s an expectant mother of multiples, you’re entering into a sisterhood bound by the unparalleled joys — and challenges — of raising twins, triplets or even quads. But if you’re also planning to breastfeed your babies, consider yourself among an even more select group of new mothers. Breastfeeding multiples — whether partially or exclusively — isn’t easy, but it is certainly possible. Knowing how to best care for your post-partum body through diet and exercise can help improve the likelihood of success in meeting your breastfeeding goals.

Eat up!

If you thought your multiples pregnancy was a calorie-fest, then welcome to lactation! Exclusive breastfeeding requires far more than you needed during pregnancy — at least 500 extra calories per day…per baby! As eager as you may be to lose your pregnancy weight, breastfeeding is not the time to follow a restrictive, low calorie diet. Research shows that low calorie diets often result in reduced milk supply — a particular concern for nursing moms with more than one mouth to feed. Besides, breastfeeding multiple babies is probably the only opportunity you’ll have in your life to eat anything you want… and still lose weight without trying! Many expectant mamas plan ahead so that they won’t need to worry about food once their babies arrive. For example, you can batch cook and freeze hearty meals like stews, chili, lasagnas and casseroles so that a quick, nutritious meal is always

on hand. Some parents create online meal registries, like Mealbaby or Mealtrain, to help friends and family coordinate meal delivery for those hectic first few months. Using grocery delivery services is also a great way to keep the fridge stocked with nutritious staples without having to go food shopping. When thinking about pantry staples, consider foods that can be eaten one-handed or with a spoon — like yogurt, peanut butter sandwiches, thick stews and oatmeal. After all, chances are you’ll be holding a baby when it comes time to eat!

Stay hydrated. Breastfeeding

infants drink 25-30 ounces daily, so nursing twins means that about seven cups of fluid will exit your breasts daily (scale that up if you’re breastfeeding triplets!). This fluid requirement is in addition to the several cups per day you require to meet your own body’s needs. In other words, if you’re not eating or sleeping, you probably should be sipping. All fluids count toward meeting your needs — including coffee, tea, broths and soups. Even watery foods like watermelon, Jell-O, popsicles and cooked cereals will make a contribution. Try keeping a pitcher or large refillable water bottle next to your preferred nursing station so you can sip on liquids while feeding your babies.

Beware of unnecessary diet restrictions. Busy new moms

have a hard enough time feeding themselves, so adding unnecessary diet restrictions to the mix just makes things more difficult. Contrary to Internet lore, avoiding certain multiplicity

foods during lactation does not help prevent food allergies in babies (and in fact, may have the opposite effect), nor will it improve gassiness or discomfort in colicky babies. Newborn babies are gassy, and this is a function of their immature digestive systems, not the result of anything in the mother’s diet. “Gassy” foods like beans and broccoli — or even spicy foods or dairy — have no bearing on a healthy newborn’s digestive function. Breastmilk is generated onthe-spot in a woman’s breasts, and there is no mechanism by which gas generated in mom’s own intestines from her diet can travel through the blood, into her mammary glands and transfer itself into baby’s milk! One exception to the rule is in cases where babies have an actual allergy to a food in mom’s diet (most likely cow’s milk), a situation that affects a small minority of infants. In such cases, symptoms are more likely to be rashes, vomiting or reflux, diarrhea or failure to gain weight. There are no benefits to avoiding spicy or pungent foods if you enjoy them, either. While compounds from foods in mom’s diet may affect the flavor of breastmilk, this phenomenon may actually facilitate acclimation and acceptance to the family’s usual diet by infants with a developing palate. So indulge in all the asparagus, wasabi, garlic and Indian food you like!

Safety first.

Virtually all of the food safety restrictions from pregnancy are relaxed during lactation: favorites like

sushi, deli meats, young cheeses and coffee are all safe to enjoy again. However, a small handful of safety-related considerations persist during lactation due to the potential effects of certain food-borne compounds on a baby’s developing nervous system. These include alcohol, mercury and endocrine-disrupting chemicals like Bisphenol-A (BPA). One to two alcoholic drinks per week is considered safe while lactating, but experts advise limiting intake to a single drink in a sitting and allowing a full two hours to pass between drinking and nursing. Also, be aware that even trace amounts of alcohol in breastmilk can interfere with babies’ sleep, and newborns are more sensitive to alcohol exposure than older infants and toddlers. To help protect nervous system development, limiting your intake of high mercury fish — as you did during pregnancy — remains prudent. If you buy canned foods like soups, beans, tuna and tomatoes, look for brands that use BPA-free can liners, including Eden Organics, Muir Glen, Amy’s Kitchen, Native Forest, Wild Planet and Vital Choice. Products sold in aseptic tetra paks are also BPA-free and safe to use during lactation.

