A review of our residency graduates since 2013 indicates that about 67 percent of our residents pursue Post Graduate Year 2 (PGY-2) pharmacy residency or fellowship training. A majority of the remaining 33 percent go on to pursue a career in the acute care hospital setting. Many of our residents stay on as members of the Upstate pharmacy family with a few pursuing academia immediately after graduation. SUNY Upstate Medical University, one of 140 academic medical centers in the United States, attracts outstanding Doctor of Pharmacy graduates from across the nation. Each pharmacy residency program draws upon the resources of our academic medical center – including a teaching hospital, four health related colleges (the College of Medicine, the College of Graduate Studies, the College of Health Professions and the College of Nursing), our research enterprise and hundreds of specialty services. Each program, accredited by (or in precandidate status with) the System Pharmacists (ASHP), offers pharmacy residents the opportunity to interact with many other disciplines. Residents are given training options within the scope of excellent pharmacy practice to pursue their interests and refine their knowledge and skills. Each resident will have the opportunity to customize a training plan based on his/her area of interest.

Within our institution the integrated role of our pharmacists has been well recognized for decades. Our residency program, and the talented individuals who make it up, have been central to this success. Our residents are often identified for their excellence in research, teaching and clinical skills during their training and are viewed as essential members of the care team. Since its inception in 1999, we have steadily expanded our program to include PGY-2 pharmacy residency programs in key clinical areas such as; Pediatrics, Ambulatory Care, Infectious Diseases and Geriatrics. We have also increased our PGY-1 program capacity while preserving a well-versed training experience involving close mentorship and guidance from a collection of experienced and innovative preceptors. Many of our preceptors are veterans of our PGY-1 and PGY-2 programs bringing a unique perspective and continual improvements to the experience. Our residents’ involvement in medication management and advanced clinical pharmacy services more than adequately prepares them for success in PGY-2 residencies, fellowship or establishing themselves as practitioners. As executive director of pharmacy at Upstate, it is my great pleasure to continue supporting and advancing the development of future pharmacy leaders through our residency training.”
Our residency training at Upstate University Hospital provides both PGY-1 and PGY-2 pharmacy residents the opportunity to develop proficiency in research, manuscript writing and the publication process. With the introduction of our innovative Pharmacy Clinical Research Micro-credential program, our residents have the opportunity to gain a strong foundational knowledge in research. This program includes 26 hours of didactic lecture sessions presented over a 10month period. The residents will complete a research project, write a manuscript and submit the it to a peerreview journal for publication after it has been reviewed and approved by the research committee. All residents, both PGY-1 and PGY-2 are required to complete this program. A highlight of the subject areas discussed includes: regulatory and ethical issues in research; understanding the basics of statistics and statistical software; presenting a research projects; and finally completing a research project. The various lectures are offered to meet the needs of the residents during the research development process. Details of our research training program is available through our pharmacy residency website at upstate.edu/pharmacy. This innovative program is also participated virtually by seven pharmacy residents from other PGY-1 Pharmacy residency programs. Currently, residents who complete the Pharmacy Clinical Microcredential program are offered a signed certificate. Work is far advanced to offer a digital badge to all graduates of the program. The Upstate pharmacy services translational research team (UPSTART), in collaboration with the Upstate pharmacy residency programs, have worked tirelessly to create a welcome environment for research and publication production. This welcome environment has motivated both pharmacists and pharmacy residents to embrace research as a vital learning experience and an important component of our residency training program. Our pharmacy residency research projects are designed to be publishable. Residents are assigned a research mentor/preceptor who is experienced in research and the publication process. These preceptors are very instrumental in the resident’s research experience and work together with the residents to ensure a successful completion of their research project(s). The residents become the primary authors of any research publication.

BCPS Preparatory Program A Board-Certified Pharmacotherapy Specialist (BCPS) preparatory program is now being offered to our residents. The purpose of the program is to familiarize residents with the exam content and format. Under guidance of board–certified preceptors, the residents will develop didactic and case-based lectures over the course of their residency training, covering a wide variety of topics on the exam. Personal preceptor experience with the exam, studying, and on-going certification will be discussed as well. The residents will gain the confidence and skills needed to independently prepare for the exam by the end of their residency.

