Kia ora Practice Managers
PHOs are required to provide aggregated information to Te Whatu Ora regarding how practices in their respective networks have used the pay disparity funding. Te Whatu Ora explain that collecting this reporting information is an important part of the initiative, as they need to demonstrate to Ministers, Treasury and Manatū Hauora on the outcomes delivered by the funding provided.

This information will be provided to Te Whatu Ora to fulfil reporting requirements for two pay periods:
- The payday immediately preceding 14 December 2022
- The payday immediately preceding 31 August 2023.
This form will be open for submissions until 5pm, Friday 15 September
Before you begin
Gather the information that you need before beginning this form. You will not be able to save your form as a draft.
You will need to provide information on how you have utilised the additional funding to: Increase base pay rates
• Add or increase penal rates
• Add or increase other allowances.
You will also need FTE, total hours paid in the payrun, and base pay rate (minus allowances, penal rates and overtime), for each of the following roles at your practice:Enrolled Nurses
• Registered/Community Nurses
• Designated Senior Nurses
• Nurse Practitioners
• Kaiāwhina (see FAQS if you're not sure of the definition).
