Letter from the Publishers PUBLISHER Angela Striligas

She Says...

He Says...

CO-PUBLISHER Brian Hullette

Hello friends! It is hard to believe that we are more than halfway through 2012! This year is passing so quickly that I am beginning to get a little dizzy!

Hello VIPSEEN fans!!

COPY EDITOR Carolyn Koesters

Many of you may have noticed some of the changes in the magazine over the last few months…the double calendar, the two-month issue titles, so forth and so on. Allow me to explain what that is all about.

DESIGNERS Amber Barrack John Connor Trimmer

I had a meeting with a client a few months ago and he shared some insight as to the perception and others’ expectations of the magazine. We truly value the opinions of both our clients and our readers, so we took this under advisement and made some much needed changes.

ACCOUNT MANAGERS Kirsten Binkley Kelli Rogers Shelley Parham April Taylor Holly McBride

We are a monthly magazine providing a post report of amazing events hosted by our local non-profits, philanthropists, and all the people who gather together to support their cause. Events and business functions take place all month long, so we are constantly covering. It has never been our intention to come out on the first of the month, we usually land between the 7th and the 10th, and have been a couple days beyond that if there is a holiday in the middle of production (the printer closes). So this has caused some confusion and there are some who thought we came out late consistently, when in fact, we hit the streets right on time. It was suggested that we revise our events calendar and make a few changes to ease some misperceptions.

CONTRIBUTING WRITERS Rita Dykes Lynda Fontaine DISTRIBUTION TJ Furchess Daniel Gentry Michael Varner

This is why you now see the double calendar in every issue and on the cover it will have a two-month date instead of the original one month, (for example, this issue is August/September 2012). VIPSEEN is still a monthly magazine and we will still print 12 issues a year.  Thank you for sharing your opinions and insight! We really do listen and care about your feedback!

WEBMASTER Robert Neilson Wired Web Development

This month our special feature is “The Doctor Is In…” I am so thrilled at the number of local medical professionals who participated and contributed to making this our largest and best issue to date! Many of your local professionals took the time to write informational articles so that you may be better informed about what they do and how they can help you or a loved one!

PHOTOGRAPHY Mickey Baker Lynda Fontaine TJ Furchess VIPSEEN, Inc. Tri-Cities 247 Broad Street Suite 205 Kingsport, TN 37660

A special thank you to the entire group of medical professionals that contributed! We know your time is valuable and the fact that you took the time to write to our readers, really speaks volumes about your patient care! Last but certainly not least, THANK YOU David J Clapp Photography for all that you did for our cover and us! We know this project was quite the undertaking and a lot more than we anticipated in the beginning! A million thank yous are not enough!

Can you say “Moving on UP?!” Angela and I are excited to announce that VIPSEEN has officially moved office locations due to our continued growth. VIPSEEN is now on the 2nd floor in the Progress Building in Suite 205. Thanks to our dedicated staff and our loyal advertisers, we continue to plow forward and beat our personal expectations. I am proud to say this is our biggest issue to date. This month is our 1st annual medical edition. We are excited to introduce to you several of our doctors from around the region to help educate you on their expertise and services. Angela and I are finally starting to see UNITY with our entire staff. There are days I want to break Angela’s neck and I’m sure the feeling is mutual, but each and every month our partnership continues to grow stronger, and we continue to grow stronger as a company. Each and every day I continue to feel so blessed with my life and the journey I am on. I love waking up each and every morning and coming to work and seeing what such a positive impact VIPSEEN has made in such a short amount of time. I continue to meet so many great people in my life; words can’t express how thankful I am to have so many wonderful friends that have made such a substantial impact on my life. I will close by saying, we want to invite you to come by our new office location and visit us. Also, don’t forget if you have events or ideas you would like to share with us, please email us at brian@ vipseenmag.com or info@vipseenmag.com. Until next time… come out and be SEEN!

As always, we hope you enjoy this month’s issue! ads@vipseenmag.com events@vipseenmag.com info@vipseenmag.com WWW.VIPSEENMAG.COM ON THE COVER Images courtesy of David J. Clapp See page 65 for complete list of names.

2/21/54~7/28/12

Last month, we were honored to have Merle Dickert grace our cover. When we went out into the community searching for people who are well known for doing great things for their downtown communities, she came highly recommended. I asked Merle if she would mind being on our cover, she was so gracious and humble, as if it was I that was doing something for her, when in fact, it was her doing something wonderful for VIPSEEN. She was the first of eight to arrive at the photo shoot that morning, which allowed us to capture pictures of only her as test shots for lighting and background. Merle e-mailed me after the magazine came out from print thanking me profusely for the promotion of the Paramount and also included “I was also proud to be in the photos with so many hard-working, outstanding folks!” but also added that we had misspelled her last name in the credits for the cover and asked if we could correct it at some point, that she and her family would appreciate it. We extend our sincerest apologies for missing that in the proofing.

10

We are deeply saddened by the unexpected loss of Merle Dickert. She was always warm and welcoming to us here at VIPSEEN and the events at the Paramount were some of the best we have attended. Merle was a jewel to those who knew her and will be cherished forever.

Much-needed rains and soggy summer weather certainly did not deter or dampen the spirits of dedicated Friends of Allandale (FOA) in July. Devoted supporters gathered in Kingsport for an evening of fun and fellowship at the group’s annual picnic and meeting on the grounds of the city’s historic mansion in the Allandale Picnic Pavilion. Past president Winkle McKinley was recognized by the group and Bill and Sheila Moss

Carter Bagnell - Bristol Youth Leadership

It was bright and early at the Bristol Chamber-sponsored breakfast and the color orange that dominated the room was not only the breakfast juice, but the predominant shirt color! Dave Hart was the reason for the show of support by many who came out early to have a chance to hear the inspirational words from the head honcho of UT Athletics himself. Dave Hart, the Vice Chancellor and Director of UT Athletics, gave a motivatational talk about the importance of good leadership for a successful business or organization. Hart took over the post last September and had only positive and hopeful remarks regarding the future of UT and the Athletic Department. Hart relayed how impressed he was by the support of the fans in Tennessee and the potential he sees in the future of UT and the coaching staff.

Steve Wilmoth and Mike Musick

The event held at King College not only showcased Dave Hart and his wisdom, but also gave insight to the Bristol Youth Leadership Program and what it had to offer the area youth. A slideshow presentation was given by Carter Bagnell, one of the youth involved in the organization, and educated the audience on the sevenmonth leadership training program and how to get involved. The organization is sponsored by the Bristol TN/ VA Chamber of Commerce. For more information regarding BYL, contact Chelsea Gammon at 423.989.4859.

Independence Day Parades in the Tri-Cities Communities across the Tri-Cities know how to celebrate the independence of America with a bang! Traditional colors of “Old Glory” were everywhere, from homes, business, and cars to beautifully decorated floats, painted hair, and festive clothing. July 4 is one of the most celebrated holidays nationwide and for a good reason; it represents the day America gained its freedom. Many look forward to a day of parades, family, friends, burgers, music, games, water sports, and fireworks. The 4th of July parades are among the largest celebrations in the area. Crowds of adults and children of all ages lined the city streets in anticipation of the colorful floats, marching bands, beauty queens, unique vehicles, fire trucks, police cars, clowns, candy, balloons, and everything else that made its way along the parade route. This year, VIPSEEN attended the parades in Rogersville, Blountville, and Elizabethton. As you will see in the pictures, each city had excellent participation and everyone seemed to have a blast. Each city’s parade was different; however, they were all celebrated with much enthusiasm, patriotism, and grandeur. VIPSEEN would like to take this opportunity to express its deepest appreciation to every person that has served and is currently serving in the United States Armed Forces. Without the sacrifice of these amazing, dedicated people fighting for our freedom, there would be no Independence Day to celebrate. Always remember the ones who fought and gave their lives to make our wonderful country free. The next time you see a soldier, take a minute and thank them for their service. Happy Birthday, America!

Olympic Day 2012

ETSU Army ROTC

Donna and Jackson Noland

Let the games begin! This is the familiar cry that instantly brings the Olympics to mind, and it was heard June 29 at the ETSU Dome when Olympic Day 2012 commenced. Kicking off at City Hall, the torch bearers ran through the streets of Johnson City and ended at the ETSU Dome. ETSU first lady Donna Noland and her son Jackson carried the torch on the final stretch into the dome, while Dr. Paul Stanton waited for the hand off to light the ceremonial torch. The ETSU Army ROTC performed the presentation of flags and afterward, those attending heard a beautiful rendition of our national anthem. Special guest speaker Dr. Stanton spoke about the excitement of having the school be recognized as an Olympic Training Center for weight lifting and credited Meg Stone, director of the ETSU Olympic Training Center and two time Olympian, for bringing the idea to the university. In turn, Stone spoke of the hopes in turning ETSU into an Olympic Training Center for more sports in the future and what being an Olympian has meant in her life. After the ceremony, children were invited to come onto the Dome floor to partake in various “Olympic Games” with the aid of volunteers from the university. As the children were getting ready, once again the mantra, “Let the games begin!” echoed throughout the Dome, setting the perfect tone for the upcoming global competition. Outdoors, it was a day of sweltering, record-breaking temperatures; however, inside the International Storytelling Center, the cool air was blowing and the art was amazing, which made the trek through the hot streets worth the journey. Mr. Skaggs’s art is impressive to say the least, and he has proven once again what a talented artist he is, only this time in the genre of photography. Karen Hall, owner of Designing Images and friend of Skaggs for the last twenty-plus years, helped Skaggs with the idea of turning his one-time hobby into a lucrative business. Hall is trained in art and framing, and after one look at Mr. Skaggs’ photographs, she suggested he have a showing. The images are as cool and interesting as the artist himself. If you ever get the opportunity to attend one of Skaggs’ shows, you will not be disappointed. Don’t be surprised if you end up with one of his prints, as they are hard to resist! Saturday, July 7 marked the twenty-third annual Mount Carmel Block Party, a kickoff to Fun Fest. Back in 1989, when Mount Carmel held its first block party, there were 4000 people in attendance. These days, more than 15,000 people flood Main Street in celebration of summer and the beginning of Fun Fest in Kingsport. The band Zaffire headlined the event in 1989 and returned to the main stage this year to rock the streets once more. The band performed for several years in the late ‘80s and reunited in 2009. The only non-original band member is Regina Hodges on lead vocals. The remaining members of the band are Tina Brickey, Steven Brickey, Scott Calton, Anthony Crawford, Jennifer Crawford, and Anthony Hodges. Divas

