Cycle for a Cause WESTFIELD - The YMCA of Greater Westfield will be hosting its second indoor Cycle a-thon. The event will be held on Saturday, November 8th, 2014, 8:00AM-12:00PM in the Y’s General Purpose Room. Monies raised from this fundraiser will go towards the YMCA’s Annual Campaign which provides scholarship monies for individuals and families who would otherwise not be able to participate in Y programs and services. Individuals may choose to participate in the Cycle-athon in three different ways: Become a sponsor and promote your business for a good cause, be a Team Captain & Join a Team or help pledge the riders. DJ, Rob Charette, will spin the music while bikers participate in some team games. The YMCA gymnastic team will cheer everyone on. Bikers can enjoy a chair massage while they wait to ride.

Know your risk of developing
Breast Cancer
We don’t all have the same risk of developing breast can-
cer. While American women, on average, have a 12% risk of developing the disease, the risk is much higher for certain groups. The American Cancer Society considers women who have a 20% or greater risk to be at “high risk”. Women with a family history of breast cancer, or have had biopsies showing “atypia” and other factors have a higher risk. The good news for these women is that we have specific management strategies to watch them more intensely, or even better, reduce their risk. About 2% of women are considered “high risk”. An even smaller number of women – 0.2%- have dramatically higher risk due to inherited mutations in several genes, most commonly genes named BRCA1 and BRCA2. Everyone has two copies of BRCA1 and BRCA2 – one is inherited from the father, and one from the mother. The function of a gene is to ultimately make a protein that has a specific purpose. These two genes make proteins that repair broken DNA. If the gene is “misspelled” the correct protein will not result, and breaks in DNA will not be repaired. This is a way cancers can start. Using highly sophisticated DNA sequencing technology, “bad” copies of these genes can be identified in an individual. Why would we test someone to see if a “bad” copy – or deleterious mutation – in BRCA1 or 2 exists in his or her DNA? We know that women who have these misspelled genes have a very high risk of breast and ovarian cancer – with a breast cancer lifetime risk that approaches 85%. So what can be done? The first step is aggressive screening for breast cancer, beginning by age 25. If we detect an early stage cancer, we have the best

success for a good outcome. Medications that can reduce breast cancer risk substantially are also available. Removing a woman’s ovaries by age 40 also substantially reduces her risk of breast cancer. Finally, removal of the breasts (prophylactic mastectomy) decreases breast cancer risk by more than 90%. Reconstruction techniques have improved, making the last option more attractive to many women who have an extraordinary risk of breast cancer than in the past. Women who have the BRCA gene mutation also have an increased risk of developing ovarian cancer by up to 45%. Unlike breast cancer, there are no effective screening techniques for diagnosing an early stage ovarian cancer, and as a result, almost 80% of women diagnosed with ovarian cancer will die from the disease because it is most often discovered at an advanced stage. Current guidelines suggest removing the ovaries (and fallopian tubes) by age 35 to reduce cancer risk by 96%. Yes, a woman can develop ovarian cancer even if her ovaries have been removed, but it is unusual. Let us not forget the men. Men with BRCA2 deleterious mutations have up to an 8% lifetime risk of breast cancer, and higher than expected rates of prostate cancer. We encourage the men to tell their doctors if they feel a breast lump. We can protect individuals who are found to have BRCA1 and 2 mutations with aggressive screening, and can even prevent some or most of these cancers. Be aware of your family history and talk to your doctor. Screenings for these gene mutations are available at the Breast Care Center at Mercy Medical Center.

mastectomy mastectomy By Keith O’Connor While many women are opting to have a double mastectomy for early stage breast cancer, studies are beginning to show that the procedure is not linked to a lower risk of death than other options, such as breast-conserving therapy in conjunction with radiation. “This is a trend that we have seen here in Western Massachusetts. Many of the women we see who have been diagnosed with breast cancer are concerned about the future risk of breast cancer in the other (non-affected) breast and, therefore, are asking about the double mastectomy as a way to improve survival,” said surgical oncologist Holly Mason, who serves as co-medical director of the Baystate Breast & Wellness Center, a service of Baystate Medical Center. “As evidenced in this study, as well as in other recent studies, this is not the case. For the individual patient, optimizing treatment of the cancer that we know about now (and not a potential future cancer) is the treatment that will improve survival,” she added. The study was published in the Journal of the American Medical Association (JAMA) on September 2. In the study, out of Stanford University School of Medicine, the authors
noted that previous research has shown that patients with early-stage breast cancer have similar survival rates whether treated with a combination of breastconserving therapy and radiation or a mastectomy. Yet, they note in their published study that the number of breast cancer patients undergoing a mastectomy, in particular, a double mastectomy, is increasing.

