many programs are available on Vancouver Island, including Victoria, Nanaimo and Parksville.