Ease into exercise.

Producing over 1,000 calories worth of milk daily, you’ll burn more by breastfeeding than you could ever expect to walk off on a treadmill. That’s why most nursing moms of multiples lose a substantial amount of baby weight without ever donning a pair of sneakers (let alone a bra!). That’s a good thing, since breastfeeding multiples tends to be a round-the-clock enterprise for the first several months, one that leaves little time and energy for the gym. As your babies grow and the demands of nursing subside, you may find yourself ready to get moving again.

Similarly, poor hydration from excessive sweating can also reduce the amount of breastmilk you produce. For these reasons, light physical activity that focuses on fitness, strength and stress relief (i.e. walking, yoga or pilates) may be more compatible with your demanding lactation regimen than intense workouts like running, interval training and spinning. This rings true at least in the first several months of breastfeeding before your supply is well-established. It’s a huge challenge to prioritize self-care when faced with the overwhelming task of caring for your newborns, so enlist help from your partner, relatives and friends in keeping yourself fed, watered and rested. Raising multiples is a marathon — not a sprint — so consider taking care of you as a form of endurance training! ags packed, heading to the hospital today. Your twins will be born and you’ll have the support and resources of the hospital while there, but what can you expect once you bring them home? There are some things you can do to make your first few days home more manageable. I found that lots of lists, charts, advice from books, and some simple organizing made our first weeks and months a little easier. Once you are home with your little ones, you will realize now you have two newborns to care for. In combination with your lack of sleep, your memory will be put to the test. You will need to use a notebook, printable chart and a computer, ipad or iphone app to keep track of feedings (amounts and times), diapers (wets or dirties), sleeping, or any questions you may have for the pediatrician at the next visit. My husband and I used printed charts he made, but many websites have downloadable charts, as well. I have heard a few MOMs recommend apps too, like iBabyLog & the free Similac Baby Journal app. The main thing is you will need to be able to bring these charts with you to your doctor appointments so that you can refer to them when asking and multiplicity

answering questions for the pediatrician.

enlist help and never say “no”

Hopefully, the first few days or weeks you can enlist help from family, friends or local church members. Don’t ever say “no” because you feel the need to do it all. You can’t do it all and you should take trusted people up on every offer they make to help. They can be crucial help with meals, chores and just getting into a routine. Enlist a friend to solicit people who plan on helping with meals or use meal scheduling websites so you know you are covered for dinners for at least the first week. It is important to let everyone know allergies, diet needs, like diabetes, and dislikes in advance. In the event you recently moved, I suggest stocking your freezer with a few microwave or frozen ready-to-bake meals to get you through one or two weeks. If you have family moving in temporarily or friends offering to help clean, make a chore list similar to the one you can find on Twiniversity’s webpage along with specific instructions on how to use your washer, dryer, bottle steamer, etc. I found being unprepared limited my help on what they could do for me. Extra hands can help sterilize bottles, do laundry (minus the underwear if your hubby is modest!), put laundry away, help care for older children or pets, etc. This is a perfect scenario and what we all as twin moms would love. However, you may encounter the person who is less than helpful. Do not feel at all obligated to entertain them the first few weeks after your kids come home. Get your husband involved if necessary because two crying newborns can be stressful without the thought of keeping the house tidy. In all honesty, your partner can be the most helpful and hopefully, they can take some time from work the first few weeks to help out. Make sure you both understand the feed/ pee/poo/sleep charts, that they are familiar with the changing station or you have labeled everything for them, and you both know what schedule you are hoping to get the twins on. When my husband was home, we tag-teamed it; he took care of one then I did the other and we switched the next time so that we got bonding time with both. I believe a dad can only have that true bond with their kids when 16

they are gagging along with you changing those wonderful newborn diapers. Ha! Those diaper changing times are when you have just one baby to reflect on and it is special. Just remember: you and your partner are a team; communication is key. If you need a break, don’t assume your partner knows; you have to be honest and tell them when you need help.

