Is it a bump or lump? Be breast aware this sport season Bumps and bruises are part of the game, but beware: your injury could be hiding something more serious. That’s the message from Breast Cancer Foundation NZ as Kiwis head into the winter sport season. Chief Executive Evangelia Henderson is urging women – and men – to be vigilant if they sustain a knock to the chest. “Bumps themselves don’t cause breast cancer,” Mrs Henderson says. “However, if you notice a lump that remains after bruising, swelling and soreness resolves, we strongly recommend that you have it checked by your doctor.” The Breast Cancer Foundation’s warning comes after a Palmerston North roller derby player, Jody Hare, was diagnosed with an aggressive form of breast cancer following a bruising encounter on the track. Jody went to her doctor with soreness, bruising and a lump that, at first, was thought to be a haematoma. When the lump did not disappear, Jody returned to her doctor and was sent for further tests. She was diagnosed with ductal invasive carcinoma and has since had chemotherapy, a mastectomy and drug treatment. Throughout, she continued with her sport and even represented New Zealand at the world championships in the UK – just one week before having her mastectomy.

Jody is grateful that her injury led to the lump’s chance discovery. “If it had been any later, it would have been a whole lot more serious,” Jody says. Mrs Henderson says, “Jody’s story is a reminder that everyone should be breast aware. We encourage you to ‘Know your normal,’ and if you notice a change, see your doctor.” More than 3300 women are diagnosed with breast cancer in New Zealand every year. Winter feels like it is well and truly here, and seasonal fruit and vegetables play a vital role in boosting immunity and fighting off unwanted ills and chills. Niki Bezzant, food and nutrition writer and editorat-large for Healthy Food Guide, says eating seasonally is not only economical, but ensures we are getting the right vitamins and minerals in our diet. “Getting lots of fruit and vegetables in winter is especially important to keep up our intake of vitamins and minerals to boost immunity. Fruit and vegetables also contain fibre which is beneficial for gut health, a very important part of our immune system,” she says. With soup season in full swing, winter produce such as kumara, carrots, parsnips, broccoli and cauliflower lend themselves perfectly to creating hot, creamy and nutritious soups and are packed with dietary fibre and vitamins. Kumara and broccoli in particular are bursting with Vitamin C, both containing more than the recommended daily dose, as well as Vitamins A and B6.

Bezzant says another cost-effective idea is batch cooking winter vegetables, it is a simple way to plan meals and a great time saver for our busy lives. “Roast up a big dish of kumara, pumpkin, parsnip, potato and carrots at the start of the week and use them in salads for lunches, frittatas for breakfast or casseroles for dinner. “I also love dark leafy greens like silverbeet, spinach and kale in winter, they add goodness to almost anything, particularly soups and casseroles. Citrus fruit is abundant now and perfect as a snack or dessert,” says Bezzant. Dudley says increasing your intake of fruit and vegetables can make a big difference to the way you feel in just a couple of weeks, helping to lift your psychological wellbeing, lowering stress levels, and boosting vitality and motivation.

Are You Planning A Pregnancy? If you are thinking about starting a family it is a great idea to consider pre-conception planning and see your GP to discuss any health issues you have and to get your blood tests done, your weight checked – to ensure all are within normal ranges; and to discuss any travel plans you may have – because of Zika virus. In New Zealand pregnancy care is free to eligible New Zealanders. When you first become pregnant, it is important to engage with your GP to establish a baseline early – within the first 12 weeks of pregnancy. We screen you for: • Your blood group • Your haemoglobin – to make sure you are not anaemic and that your baby will have enough oxygen • Your immunity to Rubella –as this can cause deafness and heart defects if you have no immunity and get this infection in pregnancy • The presence of any infections like syphilis and HIV At this first antenatal visit we provide an information pack about your pregnancy and also check how you are coping with the pregnancy. If your pregnancy is

