Getting your medicine at a cheaper price By Michelle Sherrard
M
any elders may find themselves financially struggling around the end of the year because of the Medicare D coverage gap (the notorious “donut hole”). Some may be unemployed and without insurance, while others simply want to save money on medication costs — who doesn’t?
To find out how to save money on prescription drugs, I contacted two local pharmacists who graciously shared insider knowledge. They recommend that patients speak candidly with doctors and pharmacists, take advantage of available discounts, shop around, and if needed, seek financial assistance from local organizations.
Talk to the Doctor and Pharmacist Traci Ullom, head pharmacist and owner of Audubon Pharmacy, says the best advice she could give elders is to “speak up” to get cheaper prices on their prescriptions. Patients should communicate with doctors about their need to save money. If doctors understand that finances are a concern, they might be able to provide free samples from the drug company. They might also prescribe a generic instead of the brand name if it’s available and if they are confident that the generic will work just as effectively. The price 10

difference between the two can be mind-boggling. Pharmacist Roger Hillis of Medical Towers Pharmacy offers a clear explanation for this discrepancy: When a drug company gets a patent, they own the drug for 17 years and try to earn back the money spent on research. After that expires, he explains, “anyone can copy that chemical formula and sell it cheaper.” Some medications don’t yet have a generic because their patent hasn’t expired. In the meantime, Hillis says that many doctors will switch patients from the brand-name only medication to a drug in a similar class that does have a generic available. For example, since Lipitor didn’t have a generic until November, many doctors switched their patients to Zocor. Pharmacists can also be great resources when it comes to saving money. For example, Ullom proudly recalls, “I had a good friend with three prescriptions that cost $300, and we were able to get it down to $50. It takes patience and time, but most pharmacies, including the bigger ones, will work with you.”
Pharmacy Discounts Many pharmacies offer clubs, programs, and special pricing that save customers money. For example, the free Rite-Aid Savings Program, available to everyone, offers 300 medicines at $8.99/30-day supply or $15.99/90-day. Walgreens has the Prescription Savings Program for those who do not receive money from federal or state health care programs. For one individual, it costs $20 a year, and if for some reason a customer doesn’t save more than $20 in a year, he/she will be reimbursed. Wal-mart and Target have continued on page 12 t o d a y ’ s

Without a will, the state will decide who inherits your stuff which leads to increased costs and family feuds. With a will, even if you just have $200 in the bank, you get to determine who gets it, not the state. You might prefer giving a bit of money to the homeless shelter than have the state give money to your brother who will just spend it on beer. (I’m just saying.) If you have significant assets, contact the financial advisor at your bank or an independent firm to figure out strategies for minimizing the tax effect on your estate, such as moving your IRA into an insurance product to make it tax free to your heirs. The estate plan should spell out: your last wishes for who gets what, your medical directives if you become incapacitated, your arrangements for dependents, your funeral wishes, and the moral values you want to pass on.
Here are 14 smart things to do that will make a difference for your grieving family — just in case...
1.
Write a Will.
The centerpiece of an estate plan is a will, which will spell out the transfer of your assets after your death. Your assets could include your house, car, stocks, mutual funds, insurance policies, jewelry, collections, antiques, family heirlooms, other household contents, and personal items. Decide ahead of time who gets what. That will go a long way toward avoiding family arguments. There are other avenues, such as having all of your assets auctioned off and dividing the proceeds among family members, or giving all your possessions to a charity, or spending every dime on yourself. If you are older, you might also want to consider giving gifts before you die. It’s best to consult an attorney for such an important document. State statutes vary, so you might have to have your will re-written if you move to another state. Most estates are overseen by probate court, which makes sure your estate passes to the rightful beneficiaries. Anything filed in probate court is public.

Designate a Power of Attorney.
This gives someone the power to do anything you could do for yourself, such as pay your bills, do your banking, sell your property. 16

In case you become incapacitated, it addresses end-of-life decisions. It lets family members know your wishes about medical treatments to prolong your life.

You’ll need designated guardians for minors and special-needs adults who can’t care for themselves. You also might need a plan for elderly parents.

