Signature Requirements for Electronic Enrolments

The latest enrolment reference documentation, Enrolment Requirements for Contacted Providers and PHO version 4.1 accepts that the enrolment process can be electronic or hard copy and the signature can be ink or digital pen. As shown in the highlighted part of the table below from Section 2 Enrolment Overview:

Signing of an electronic enrolment form The signature section on the electronic enrolment form should allow the enrolee to move the computer mouse or digital pen to form the signature, (although the signature may be slightly messier) the intention of the Enrolment Requirements will be met (i.e. that the enrolling person actively and personally confirms their enrolment) as advised by the Ministry of Health. A copied and pasted signature or a person’s name typed on an electronic enrolment form requires no involvement from the enrolling person and in that sense is not the same as a manual entry (in digital or ink format) so would NOT be acceptable. If an enrolee has completed an electronic enrolment form, there is no need for them to also complete a hard copy. However, the electronic form must be able to be printed in a format that presents all Enrolment Form requirements.