In an accompanying editorial addressing the study, the point was made that doctors must inform their patients of the surgical risks – especially that there is still a chance of new or recurrent breast cancer.
“This is a very important part of the surgical consultation for breast cancer. It is essential for patients to understand the multidisciplinary approach (surgery, medication, radiation) to breast cancer treatment. A more extensive surgery does not necessarily improve survival,” said Dr. Mason.
The Baystate breast surgeon noted that although the idea of the double mastectomy may be appealing, it is a significantly larger surgery than breast conservation. “We are seeing data that shows that the complication rate (the risk of bleeding, infection, wound breakdown, or need for reoperation) is higher in patients undergoing the double mastectomy than in patients undergoing breast conservation or a unilateral mastectomy. In terms of breast cancer treatment, this can mean a delay in the start of the other treatment that is absolutely necessary to control the breast cancer, such as chemotherapy or radiation,” said Dr. Mason. However, Dr. Mason noted that the arguments over the double mastectomy do not necessarily apply to the BRCA gene mutation carriers or patients with a strong family history for breast cancer. “For these patients, the question may be whether or not the double mastectomy should occur at the time of breast cancer treatment or at a later date, once breast cancer treatment is completed,” she said.

BreasT Lumps or LumpIness Many women may find that their breasts feel lumpy. Breast tissue naturally has a bumpy texture. For some women, the lumpiness is more pronounced than for others. In most cases, this lumpiness is no cause to worry. If the lumpiness can be felt throughout the breast and feels like your other breast, then it is probably normal breast tissue. Lumps that feel harder or different from the rest of the breast (or the other breast) or that feel like a change are a concern and should be checked. How frequently should women undergo mammography and breast examinations? The American Cancer Society (ACS) recommends a baseline mammogram for all women by age 40 and annual mammograms for women 40 and older for as long as they are in good health. In women with “lumpy breasts” or breast symptoms, and also in women with a high risk of developing breast cancer, sometimes a baseline mammogram at 35 years of age is recommended. This recommendation is somewhat controversial, and there are other viewpoints. However, the U.S. Preventive Services Task Force (USPSTF) recommends against routine mammography screening for women before 50 years of age and suggests that screening end at 74 years of age. The new USPSTF recommendations are in opposition to other existing breast cancer screening guidelines from organizations such as the American Cancer Society as described above. The USPSTF guidelines also recommend changing the screening interval from one year to two years and suggest that women 40 to 49 years old who are at high risk for breast cancer consult with their doctor regarding the time to begin regular screening mammography. It is important for women who are concerned about when to begin mammography to discuss the situation with their health care professional. He or she can help you make an informed decision about breast cancer screening that is appropriate for your individual situation.
Mammograms and young women There is a special issue regarding mammograms in young women. Since young women have dense glandular breast tissue, routine mammograms have difficulty "seeing through" the dense breast tissue. Therefore mammograms may not be able to detect cancer in the breast because the dense breast tissue around the cancer obscures it. However, this problem can be partly offset by the use of special breast ultrasound, which is now an extremely important additional imaging technique used to supplement mammography in difficult cases. Ultrasound can make visible a lump hidden within dense breast tissue. It may also detect lumps and early breast cancers when mammograms fail to identify a problem. Ultrasound can also help doctors locate specific areas in the breast for biopsy (obtain small samples of tissue to study under a microscope). Sometimes doctors also suggest the use of magnetic resonance imaging (MRI) screening (see below) in younger women with dense breast tissue. Magnetic resonance imaging (MRI) scanning Recent research has shown that MRI scanning may be a useful screening tool for breast cancer in certain high-risk populations. In 2004, a team of Dutch researchers published a study of over 1,900 women at high risk for breast cancer in the New England Journal of Medicine. These women underwent breast cancer screening that included physical exams every six months along with yearly mammograms and MRI scans of the breasts. While conventional mammography did detect many cancers at an early stage, some tumors were identified by MRI that were not detected by mammography. Overall, MRI led to the identification of 32 tumors, of which 22 were not seen on the corresponding mammogram. Likewise, some tumors appeared on mammograms that were not visible on the MRI scan. Mammography detected a total of 18 tumors, of which eight were not identified by MRI. The routine use of MRI, however, has many limitations. While it enabled the detection of some tumors in high-risk women, it also detected more noncancerous lesions (falsepositives), which lead to many more follow-up examinations and potentially unnecessary surgeries. In fact, MRI led to twice as many unnecessary examinations and three times

as many unneeded surgical biopsies of the breast than screening by mammography alone. MRI is also approximately 10 times more costly (average cost $1000-$1500) than mammography. Because of these limitations, experts believe that screening with MRI is impractical for women who do not have an elevated risk of developing breast cancer. However, its benefits appear to outweigh its limitations in certain high-risk populations. The American Cancer Society recommends that women at high risk for breast cancer (greater than 20% lifetime risk) should get an MRI and a mammogram every year. Women at moderately increased risk (15% to 20% lifetime risk) should talk with their doctors about the benefits and limitations of adding MRI screening. Women at high risk for developing breast cancer include those who: have a BRCA1 or BRCA2 mutation, indicative of a strong inherited risk of breast cancer; have a first-degree relative with a BRCA1 or BRCA2 mutation but have not been tested for the mutation; and received chest radiation -- to treat Hodgkin's disease or other cancers, for example -- between 10 and 30 years of age. Women with significant risk factors may want to discuss their screening program with their physician to better determine whether MRI may be useful in their own case. It is important to note that MRI should not be considered a substitute for regular mammography, and mammography is the only screening tool for which a reduction in mortality (death rate) from breast cancer has been proved. Breast self-examination and breast examinations by your doctor Both the American Cancer Society and the USPSTF do not call for regular selfexamination of the breasts in their guidelines. The ACS states that breast self-exam is optional, while the USPSTF states that doctors should not teach women to do breast self-examination. The ACS further recommends a clinical breast exam (CBE) by a health care professional about every three years for women in their 20s and 30s and every year for women 40 years of age and over.