Residency Program Structure and Design
The program begins with a two-week orientation that is anchored by an internal medicine based learning experience designed as a repeating, four-week rotation, alternating with fourweek, core learning experiences and one elective rotation. The program is designed to offer PGY-1 pharmacy residents the opportunity to progressively increase their independence in a professional, friendly environment in which they are able to become proficient pharmacy clinicians by the end of their residency training. Site: Upstate University Hospital Upstate University Hospital, the area's only Level-1 trauma center, has a centralized pharmacy distribution paired with numerous satellite offices. The decentralized pharmacy services cover every patient care area, including the Emergency Department. Upstate University Hospital also houses the Central New York Poison Control Center, which serves 54 counties. The Poison Control Center is run by PharmD clinical toxicologists and serves as one of our training sites.
The program structure follows ASHP accreditation standards and required competency areas, goals and objectives for PGY-1 pharmacy residency programs. The training experience is a 12-month , fulltime, curriculum designed to address the ASHP competency areas and individually tailored to aid in developing residents to become independent clinicians as well prepare them for their next endeavor, whether it be academia, fellowship, PGY-2, or patient care providers.

“The PGY-2 Ambulatory Care residency is a cosponsored program between Upstate Medical University and St. John Fisher University (SJF) –Wegmans School of Pharmacy (WSoP). The residency, which began in 2011, primarily focused on developing services in an Internal Medicine medical residency training site. However, over the last several years the outcomes achieved by the clinical pharmacist and residents have influenced expansion of collaborative pharmacist practice to other adult outpatient specialty areas, such as Inclusive Health Services, Neurology and Rheumatology. It is the philosophy of the program to develop a strong clinical practitioner with a foundation in primary care but also educate the resident on critical aspects of sustainability and the elements of developing a successful business plan. The partnership with WSoP enhances the resident experience by developing academic teaching skills and mentoring pharmacy students for future opportunities in pharmacy practice. The blend of academic and clinical pharmacy provides the resident with the skills and abilities to develop a practice in either setting. The role of academic pharmacy is to drive innovation and I believe our program accomplishes that. I look forward to continuing to pioneer innovative pharmacy practice models in ambulatory care.”

The Ambulatory Care pharmacist preceptors are embedded in hospital-based clinics. Core rotations include an emphasis on the management of diabetes, anticoagulation and disease states pertaining to the following specialties: Neurology and Pharmacogenomics, Rheumatology, Family Medicine, and Inclusive Health Services. Our focus is on the management of high-risk disease states and high-risk medication use. Our Internal Medicine clinic served as a pilot site for the Collaborative Drug Therapy Management (CDTM) practice in New York State (NYS) and provided data to support CDTM legislation in NYS. Our pharmacistdriven CDTM services include management of medications related to pharmacogenomic screening results, anticoagulation, diabetes and cardiovascular risk reduction.

Program Description Our PGY-2 Ambulatory Care Residency is offered in conjunction with St. John Fisher University-Wegmans School of Pharmacy (SJF-WSOP), in Rochester, NY. It is a 12-month program that builds upon the principles and experiences of a PGY-1 residency with additional expectations that the resident gain exposure, experience and independent skill in delivering pharmaceutical care services to ambulatory adult patients. Additionally, the joint venture with SJF-WSOP offers the resident the opportunity to develop and refine academic teaching skills. Upon completion of the residency, the pharmacist should be prepared to establish and maintain a practice-based clinic in the care of ambulatory adult patients and feel confident to pursue a career in academia if so desired.

The PGY-2 Ambulatory Care Pharmacy Residency at Upstate University Hospital is a joint venture with St. John Fisher University - Wegmans School of Pharmacy, where the resident serves as an adjunct preceptor and teacher. This collaboration gives the resident a well-rounded experience at multiple practice settings from academia to clinical practice.
The resident is exposed to practice management models focused on quality improvement initiatives that optimize patient outcomes and business practice models that educate the resident on the business of pharmacy and justification of pharmacy services. The resident is afforded the tools needed to independently establish a successful ambulatory care career at the completion of residency.

Scholarly Activity Residents analyze outcomes of existing programs and the design, implementation and assessment of an independent research question. The results of the investigation are expected to be published in the pharmacy literature. Elective Opportunities Residents can refine their learning experiences by choosing rotations in other ambulatory clinics such as advanced endocrinology and advanced specialty pharmacy practice with the specialty pharmacy team.

Upstate Golisano Children’s hospital is where I found my passion for becoming a pediatric clinical pharmacist and I am so honored to be able to continue my residency training here. Pursuing a residency at Upstate has been a dream of mine since my first introductory rotation as a student at Upstate. The PGY-2 infectious Diseases residency is designed to transition PGY-1 residency graduates from generalized practitioners to independent specialized practitioners with a focus on patients with infectious diseases. Residents will see patients with a variety of infectious diseases through their participation with the infectious diseases (ID) consult service, antimicrobial stewardship, and HIV clinic. Additional learning experiences include: critical care, pediatric infectious diseases, emergency department and laboratory.microbiologyInadditiontodirectpatientcare,theresidentwillcompletea research project, and build leadership skills by being involved in decision making pertaining to antibiotic use through the antibiotic subcommittee. Teaching opportunities are ample and include pharmacy rounds, ID grand rounds, didactic lectures, and coprecepting PGY-1 residents and APPE students.
Site: Upstate University Hospital Residents will have exposure to patients in inpatient areas including: ICUs, Emergency Department, and pediatrics as well as clinics affiliated with our Infectious Diseases group.

When first applying for a PGY-1 residency, I was drawn to the extensive opportunities offered at SUNY Upstate University Hospital including: clinical rotations of various pharmacy specialties, participating in interdisciplinary rounds at an academic medical center, creating my own research project with involvement in the research micro-credential, and educating pharmacy students through the teaching certificate program. I am very fortunate to have learned from such dedicated colleagues during my PGY-1 training here at Upstate, whose guidance and support taught me how to become an independent, wellrounded pharmacist. I had entered PGY-1 with interests in infectious diseases and academia, and I am so thankful to continue my residency training as the PGY-2 Infectious Diseases Pharmacy Resident here at Upstate. This PGY‐2 program in Geriatrics is a 12 month program that builds on Doctor of Pharmacy (Pharm.D.) education and the PGY-1 pharmacy residency program to contribute to the development of clinical pharmacist in the specialized area of geriatrics. The program fosters critical thinking, thereby enabling the resident to integrate their understanding of pharmacokinetics, pharmacodynamics and biomedical literature into an effective approach to optimize pharmacotherapy for geriatric patients. This program includes research in a sustainable improvementqualityproject or initiative for the populationgeriatricinapracticeareaof the resident’s Additionalchoice.researchopportunitiesareavailable based on level of interest and are fostered by both Upstate and SUNY Binghamton’s School of Pharmacy. These research projects are designed to be publishable. This PGY-2 residency provides the resident with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care of older patients or other advanced practice settings. Residents who successfully complete this accreditation eligible PGY2 pharmacy residency are prepared for advanced patient care, academic or other specialized positions, along with board certification in geriatrics.

Our geriatric program strives to help the senior patient from a pharmacotherapy perspective in many phases of care: from emergency room admission, stabilization of disease state, to outpatient management in specialized clinics. On a personal level, the relationships that develop between patient and provider make our interventions meaningful and professionally rewarding. I chose to pursue my PGY-2 training in Geriatrics at Upstate University Hospital knowing it would be an excellent match for my goals to become a boardcertified geriatric pharmacotherapy specialist with strong opportunities in emergency medicine, teaching, and leadership. During my interview process, I learned how the Geriatrics program at Upstate is a ‘powerhouse’ in that they are busily improving patient care for the geriatric population by thinking outside the box. I was struck by the sincerity and dedication displayed by everyone as well as the diverse presence of the preceptors, and the focus on collaboration.interprofessionalTheprogramoffers many unique opportunities that will enable me to become a well-rounded geriatric pharmacist including exposure to unique practice settings such as the Geriatric emergency medicine department and hematology/oncology,geriatricthe opportunity to complete the Clinical Research MicroCredential, and the Humanitarian Healthcare elective longitudinal. I appreciate how this program has a strong component of research projects that are designed to be publishable which will help me grow my research skills.