set a schedule

Schedules are a big deal, so you may want to consider learning about how to begin. It helps you as parents get a better feel for what comes next with them, and for scheduling the rest of your life outside of babies. Obviously, it’s much easier to feed two at a time, have them sleeping at the same time, etc. Putting the babies on a schedule will take some time, as they often need to eat on demand in the beginning to gain much-needed weight, and it may even take them a bit to get their sleeping cycle adjusted (day vs. night). For us, we put our twins on a routine of every two hours needing to be changed, fed and put back to sleep, but there will always be times your routine gets changed. Schedules for us changed when each of us returned to work. However, the biggest challenge on our routine was severe acid reflux and lactose intolerance in both babies. There is nothing like a projectile vomit or a high fever to throw you off schedule. This is where books, WebMd.com and nurse friends were lifesaving to me. I loved What to Expect When You’re Expecting and Baby 411, as those two books were highlighted and flagged and kept me from making a lot of calls to the pediatrician’s office when unnecessary. Lucky for you, now you can get a copy of Twiniversity’s twin-specific book What to Do When You’re Having Two, full of helpful information on tandem breastfeeding, NICU information, sleep and feeding schedules, how to register for twins, what you really need when they arrive, etc. I encourage you to use whatever resources you have, but if your mommy instinct says something is wrong, call your doctor as soon as possible, ask your MOM group, or take your little one(s) to the ER. Once your little ones are feeling better, try to get them back on schedule as quickly as possible. Other schedule changers are when you will have

those visits from friends, family and sickness. The first few weeks, your twins are still building up their immunity systems. I personally limited visitors for three months because they were born in flu and RSV season, but this is not always an option. Ask friends and family to not come over if they are sick or chose to not vaccinate. If they come anyway, either politely turn them away or offer a paper mask and hand sanitizer. Yes, you will get crazy looks and you possibly may offend someone, but at the end of the day, you will be the one taking care of a sick newborn all night long. Because we had winter babies, we washed everything given to the twins the first few months to limit exposure to sickness. When we were able to take them to the store we used carseat covers to limit strangers from feeling the need to touch them. We used hand sanitizer ourselves after leaving the stores and doctor offices. Again, this may seem extreme, but I like sleep too much to be rocking a sick baby all night long.

prioritize & include the family

Another biggie when bringing the babies home is how much space you have for all of the things you’ll need on hand to either entertain, pacify or just plain live with two babies. You’ll need to consider whether you’re getting two of everything or if one of each will suffice. Of course, budget always plays a role in this, too, and honestly, it’s whatever works best for your family. Many families get by with just one swing and even have the babies share a crib. If you have older children or pets at home, you’ll want to introduce them to the new babies slowly. Two new additions will take some getting used to, but having your older children help where they can based on age and level of responsibility (i.e. fetching diapers and bottles, or helping feed, etc.) will certainly make the transition go much more smoothly.

keep your chin up & take care of yourself

The most important thing to remember is you are human; you are not perfect. Things will happen, the twins will get sick eventually or be fussy for no known reason, and sometimes you will need to put them in their crib and walk away for a moment. My husband and I found our best routine was change them, feed them, swaddle them, comfort and rock, give a pacifier, repeat. If they did not stop crying we tried putting them in the swing or laying them in their cribs and walking away. You can use a video monitor to keep an eye on them. Take care of yourself through these first weeks. Take breaks as needed, ask for help, eat three meals a day and try to rest when the babies rest. After all, you just gave birth to two precious babies and that is hard work. The first few weeks may be difficult, but are just the beginning of a wonderful new phase of your life! multiplicity

here is something about having twins. They fascinate people. They cause complete strangers to stop, stare, ask questions, and say the most preposterous things to you. There is no escaping it. As a parent of multiples, you will forevermore be treated to “20 Questions,” each time you venture out with your children. So have a little fun with it...if only in your head! In the two years since I became a mother to twin boys, I have probably heard it all. Some questions and comments are kind, some are humorous, and some are downright ridiculous. I have thought up my own witty, sarcastic responses to give in these circumstances, things I would probably never say aloud. Nevertheless, thinking them helps me to have a little fun with our constant celebrity status because as a parent of multiples, you just became

something of a celebrity, too. Here are just a few things you’re likely to hear along the way:

Are they twins?

