PrescriptionCosts$500foraMonth'sSupply DEARDR.ROACH: Whatcanyoutellme aboutaprescriptionof145mcgofLinzess,and whyadoctorwouldprescribeapillover$500? Whataretheadvantagestotakingthis?Surely, theremustbesomethinglessexpensivethatwill providethesameresults.--J.P.

ANSWER: Linaclotide(LinzessorConstella) isamedicineusedforconstipation,eitherin peoplewithirritablebowelsyndromeorin peoplewithconstipationfornoparticular reason.Itworksbyactivatingaproteintoallow chlorideandbicarbonate(alongwithfluid)into theintestine.

Sinceitisnotabsorbedintothebody,ithasfew sideeffects.Themajorsideeffectisthatitcan worktoowellandcausediarrhea.Thisis enoughofaconcernthatthemedicineisnot usedinchildrenundertheageof2.Thereare threedifferentstrengths:72,145and290mcg. Ifyouareaskingmetodefendpharmaceutical pricing,Ican'tdothat.Iveryseldomusethis medicinebecausetherearesomanyother optionsforconstipation--startingwitha differentdietwithoutusinganymedicineatall. Butinpeoplewithoccasionalconstipation despitehavingagooddiet,Iusuallystartwith fibersupplements.

Ifthosedon'twork,Irecommendasurfactant suchasdocusate(Colace),whichallowsmore watertoenterthestool.Yetanotheroption wouldbenonabsorbablesubstanceslike polyethyleneglycol(MiraLAXandothers), lactulose,sorbitolandmagnesium.Allofthese aremuchlessexpensive.

However,therearestillpeoplewhodon'tdo wellwithanyoftheseless-expensive(andalso tried-and-true)alternatives.Linzesshasmadea realdifferenceinthelivesofseveralofmy patients.Itisusuallyprescribedbyanexpert(a gastroenterologist)atleastinthecaseofmy patients.

Finally,Linzessisroughly$20apillorabout $500foramonth'ssupplyintheU.S.without

insurance.Somepeopleonlyneedit occasionallyandareverywillingtopay,evenif insurancedoesn'tcoverit.

DEARDR.ROACH:Theotherday,whenwe werechangingourbedsheets,myhusbandasked meifIknewwhythesheetsandpillowcaseson hissideofthebedwerestainedyellowandthe onesonmysideweren't.Myonlythoughtwas thathehasaverystrongbodyodor.Myhigh schoolbiologyclassescamebacktome,andI rememberedthatoursenseofsmellisalockand-keymechanism,whereodorcomesfroma tinyparticlethatlocksintothesensorswithin ourolfactorysystem.So,Ithoughtthatthe particlesmightbestainingthebedding.Could thisbeso?--J.M.

ANSWER: Chromhidrosisisthenamefor coloredsweat,anditisn'tuncommon.Bothmen andwomencanhavechromhidrosis.Thereare severalpossiblecauses.Oneisthatthesweat glandsthemselvescanexcreteacolored substance,suchaslipofuscin,whichoften causesayellowdiscoloration.

Iadmireyourusinghighschoolbiology,butI don'tthinkyouarerightinthiscase.Theodorin sweatcomesfrombacteriathatlivesinand aroundthesweatglands.Thebacteria themselvesrarelycausediscoloration,but yellowisn'ttypicalforbacteria.

Occasionally,medicinescancausesweattoturn adifferentcolor.Thelaxativebisacodyl (Dulcolaxandothers)cancausepeopletohave yellowsweat,andsomeantiperspirantswill causesweattohaveayellowcolor.

Sinceit'sboththesheetsandpillowcases,I suspectthatit'sthelipofuscininthiscase. Treatmentmaynotbenecessary,butcapsaicin cream(0.025%)hasproveneffective,although themechanismbehindhowitworksis unknown.