Develop a Business Succession Plan. Prepare for the future of your business.
Write a Legacy Letter.
Pen a letter about your life, and what you appreciate about your family. You could talk about the good times you have shared, the difficulties you have overcome, your hopes and dreams for your children, the things and causes that were important to you, issues you’ve resolved, the places you love, the values you want passed on, etc. It can be a one-page letter, or a video, or if you’ve got the ambition, a memoir. Lots of older adults are taking writing classes and selfpublishing memoirs.

De-Clutter.
If you are a senior, now is the time to down-size, get rid of junk and save your family the headache of guessing about what to keep and what to toss. Even if you are young, purging is important so that other people don’t have to guess about what to keep and what to toss. Consider getting a document scanner to scan in and file receipts, taxes, business cards, etc, then shred the paper.
A visit to a financial advisor can help you plan for the future.

Purchase Insurance.
Women are often under-insured — whether they are the main bread winner or the stay-at-home mom. Life insurance will provide cash for your family after your death so they can handle funeral costs. Also, if the stay-at-home mom dies, life insurance would help Dad pay someone to come in and help take care of the kids. Disability insurance will protect your income if you are employed outside the home. Consider long-term care insurance, or in lieu of that, consider a hybrid life insurance policy with a long-term care rider. The money that’s not used goes to your death beneficiary tax-free.

Make Several Lists.
Do an inventory of all of your assets — financial, physical, paper, including updating beneficiaries on financial accounts. Also, create a list of your debts. While you’re at it, make a list of charities you support. 18

Organize.
Set up a filing system and put those lists together in a binder with all of your important documents such as your will, insurance policies, car title, social security card, retirement accounts, credit card information, debts, etc. In this digital age, your papers should also include information and passwords for automatic bill payments, on-line financial and shopping accounts, as well as social media, such as Facebook, so that your family can stop or close these accounts. You can include household security codes, computer passwords, safe combinations, the location of safety deposit boxes and keys, etc. Now put these lists and all your important papers in one place, such as a bank safety deposit box (best), or a locked file box in a closet (okay). And tell someone. However, because safety deposit boxes are often opened well after you are gone, give a copy of your will to a trusted family member, so they know your funeral and burial wishes. With the will, they can go to probate court, and get appointed executor and get authority to enter your safety deposit box. Also consider putting a trusted person’s name on your safety deposit box with you, and tell them where the key is. Review this binder yearly. Update the documents at regular milestones, such as marriages, births, moves, and major changes in your life. You’ll be missed, but appreciated.
Up a Pre-Paid 13. Set Funeral.
Family members, says Faller, can be assured that you are getting what you want, and it prevents your loved ones from spending too much money on your funeral. “When folks are stressed because someone has died, they don’t think very clearly and are more gullible and thus pay more for the funeral.”

y mother is only 5 feet tall, but I’ve always measured her by a different yard stick. She was the one who kept the family together. She taught the six of us children all the necessary momism’s to survive... “Always wear clean underwear. You never know if you’re going to be in an accident;” “Don’t run with scissors, you’ll fall and poke your eye out;” and “Give old ladies your seat on the bus...someday that will be you.”
When I was a kid, Mom had all the answers. I often thought that she should strike a pose and sit on a mountaintop. Since then, I’ve discovered that her wisdom was acquired at my grandmother’s kitchen table. Mom ruled our home with an iron fist. If she raised her eyebrow in a certain way, I felt like I had a frozen turkey in the pit of my stomach. But I’ve always known that if I were in trouble, she’d fight like a savage tiger to save me. I never had to go to a Comedy Club for entertainment. My mother was Lucille Ball, Gracie Allen, and Imogene Coca all rolled into one. This past Labor Day weekend, I visited my mother. On the eight-hour drive between my home and hers, I thought about how lucky I was to have had my 89-year-old mother. Granted, macular degeneration had robbed her vision and arthritis had limited her mobility, but Mom’s mind was still as sharp as a razor’s edge and for this I was so grateful. We spent the first few days of my visit “doing” lunch, going to the movies, talking, and joking. On the third day of my trip home, I made a side trip to console a friend who had lost her father. I spent the night with my friend. When I returned the next day, Mom’s world had changed. She wouldn’t or couldn’t let me in the house. With the help of the fire department, we forced our way in. Mom was locked in her bathroom. She couldn’t get out. In words I didn’t continued on page 22

