Health and Wellness Services Workshop Wednesday 25th November 2020
The PHO’s Health and Wellness Services are available to support the enrolled population of General Practices in the community. For those not currently enrolled with a GP, we encourage and support people to enrol with a GP of their choice. Our focus is on extending Health and Wellness Services into the community as the eyes and ears for your practice. While we have a walk-in clinic in First Avenue, many services are delivered from community locations such as marae, night shelters, refuges, community centres, and in some cases, people’s homes. The services we offer include immunisation outreach, wound care, long-term conditions management, lifestyle wellness and diabetes self-management groups, and our Workplace Wellness programme. While there are some limitations to the support we can offer, we work hard to identify solutions and engage patients with General Practice. We would like to take this opportunity to ask General Practices how our services can meet the future needs of your practice and community: • • • •

The Health and Wellness Services team, assisted by Blink PR, will be facilitating this session so bring along your feedback and ideas and we will use this information to continue developing our services to meet the needs of your enrolled population.