This year’s block party was kicked off with the TZ3 race at 9:00 a.m. while the main event began at 5:00 p.m. with an array of festivities. A selection of food vendors lined Main Street with mouth-watering, crowd-pleasing food while the activities made way for fun for all ages. A car show, a motorcycle run, kids’ carnival, wrestling, and face painting were just some of the events of the day. In addition to Zaffire, a variety of musical entertainment was available throughout the evening; gospel music in the former Food City parking lot, bluegrass music by The Shadow Drifters on the main stage, and Stacy Vaughn performing his musical comedy on the BB&T Bank stage. Closing out the evening was a magnificent display of fireworks. For more information, visit the Mount Carmel Block Party on Facebook.

Girls just wanna have fun

To continue celebrating Rascals 25th anniversary year, Greg McClellan, Cindy Cox Steed, and Dennis DeVinney organized an ‘80s high school reunion fundraiser in support of the organization. Rascals was a cause close to home, especially for Greg, since he had been one of the first

DJs to work for the non-profit organization upon its opening, in 1987. Greg and the director, Amy Cradic McAtee, had been friends in high school. When he approached her with the original idea, she was thrilled. She stated, “July is always a tough month for us due to family vacations, summer ball games, and having to close for Fun Fest activities. Having this fund raiser was a blessing during struggling times.” Invitations were sent out via Greg, Cindy, and Dennis’ Facebook friends list in January to gauge a response to having a party, and it was overwhelming. The party/fundraiser was set for Friday, July 20 at Rascals, 125 Cumberland Street, in downtown Kingsport. (They hoped that Fun Fest would bring in lots of people from out of town that could also celebrate and support the fundraiser.) The ‘80s high school fundraiser was a huge success! We had over 200 people in attendance dancing to all ‘80s tunes and socializing like they had many years ago. Some people came from as far as Missouri to attend. The event brought in over $2,500, which directly benefited

Rascals Teen Center caters to 5th and 6th grade students the first, third, and fourth Fridays of each month, from 7:00 to 10:00 p.m. In conjunction with Forever Young Teens, they also hold a high school event about once a month. Artist Profile

Fine Art Photography P

hotographer Ansel Adams said, “There are always two people in every picture: the photographer and the viewer.” When making an image my goal is to capture not only what I am seeing, but also the essence of what I am experiencing with my other senses and to share that with the viewer. I’ve taken photos all my life, but only began selling my images in 2004 when a gallery asked to show my work. I was thrilled when my prints began to sell which gave me the confidence to approach additional galleries. Over the next few years my gallery representation grew, I had several solo and group shows, and I also began teaching photography workshops I designed for the Art Association of Harrisburg. At that time my full-time job was working as the Vice President of Senior Living Services for Complete HealthCare Resources, based in Dresher, PA. It was a wonderful job working for great owners, but in the back of my mind I wondered if I could turn my love of photography into my full-time business. My wife and I decided to give it a try and we left jobs, family, and friends behind in Pennsylvania and moved to Kingsport in 2007. We believed Kingsport would make a great base of operations since it was located near the mountains of Virginia, Tennessee, and North Carolina and we also believed that Kingsport’s downtown could become a future arts destination.

I knew I needed to build a brand-new portfolio of images of the southeast and worked nonstop to do so. I normally leave the house at 4am to drive to Roan Mountain or Cades Cove in the Smokies, and then hike or bike several miles to be in place for a sunrise. It was a lot of work, but I was doing what I loved and was surrounded by the beauty of this area.

snows from the Great Lakes blanket the area from early December into March and it’s not unusual to find 15foot snowdrifts. In December 2010, I tried to hike along the AT to make some images after a recent storm but gave up after the snow reached my thighs. I decided I better purchase snowshoes with money I received for Christmas and have enjoyed traversing more snow than I ever saw in Pennsylvania.

As I began to build my portfolio, I found a connection to Pennsylvania that I didn’t expect. I grew up near and hiked on the Appalachian Trail and two of my favorite photography locations in the southeast are also along the AT; Roan Mountain and the Grayson Highlands.

While many of my images are made in the wild, I also enjoy cityscapes and architecture, and recently I began a series of steam locomotive images.

The Roan Mountain Highlands are incredible anytime of the year. I’ve hiked to Grassy Ridge Bald to make images of the rhododendron in June and to the top of Roan Mountain in the winter. In 2008, I began a series of images titled The Goats of Roan. The images are of Angora goats that graze along the AT between the Jane and Grassy Ridge Balds and are there as part of the 10-year Baa-tany Goat Project.

As I built my portfolio of new images, I also strategized on how and where to sell them. In the summer of 2007, I approached Jennifer Phipps who had owned Picture This in Kingsport to show her my work. Picture This was the first location in this region to show my images and I’ve always appreciated Jennifer for giving me that opportunity. I then approached other galleries, with Twigs and Leaves in Waynesville being my first gallery in North Carolina and the gallery in the southeast that has shown my work the longest.

In addition to its stunning beauty, the Grayson Highlands in southwest Virginia contain some of my favorite subjects -- the wild ponies that graze on the balds. Early June in Grayson is a photographer’s dream come true with wild ponies grazing alongside the blooming rhododendron. Some of my favorite pony images have also been made in the fall and winter. I’ve found that the Mount Rogers’ area, which includes Grayson, is like a snow magnet. Lake effect

My images can now be found in galleries in Arizona, New Jersey, Pennsylvania, Tennessee and Virginia. In the Tri-Cities they are available at Blowfish Emporium in Bristol, Robin’s on Roan in Johnson City, Star Trails Downtown in Kingsport and regionally at Gallery Mallory – Abingdon. They are also available on my website: www.JeffreyStonerPhotography.com.

Cash Mob at The Carriage House

Terri Willis Jones- Owner of Carriage House

The latest “victim” of the Cash Mob concept was the popular retailer The Carriage House in downtown Kingsport. The business was bustling in full swing as customers weaved in and around the various offerings the merchant displayed. The phenomenon sweeping the nation referred to as a “Cash Mob” is quickly becoming a fun way to financially support local businesses. The idea originated from the popular fad called “Flash Mob” in which John and Karen Barber

Kristi and Jeff Fleming

people gather in a specific area and seemingly spontaneously break out in song and/or dance. A Cash Mob is similar in that people gather en masse at a specific spot; however, they differ in that instead of singing and dancing, they break out their wallets and support the business where they have gathered.

The Carriage House specializes in custom framing with over 35 years of experience to its name, as well as a full-service needlework shop offering knitting, needlepoint, needle punch, needle punch rug hooking, along with various classes and stitching groups. The Carriage House is known as the place to go to get the in-demand “Life is Good” products. The Carriage House is open Tuesday through Friday from 10:00am – 5:30pm and Saturday from 10:00am – 1:00pm. For more information regarding the Cash Mob that takes place the first Thursday of each month, you can call the Kingsport DKA office at 423-246-6550 or you can visit the Cash Mob Facebook page. The 31st annual Fun Fest extravaganza kicked off with its enormous parade and Mardi Gras party on Friday, July 13. Fun Fest, a Chamber of Commerce event, is the largest event of the year with over 140,000 people in attendance. Fun Fest was organized to unite the greater Kingsport area with family-oriented fellowship and fun to confirm that Kingsport is an excellent place to live and work. Kingsport offers numerous opportunities for people to come together while appealing to the interests in culture, athletics, art, and entertainment. Fun Fest compiles all of these aspects in just one week; however, many activities can be found in Kingsport all year long. Fun Fest features fun for the whole family. Events include the Crazy 8s race, activities at the beautiful Bays Mountain Park, hot air balloons, concerts, car show, dog show, Miss Kingsport pageant, cardboard boat race, bird watching, Taste of the Tri-Cities, fireworks, and much more. Although the weather did not cooperate for this year’s festivities with rain and storms almost every evening, it did not stop the fun. Fun Fest organizers always have an alternative plan for uncooperative weather. Among all of the festivities, attendees look forward to concerts and Taste of Tri-Cities, and this year was no different. Thousands flooded the street of Fort Henry Drive and J. Fred Johnson Stadium to partake in some local restaurant cuisine and get their groove on with extraordinary musical performances. There were 16 different restaurants represented, serving pizza, Mexican, Chinese, Japanese, seafood, ice cream, and much more. There was a variety of musical entertainment sure to have pleased most any preference, including Travis Tritt, Lynyrd Skynyrd, Folk Soul Revival, Drake White, and Jeremy Camp. Fun Fest came to an impressive end on Saturday, July 21 with Eastman’s spectacular fireworks presentation. For more information, visit www.visitkingsport.com/funfest. Story by: Rita Dykes

Pappy and customers

It was a rainy afternoon, but those big, wet drops were a welcoming gift to the dry spell that had been our usual fare for the last month! No matter what the day held for busy residents, people were out and about, as was the case with the customers at Pappy’s Motorcycle Parts and Custom T’s in downtown Kingsport.

