Expressions of Interest Clinical Leader Diabetes The Clinical Leader Diabetes (CLD) provides clinical advice and support to develop strategic direction for diabetes prevention and care in Aotearoa. The CLD contributes to the leadership and development of the National Diabetes Programme and works with the Ministry of Health (the Ministry) and the health sector to improve outcomes in diabetes across the country. The Population Health and Prevention Directorate works closely with the sector to support the implementation of Government’s strategic priorities in health through using evidence to support implementation, building capability and sharing innovation. The Ministry has a strong commitment to supporting Māori, Pacific, Indian and young people to improve diabetes outcomes in the context of their whanau and communities. The work directly contributes to the aims of • Whakamaua Māori Health Action Plan and • Ola Manuia: Pacific Health and Wellbeing Action Plan 2020–2025. The CLD will have a clear commitment to enacting Te Tirirti o Waitangi (Treaty) principles and fostering manaakitanga (care, inclusion, respect, support, trust and kindness) in Aotearoa’s response to diabetes. The National Diabetes Work Programme and the continued implementation of the Ministry of Health plan Living well with Diabetes: A plan for people at high risk of or living with diabetes 2015–2020 across district health board (DHB), primary health organisation (PHO) and primary care. The CLD will support the development of an integrated programme that covers the Ministry, DHBs, and national and regional diabetes activity across the continuum, to improve Māori health outcomes and implement the Government’s priorities for diabetes. Purpose of the role The CLD is accountable and responsible to the Manager, Long Term Conditions for contributing to the leadership, development, innovation and implementation of a National Diabetes Work Programme. This will be achieved by working with sector leaders in primary and secondary care, Māori and Pacific health to improve health equity and improve the delivery of diabetes services. A key feature of the role will be to contribute to the design and implementation of an agreed work programme. The CLD will act as the diabetes clinical spokesperson and provide clinical advice for diabetes service management, and further development of the work programme. They will have leadership responsibilities within a Ministry National Diabetes working group and contribute diabetes clinical leadership to Long Term Conditions leadership forums.


Objectives of the role The objectives of the role are to: • provide clinical advice and leadership on issues relating to diabetes • provide leadership in addressing the health inequities in diabetes care • provide guidance on the quality improvements that can be made in acute and chronic services to improve diabetes care • work with the DHB diabetes leadership groups or Diabetes service alliances to implement key recommendations from the strategic priorities for diabetes • understand the VDR and contribute to data improvements related to diabetes • support and participate in the DHB reporting process • provide input into the Ministry diabetes work programme • support the development of a revised plan of action to clearly identify and improve the health of vulnerable and target groups across Aotearoa • support the health sector to improve diabetes services in New Zealand • support development of education and training for the diabetes workforce • identify national diabetes workforce issues • provide leadership within the Ministry Diabetes working group and broader Long Term Conditions forums • support quality assurance for diabetes services using MOH Quality Standards for Diabetes Care • support the development of diabetes clinical guidelines with the relevant sector groups including Māori and Pacific Working relationships The CLD will be required to develop and maintain effective working relationships, both within and external to the Ministry. These relationships will support improved outcomes for people with diabetes and identify and support equitable access to diabetes care. The CLD will have a close relationship with Māori and Pacific communities and teams within the Ministry as well as a direct working relationship with the diabetes team at the Ministry. The CLD will also be expected to take a systems wide approach and work effectively with: • Māori Health • Pacific Health • nurses • doctors • allied health professionals (eg, podiatry, ophthalmology and dietetics) • NZSSD • DHB management • primary care, PHOs and GPs • DHB diabetes networks • NGOs, Diabetes NZ, Diabetes Foundation Aotearoa and other allied NGOs (eg, Heart Foundation/Stroke Foundation) • PHARMAC • consumer groups • relevant Ministry of Health work streams, and specifically leadership and relationship-building are important components of this role.

Term of the role The funding for this role is for a fixed-term of one year from 1 November 2020 to 31 October 2021, with possible renewal for a further two years as mutually agreed by the individual, the individual’s employer, and the Ministry. This role requires a commitment of eight hours per week for 42 weeks. There is flexibility as to how these eight hours are committed, and may not necessarily mean eight hours each week, however there is an expectation of availability for queries and timely responses. This is a sector-based role and will constitute employment on a fixed-term contract. There is some flexibility around the approach and options include: • employment on a fixed-term contract for one year • secondment into the role. Travel and other associated expenses are managed by advance agreement with the Long-Term Conditions Manager. Experience The role requires a person with a clinical background who has an interest in improving diabetes management and outcomes and in reducing inequities in diabetes service delivery and access across Aotearoa.