unplanned there can be concerns about alcohol and drug intake and travel destinations visited in the time period before your pregnancy was detected. Folic acid is an important medicine before conception and in early pregnancy, as is iodine – because soil levels in New Zealand are low. Our doctors can prescribe these for you. Optical Coherence Tomography or OCT is a non-invasive imaging test that uses light waves to take cross-section pictures of your retina and optic nerve, the light-sensitive tissue lining the back of the eye. This sophisticated scanning system produces highly detailed images of the retina. It is often likened to an MRI or x-ray of the eye. This procedure is currently the only one that shows in-depth images of the eyes internal structures. Other procedures only show the surface of these structures. ESR scientists say the flu strain (influenza A(H3N2)) associated with an increase in hospitalisation and deaths in the Northern Hemisphere is part of this year’s vaccine in New Zealand. The environmental health science agency’s Public Health Physician, Sarah Jefferies, says countries such as the UK reported moderate to high levels of influenza and influenzalike illness during their 2017/18 season. Currently, there are four seasonal influenza viruses circulating globally - influenza A(H1N1), influenza A(H3N2), influenza B/Yamagata lineage and influenza B/Victoria lineage. Dr Jefferies says the 2018 Southern Hemisphere seasonal influenza vaccine aims to cover these four viruses by including inactivated strains. This year the publiclyfunded influenza vaccine includes all four strains (i.e. it is a quadrivalent vaccine). The challenge for scientists and medical authorities is virus mutation, with the influenza virus able to rapidly change as it spreads through the population. This means some strains of virus may not be recognised by the body’s immune system in people who are vaccinated or have been previously exposed to influenza virus. “For example, A(H3N2) is notorious for being able to rapidly change genetically, and this is the type of seasonal virus commonly detected in parts of the Northern Hemisphere during their recent flu season. “But, it’s important to note that our Southern Hemisphere vaccine has been update, says Dr Jefferies. “The WHO recommended a change in our Southern Hemisphere seasonal influenza vaccine for 2018 which better covers some strains of influenza detected in the 2017 Southern Hemisphere season and the Northern Hemisphere 2017/18 season.” However, she says it is difficult to predict exactly what strains of seasonal virus will end up circulating in New Zealand’s 2018 season. “The health impacts of flu this year will depend on how well New Zealanders use preventative measures like immunisation and good hygiene practices, how the strains which circulate compare to viruses we’ve had previously, which cause natural immunity, and to the strains in the vaccine, which also generate immunity.” Dr Jefferies says the best option for prevention is immunisation, and the vaccine is free for older people, pregnant women and those with certain medical conditions. ESR is New Zealand’s Crown Research Institute which specialises in science that helps safeguard people’s health. ESR manages the national influenza surveillance programme on behalf of the Ministry of Health.

Asthma: Be more vigilant Whanganui parents are being warned to be more vigilant in managing their children’s asthma with a new report showing high numbers of children being hospitalised for the disease in the region. Auckland parents are being warned to be more vigilant in managing their children’s asthma with a new report showing high numbers of children being hospitalised for the disease in the region. The Massey University report found an increase of 45 per cent in the rates of Kiwi children hospitalised with asthma from 2002 (473 per 100,000) to 2016 (688 per 100,000). According to the report authors, more than 6000 children under the age of 15 were hospitalised with asthma in 2016. The report found Auckland has the country’s highest rate of child hospitalisation with asthma with a rate of

1076 children aged under 15 admitted per 100,000 - a 56 per cent higher level than the national average of 688 per 100,000. While many children grow out of asthma as they age, one in every nine adults are believed to be living with the disease. New Zealand’s high rates of asthma carries a significant cost burden – estimated to be more than $850 million annually, including the loss in productivity at work and the direct costs of medical treatment. Associate Professor Jim Reid from Otago University says New Zealand has one of the highest rates of asthma in the world with around 70 people dying from the disease each year. Recent research shows the prevalence of asthma in New Zealand children is approximately 15 per cent , with 11 per

cent of adults also affected. More than 521,000 Kiwis are estimated to need medication for the disease. Professor Reid says the key to getting the nation’s asthma burden under control is a simple Asthma Control Test (ACT) which is done at either a pharmacy, doctor’s surgery or online at asthmacontroltest.com.

‘Low-cost prevention measures the answer’ The recent Healthy Auckland Together monitoring report shows poorer oral health outcomes for children in the poorest neighbourhoods, being 12 times more likely to have worse oral health compared to children from affluent areas. The New Zealand Dental Association (NZDA) says while recent debate focuses on expensive measures, such as increasing government funding of dental care, there are many low-cost preventative measures that would improve oral health. “Many of the measures we’re calling for don’t carry a great cost, they simply require political will,” said NZDA spokesperson Dr Rob Beaglehole.