Lori Dockery

Pappy’s was the business chosen for a cash mob event recently, and the customers not only came out to show their support, but they were also treated to an indoor picnic as a show of appreciation from the storeowner himself. Pappy Vineyard has been in business downtown for 39 years, 35 of those at his current location of 244 East Main Street. Even though Pappy’s started out as a motorcycle parts shop, it quickly

He’s headed up many philanthropy projects over the years, which includes the notable bicycle recycling project. Every year, more and more people donate used bicycles to Pappy who then takes them to the state penitentiary to be worked on and rejuvenated by the prison group known as the ”Foggy Mountain Club.” After the

According to Sylvia Musgrove, director of Kil’n Time, more than 75 artists worked to create 120 hand-painted bowls for the benefit. “It’s very satisfying to see so many segments of our community — painters, potters, musicians, chefs, students, teachers, medical professionals and more — come together to make this night a success,” she noted. “Bristol is using art to impact the lives of others, and that’s deeply nourishing.” Proceeds benefited Healing Hands, a Bristol-based charitable clinic that offers free medical and dental assistance for the working uninsured. To date, the nonprofit organization has provided 40,000 patient visits and more than $9 million in free medications to area residents. Helen Scott, executive director of Healing Hands, said the event reflects a difficult choice many have to make. “Lots of people find themselves having to budget either for their own well-being or for essential supplies. Community support helps us reach more people so they don’t have to make the choice between

Music Profile Music Profile

BRAD BLACKWELL 1. So how did you get started writing and performing music?

Well, I got my first guitar when I was a junior in high school for Christmas. As the story goes, I didn’t exactly ask for it and at the time was trying very hard not to let mom see that I was a little bit bummed out at the present. But, it wasn’t long before I completely fell in love with the instrument and from there, I suppose, the rest is history. 2. When did music change from being just a hobby to becoming a full-fledged career opportunity? Things really started to take shape when I met my current manager and co-songwriter, Jim Denton, and also when I moved to Knoxville. Jim had been a songwriter in Nashville during the 90s and had actually written the number one song “Rope the Moon” for John Michael Montgomery in 1995. Through his successes, he kind of knew the ins and outs of the business and had a lot of contacts in Nashville. So, while going to UT, I was able to work with him, play shows, and build my fan base while at the same time make trips to Nashville to write and play with industry execs that he had already established relationships with. I was able to do all that while also being a full-time student at UT. It all has worked out pretty nice though. 3. You recently sold out a show at the Historic Bijou Theater in Knoxville, Tennessee. Please tell us a little bit about what that was like. It was truly a dream come true. I have been working towards that goal for quite some time. I must say that it felt

Quick Facts about Brad: • Born May 18, 1989 • Kingsport native • Graduated from DobynsBennett in 2007 • All-conference basketball player at DB, played one year of college basketball at Maryville College • Rabid basketball fan and voracious reader, especially books about Pete Maravich, Larry Bird, and Abraham Lincoln • Graduated in May from the University of Tennessee with a degree in Advertising

pretty incredible to stand out in front of the crowd and realize it all playing out so nicely. I have been incredibly lucky to have so much support from so many people and the fact that so many people have given me such great responses to the music. 4. We understand that former University of Tennessee head football coach, Phil Fulmer introduced you at the show. How did that come into fruition and how did you two meet? I was introduced to him through a professor that I had while at UT. This professor also happened to be the head of a wealth management company in Knoxville where coach Fulmer currently works and was kind enough to pass my stuff along to him. Long story short, Coach Fulmer is a remarkable man and there is no reason, other than him just being an incredibly nice person that he should have much interest in what I’m doing. I just am very lucky and thankful of all the help he’s provided. 5. You have got some pretty incredible shows coming up. September 9, you will be playing a the Tennessee Valley Fair and then on October 14, you will be opening up for Darius Rucker at the Foothills Fall Festival in Maryville. How did these shows come about? I give the credit for these shows to all the people who have really just believed in what I’m doing throughout and who have helped to get the music into the right people’s hands. I am obviously very excited about both shows and feel very fortunate. I am happy, but not satisfied (laughs). I want to take things as far as I can take them!

BARIATRIC SURGERY (Weight Loss Surgery)

The safety and efficacy of these surgeries, when performed by experienced surgeons in well prepared centers, has been proven repeatedly. The morbidity and mortality rates with Bariatric surgery at Centers of Excellence Andrew P. Kramer, D.O., F.A.C.O.S. are very similar to other common surgeries like gallbladder removal. One must remember that morbidly obese Surgical Associates of Kingsport patients, by definition, are unhealthy, and thus are at a higher risk for any surgical intervention. Without intervention, morbidly obese patients typically live about 15 years less than normal weight individuals.

BARIATRICS

Obesity is the number one health care epidemic in the world today. It is the root cause of many of the most common and deadly diseases that affect us and our loved ones. These include type II diabetes, heart disease, obstructive sleep apnea, hyperlipidemia, depression, and even many forms of cancer. Bariatric and metabolic surgeries have been definitively proven to be the only effective method of obtaining significant and long term weight loss, and are currently the most effective way to treat and CURE diabetes and reverse hypertension, hyperlipidemia, and sleep apnea. By reducing obesity rates worldwide, we will lower rates of cancer, heart disease, and early death.

Dr. Andrew P. Kramer has performed more than 1500 Bariatric procedures. He also has experience as a Bariatric patient, having undergone laparoscopic gastric bypass himself in 2004. Dr. Kramer received his medical degree from the New York College of Osteopathic Medicine/New York Institute of Technology in 1987 and completed his surgical residency at UMDNJ/Kennedy Memorial Hospital in 1992. After practicing surgery in SW Virginia, he incorporated Bariatric surgery into his practice in Lebanon, VA in 2000. Dr. Kramer developed one of the first comprehensive Bariatric surgery programs in a small hospital rural setting, and he and his program were nationally recognized as one of the very early Bariatric Surgery Centers of Excellence. He brought his program to Holston Valley Medical Center in 2007 and The Comprehensive Weight Management Center was recognized as a COE in 2008. Dr. Kramer is board certified by the American Osteopathic Board of Surgery, and is a Fellow of the American College of Osteopathic Surgeons. He is a partner with Surgical Associates of Kingsport, Inc. and serves as the medical director of The Comprehensive Weight Management Center at Holston Valley Medical Center. Dr. Kramer is active in medical education as an associate professor of Surgery at ETSU and with several medical school programs. He was a founding member of the Virginia Bariatric Society and is a member of the Tennessee Bariatric Society. Dr. Kramer is an avid runner and boater. He is married to Pamela (for 25 years) and father to Zak (age 21) and Mollie (age 17).

I was concerned that carrying the extra weight was going to cause serious health issues as I got older. I wanted to be healthier for myself and my family. Once I made my mind up to have the gastric banding the journey was not that difficult. Since my weight loss, I am healthier and have a lot more energy. Also, my eating habits are much better. I am more involved in various activities such as running and bicycling. It is not a quick fix journey, it is a life style change. It takes time, but it is well worth it. HeartSHAPE offers hope and saves lives ®

Heart problems can be detected early with a simple, inexpensive, noninvasive test. cardiology

“Since we screen patients without symptoms, the test generally doesn’t detect a level of disease that requires surgery,” Dr. Ladley said. “The test helps us see the problem if there is one, and we can start working on prevention before a person has symptoms.” The Wellmont CVA Heart Institute’s HeartSHAPE® program focuses on early detection of coronary artery disease and heart attack prevention, and it takes less than five minutes to perform. A 64-slice ct scanner is used to take pictures of the coronary arteries from outside the body. From those images, a coronary calcium score is determined.

Don Osborne provides Carolyn Burgess with a HeartSHAPE® coupon and discusses the benefits of calcium scoring. Don Osborne was near death – and didn’t even know it.

A local farmer, active church member and community volunteer, he had been living with a life-threatening heart condition without having any of the classic symptoms. If it weren’t for a simple, inexpensive test, he might not be here today. The test, called HeartSHAPE®, is a noninvasive cardiac calcium scoring test that saves lives by detecting heart problems in patients who have no symptoms of heart disease. For Osborne, telling people about the test has become a mission. “I talk to everyone about it,” he said. “I have probably talked to more than 300 people, and I’ve driven friends and neighbors to the Wellmont CVA Heart Institute to have the test done.”

To participate in a HeartSHAPE® test, you must be referred by your primary care physician or meet certain criteria. It is generally recommended for men over 45 and women over 55 with at least one cardiovascular risk factor. If someone has two or more risk factors – such as diabetes, obesity or smoking – it may be appropriate to screen below the recommended ages. “At the time I had the test, I thought I was healthy,” Osborne said. “I would have said I was in the top 10 percent of people my age. But instead, I was ready to die, and I didn’t even know it. This test saved my life.” Get a picture of your heart health and fight heart disease early. Or enjoy peace of mind knowing your heart is healthy. For a limited time, take advantage of a HeartSHAPE® special two-for-one price of only $49 for you and a loved one. To learn more or to schedule a HeartSHAPE® test, call Wellmont Nurse Connection at 1-877-230-NURSE.