quite understand, she tried to tell me that she was fine. Mom was not fine. And I was terrified. When I unlocked the bathroom, I realized that Mom had had a stroke. Over her many protests, I called EMS and Mom was transported to the hospital. Guilt curled around me like smoke in a house fire. Why had I left her? If I’d only been home, I could have done something. I wasn’t there when she needed me. Irrational as it often is, guilt is an unwelcome guest at most family tragedies. When her stay in the hospital revealed other medical problems, her anger with me for calling 911 subsided. After the cardiologist and neurologist told her their diagnosis, they left her bedside. Mom said, “I guess you were right...I do belong in the hospital.” I haven’t heard my mother admit to being wrong too many times. I wished that this hadn’t been one of them. Then Mom looked at me and said, “Well, I guess it’s my turn.” No! I thought. Don’t say it. Don’t even think it! But then I realized. It was...her turn. And... my turn too. My turn to begin the journey of losing my mother. Her turn. My turn. Mom was transferred to a rehabilitation hospital to begin occupational therapy, physical therapy, and speech therapy. It was our effort to try to recapture and rehabilitate. I knew that part of my mom was gone, and I was grieving the loss. Ten years ago, I wrote a short book of family memories. On a recent visit to the rehab hospital, I read many of the vignettes to Mom. She could remember the stories of our childhood and she was delighted to hear the tales once again. One afternoon, Mom asked me to 22

tell her another story. I complied. I read a short vignette about the times she told me bedtime stories. When I had finished reading, I looked up and Mom had fallen asleep..... Roles had reversed. My turn.
In sorting through some of Mom’s things in her home, I came upon a newspaper clipping that she’d cut out. Maybe she saved it because of some sort of premonition. Maybe she knew that her turn was coming. It reads:
To My Children: When I spill some food on my nice clean dress Or maybe forget to tie my shoe Please be patient and perhaps reminisce About the many hours I spent with you. When I taught you how to eat with care Plus tying laces and your numbers, too, Dressing yourself and combing your hair, Those were precious hours spent with you. So when I forget what I was about to say, Just give me a minute — or maybe two. It probably wasn’t important anyway, And I would much rather listen just to you. If I tell the story one more time, And you know the ending through and through, Please remember your first nursery rhyme When I said it a hundred times with you. N
ow that the baby boomers have hit 60, aging is being re-packaged into something other than nursing homes and death sentences.
Commercials now show silver-maned seniors playing golf at retirement centers that look like vacation resorts. All this talk about aging is getting old. I do not intend to go gently into that good night. I plan to go screaming and kicking on the way out just as I came in. I may be getting older, but, hopefully, wisdom comes with age. Providing I can remember what I’ve learned. Since turning 60 all I seem to retain is water. I tell myself it’s no big deal. I tell myself it’s just another birthday. I tell myself 60 is the new 50. I tell myself I should not be talking to myself so much. (Warning, Will Robinson, Warning!) Yes, it is hard to believe I am 60, rapidly approaching 61, but I slowly surrender to the idea since there are so many adjustments I have had to make that start when I look in the mirror. I don’t know why aging comes as such a shock because the reminders are anything but subtle. The spots on my skin that were once freckles have suddenly darkened, widened, and become what I remember my grandmother calling “age spots.” The “power-walking” I did so religiously in my forties has turned into something powerfully less like 24