The seven actions are:

1) I ntroducing an icon on drinks indicating, in teaspoons, the amount of sugar in each drink. 2) I ndependent monitoring and evaluation of food marketing, with an emphasis on marketing that influences children. 3) Urging the government to adopt WHO limit guidelines. 4) Encouraging public to switch to water by; introducing warning labels highlighting sugary drinks as risk factors for obesity,

diabetes, and tooth decay, and a nationwide social marketing campaigns such as ‘Switch to Water’. 5) W orking with schools and the Ministry of Education to introduce ‘water only’ policies. 6) I ntroducing local council ‘water only’ policies at council facilities and events. 7) I ntroduction of a ‘sugary drinks’ tax in line with WHO recommendations.

DEMENTIA FRIENDS Alzheimers NZ is issuing a call to action as part of a new outreach and awareness programme launched recently. Dementia Friends learn about dementia and the impact it has. They then make a commitment to action to help those living with dementia to live well. Dementia Friends choose the action or actions they are comfortable with and able to offer. Dementia Friends has been informed and shaped by the voices and input of Kiwis living with dementia. Whether you are a school kid or a chief executive, a Cabinet Minister or someone in between, you can be a Dementia Friend, Alzheimers NZ chief executive Catherine Hall says. Ms Hall hopes thousands of Kiwis listen to Alzheimers NZ’s call and join the Dementia Friends community. “We are inviting everyone in New Zealand to become a Dementia Friend

Health professionals welcome this month’s public consultation launch for the Zero Carbon Bill, saying it is crucial for New Zealanders’ health. However, they warn that the Bill will need to be seriously strengthened for faster, fairer protection of a healthy climate. The Bill (modeled on the UK’s successful Climate Change Act 2008), proposes a consistent policy framework for NZ to reach net zero climate pollution in the coming decades. This is vital for protecting human health and wellbeing, in the context of global action. “As health professionals, we know that climate change is a medical emergency,” says Dr Rhys Jones, co-convenor of OraTaiao: The NZ Climate and Health Council. “Quickly reducing climate pollution to net zero is an essential prescription for the health of New Zealanders.” “We know that legislation like a Zero Carbon Act works. The UK Climate Change Act has led to real climate action, including by the health sector. The UK’s National Health Service has reduced emissions by 11% since 2007 – despite an 18% increase in healthcare activity, and the UK’s overall emissions are falling, unlike ours,” says Dr Jones. However, OraTaiao has two major concerns with the Bill as currently proposed. First, the target of zero emissions by 2050, while more ambitious than previous targets, is still too little, too late. “Because of our previous inaction, alongside inaction by other countries, NZ’s emissions now need to be net zero in the 2030s to meet our promises under the Paris Agreement. For the sake of our Pacific neighbours and others living in low-lying areas, we must reach net zero emissions faster.” Second, there are major issues of fairness that need to be addressed in the Zero Carbon Bill’s development. “Equity and Maori wellbeing have to be at the heart of climate policy to support those being hit hardest by climate change,” says Dr Jones. “The process has so far failed to fulfil the government’s partnership requirements under the Treaty of Waitangi.” “We know that NZ can rapidly cut emissions in ways that are fair, honour Te Tiriti o Waitangi, and improve our health in both the short and long term. Cutting our emissions well before 2040 is critical for New Zealanders’ health. We need a fast, fair, Treatybased transition now,” says Dr Jones.

FOR RESEARCH PROGRAMME Major research grant awarded to assess effectiveness of primary health care A Victoria University of Wellington-led team of researchers has received a $4.78 million Health Research Council grant to assess the effectiveness of New Zealand’s primary health care system. The grant, which is over five years, is for a programme of research to be led by Professor Jacqueline Cumming (pictured), Director of the Health Services Research Centre in Victoria University of Wellington’s Faculty of Health. The programme, Enhancing primary health care services to improve health in Aotearoa/New Zealand, will look at the current models of delivering health care via primary health care service providers such as general practices, nursing clinics and pharmacies, and how this model might be improved. A major part of the research will focus on the views of those using or needing to access care. The programme is in collaboration with the School of Health at Victoria University of Wellington, the Health Systems Group at the University of Auckland, the Department of Primary Health Care and General Practice at the University of Otago (Wellington), Pacific Perspectives Ltd, Whakauae Research Services, CMDHB and the Health Services Management Centre in the University of Birmingham and the

Department of Health Services Research and Policy at the London School of Hygiene and Tropical Medicine in the UK. Professor Cumming says that since 2001 New Zealand health policy has focused on improving access to and increasing the use of primary health care services, in an effort to keep people well, but also to reduce pressure on hospitals.