Osborne’s journey with HeartSHAPE® started after he nearly passed out while working in his garden. Concerned, his physician performed an ekg and stress test. “My doctor said my test results were good and my blood pressure and cholesterol had been good,” Osborne said. “I saw an ad for the heart test in the newspaper, and I thought I’d go have it done, just in case. I didn’t think they would find anything, but I was curious.” Osborne’s curiosity may have saved his life. This test revealed his cardiac calcium score was high. Even worse, the test found Osborne had an aneurysm that measured two and a half inches and required immediate surgery. Dr. Herbert Ladley, a cardiologist with the Wellmont CVA Heart Institute, points out that most patients who have the test don’t have cases as dramatic as Osborne’s.

Mike Needham, CT technician, places Don Osborne in the scanner for a HeartSHAPE® test. 71

Can Your Smile Age You? A recent survey concluded that an unattractive smile not only can make you look older than you are, but it can make people perceive you as less intelligent else, an improved smile resulted in people being perceived as younger, more successful, and more attractive to the opposite sex. It also, had a positive influence on both personal and professional relationships. If you are not quite ready to undergo cosmetic surgery to address fine lines, wrinkles and deep folds, a nonsurgical alternative using veneers may be an option. When the back teeth are worn or missing, the facial structure they support tends to collapse or fall in, which can lead to deep nasolabial folds. Building out the back teeth with veneers or crowns helps to lift the cheekbone area of the face and widen the smile, thereby making you look younger. When the teeth that support the lips are built out, the blood supply is stimulated and lines around the lip area are less noticeable. The end result is more balanced appearance, enhancing the prominence of the cheekbones for an overall younger look. Veneers are one of many cosmetic options to improve your smile.

Martin Dentistry My husband Joe and I have been very blessed to travel many wonderful places around the world; but, after being diagnosed with sleep apnea last year, this diagnosis really put a “damper” on our active lifestyle and travel plans when having to deal with a CPAP machine and all it involved. I went through six or seven different mask options throughout the last year, but none of these worked for me. The Sleep Clinic then suggested I might want to see Dr. Tim Martin of Martin Dentistry. What a blessing that turned out to be for me! After my initial consultation with Dr. Martin, he completed a thorough examination and evaluation of my situation. He was so patient, understanding, and so knowledgeable about the issues of sleep apnea. (You need to check out his credentials!) He then fitted me with a small, non-invasive dental appliance to wear as I sleep at night. From the very first night, my symptoms disappeared and have not returned. It’s really difficult to describe with words how much this has changed my life! It’s been amazing!! Dr. Tim and his warm, wonderful staff are such caring, compassionate people, and I cannot express enough how wonderful it is not to have to carry a CPAP machine with me everywhere I travel, as well as dealing with it at home. It has given me a new lease on life. I would encourage anyone who is having difficulty with sleep apnea to contact Dr. Tim and his amazing team!

Michael Austin IT Relationship Manager Eastman Chemical Company Like many others, I had tried using a CPAP and sleep medications to help with my sleep apnea and fatigue; neither worked well for me. I could not tolerate the CPAP and actually rested much less with it. The sleep medications had very negative side effects for me. While I know many find these helpful, they were not effective options for me. Dr. Martin introduced me to the sleep appliance and I found it to be very helpful. I have not completely stopped snoring, but my snoring has greatly decreased. I sleep better at night now and wake up more rested in the morning. Like all medical devices, there is a short period of time it takes to get used to the device. The other thing I liked about the sleep appliance is you can adjust it very slowly so you can get to your maximum improvement level. It’s very easy to travel with and fits easily in a toiletries case, whereas the CPAP was much more troublesome to take on trips, especially trips that required air travel. Overall I’ve been very pleased with the sleep appliance, its simplicity in design,

and effectiveness in results. Dr. Martin and his staff have been a pleasure to work with and you know they have your best interest at heart. You’re more than just a patient to them, you’re a real person that matters!

Bradley Melone CMPM, CPMA Practice Manager First Choice Neurosurgery I was diagnosed with severe, obstructive sleep apnea following a sleep study in January 2007. As expected I was placed on CPAP to treat this serious condition. For the next year I tried numerous masks, too many to count, and even went onto BiPAP in hopes of finding a mask/machine I could use regularly. After many sleep-interrupted nights struggling with the mask fit/seal, I finally gave up on trying treating my sleep apnea. This was a tough decision for me, as I am in the medical profession and know the serious health affects this condition poses, but I simply could not find a mask that I could effectively use. A couple years later I saw a flyer from Martin Dentistry and Sleep Medicine talking about a dental appliance that could be used in treating sleep apnea. The local physician that I work for and I researched this and found that yes, indeed, these devices do exist and actually have a proven track record of success, so I made an appointment with Dr. Martin to discuss my particular case. We both felt that the oral device was worth trying in lieu of not treating my sleep apnea, but had some reservations of how successful the device would be, due to the severity of my sleep apnea. The sleep appliance dental appliance has worked beyond my furthest expectations. The sleep study performed while using my oral appliance also showed a remarkable improvement. I am sleeping through the night, have more energy during the day, and have peace of mind that I no longer stop breathing during the night. The sleep appliance is quite comfortable to wear, easy to take on trips and has been a true blessing for me and my family. Dr. Martin and the staff of Martin Dentistry and Sleep Medicine are exceptional people who obviously enjoy what they do, as you could not find a better group of people to make sure all your needs and questions get answered. They handled everything from fitting the appliance to working with the insurance companies to pay for the appliance -- all I had to do was come in for my appointments and wear the device. I strongly urge anyone who has sleep apnea to seek medical treatment for this serious condition and not continue to ignore it. There any many options out there and I can think of no better place to get that care than Martin Dentistry, you will feel welcome from the minute you walk in the door.

Joe & Tori Nance, Kingsport

My name is Debbie Hyder and I would like to tell you about my experience at Dr. Kevin Martin’s office. When I walked into Dr. Martin’s office, I was truly a traumatized and broken person from the procedures I had went through for over a year to get my dental implants. My life had been on hold for so long because of my teeth. I truly found a dentist that had compassion and concern for his patients. He saw I needed my smile back as soon as possible. He had his wonderful staff work me in his schedule, even stayed over late sometimes and got my teeth fixed in half the time it should have taken. My teeth are absolutely beautiful. I smile all the time and can eat things that I haven’t eaten in years. He positively, without a doubt, gave me my life back. I recommend Dr. Martin to all my friends and if you want a dentist who truly cares about you and does number one work… I highly recommend Dr. Kevin Martin.

Sharon Grass Teacher City of Kingsport Disappointed with the shifting my teeth had done over the years, I was even more discouraged when an orthodontist told me that the only recourse was a minimum of two years of metal braces, complete with elastic bands and night-head gear. Hoping for something a bit more discreet, I let the idea go until Dr. Martin’s office began offering Invisalign. I was pleased to learn I would be a good candidate for this procedure even with the significant amount of straightening needed on my bottom teeth. After just over a year, my teeth are beautifully straight and aligned. I am truly delighted with the results and highly recommend this convenient, comfortable and affordable way to recapture confidence through a dazzling smile.

Justin Jenkins Police Officer Johnson City, Tennessee My name is Justin Jenkins. After trying CPAP and surgery without any success, I decided to talk to Dr. Martin about oral appliance therapy. I’ve noticed a huge difference in the way I feel during the day after wearing my appliance. A month ago, my dog used my appliance as a chew toy and I really noticed a difference in the three weeks I was without it. Before appliance therapy I was tired all the time and had no energy; now, I cannot imagine being without it.

many thousands of times have we seen those toothpaste or mouthwash commercials? They are all very similar. It begins with the patient in the dental chair looking sad, receiving some bad news from the dentist. We then hear the voiceover say something like this: “4 out of 5 dentists recommend brand A toothpaste/mouthwash to prevent gingivitis and gum disease. It has been the same message for years. Most people have seen it so many times that they simply tune out and never give it another thought. But what is gingivitis or gum disease and why should it matter to you? Gum disease, or as it is referred to by dental professionals, periodontal disease, is the inflammation of the gums and its subsequent effects on the soft tissue and bone that supports the teeth. It is caused by bacteria found in plaque and the toxins that are released by these bacteria. There are three stages of the disease process: gingivitis, periodontitis and advanced periodontitis. The disease is most commonly characterized by swollen, bleeding gums. This can lead to a loss of bone around the teeth and eventually loss of teeth. The startling fact is that until the later stages of the disease, aside from some minor bleeding during routine brushing and or flossing, gum disease has few discernible symptoms or warning signs and rarely is painful. By the time most people realize they have a problem irreparable damage has been done. It has been reported that up to 75% of the population over the age of 35 have periodontal disease. Gum disease is treated in many ways. Typically it consists of a combination of manual scaling of the tooth and root surface and antibiotic treatment. The scaling removes the bacteria containing soft and hard deposits from the gums and teeth while the antibiotics aid in reducing the overall amount of harmful bacteria present.

increasing number of relationships between periodontitis and other diseases in the body, most information is known about its notable effects on diabetes, heart disease and pregnancy.

