Sending ambulance care summaries to general practices Information for general practice teams The St John and Wellington Free Ambulance electronic patient report form (ePRF) system has recently been enhanced to enable ambulance care summaries to be sent to GPs via Healthlink when their patient is seen by ambulance staff and not transported to a medical facility. Sending ambulance care summaries to general practice teams will improve patients’ continuity of care, ensure you are aware of an ambulance attendance to your patient and improve information sharing within the health sector. The ambulance care summary has been designed specifically for general practices, with feedback incorporated from the Royal New Zealand College of General Practitioners. We will start sending ambulance care summaries to general practices in the Wairarapa, Southern and Bay of Plenty DHB areas for two weeks commencing on 5 June 2020, prior to going live across the rest of New Zealand on 19 June. During the two week pilot phase, general practices outside the Wairarapa, Southern and Bay of Plenty areas may receive an ambulance care summary if one of their patients is attended to by ambulance staff within the Wairarapa, Southern or Bay of Plenty areas (for example, if a patient is on holiday in one of these areas). Calling all GPs New ambulance care summaries from St John and Wellington Free Ambulance St John and Wellington Free Ambulance are committed to working with general practice, to ensure information is connected to a patient’s healthcare home when they are assessed and treated by ambulance personnel in the community. St John and Wellington Free Ambulance will soon start sending ambulance care summaries to GPs where the patient has not been transported to a medical facility. Ambulance personnel attend over 500,000 patients each year. Of this number, over 80,000 patients with a low acuity condition are assessed and treated by ambulance personnel in the community, without requiring transport to a medical facility. Sending an ambulance care summary to the GP will keep the patient’s general practice team informed when a patient is seen by ambulance personnel.
What is an ambulance care summary?
What you’ll see
An ambulance care summary lets you know that ambulance personnel have assessed and treated your patient in the community. The ambulance care summary includes the history taken, a summary of the patient’s clinical problem, any treatment administered and recommendations provided to the patient. You will also receive a copy of the ACS for deceased patients
GPs will receive the summaries via HealthLink into their PMS system and the summaries will have an NHI number to allow easy filing.
How will the ambulance care summary be sent?
If General Practice Teams wish to download the full ambulance care summary, the ACS access code on the ambulance care summary, along with the patient’s DOB, can be entered into https://acs.stjohn.org.nz/
The ambulance care summary will be sent via Healthlink using HL7 protocol, ensuring security and privacy.
What an ambulance care summary is not An ambulance care summary is not intended as a transfer of care; if there is ever a need for a transfer of care or urgent action to be taken, ambulance personnel will phone you.
You can choose to read the summaries as they come in, look at them the next time you see that patient, or have another member of the general practice team (e.g. practice nurse) review them.

We hope ambulance care summaries will be of use to you in future consultations with your patients and we’re keen to hear your feedback on their format and clinical content.