I have always dreamed of answering this with a sassy response, “no, I just found the second one in the parking lot and thought I would take him shopping with us.” The reality is I have two children who are the same size and look remarkably similar. Do you really need to ask me if they are twins? Yet you will probably hear it everywhere you go and that only begins the trip down the rabbit hole. The questions typically get more complicated and more personal from there!

Double trouble!

While I sometimes jokingly refer to my boys as “double trouble,” I find it rather presumptuous

coming from a stranger. It doesn’t seem fair that my children should automatically be labeled as troublemakers, particularly when they behave well in public. When I hear this, I respond along the lines of, “nope, double blessings.”

Boy, you have your hands full!

Seeing as how my twins also have a brother who is 22 months older, I hear this one every time we head out. In my head, I say, “Well let’s see…I have three children and only two hands…so based on simple arithmetic, yup, my hands are very full!” In reality, I just laugh and tell them my hands are full of love.

Are they identical?

Some questions truly make me wonder what is being taught in high school biology classes these days. My boys happen to be identical, but I know countless other moms who have boy/girl twins and still hear this question. All I can think is the inquirer was playing hooky the day DNA and X and Y-chromosomes were discussed. If you feel up to giving a brief science and anatomy lesson, I say go for it. It certainly couldn’t hurt!

Did you plan to have twins?

“Yes, I had a long, hard talk with my body to make sure it knew I wanted to grow two babies at once.” I am guessing this question also originates from a lack of a good science education. If you have the desire and the time, feel free to answer with a quick explanation of how twins come to be. Or if you are like me and always in a hurry, it is okay to just say no, smile, and be on your way.

Are they natural?

We all know what they mean by this question; they want to know if some sort of fertility treatment was involved with the conception of your children. What they do not realize is that by asking you this, they are asking a very personal question that is none of their business. Moreover, by using the term “natural,” they are insinuating that children conceived through reproductive assistance are somehow “unnatural,” which could not be further from the truth. I usually politely tell them my twins were spontaneous, and explaining why “spontaneous” is a more tactful term than “natural.”

My two kids is like having twins.

No, it is not. Trust me, it is not. “You didn’t watch your body stretch to the brinks to afford room for multiplicity

two. You did not have to figure out how to soothe two screaming newborns at the same time. You did not go months without sleep while your babies were on different schedules, and I seriously doubt you’ve ever had two diaper blowouts at the exact same moment.” That’s what I want to say. But let’s face it, only those of us who have been through the newborn phase of multiples understand just how hard it really is. And that’s okay.

Who is older?

The response in my head is always, “why does it matter?” My boys were born five minutes apart. Did my one son gain some sort of amazing life experience in those extra five minutes of breathing oxygen before his brother arrived? Somehow, I think not. However, for whatever reason, inquiring minds want to know.

I guess you are done now.

“Why thank you perfect stranger for doing my family planning for me.” I get this comment about as often as I am asked if we will try for a girl next. I really don’t understand why a stranger feels the need to share their opinion on the size of my family. I would imagine moms with a boy/girl twin set get similar statements since they have one of each.

Better you than me.

This statement almost sounds like an insult, as if I did something wrong and am being punished by having twins. The reality is just the opposite. My twins are a blessing. While on the outside I will smile and nod, on the inside I am thinking that it is better me because I can truly appreciate this amazing double gift that I have been given. As a new parent of multiples, you will likely hear these questions and comments more times than you care to. For better or worse, it comes with the territory. Handle these strangers in the way that feels best to you, whether you answer them honestly or with a little humor and fun. They think they have the right to ask intimate details about your life and family choices, but you always have the right to smile and politely walk away. When you find out you’re having twins, the rules change. Heck, it’s safe to say the game changes. It really takes your breath away and makes you reexamine everything you thought was true about the life of a future parent.