Dental

How

There appears to be a bidirectional relationship between gum disease and diabetes. Approximately 95% of diabetics also have periodontitis. Diabetics have been shown to be more likely to develop gum disease due to their greater susceptibility to infections. This in turn can increase blood sugar and place these individuals at much higher risk for diabetic complications. Researchers have shown that those with gum disease are almost twice as likely to suffer from coronary artery and heart disease. It has even been proposed that this oral bacteria can infect arterial plaques associated with atherosclerosis further contributing to their dangerous nature. Newer studies have suggested that women with gum disease are 2 to 7 times more likely to suffer abnormal pregnancy outcomes such as premature birth, low birth weight, preeclampsia and growth restriction. The American Academy of Periodontology strongly recommends pregnant women receive at a minimum a screening for gum disease and encourage the maintenance of routine professional dental care. The most important factor in the prevention of gum disease is twice a day brushing and daily flossing in addition to visiting your dentist every six months for a cleaning and comprehensive oral examination. Unlike a lot of systemic diseases, gum disease is one that is 100% preventable. With proper home care and some assistance from your dentist or dental hygienist, you can avoid the potentially dangerous effects of gingivitis and gum disease.

What poses the greatest amount of concern with gum disease are the systemic conditions with which it has been linked. While new research is showing us a rapidly 77

Step 4 works at an even deeper level, working on wrinkles and deep folds. Products such as Botox® work on the muscles responsible for creating the lines. Repeated use can lift out stubborn lines or prevent new ones from forming. Lines that are still too deep may require a filler, such as Juvederm®, Restylane®, or Radiesse®, to fade away the etching in the skin or fill in a deep fold. Disappearing lips can be revolumized, sunken areas around eyes corrected, and downturned corners of the mouth can be lifted. The results of combining these procedures can be very dramatic.

Wellmont Health System has provided superior, compassionate health care to thousands of people in the Tri-Cities through its hospitals and other facilities. Now, the health system has introduced its new physician enterprise. Wellmont Medical Associates, formerly known as Wellmont Physician Services, is an organization that allows its physicians to lead the way in redefining how quality care is delivered to their patients. Wellmont recognizes physicians know best how to treat their patients. So by giving doctors a more direct role in the decision-making process, they can provide even better care for you. But what makes Wellmont Medical Associates unique? This organization has quality providers, high standards and the backing of a phenomenal health system – and we are growing rapidly. Wellmont Medical Associates

One way to achieve Wellmont Medical Associate’s goal to help our community become healthier is by becoming the first group in the region to meet the criteria for the 2011 National Committee for Quality Assurance PatientCentered Medical Home. A medical home is a healthcare setting that facilitates partnerships between individual patients and their personal physicians and, if needed, the patient’s family. This program helps give our practices information about organizing care around patients and having a more team approach in providing care. But we know we can’t create a healthy community alone. We can’t create a healthy community without the partnerships we have with Wellmont and its hospitals. But more importantly, we rely on the partnership we have with our patients. A great doctor once said, “Unless someone like you cares a whole awful lot, nothing is going to get better. It’s not.” This famous doctor was Dr. Seuss. His books may be lighthearted with silly rhymes, but he knew the importance of working together to make a difference. Through Wellmont Medical Associates, patients are empowered to take part in their care. Through this personal collaboration, rest assured you are receiving the best health care in the region.

Our physicians apply best practices in quality and service at all our physicians’ offices to ensure our patients receive optimal care, and our providers want to form long-term relationships that extend beyond the office walls. Our board of directors is a mix of physicians and administrators working together for one cause. This group’s diversity is beneficial to you because it is the perfect mix of medicine and business. While quality care is paramount, we also want to lower costs and be more efficient with our resources to better meet your needs. 85

Thyroid, Insulin, Cortisol, Estrogen, Progesterone, Testosterone, Vitamin D, DHEA The balance of these hormones is crucial to losing weight and KEEPING it off. Especially as you get middle-aged! To keep weight off you have to first get your hormones in balance and second, learn how to eat right! Contrary to a lot of traditional nutritionists and dieticians, I believe the following to be true: • • • • • •

Calories IN do not equal calories OUT. The FATS do not make you FAT. WHOLE GRAINS are not necessarily good for you. Although exercise is good for you, exercise alone will not make you lose weight You will GAIN MORE WEIGHT (eventually) drinking diet sodas than regular sodas, and FAST weight loss is more effective long term than SLOW weight loss.

That’s why I prescribe HCG ( also a hormone) as a tool for rapid weight loss. It is one of the most effective ways for patients to lose unwanted fat FAST. I have personally treated over 7, 000 patients with the HCG hormone and part of my protocol is to have their hormones checked before they begin. Why? Because if I know what is causing their weight gain, I can tailor their weight loss program so that it will be successful long term. If patients don’t learn how to eat, exercise, sleep and balance their hormones they will gain the weight back! Performance Medicine will help you get there. What’s next to learn about your total health? THE WHEAT BELLY! I will tell you all about it in the September Issue. See you at Performance Medicine. –The Doc

RESULTS!

After

At the age of 32, I had seen one of my co-workers, who is 20 years my senior, nearly die of a massive heart attack. I’d seen another co-worker who is 10 years my senior go to the doctor and find out that he was diabetic, obese and had high blood pressure. I could see where my life was headed if I continued on the path I was on. On Jan. 9th, 2012, I went to visit Willie and the gang at LiveFit Medicine. My weight that day was 267 pounds! That was Day One of a different lifestyle. As I sat down and began to talk, Willie asked what my goals were, and I honestly didn’t have an answer. “When are you going to run your first half-marathon?” Willie asked. “HE MUST BE CRAZY,” I thought, “I could NEVER run that far!!!” I must admit, I was very skeptical of the HCG program and the concept of “Transformation.” But as I write this today, I am no longer a skeptic - I’m a believer! Over the last few months, the team at LiveFit and Anytime Fitness has truly helped me to regain my life. I am doing things now I would never have imagined possible a year ago. Today I weigh 202 pounds, and I am an avid CrossFitter, preparing for my level one CrossFit certification. I’ve accomplished several fitness goals I never thought possible, such as running a Warrior Dash, several 5Ks and the Crazy 8s race, and in October I plan to run my first half-marathon! My total weight loss is 65 pounds, but I feel like I’ve lost an entire person. I am literally in the best shape of my life. I can lift harder, run farther, and I feel better than I ever have. At LiveFit they advocate “Weight Management” and not just “Weight Loss.” Every single member of the team is super-friendly and committed to your success. I can’t say thanks enough to Willie and his staff for helping me every step of the way. Don’t just lose weight … LiveFit

The HCG Transformation program at LIVEfit Medicine Health and fitness

The HCG Transformation Program at LIVEfit Medicine is based on a diet developed by Dr. ATW Simeons and published in 1954. The diet itself is relatively simple. You are allowed two small servings of protein, two vegetables, two fruit, and pieces of melba toast. The hormone is HCG (the pregnancy hormone) and the diet is a calorie-restricted diet of less than 500 calories per day with no fewer than 23 injections and no more than 40. The average weight loss for men is 1-2 pounds per day and for women between ½ - 1 pound per day. You feel surprisingly well with sufficient energy to perform your daily tasks with a few minor side effects. The first week you may develop a mild headache and possibly a little constipation. After the first 3 days of the diet, it is important to follow the protocol to a “T.” Accountability is crucial in your journey with HCG; therefore, you must weigh daily. Occasionally you may hit a stall and if you do, we will need to make adjustments to get the weight moving again. The amount of exercise is limited to low intensity cardio for no more than 45 minutes per day. My own personal struggles with obesity it what has led me to change my practice from Endocrinology to Lifestyle medicine. After serving eight years in the U.S. Army and after being honorably discharged, I finished my degree as a Family Nurse Practitioner which led to me gaining up to 265 pounds. After developing early signs of type 2 diabetes, I knew something had to change. I began evaluating my lifestyle and started to eat cleaner. I tried running again, but my knees hurt to the point that it was too difficult to run, so a friend of mine pushed me into getting a bike. I began riding and noticed that my knees no longer hurt and I was feeling better and kept pushing myself harder each week. My blood sugar was normalizing and after three years, I was able to drop 95 pounds. After practicing Diabetes with Dr. Matthew Beasey for almost five years, I read the manuscript, “Pounds & Inches” by Dr. ATW Simeons. Little did I know that this manuscript would change my life. My first patient was a friend I had been treating with Diabetes and severely elevated Cholesterol. He was on every medicine you could take besides insulin and had an Hgb A1C of 10.5. He was a truck driver and could not take insulin. When he first agreed to go on the HCG diet, I was a bit skeptical, but after only one week his blood sugar levels were dropping and I had to take him off medications. By the end of the 40 days, Billy Dishner was off every medication for diabetes, cholesterol, and high blood pressure. Next, I put my 330-pound brother on the diet and he dropped over 40 pounds, and has since dropped over 100 and maintains his weight. I knew that there was something missing from the HCG diet because you could only be on the diet for 6 weeks, then you had to be off for at least 6 weeks. From this point I knew that adding a structured exercise program to the HCG would be beneficial, but I had no idea the impact it would have. So I decided to open a fitness center (Anytime Fitness) and combine medicine with fitness. My first client in the HCG Transformation Program was John Goddard and after a year of working with John, he was able to lose 226 pounds. Since a little over a year and a half ago, we have had almost 78 people lose over 100 pounds and within the next few weeks our 5th client will have dropped over 200 pounds. So what makes us different? Our program is based on the original Simeon diet. There are 4 phases to our program and we walk you through each step of the way. Phases 1 and 2 are the 40 days of the diet and phase 3, we have developed into a low carbohydrate diet and this is where we begin our 6-8 week strength conditioning program. After you reach your goal, we do metabolic testing and teach you how to keep your weight normal without dieting. So basically we are offering a lifestyle change. “It’s not about how you look; it’s about how you live.” Facts about Willie Hamilton • Board Certified Family Nurse practitioner • Board Certified in Nutrition and Sports medicine • Crossfit level 1 Trainer • Certified Personal Trainer • Honorably Discharged Veteran of the US Army • Certified level 2 cycling coach • Avid runner and cyclist If you have any questions about the HCG Diet or any other health or fitness questions you may add me to Facebook or email me at info@LIVEfitmed.com. different from that of today. To prepare for new types and lower levels of reimbursement and to navigate a challenging health care environment, innovative approaches to health care delivery are needed to offer quality care to patients while maintaining sustainable business models. Integrated Solutions Health Network LLC (ISHN) has taken a calculated risk to venture into this new health care paradigm by developing a set of populationcentric solutions. These fiscally conscious solutions are in various phases of implementation and are designed to improve patient outcomes and experiences without sacrificing quality of care. The key to ISHN’s success is an indivisible partnership with providers sharing the same goal of providing exceptional health care. ISHN is an affiliate of Mountain States Health Alliance (MSHA). The communities ISHN serves are located in Northeast Tennessee, Southwest Virginia and Western North Carolina. ISHN is owned by providers, led by physicians, and created to provide the community with a new, improved model of health care delivery. ISHN is focused on providing a new type of health care delivery system that follows the tenets of the Triple Aim™ initiative to improve the health of the community and optimize the patient experience while controlling costs. ISHN’s provider network seeks to offer quality care by incorporating evidencebased approaches to improve patient outcomes.