breast cancer Patients with breast cancer have many treatment options. Most treatments are adjusted specifically to the type of cancer and the staging group. treatment options should be discussed with your health care team. Below you will find the basic treatment modalities used in the treatment of breast cancer. treatment modalities are always changing and developing. it is important that you discuss the different options with your health care team. Surgery Most women with breast cancer will require surgery. Broadly, the surgical therapies for breast cancer can be divided into breast conserving surgery and mastectomy. BreaSt-conServing Surgery this surgery will only remove part of the breast (sometimes referred to as partial mastectomy). the extent of the surgery is determined by the size and location of the tumor. in a lumpectomy, only the breast lump and some surrounding tissue are removed. the surrounding tissue (margins) is inspected for cancer cells. if no cancer cells are found, this is called "negative" or "clear margins." Frequently, radiation therapy is given after lumpectomies. MaStectoMy During a mastectomy (sometimes also referred to as a simple mastectomy), all the breast tissue is removed. if im-
mediate reconstruction is considered, a skin-sparing mastectomy is sometimes performed. in this surgery, all the breast tissue is removed as well but the overlying skin is preserved. raDical MaStectoMy During this surgery, the surgeon removes the axillary lymph nodes as well as the chest wall muscle in addition to the breast. this procedure is done much less frequently than in the past, as in most cases a modified radical mastectomy is as effective. MoDiFieD raDical MaStectoMy this surgery removes the axillary lymph nodes in addition to the breast tissue. Depending on the stage of the cancer, your health care team might give you a choice between a lumpectomy and a mastectomy. lumpectomy allows sparing of the breast but usually requires radiation therapy afterward. if lumpectomy is indicated, long-
term follow-up shows no advantage of a mastectomy over the lumpectomy. raDiation therapy radiation therapy destroys cancer cells with high energy rays. there are two ways to administer radiation therapy: external BeaM raDiation this is the usual way radiation therapy is given for breast cancer. a beam of radiation is focused onto the affected area by an external machine. the extent of the treatment is determined by your health care team and is based on the surgical procedure performed and whether lymph nodes were affected or not. the local area will usually be marked after the radiation team has determined the exact location for the treatments. usually the treatment is given 5 days a week for 5 to 6 weeks. Brachytherapy this form of delivering radiation uses radioactive seeds or pellets. instead of a beam from the outside delivering the radiation, these seeds are implanted into the breast next to the cancer. cheMotherapy chemotherapy is treatment of cancers with medications that travel through the bloodstream to the cancer cells. these medications are given either by intravenous injection or by mouth.. chemotherapy can have different indications and may be performed in different settings as follows:

tain treatment intervals followed by a rest period. the cycle length and rest intervals differ from drug to drug. horMone therapy this therapy is often used to help reduce the risk of cancer reoccurrence after surgery, but it can also be used as adjunct treatment. estrogen (a hormone produced by the ovaries) promotes the growth of a few breast cancers, specifically those containing receptors for estrogen (er positive) or progesterone (pr positive). Whenever a disease has the potential for much harm and death we search for alternative treatments. as a patient or the loved one of a patient you want to try everything and leave no option unexplored. the danger in this approach is usually found in the fact that the patient might not avail themselves of existing, proven therapies. you should discuss your interest in alternative treatments with your health care team and together explore the different options.

RAY: So if you were to fill your Mazda here in the states with imperial gallons and calculate your "miles per imperial gallon," your combined mileage would instantly be 38, not 32. TOM: And the other major factor is the mileage test itself. The European test tends to use slower acceleration, lower top speeds, and fewer starts and stops than the U.S. test. It also doesn't take into account things like air conditioning use and cold-temperature use. RAY: So if you took the same exact car, with same exact engine and same amount of fuel, and ran them through the two different tests, the European mileage number would be, on average, about 20 percent higher, just because the test is different. TOM: So if you convert for imperial gallons, and then convert for the difference in testing methodology, your car would be rated at about 45.6 mpg. That's pretty close to the 50.4 advertised in the U.K. RAY: Of course, you can't count on getting ANY of these numbers in real life. While the U.S. test is more realistic than the European test, neither of the tests really mimics your own driving. TOM: So they're always best used as relative measures, to compare one car with another. RAY: The best measure is the one you do at the pump. You've done that, and as you say, you're "thrilled" with your 32 mpg combined. So enjoy