So, many crazy things happen to you when you have twins. It’s truly remarkable to think that people go out of their way to give you special treatment, and you’ll be baffled by the fascination (and the ignorance) that people have when staring at two (or more) children that share the same birthday. My experience motivates me to offer some tips because they are important pieces of advice. Some are funny, some not so much, but many will make life just a little easier for parents expecting twins. I don’t hold these truths to be self-evident (if you don’t know your history, Thomas Jefferson started the Declaration of Independence that way…not like you care), but I do feel as though they will be helpful. To be fair, this list mostly pertains to twins under the age of one. Mine are over two-and-a-half now, so don’t ask me for advice on toddlers yet. Without further ado… 1. You will likely keep the kids on the same schedule as much as possible. 2. You will not let the schedule dictate your life…your twins are still individuals. 3. You will NOT wake a sleeping baby unless you are feeding at night…do this very quietly. 4. As much as possible, you will not refer to your children as “the twins”. 5. When in doubt, feed the babies. 6. You will accept help from anyone you trust. You can pretend to do it all by yourself, but when you have hit the bottom, you will ask, and gladly accept. 7. You will accept diapers/gifts from anyone and everyone. They will buy you more because you’re having two babies, and you will store it away and find a way to use it or apply it to something you need. 8. You will not be picky when choosing colors of strollers and high-chairs. 9. You will not feel guilt when you have to miss work; your kids will be sick often the first couple of years and few understand the plight of a twin parent. 10. You will have Goldfish or Cheerios with you AT ALL TIMES. 11. You will always have at least four diapers in the bag AT ALL TIMES. 12. When alone with your twins, any place makes a good changing table.

13. You will have a supply of Tylenol at home and in the bag AT ALL TIMES.

14. You will not care about the brand of diapers when you reach for one in a time of need.

31. You will hate bath time, then you will love it. Then you will hate it again, but the twins will love it. Get used to this love/hate relationship.

15. You will accept checks from formula companies to receive discounts on formula, and you will wait at the register until the cashier rings it up properly.

32. You will not eat the puffs that are designed for babies. They are gross, but your twins will like them a lot.

16. You can use boppy pillows for feeding, at times. If you’re alone, and they get hungry, you’ll understand everything I’m saying.

33. You will have weird dreams; it comes with the lack of sleep and heightened awareness. 34. You will own a calming noise machine. This will last much longer than you anticipate and don’t feel guilty if you must borrow it for yourself from time to time.

17. You will sing James Taylor songs to rock them to sleep (what were you going to sing, Ke$ha?). 18. You will face tantrums during toddlerhood, and you will want a strategy for those difficult days (on weekends, a 45-minute drive does wonders).

35. You will not forget your significant other. There’s a reason you “ended up” with twins.

19. You will not, under any circumstances, underestimate the value of Goldfish.

36. You will hear others tell you to “sleep when the babies sleep”. You will then laugh at them, because it’s not that easy.

20. You will not feel shame in hauling a red wagon through a shopping mall, or a food truck rodeo, or a farm.

37. You will celebrate joyously when they turn one and the doctor tells you to switch to cow’s milk.

21. You will buy a thermometer that takes temperature by reading it through the temple. Unless, of course, you insist on doing it other ways…good luck with that.

38. You will buy lots of milk (see #37). 39. You will read to them, even when they appear not to be listening; they will surprise you one day.

22. You will dress your twins alike regardless of how different they may look, that is, until they start school. Or if they are boy/girl (obviously).

40. You will laugh at teething toys, teething necklaces, teething tablets, etc. --- until they start teething. You will cling to anything that will make them stop crying.

23. If you have boy/girl twins and someone at Walmart (because that’s always where it happens) asks you if they are identical, you will remember they know nothing about twins and tell them they are different in one very important way.

41. You will find yourself angry, frustrated, tired, irritated, annoyed, frazzled, clueless, and sometimes feeling hopeless. This will pass, and your babies will get older. They will talk, they will crawl, they will walk, and they will interact with each other in ways that “normal” siblings cannot. They will share a bond that they will take with them throughout the rest of their lives and it will be awesome to witness and foster. And you...you will be a hero. You were, and are, the parent of twins.