CrestPoint works in partnership with members, employers and other health care sponsors to manage health care costs, improve clinical quality and outcomes, and create a culture of health that motivates individuals to participate in improving their personal health and wellness.

About AnewCare In 2011, ISHN formed AnewCare in response to opportunities presented by the Affordable Care Act. AnewCare has created a network of providers who will coordinate care for the communities of Northeast Tennessee, Southwest Virginia and Western North Carolina. AnewCare will be able to accept new payment models such as bundled payments and will participate in shared savings programs such as the Medicare Shared Savings Program (MSSP) (accepted in July 2012). ISHN’s provider network possesses many of the capabilities required to operate successfully within an Accountable Care Organization (ACO) framework, and ISHN established AnewCare to be the ACO framework for its provider network. AnewCare is organized around locally managed Physician Organized Delivery Systems (PODS) which are clustered around key hospitals or service area hubs. These PODS will drive the implementation of the Patient-Centered Medical Home (PCMH) model of care delivery and other reforms needed to ensure success. AnewCare is also collaborating with community organizations to address high hospital readmission rates in a predominantly rural, underserved population in Central Appalachia.

the patient did not have any complications from high potassium, if he did, then the liability is borne by me and the hospital, but not the software vendor. The unanticipated consequences of computerized physician order entry implementation present the risks of mismatches between workflow and clinical information systems like juxtaposition errors, (mistakenly selecting an adjacent patient name or medication), and illusion of communication, (physician assumes the medications that were ordered have been administered, or that order entry implies that others will see and fulfill the orders). Even though the intention of expanding electronic medical records in the present healthcare bill to provide better patient care is respectable, no accountability has been placed on software vendors for sub-standard or faulty software that can cause medical errors. The software vendors avoid liability by clauses in the contract with language that states “vendors are not liable for errors in their systems because physicians, nurses, pharmacists and healthcare technicians should be able to identify and correct any errors generated by the software faults.” The software vendors should be held responsible

The hospitals, physicians and various ancillary healthcare providers can negotiate to limit or shift some of liability risk to the software vendors by including in the contract the indemnity clauses, risk sharing, extended warranties, including different types of damages to be reimbursed by the vendor, and define the standard of care. The hospitals and healthcare providers should consult with medical malpractice attorney to address the medico-legal risks that the software could cause. The providers also need to consults with the malpractice insurance carriers before adopting and implementing healthcare software. In conclusion, is it the appropriate time for the patients to ask their healthcare providers and hospitals if they are using certified and well evaluated software programs? Is it reasonable for the doctors to ask the hospitals whether the software programs implemented are certified, foolproof for medical mistakes and patient safety? Is it time to include software vendors as medical malpractice defendants?

Breast cancer is a devastating illness that nearly 200,000 women are diagnosed with each year in the United States. There has been exceptional progress made in this disease in terms of diagnosis and treatment over the last few years, and it is important that a patient be in the hands of a well-trained physician in this disease process. The steps and timing in treatment can vary with the needs of each patient. If breast conservation is the goal, then that is often possible. It is important to be cared for where a multidisciplinary team approach is established. Integrating the components of breast cancer care expertise in an accurate and efficient manner is at the heart of the care of the breast cancer patient.

you have cancer. Cancer... it’s one of the worst diagnoses a patient and physician face together. There is fear of the unknown and even fear of treatment. The good news is that innovating techniques are available to lessen the fear and help with your treatment. A common cancer diagnosis is liver cancer. Cancer in the liver may come from somewhere else in the body like colon cancer or it may originate in the liver. Last year there were 18,500 cases of cancer originating in the liver. This is a cancer that strikes men twice as often as women. These tumors are usually a cancer called hepatocellular carcinoma. The leading contributing cause is cirrhosis. Several things that cause cirrhosis of the liver include Hepatitis B or C and alcohol abuse. There are many other causes. Cirrhosis occurs when the liver is injured and attempts to repair itself. Over a period of years, this can lead to tumor formation. Hepatitis C has lead to a worldwide increase in hepatocellular carcinoma (HCC). In addition to HCC, which is a primary process, any cancer from any part of the body can spread to the liver. One of the most common cancers to spread, or metastasize, is colon cancer. These tumors start as a small number of abnormal cells that grow for months or years before being detected. Of the 140,000 newly diagnosed patients in a year roughly half of them will develop spread to the liver. Of those, only 1 in 10 will have a surgical cure. There are a number of blood tests and X-rays, including CT scans, that help the diagnostic process. A biopsy is the only way to determine what type of cancer is there. Most biopsies are easily performed by the Radiologist using a small needle, local numbing medicine and medicine in your vein to take the edge off of the procedure. If a liver cancer is confirmed, there are many treatments. Bristol Regional Medical Center provides many of those treatments. One of the newer and most interesting is procedures provided by Interventional Radiology. These newer treatments: • Are less painful and debilitating • Result in quicker recoveries • Have fewer side effects and complications

Interventional Radiologists are uniquely skilled in using the vascular system to target tumors from inside the body through small tubes called catheters without medicating or affecting other parts of the body.

oncology/radiology

One of the most frightening things to hear from your doctor is that

Tumors need a blood supply to feed themselves and grow. Interventional Radiologists can deliver treatment through both medications and radiofrequency ablation from inside the body. A common treatment, embolization, involves blocking the blood flow to a cancer. Radioembolization is the combination of blocking the blood flow and giving radiation with small radioactive beads that float in the blood to your tumor. The beads are carefully given only within the blood vessels feeding the tumor. Another promising technique uses a small needle to deliver microwave heat to kill a tumor in the liver lung or kidney. This heat ablation is a separate method of treatment where radiologists direct microwave or radiofrequency heat directly through the skin to the tumor. Another technique uses cold to freeze the tumor. Chemoembolization, or targeted chemotherapy, is a minimally invasive treatment for liver cancer that can be used in larger tumors or when the tumor is in a bad location. This process delivers a high dose of cancer killing drug directly to the liver while additionally blocking the blood supply to the tumor. The Interventional Radiologist does this by threading a tiny catheter from the artery in the groin into the liver. The drug stays in the tumor because the blood supply that would normally wash it out is slowed or stopped. Therefore a higher dose of chemotherapy can be used. The procedure usually involves a hospital stay overnight up to two or three days and can leave the patient feeling like they have lower than usual energy for about a month. The Blue Ridge Radiology interventional radiologists at Bristol Regional Medical Center are also experts in all things relating to your blood vessels. They are often involved in diagnosing and treating leg pain and wound healing issues. From the very first cardiac catheterization performed by Dr. Charles Dotter, a radiologist, until now, Vascular Interventional Radiologists stay on the forefront, teaching and developing innovative treatments. As always, our goal is help every patient to receive the best care delivered with compassion and excellence. 99

