Start completing a BOP CBAC patient form and a Covid-19 lab form

Forward requests to PHN.Referral@bopdhb .govt.nz

Swab patient, complete BOP CBAC patient form and Covid-19 lab form

Email BOP CBAC patient form to WBOPPHO for data collection elizabeth@wboppho.org.nz

Drop Pathlab form and swab directly to Pathlab

If team arrive to site and unable to safely carry out test. He team is to contact Karen Thomas (027 590 3340) who will then escalate to Clinical Director for guidance.


Information CCC need to collect for CBHS Clinical Director -

Person’s mobility (can they get in/out of car?) Number of symptomatic people within household/bubble Location of person Are they palliative, immunosuppressed, do they have significant health issues/symptoms/comorbidities? - Any risks/hazards identified that would prevent a transport team or home-based testing team safely entering te person’s property? - The person’s social situation (vulnerable person i.e. single mother with a new born Guidelines only for CBHS Clinical Director Send home-based testing team to person’s home - Person has poor mobility/can’t get in and out of a car - 3+ number of people in the house with symptoms - There is an available and safe area outside the house to carry out test

Potential No test – identified as probable case not tested - Person has significant health issues/symptoms - requires clinical assessment (this includes Palliative and Immunosuppressed) - There is identified risk to either the transport team or the testing squad team to enter the person’s property - Rural location of the person (impact of sending a team to test person at home vs. transport a person to CBAC site) - Any person’s situation that doesn’t fit into identified situations.