walking and more like strolling — and more often in the direction of the refrigerator. I could look at 60 and see all the negative adjectives it evokes. The words senile, slowdown, sluggish, and senior are only a few that come to mind. I decide instead to concentrate on the positive descriptions such as significant, strong, spirited, and showstopper, and I feel myself start to shape-up. Sixty, like any age, is what you make it. You can choose to shrink or shut yourself off, or you can choose to shine and show yourself off. Sixty can be a step up in your life as a time for strengthening your mind, body, and spirit. Sixty can be the time for a new strategy. Surrender your fears and set your sights on spontaneous life experiences you never had time for or talked yourself out of for whatever reasons. Sixty is the time to be straight-forward in your thinking. It is the time to do those things you thought you could not do or were too afraid to try. Do I smell “bucket list?” Of course, who do I think I’m kidding? Anyone who says getting older isn’t so bad never watched television. Television is strictly youth-oriented. No one on TV is under 40 unless he’s on a commercial for bran flakes or erectile dysfunction, or a movie of the week about Alzheimer’s disease. Reality shows such as “The Bachelor” or “America’s Next Top Model” are about gorgeous young people. If they made a reality show about a nursing home, maybe they could call it “Lost.” I’m wait-
ing for a reality show about senior citizens called “America’s Next Top Octogenarian.” Now that would be a show worth watching. The older people in our society (me included) have so much to say, if only someone would be willing to listen. I guess since I’m so much older and wiser, I should be happy to reach such a pinnacle, since it definitely beats the alternative. As in the Bee Gee’s hit from the ’70s, I should set my sights on “Stayin’ Alive” as my theme song. All of a sudden, I realize I’m putting my pajamas on earlier every winter, just as my mother did. By next winter, I’ll probably be cooking dinner in them. By the time my husband and I reach retirement, we’ll probably be too exhausted from the rigors of middle age to enjoy it. All of our friends are enjoying their grandchildren. If my sons don’t marry soon, I won’t have strength enough to hold my own grandchildren. In my frustration, I used to tell my boys I hoped they’d have a dozen kids to make their lives a living hell. Now I’m just scared I’ll have to babysit for them. I really don’t know what I’m complaining about. If I live long enough for my sons to have children, there’s a good chance I won’t recognize them anyway. Of course, by that time my sons will be middle-aged themselves and wondering how they got there.

Kentucky Opera concludes its 2011/12 Season with Franz Lehár’s romantic story set in the glittering world of 20th century Paris, where a wealthy widow looks for love — ultimately finding it with a former flame while also keeping her country solvent. Victor Léon and Leo Stein based their libretto on the Henri Meilhac play, L’attaché d’ambassade (The Embassy Attaché). The production made its world premiere in Vienna in December of 1905 and subsequently enjoyed great international acclaim. Last performed by Kentucky Opera in 1985, the operetta is in three acts, and is performed in English. Make sure an empty home is secure By Lisa M. Petsche
I
t may happen unexpectedly: your parent or other senior relative who lives alone must spend a period of time in a hospital or a rehabilitation center, or convalescing in a care facility. If
he or she (for simplicity, the latter will be used here) is a house dweller, looking after her home and its contents can be a complex task. Read on for valuable tips on how to keep her property safe during her absence. Indoor security measures • Remove any keys hanging by the door or stashed in a drawer. • Ensure all points of entry are secured. Place a snugfitting piece of wood in the bottom of sliding window and glass door tracks to increase security. Disconnect the automatic garage door opener and manually lock garage doors. • Arrange for mail to be redirected and suspend or cancel newspaper delivery. • Notify a trusted neighbor that your relative is away and ask him to watch for suspicious activity and call police if necessary. Ensure this person knows how to reach you should any problems arise. Also ask him to do one or more of the following to make the house appear occupied: remove flyers from the mailbox and porch; park a vehicle in the driveway, or brush off your relative’s car and make footprints to the door after a snowfall; place a bag of garbage at the curb on waste pickup day. • Keep some blinds open, and put indoor lights on timers. • Disconnect the answering machine. • Unplug electrical items that are susceptible to power surges. • Catalog, photograph, or videotape room contents. Record the serial numbers of valuable items such as televisions, stereos, and VCRs. Remove smaller valuables and store them in a bank safety deposit box. • Review your relative’s homeowner’s insurance policy to determine whether it provides coverage while the home is unoccupied for an extended time, and if so, under what conditions. Ensure any conditions are met. • Perform regular checks for pests, ruptured pipes, water leakage, and other problems, especially after storms and during extreme weather.