Dr. John H. Phillips, a native of Dallas, Texas, received

My approach to sports medicine is patient centered and fundamentally down to earth. Sports medicine and arthroscopic surgery are some of the most technical procedures done today, yet the key to a successful outcome involves the relationship I build with my patients. My first goal when discussing surgery with patients is to insure they understand the nature of their injury and what their various treatment options may be. We spend time at every appointment reviewing the injury itself – reviewing diagrams and models and discussing the natural history of the condition. Patients will often come in with pages from the Internet or magazine articles, which we can review and discuss as well. Similarly, before I discuss treatment options I want to understand how an injury affects my patient in his or her daily life and goals. I will often inquire about specific athletic goals like college scholarships or occupational tasks such as physical requirements for emergency evacuation from a coal mine. By understanding exactly how an injury affects a person I can then offer a surgery designed to allow them to return to play as an athlete or perhaps return to work in law enforcement or even return to chores in the house rather than simply describing a repair of a rotator cuff or biceps anchor in the shoulder. This discussion then forms the foundation, which allows us to develop the right treatment plan for each patient. A particular advantage of sports medicine injuries is that they are rarely life or limb threatening, so potential surgery can be planned electively when most appropriate or most convenient for my patients. However, if surgery is needed but by necessity delayed, there may be other treatments which can help an athlete finish his season

his undergraduate degree from Vanderbilt University in Nashville, Tennessee and his medical degree from Baylor College of Medicine in Houston, Texas. His residency in general and orthopaedic surgery was at Eisenhower Army Medical Center in Fort Gordon, Georgia. Dr. Phillips, a prior Army Officer, served as Chief of Orthopaedic Service, while stationed at Fort Sill, in Oklahoma, both before and after his deployment to Tallil, Iraq. Dr. Phillips is Board certified by the American Board of Orthopaedic Surgery, and is a member of the American Academy of Orthopaedic Surgeons and the Southern Orthopaedic Association. He has extensive experience in orthopaedic trauma care, upper and lower extremity surgery, joint replacement surgery and sports injuries. Dr. Phillips resides in Blountville with his wife, Sarah.

or a working adult make arrangements to allow for time off during recovery. When we discuss surgery, we may review different treatment options appropriate for different patients at different points in their life, with varying needs. For example, does every knee with a torn anterior cruciate ligament require a complex reconstruction? No, but every knee with this injury can be treated and made better in some manner. My job is to give the patient the information they need to decide if and when they are ready for a surgery. When we do decide to pursue surgery I spend as much time planning the rehabilitation and recovery as the operation itself. I will frequently brief patients that many procedures are “day surgeries” thanks to the advances of medicine, we cannot accelerate the time it takes the body to heal. Before we ever commit to a surgery, both my patient and I understand exactly what will be planned and how the recovery period will play out. More than just healing time, recovery includes both formal therapy as well as a personal commitment to return to normal activities when safely able. All patients are started in therapy within a week of surgery (some on the same day), and at every follow up I review what a patient’s work and life activities would entail in order to allow them to return to as much activity safely as possible. While surgery does involve the majority of my week in my day-to-day practice, the majority of my interaction with each individual patient occurs in the months prior to and following a surgery. While we as orthopaedic surgeons are often compared to carpenters and mechanics, I tend to see myself in terms of a consultant and coach and above all partner in the care of my patients. So, while I enjoy pursuing the science of medicine to offer the most advanced surgical treatments available, I am most satisfied by the relationships I form with those whom I can help. – Dr. John Phillips

Billie J. Minton

The Drug Expert

“Life is your private party, celebrate it with exercise! Your body should be the best outfit you ever wear.” –Dr. Billie J. Minton

Dr. Minton is an entrepreneur and a true pioneer of Medication Therapy Management. She is the Founder, CEO and co-owner of six established companies. Dr. Minton is well-known for her effective leadership, compassion, dedication and is truly a person of integrity.

Dr. Billie J. Minton's goal is to get patients on the least amount of medication for the maximum benefit while still targeting disease states and symptomology. She provides vision and leadership in Pharmacy Practice through her private practice with her patients and colleagues.

Billie’s parents have shaped her into the woman she is today. One of her favorite quotes came from her father, Philip F. Minton, “Do not accept an answer of NO from someone that’s not qualified to give a YES. He also said, "reality is the biggest pill that you will ever swallow."

"Billie is intelligent, caring, loving and lives by example. You cannot ask for more in a doctor, person or friend!" --Doug Davidson, film/tv actor, The Young & The Restless, Los Angeles, CA

“I grew up with four sisters, three older and one younger. My second eldest sister had to take lots of medications just to survive. This always made me both sad and curious because I wanted to know why she had to take all of these medications. Also, I wanted to know whether or not these medications were really necessary; helping her or even making her worse. I watched these medications cause many problems for my sister such as side effects, adverse effects and drug interactions. My Mother and a nurse, Evelyn Minton, had to dose these pills out to my sister on a regular basis. Everyone in our family would call my mother when they were sick or needed to learn more about diseases and medicine. I knew then that Mom was special and that I desired to be just like her! Your life is only as good as the people that are in it. You should touch everyone in your life with respect." –Dr. Billie J. Minton Dr. Billie J. Minton has dedicated her life to the professional values of patient-centered private practice. Dr. Minton is the CEO and Founder of Medication Management Center, LLC. located in East Tennessee, providing medication counseling and disease management for adults and children. She has been in private practice since 2000. She provides clinical service evaluations, while monitoring all aspects of health for the patient in relationship to the disease profile including prescribed medications, over-the-counter (OTC) medications, herbal preparations and lifestyle. Her goal is to educate to the public that “there is not a pill for every ill.” “Dr. Minton has had the right idea for years. Less medication is better. Prescription medication should be a last resort. She has always been ahead of the curve on this. I’ve been referring patients to her for years. She knows how important nutrition & lifestyle are.” –Dr. Tom Rogers

"Dr. Minton, I would like to thank you for helping me feel so much better, life is worth living again. Thank you so much for your help. God bless. Keep up the good work! -- Dolly Dianna Bowman, Kingsport, TN Instead of medications, Billie Minton, PharmD, dispenses life-altering advice to patients who feel they have nowhere else to turn. As a private medication consultant with a community practice, Minton reviews patients' medications, diet, and exercise habits during one-on-one sessions, uncovering problems that may otherwise go unnoticed and making recommendations to help patients feel better. Counseling and educating patients is as important as prescribing and dispensing medication. Many deaths and tragedies have occurred due to lack of education and communication concerning patient's medications and disease states. "When I was on forty pills I couldn't even get out of bed. Then I came to see Dr. Minton. She has worked with me where I can get down now to 6 or 7. I feel great. I feel wonderful, like a new person." -- Rosetta Vinson, Bristol, Tennessee "Dr. Minton is extremely thorough and conscientious. There's no question that her services are a benefit to the community. Patients say she spends extra time with them. Because she has an unusual area of expertise, she's able to offer patients more information than they can get from pharmacists or other health professionals." --Margaret Robbins, M.D. "I feel Dr. Minton is genuinely concerned with my well being. She's personable, caring and very consciencious. Anything she could do to help get my life back and to get healthy, she's there. She has made a real difference." -- Brenda Walker, Kingsport "As a liscensed nurse, I was so impressed with Dr. Minton. She puts time, effort and knowledge into every patient. Her caring about the well being of her patients is a wonderful thing in today's world.

She is a Godsend to people who want to take charge of their lives and make every day a "feel better day." Lord Jesus bless her in all she does." -- Shelia Hooven, Kingsport A few of Dr. Billie J. Minton’s accomplishments: Dr. Billie J. Minton coauthored Medication Management Bill with TN State Rep. Nathan Vaughn, SB 1899 Dixon/HB1958 Vaughn Reimbursement for Medication Therapy Management Services, which PASSED both the Senate and House in May 2003 and became law that year. • In September of 2005 Dr. Minton was in “Pharmacy Today” with a feature article entitled “Consultant Pharm.D. Solves Patient’s Medical Mysteries.” Dr. Minton describes how medication reviews lead to patient-specific solutions • Dr. Minton’s weekly radio show in Kingsport, TN educated the public to the fact that “there is not a pill for every ill.” • She appeared as a guest speaker on National Public Radio (NPR) and has traveled the country educating the public about medication counseling and disease management. • Dr. Minton was featured in the Mercer University Publication, Mercer Health; “Pharmacy Alumna Dispenses Advice Instead of Medications in 2002.”

"Sometimes we get a glimpse of God's grace through others. God wants us to be a willing vessel." – Dr. Billie J. Minton

* She was featured in a Kingsport Times newspaper article entitled, "Dr. Minton is solving pharmaceutical mysteries" in May of 2002. "I engage my patients with a no nonsense, common sense approach. Always, weighing the benefit versus the risk in every decision I make. I thank my patients for allowing me to be a professional member on their health care team. There is no one health care professional that has all the answers. We all have a special contribution to make for the good of the patient." Dr. Minton's favorite author and quote, "Common sense is genius in it's working clothes." -- Ralph Waldo Emerson This is a factoid, you are going to breathe more than 23,040 times today, so take it easy! Topical Gels Offer Chronic Pain Relief

Topical gels are applied directly to the area of pain and affect the tissues directly beneath that area. Administration of the gels is painless, with minimal amounts entering the body. This lowers the risk of toxicity (poisoning) and gastrointestinal problems. The following information is from studies in which topical gels have been used effectively to treat pain. Neuropathic pain, caused by damaged or dysfunctional nerves, can be relieved by local anesthetics and anticonvulsants1, according to Oscar A. deLeon-Casasola, M.D., of Roswell Park Cancer Institute, State University of New York at Buffalo. Undesirable side effects, such as increased sensitivity caused by nonsteroidal anti-inflammatory drugs (NSAIDs), can be avoided with the use of topical gels. Following are proven examples of such use: •

Topical ketamine can reduce the effects of substance P, a neurotransmitter implicated in pain syndromes;

Topical administration of clonidine helps alleviate a condition (sympathetic afferent activation) causing increased heart rate and blood pressure, among other things;

Topical opioids can prevent pain at the site of an injury by binding receptors in the brain, spinal cord and digestive tract. Binding receptors reduces the release of the calcitonin gene-related peptide (CGRP) and substance P from nerves, where the pain mechanism originates;

Topical application of pentoxifylline has effectively provided relief for some neuropathic conditions;

With their potential to provide pain relief without adverse side effects, topical NSAIDs are widely used to treat acute musculoskeletal conditions, often caused by strains, sprains or sports/overuse-type injuries.

pharmaceuticals

For chronic pain sufferers worried about drug interactions or drug levels in their bodies, topical ointments and/or combination therapies offer viable alternatives. Ease of use, fewer side effects and reduced costs make them attractive – and beneficial – for all patients, especially the elderly. These treatments also may be customized for individual needs.

