DEARDR.ROACH:Inarecentcolumn,you referredtoaU.K.sourcethatstated"all-cause mortalityisverysignificantlyreducedinpeople whohavehadtheCOVID-19vaccine, comparedtounvaccinatedpeople."Iwasn't surewhatyoumeantbyall-cause?Doesthis meananyandalldeaths,withorwithout COVID-19,foranyreason,havebeenvery significantlyreducedjustbygettingthe COVID-19shot?Ordoesitmeanthatanyand alldeathsassociatedwithCOVID-19havebeen reducedinthosewhohavebeenvaccinated?If itisthefirst,thenfewerpeopleshouldbedying, peopleshouldbelivinglonger,andweshould notseeadecreaseinlifeexpectancy(exceptfor theadditionofthosedyingfromCOVID-19, whichbringstheagedown).Anyway,ifthefirst istrue,thenIwilldefinitelycontinuegettingthe COVID-19shotjusttolivelonger,withor withoutgettingCOVID-19!--C.P.
ANSWER:TheU.K.studylookedatall-cause mortality,whichmeansanyonewhodiesfrom anyreason.Thestudyfoundthatgettingthe COVID-19vaccinereducedthelikelihoodof dyingfromCOVID-19andnon-COVID-19 causes.
Onepotentialproblemstudiesfaceisassigning causeofdeath.Ifapersonwithaveryserious cancerwhoisexpectedtopassawaywithina fewmonthsgetsCOVID-19anddies,didthe cancerkillthem,ordidCOVID-19?Ifyouhave tochoosejustonecause,theanswercouldbe misleading.That'swhytheresultsoftheU.K. study(andaFloridastudythatshowedsimilar results)weresoimportant.Peoplewhogetthe COVID-19vaccinearelesslikelytodiefrom anycause.
InthelastfewyearsintheU.S.,the longstandingtrendofimprovedmortalityhas reversed,anddeathratesaregoingup,partially duetoCOVID-19./
DEARDR.ROACH:Myhusbandhasbeen diagnosedwithcardiacamyloidosis.Thedoctor
hasstartedhimonVyndamax,averyexpensive drug.Withoutthisdrug,heisluckytolive,atthe most,twoyears.Isthisdrugamiracle,anddoes itguaranteethathewilllivemanymoreyears? --T.W.
ANSWER:Thereareseveraltypesofcardiac amyloidosis--adiseaseinwhichabnormal proteinsaredepositedinmanytissues,including theheart,wheretheproteininterfereswithheart functionandleadstoheartfailure.Tafadamis (alsoknownbyitsbrandname,Vyndamax)is usedintransthyretinamyloidcardiomyopathy (ATTR,ofwhichtherearetwotypesthatboth gettreatedwithtafadamis).
Subjectsinthetrialontafadamiswerefollowed fortwo-and-a-halfyears.Halfofthegroupwere giventafadamis,whiletheotherhalfweregiven aplacebo,whichlookedliketherealdrugbut hadnoactivity.Ofthosewhogottheinactive placebo,43%diedinthetwo-and-a-half-year timespanofthestudy,butofthosewhoreceived tafadamis,30%died.Thirteenpercent,orabout oneineightpeople,whoreceivedtafadamis lived,whentheywereexpectedtodie.In addition,therewerefewerhospitalizationsand lesslossofabilitytoexerciseinthosetaking tafadamis,comparedwiththeplacebo.There werenoserioussideeffectsthatweremore frequentinthetafadamisgroup.
Tafadamisdefinitelyaffordsabigimprovement intheoutcomeofpeoplewithATTR-CA. However,Iwouldnotcallthedruga"miracle," noristhereaguaranteeoflivingmanymore years.Tome,amiracleiswhensomeone recoverswhennodoctorexpectedthemto.I haveseenafewinmycareer,andtheyarevery powerful.Ihaveseenmanytreatmentsthat haveincrementalimprovementsinoutcomes, andovertime,thathasledtodramatic improvementsinmanydiseases.
