LIFESTYLES CHRONIC HEALTH CONDITION PREVENTION PROGRAMS Launched in 2020, both the CDC’s year-long National Diabetes Prevention Program (DPP) and the 10-Week Weight Management platform (powered by Naturally Slim) have shown that engaged participants reduce their risk for chronic health conditions such as diabetes, stroke, and heart disease. Diabetes Prevention Program Two groups of DPP launched in February meeting weekly and rapidly moved online in March due to the COVID-19 pandemic. Since then, two additional DPP groups were launched in August with the following outcomes for 2020:
