Platinum Chain (Lid-Chain) – more flexibility for your patients The Lid-Chain is an implant, used to correct Lagophthalmos resulting from facial paralysis, specifically defects of the orbicularis oculi muscle. Various implants can be used to treat Lagophthalmos resulting from facial paralysis. Using gold implants, as first described by Illig in 1958, is a well-established procedure. When carried out correctly, this procedure can lead to complete lid closure without loss in the field of vision. The operation is technically simple, reversible without difficulty and where necessary it can be combined with other lid corrective procedures. This makes this method appropriate for cases of both reversible and non-reversible facial paralysis. Post-operative complications when using gold weight are not uncommon however (see side 4). Ultra-sound examinations of the upper lid tarsus brought to light the need for a flexible lid implant. The flexible platinum Lid-Chain was developed to reduce post-operative complications. The advantages of using platinum can be found in several clinical studies. Along with the better tissue compatibility of platinum in comparison with gold, the high density of platinum leads to a 10% reduction in volume, when compared to the usual gold implants. The flexibility of the chain enables optimal pre-tarsal alignment, both this and the maintenance of long-term flexibility are confirmed by ultra-sound examination results. This, in turn, greatly reduces post-operative complications and increases the effectiveness of this treatment greatly. In further testing, the MRT compatibility of the various sizes of Lid-Chain was proved for MRT systems registering between 1.5 and 3.0 Tesla.

Tips for fixing the Lid-Chains in position: Use only very slowly or non-absorbable stitching material (e.g. 6-0 with a P3 needle) The lid chain has several holes for pre-tarsal fixation. There should be at least 4 fixation points for the 0.6g chain, 6 for the chains from 0.8-1.6g and 8 for the 1.8- 2.0g chains. Each fixation point must be stitched individually.

Test Weights The implants are available in eight different weights, ranging from 0.6 g to 2.0 g. The optimal weight is ascertained by way of a preoperative “test fitting“. For this purpose test weights are used which are fixed with an adhesive strip onto the eyelid, in order to determine which weight is needed to result in the desired level of eyelid closure.