Outdoors • Remove spare keys hidden under mats or elsewhere outside. • Lock tools and ladders in the garage or a shed so they’re not accessible to thieves. • Install timers or put photosensitive bulbs in outdoor lights. • Perform regular perimeter checks for signs of forced entry. • If you’re not able to perform outdoor maintenance tasks, enlist the help of a reliable neighbor or contract with a reputable yard maintenance service. Another option is to hire a property manager to regularly inspect the place inside and out and arrange grounds keeping. Vehicle security • Ensure windows are closed tight and doors are locked. • Remove registration and insurance information, as well as hidden keys. Also remove anything of value. • Record the license number as well as the color, year, make, and model of the vehicle, and any visible damage. • Lock the vehicle in the garage. If it must remain in the driveway or on the street, get a security device such as an electronic alarm, kill switch, or steering wheel lock. Further considerations • Remove refrigerator contents that can spoil and any other perishable foods in the kitchen. • Store opened dry goods such as sugar and flour in sealed containers or place them in the fridge. Ensure kitchen and dining areas are free of food residue. • If your relative has a pet and you’re not able to care for it, find a friend or neighbor who will take it in. Otherwise, ask the local humane society if there’s a volunteer pet fostering program in the area; arrange for a boarding facility; or hire a pet sitter to either make daily visits or stay in the house. • Take home plants or distribute them among family and friends if your relative has an abundance of them. • Turn off the water supply to the house if regular lawn and garden watering is not part of the maintenance plan. • Close chimney flues, which can be a point of entry for birds and small animals. My mother is 79, and used to be very alert and stayed busy. She prided herself on being a well controlled, non-insulin dependent diabetic on oral meds. However, she no longer pays attention to her appearance, nor demonstrates her usual strict compliance with meal portions. She stays thirsty. She has stopped calling her friends and wants to sleep all the time. How can I best approach her so she realizes I am genuinely concerned without hurting her feelings? — Joan

Since your mother is diabetic she needs to discuss medical concerns with her physician. Elevated blood sugar can cause lethargy, extreme sleepiness, and constant thirst. So, finding out if and how she is testing and keeping track of her blood sugar readings is of prime importance. I suggest you call her to let your mother know you are bringing her favorite lunch over for the two of you to enjoy and visit. During your visit, after lunch, or whenever you feel the timing is right, share your observations and concerns in an effort to obtain an accurate account of how or if she perceives the same symptoms. Some questions you may consider asking are: • Has she been feeling okay lately or is something on her mind? Is she sleeping okay? • Go over her blood sugar testing records for the past 90 days. • Discuss abnormal blood sugar results to see how she is managing them. • Ask how she feels physically if her blood sugar is too high or too low and what she does about it? • Observe when she tests her blood sugar in order to monitor her technique for accuracy. • Glucometers — Some test machines, depending on the brand, require users to ‘calibrate’ one strip inside each freshly opened can of test strips before testing actual blood sugar. If her results seem “off,” ask her if she knows how to ‘calibrate’ one special strip or not when she opens a new container, and if so, does she always calibrate it? • Make sure she knows to use the lancet holder pens because they hurt less. • Depression is another concern, especially when diabetics become non-compliant with their diet, medications, and normal diabetic regimen. Seasonal Affective Disorder (S.A.D.) affects many during the winter months and is a very real diagnosis affecting all ages — especially the elderly.

New Community, More Jobs Masonic Homes of Kentucky’s Louisville campus will have a new addition coming next fall. Miralea is a 3-story Independent Living Life Care Community with 90 apartment residences and 12 patio homes. Common areas will include a library, living room, private dining room, Fleur de les Restaurant, wellness/fitness center, natatorium, creative arts studio, business center, media room, and court yards. Miralea residents will have access to all levels of care on Masonic Homes’ campus at a discounted rate. Seventy-five new hospitality jobs will be created with this new facility.
