Referral process for patients who are housebound

Patient with respiratory symptoms and / or suspected COVID-19 Unable to travel to GP Practice or Testing facility

Patient contacts General Practice

Client meets clinical criteria for testing and safe for homebased test testing authorised by GP/NP

GP/NP complete triage form in Halcyon (WBOPPHO) or eReferral (EBPHA) Claim Tier 1 Virtual Consultation

Refer Patient to Community Care Coordination Centre (CCC) 0800 267 222 (0800 BOP CCC) or email admin@bopccc.org.nz.

Public Health Nurses will contact patient, visit and collect swab. Visits will be done on Monday, Wednesday, and Friday unless an urgent request is made. Swab will be delivered to Pathlab for processing

Testing Completed - GP to manage results and patient notification All positive results will be followed up by Toi Te Ora

If Testing cannot be completed - CCC team will notify referring GP