24. You will keep their birth “tags” on for at least a week because, well, who wants to mix up babies? 25. You will love bouncy seats. That is all. 26. a delivery hospital with the highest level NICU available, if at all feasible. Otherwise you may be separated from your babies when they are rushed to a higher level NICU hospital than where you delivered. These hospitals typically offer multiple moms the opportunity to tour not only the Labor and Delivery floor, but also the NICU. No mom wishes to have her babies in the NICU, but some preparation for the possibility of it happening is quite important.

the nicu stay things every parent of multiples should know by susan rassette

l

earning that you are about to become the mom to multiple babies is a true blessing, but it can also be a scary prospect. More than half of all multiples are born before their due date. This may mean that your babies arrive just a few weeks early, as is typically recommended by doctors. The placenta can begin to break down early and not work well for the full 40 weeks of a multiple birth pregnancy. Many doctors consider 38 weeks full term for multiples. However, some multiples can arrive dangerously early. The typically accepted threshold for viability is 24 weeks. This is the first goal, although babies born this early are at great risk of a long-term NICU stay, long-term developmental issues, or even death. The next goal is 28 weeks. Babies born at 28 weeks still face the risk of long-term complications, but typically do much better with a 90% survival rate. Babies born between 32-34 weeks have an excellent chance of survival without suffering any long-term complications, but they will spend time in the NICU learning to feed and grow. After 34 weeks, babies do very well and may require little to no NICU time. The NICU can be a scary place. But a multiple mom should choose

Some multiple moms have difficult pregnancies and know that a NICU stay will likely be in their babies’ future. While a high risk pregnancy is scary, it gives moms time to prepare and plan for a potential stay (especially if the mom spends extended amounts of time on bed rest). If you find yourself stuck on bed rest, use the time to research, plan, and be as prepared and knowledgeable about the NICU as possible. Some helpful books are: • Preemies – Second Edition: The Essential Guide for Parents of Premature Babies • The Premature Baby Book: Everything You Need to Know About Your Premature Baby from Birth to Age One Regardless of how your babies end up in the NICU, take advantage of the help that it can offer. There should be a Social Worker assigned to you, and they will likely meet with you very early in your babies’ NICU stay, or even prior to your babies being born if you are admitted and expected to deliver premature babies imminently. The Social Worker is an invaluable resource who can help you and your family physically, financially, and emotionally. The care that you can take part


in with your premature babies will depend upon the gestation at which they are born. Try to take part in all the care that the hospital will allow. Change diapers, give sponge baths, feed them, and take the opportunity to touch your babies whenever you can. Kangaroo Care, which is the act of holding your baby against your chest, skin to skin, has been proven to be beneficial in many ways, including: • Maintaining the baby’s temperature • Regulating the baby’s heart and breathing rates • Helping the baby gain weight • Helping the baby have an easier time learning to breastfeed The special circumstance of multiples allows for one more great option in the NICU --- co-bedding your babies. Many NICUs will allow, and even encourage, that multiples be co-bedded in the same isolette once they are stable and infection free. They were womb mates, and many studies have shown that co-bedding your multiples is as beneficial as Kangaroo Care. The NICU experience will have its ups and downs. Many

parents find it helpful to keep a NICU journal, blog, or care page about the experience. The care page or blog is very beneficial to keep others updated on your babies’ progress without having to provide individual updates when all you really want to do is spend time with your babies. Try to speak with the neonatologist regularly, and document what they tell you so you can refer back to it. Ask questions if you do not understand, or ask the nurse if you think of a question once the doctor is gone. Most importantly, take any help people offer. Let your family and friends drive you, bring meals, care for your other children, etc. so you can go to the NICU. Eventually, the time will come when your babies will be ready to go home. This very well may not occur on the same day, and some NICUs will not allow the discharged infant to return back to the NICU. You will need to find someone to care for your baby at home while you visit the other(s). If your NICU allows you to bring your baby back in, take advantage of that. Your baby(ies) still in the NICU will benefit from being able to

co-bed with their siblings and it will also be helpful to you to have them back together. The NICU can be a difficult experience, but one you’ll appreciate later. Take advantage of all the resources the NICU has. The Social Worker and nurses are there to help. Even when your babies make their way home, you may still need to deal with some long-term prematurity issues. Again, the NICU can make helpful recommendations and assist you with on-going therapies that may be necessary for your premature baby(ies). Lastly, remember to thank the NICU staff for the care they provide. They work hard for you and your babies and deserve some recognition for helping them survive those first few rocky weeks/months of life. Susan is the mother of fraternal twin girls, age 7.