Sensitivity can intensify and bring about chronic neuropathic pain even when nerve transmission of information to the brain is not occurring.

In 47 clinical trial studies involving 3,455 participants, 50 percent experienced pain relief. For treatment periods of six to 14 days, topical diclofenac, ibuprofen, ketoprofen and piroxicam produced similar results. The analyses concluded that “topical NSAIDs can provide good levels of pain relief without the systemic adverse events associated with oral NSAIDs...”2 Topical creams also can be effective in treating osteoarthritis. Researchers at the University of Connecticut School of Medicine examined the effect of a topical cream in treating 40 patients diagnosed with osteoarthritis of one or both knees. Treatment consisted of cetylated fatty acids (CFA). After treatment, significant improvements were noted for knee range of motion, ability to walk up and down stairs, walking and sitting down, ability to rise from sitting, and unilateral balance.3 The professionals at Anderson Compounding Pharmacy have received advanced training and use specialized equipment, FDA-certified chemicals and cosmetically-appealing bases to customize medications that address the individual needs of each patient. We work together with physicians and other healthcare practitioners to solve medication problems. Topical tricyclic antidepressants, such as doxepin and amitriptyline, have been effective in reducing neuropathic pain.

3 Journal of Rheumatology. 2004 Apr; 31(4):767-774

When neuropathic pain is not reduced with the use of opioids, other medication – such as tricyclic antidepressants, anticonvulsants and local anesthetics – is often used. 111

BREAST RECONSTRUCTION: DOING SOMETHING POSITIVE

Hello. My name is Dr. Jim Webb. I’m a plastic surgeon in Kingsport, Tennessee, and I’ve lived here with my family for approximately four years after my military retirement. I would like to talk to you about breast reconstruction following mastectomy for cancer. If you, or a loved one, have encountered this dreaded disease, you may already have spoken to a plastic surgeon about breast reconstruction. My first encounter with breast cancer came from my grandmother, when in her 60s, she was required to undergo a mastectomy. My grandmother was a very spiritual woman and she rarely wore jewelry or makeup. Her clothing was simple and consistent with the hard-working, honest, singlemother lifestyle she was required to live raising two children. I don’t recall how we got on the subject, but I remember her telling me that after having had a mastectomy, she felt “deformed,” that she would turn the lights off when she got out of the bathtub so she would not

As I continued to encounter patients who had various deformities, either from trauma or surgical treatments, I became interested in the techniques used by plastic surgeons to turn deformity into something positive, better than just a

healed scar, but a scar which considered form and function and minimized deformity. And that is what breast reconstruction is all about – the need to feel good about yourself and confident in your presentation does not change with age, as my hard-working grandmother taught me. Certainly the goal of breast reconstruction would be that perhaps your appearance would be better than it was before your mastectomy; although, this is not always achievable. More importantly, our goal is that you feel good about yourself and have the confidence you need, whatever your lifestyle may be.

Plastics

Hello. My name is Dr. Jim Webb.

see herself in the mirror. She felt that when she went to church, people knew that she didn’t have a breast and would look at her. I doubt that was really the case, but she felt it was. Although I had not yet been admitted to medical school, it was then I learned that it’s not about what other people think, it’s about what you think about your body and how it makes you feel that is most important. She certainly wasn’t concerned about attracting men; she only wanted to feel normal. To wear her clothes with confidence and not worry about her breast prosthesis falling out from beneath her dress in the grocery store, as it did on one occasion, much to her embarrassment. She told me that rather than pick it up off the floor, she simply kicked it under the shelving until no one was around and she could retrieve it. Years later, after I had entered practice as a general surgeon, I remember looking at mastectomy flaps and feeling a bit of pride and relief when the skin I had elevated healed and the chest was closed, but I could see in the faces of the ladies, standing from the exam table, that rather blank stare and disappointment as they reached to put on their prosthesis and cover up the work that I was – for lack of a better word – proud of.

In selecting a plastic surgeon to help you with your reconstruction, you should consider that surgeon’s willingness to learn about you; not only what your health situation is, but also your social and family factors that impact what type of reconstruction is best for you. Your general health, cancer treatment requirement, body habitus, age, family situation, will significantly impact the timing and type of breast reconstruction you choose. At New Horizons Plastic Surgery, we are committed to assisting you in this decision-making process. So whether you have had a mastectomy in the past, or find yourself in the difficult situation of being recently diagnosed with breast cancer, it would be our pleasure to discuss with you your options. Be positive. Do something positive.

Dr. Chris Allen holds an undergraduate degree in Zoology from Auburn University. His doctorate in Podiatric Medicine is from Des Moines University College of Podiatric Medicine and Surgery. He completed his residency at James H. Quillen School of Medicine in Johnson City, Tennessee. Dr. Allen and his wife, Melissa, enjoy spending time together with their four lovely girls, Summer, 14, Sarah Grace, 12, Elizabeth 5, and Emmerson, 3. They recently completed a house restoration project in a historic Bristol neighborhood. Dr. Patrick Saavedra completed his undergraduate degree in Biology at the University Of Akron. He graduated from the Ohio College of Podiatric Medicine in Cleveland, Ohio and completed his residency in Podiatric Surgery at the Dunlap Memorial Hospital in Orrville, Ohio. Dr. Saavedra and his wife, Dr. Jennifer Greiner, an ENT physician with Meadowview Ear, Nose, and Throat Associates, moved to the area in 2011. Dr. Saavedra is an accomplished runner and has competed in several marathons. Dr. Saavedra enjoys all aspects of Podiatric practice but especially

those conditions involving sports related injuries and issues. Drs. Allen and Saavedra are assisted in practice by a wonderful staff that treats every patient with compassion and respect. From the moment you walk in the door and until your care has been completed, everyone treats you as one of their family and smiling faces abound. Glenda, Valerie, Bambi, Christy, Allyson, Samantha, and Markalene all contribute in making your experience remarkable. Obesity has reached epidemic proportions in the United States. According to the Centers for Disease Control and Prevention approximately 35% of adults are obese, and recent studies predict and estimated 42% of adult obesity by the year 2030. Obesity can cause and exacerbate foot and ankle conditions such as heel pain, plantar fasciitis, tendonitis and flat feet. These painful conditions can make it virtually impossible for many to exercise and lose weight. Without exercise and weight loss, other health hazards like heart disease, diabetes, peripheral arterial disease and stroke can progress and worsen. At Highlands Podiatry, we are able to treat many foot and ankle conditions so the patient can begin, or continue and complete an exercise program. Activity-limiting conditions can include heel pain, plantar fasciitis and foot deformities like bunions or hammertoes. Left untreated, these conditions become chronic, and a vicious cycle of inactivity due to foot and ankle pain occurs, having a direct effect on weight gain and lack of physical activity. Many causes of foot and ankle pain, such as heel pain, can be treated non-surgically with the use of orthotics, stretching exercises and good, supportive shoes. Some conditions, like bunions and hammertoes, may require surgery, and non-weight bearing or lowimpact exercise can be performed during the recovery period. The physicians at

Highlands Podiatry are highly trained in a wide variety of surgical and non-surgical treatment protocols for many foot and ankle pathologies. An increase in physical activity and exercise can also help to control some of the symptoms of diabetes, such as hypertension, peripheral vascular disease and diabetic neuropathy. The diabetic patient who is overweight and intends to start an exercise program needs to have a thorough examination by a Primary Care Physician. A comprehensive diabetic foot exam is recommended every 6 months, and more frequently for the diabetic who is “atrisk” due to heavy callus formation, bony foot deformities, poor circulation, or lack of sensation in the toes or feet. Wearing the appropriate shoe is also very important during exercise, and a diabetic shoe with a custom-molded insert is recommended for the “at-risk” diabetic patient. This will allow for safe participation in the specified weight-loss program.

podiatry

Highlands Podiatry provides comprehensive foot and ankle medical services to all patients who present for treatment. With practice locations in Bristol, TN, as well as Abingdon, VA, Highlands accepts most insurance plans, including Medicare and Medicaid (TN & VA). Providing medical services five days a week in both locations, Highlands is ready to accommodate any foot and ankle medical need. Dr. Chris Allen founded the practice in 2002 and Dr. Patrick Saavedra joined him in 2011. The physicians provide surgical services at Bristol Regional Medical Center and Sapling Grove Ambulatory Surgery Center in Bristol. Highlands provides orthotics, diabetic shoes and durable medical equipment to patients, as well as topical antifungal medication for nails and various accommodative pads for hammer toes and bunions.

Chronic foot and ankle pain should not stop obese patients from participating in exercise programs. Painful foot and ankle conditions like heel pain, plantar fasciitis and tendonitis can be evaluated and successfully treated at Highlands Podiatry. We strive to improve quality of life by treating foot and ankle pain, thereby allowing for an increase in physical activity in order to promote meaningful weight loss and battle the epidemic of obesity. Highlands’ Bristol office is located at 2765 W. State Street, and our Abingdon office is located at 616 Campus Drive, Suite 300 off Russell Road.
