A Withdrawal Form is available in the DGCE Office. If you withdraw after the second class meeting, you must obtain the signature of the faculty member.

ng 4 Winter /Spri2012 Non-attendance does not ensure withdrawal from a course. It is the student’s responsibility to withdraw from courses as outlined above, or a “Failure to Withdraw” grade of “F” will result. At the instructor’s discretion, a student may sometimes be reported for non-attendance and an Administrative Withdrawal may be requested, but it should not be assumed that non-attendance will result in withdrawal from a course. In addition, non-payment does not result in Administrative Withdrawal from a course. Professor’s Transcript Withdrawal Signature Notation Before first class meeting No None Before second class meeting No None Before third class meeting Yes ‘W’ By withdrawal deadline (see above) Yes ‘W’ After deadline* ($25 fee) Yes ‘W’ Administrative withdrawal for non-attendance** ‘W’ After drop-add period N/A ‘W’ Failure to withdraw N/A ‘F’ * Approved for extenuating circumstances only; must have the Dean’s approval. **At instructor’s discretion.

Financial Information Important Notice: Failure to pay within 5 days of web registration will result in the addition of the $100 late payment fee. • Both registration and educational service fees will only

be refunded for officially canceled courses; education service fee is refunded for withdrawals before the semester begins. • Registration fee is non-refundable. No refunds for Winter Session after class session begins. • Please note that online courses may have earlier start dates. • We accept checks, money orders, Mastercard, American Express, Visa and Discover cards. Cash cannot be accepted for payment over $10.

Withdrawal and Refund Schedule Refunds of tuition and instructional fee, if any, are made on the basis of the date and time of receipt of a student’s withdrawal. Non-payment does not result in withdrawal from a course. Students who officially withdraw from a course after the second scheduled class and within the allowable withdrawal period will receive a designation of W. The registration fee is non-refundable. • Before the semester begin date, students are responsible for $75 (Registration Fee). • After the semester begin date but before the first class meeting, students are responsible for $150 (Registration and Educational Service Fees), and the course will not be indicated on the transcript. • Before the second class meeting, students receive a 75% refund of total tuition and fees, and the course will not be indicated on the transcript. • Before the third class meeting, students receive a 40% refund of total tuition and fees, and the course will appear as a Withdrawal (“W”) on the transcript. • There are no refunds for classes dropped after the third class meeting. Students dropping at this time will incur a $25 Late Withdrawal Fee and require the signature of the instructor. The course will appear as a “W” on the transcript. For refund and withdrawal purposes, Online and Brick & Click courses are deemed to begin “meeting” on the first day of the semester at 9 a.m., unless otherwise noted. They will be considered “meeting” as other in-person courses that begin the first day of the term (i.e., once per week for fourteen weeks in fall/ spring, twice per week for six weeks, or every day in winter). This refund and withdrawal schedule follows this “meet” schedule and is effective regardless of individual student login.

Meal Plan Refund Schedule: Prior to the first day of classes 100% During the first two weeks of classes 80% During the third week of classes 60% During the fourth week of classes 40% During the fifth week of classes 20% After the fifth week of classes No Refund

In order to avoid being assessed a state mandated insurance charge you must waive the fee by entering your medical insurance information online at www.universityhealthplans.com

and are responsible for total payment even if they withdraw before the end of the semester and regardless of grade. The first payment is due with registration, and includes the $100 non-refundable fee. The remaining payments are due according to the following schedule: Fall Semester Last Friday in October Last Friday in November Spring Semester Last Friday in February Last Friday in March Summer Semesters At mid-point in term
