MEDICA EDUCATION CONFERENCE 2015 sheds light on the future of surgery

M

edical technological systems that require medical expertise and technological know-how to be closely linked will be the primary focus at the MEDICA EDUCATION CONFERENCE, the informational platform for medicine and medical technology. It will once again be held in Düsseldorf from 16 to 19 November 2015 within the scope of the world’s largest medical trade fair, MEDICA, with around 4,800 exhibitors and recently around 130,000 specialist visitors taking part on a regular basis. This year, a day of the event will be dedicated to surgery and new operation techniques. On 16 November 2015, the primary focus will be on innovations in the field of surgery. Within the scope of symposiums and frontal lectures on “Science and Medical Technology”, experts from the hospital, research and industrial sectors will be explaining and discussing which aids will provide support to surgeons in the operating theatre in the future. How can interventions be carried out in an even further minimally invasive manner whilst being even gentler on patients? Engineers and physicians are cooperating on a world-wide level to bring surgical systems out of laboratory and into hospitals.

The telesurgery system “MiroSurge”

One example of this includes the telesurgery system, which was developed by researchers at the German Aerospace Center (DLR) located in Oberpfaffenhofen, which will be deployable in operating theatres in a few years. A basic component of the system includes the “Miro” robotic

arm. Several of these Miro arms together with an input console with a 3D display and two tactile input devices make up the “MiroSurge” system. The surgeon controls several input arms at one time using input devices located a few metres away from the patient. Thereby, one robot arm is equipped with a stereo HD endoscope camera that transmits video images of the patient. The other two arms are equipped with DLR instruments for working with both hands within the operating field. These instruments are powered by electrically driven motors and sensors are located at the tips of them. The highly sophisticated integration of sensors allows direct interaction to take place between physicians, robots and patients. This means that the sensors measure split-second interactions that take place between the tissue

within the operating field. These are passed on to physicians as a tactile experience – they feel that they are cutting into tissue. In 2012, Stent Retrievers were associated with a mortality rate of 17.1 percent vs. pre-retriever devices in both 2008-2011 (19.3 percent) and 2006-2007 (25 percent). Additionally, the age-specific mortality following stroke intervention decreased with newer devices for patients over 80 and 65-79 years of age: 2012 (20 and 14.9 percent, respectively) compared to 2008-2011 (25.8 and 20.5 percent, respectively) and 2006-2007 (26.3 and 30.2 percent, respectively). These findings are significant as they prove that older patients can in fact benefit from IA therapy and that age, alone, is not an exclusion criteria for interventional treatment.

Out of a pool of 932 subjects, study results showed that the newer and larger Penumbra catheters provided a faster and more efficient method for treating acute ischemic stroke. In turn, when considering variables including age, baseline National Institutes of Health Stroke Scale (NIHSS) score (which assesses stroke severity), and post-treatment status of restoration of blood flow, shorter procedural times were a significant predictor of better clinical outcomes at 90 days, as measured by the modified Rankin Scale (mRS), a standard tool used to indicate level of independence following a stroke. Dr. Donald Frei, lead author of the study, president of the Society of NeuroInterventional Surgery (SNIS) and neurointerventionalist at Swedish Medical Center, echoed the optimistic sentiment espoused by Boddu. “As each new generation of devices provides improved tools for our treatment arsenal, we are able to adjust our approach to maximize efficiencies and results for our patients. Time is the most important factor when treating a stroke, so it is incumbent upon all of us to use every tool at our disposal to achieve fast and effective treatment,” Frei said. “Thankfully, each generation of new devices has proven to reduce procedure time, which raises the chance of a successful operation.”

3-D printing techniques help surgeons carve new ears week at the American Academy of Otolaryngology - Head and Neck Surgery conference in Dallas. “It’s a huge advantage over what we’re using today,” said lead author Angelique Berens, a UW School of Medicine otolaryngology - head and neck surgery resident. “You literally take a bar of Lever 2000 while the attending is operating and you carve ear cartilage. When surgical residents need to practice a complicated procedure to fashion a new ear for children without one, they typically get a bar of soap, carrot or an apple.

o treat children with a missing or under-developed ear, experienced surgeons harvest pieces of rib cartilage from the child and carve them into the framework of a new ear. They take only as much of that precious cartilage as they need. That leaves medical residents without an authentic material to practice on, as vegetables are a pale substitute. Some use pig or adult cadaver ribs, but children’s ribs are a different size and consistency. Now, a collaboration between a University of Washington otolaryngology resident and bioengineering student has used 3-D printing to create a low-cost pediatric rib cartilage model that more closely resembles the feel of real cartilage and allows for realistic surgical practice. The innovation could open the door for aspiring surgeons to become proficient in the sought-after but challenging procedure. Their results are described in an abstract presented this 8 | September-October 2015 | Medical Device ASIA

As part of the study, three experienced surgeons practiced carving, bending and suturing the UW team’s silicone models, which were produced from a 3-D printed mold modeled from a CT scan of an 8-year-old patient. They compared their firmness, feel and suturing quality to real rib cartilage, as well as a more expensive material made out of dental impression material. All three surgeons preferred the UW models, and all recommended introducing them as a training tool for surgeons and surgeons-intraining. Kathleen Sie, a UW Medicine professor of otolaryngology - head and neck surgery and director of the Childhood Communication Center at Seattle Children’s, said the lack of adequate training models makes it difficult for people to become comfortable performing the delicate and technical procedure, which is called auricular reconstruction. There’s typically a six- to 12-month waiting list for children to have the procedure done at Seattle Children’s, she said. “It’s a surgery that more people could do, but this is often the single biggest roadblock,” Sie said.

“They’re hesitant to start because they’ve never carved an ear before. As many potatoes and apples as I’ve carved, it’s still not the same.” Another advantage is that because the UW models are printed from a CT scan, they mimic an individual’s unique anatomy. That offers the opportunity for even an experienced surgeon to practice a particular or tricky surgery ahead of time on a patient-specific rib model. Co-author Sharon Newman, who graduated from the UW with a bioengineering degree in June, teamed up with Berens while they both worked in the UW BioRobotics Lab under electrical engineering professor Blake Hannaford. Newman figured out how to upload and process a CT scan through a series of free, open-source modeling and imaging programs, and ultimately use a 3-D printer to print a negative mold of a patient’s ribs. Newman had previously tested different combinations of silicone, corn starch, mineral oil and glycerin to replicate human tissue that the lab’s surgical robot could manipulate. She poured them into the molds and let them cure to see which mixture most closely resembled rib cartilage. “I would go to the craft store and Home Depot and say I want to make models - what aisle should I go to?” said Newman. “It turns out a lot of these ideas were based off of materials people use for arts and crafts like rings or other jewelry.” The team’s next steps are to get the models into the hands of surgeons and surgeons-in-training, and hopefully to demonstrate that more lifelike practice models can elevate their skills and abilities.

Fighting fakes with the first integral 3-D barcode The first 3D barcode which can be built into products during manufacture has been developed by UK engineers. The anti-counterfeit marker is virtually invisible to the naked eye and impossible to detect by touch. It can be read using a laser scanner, allowing anything from phones to pills to be tracked and verified as authentic. Made public at the British Science Festival in Bradford, UK, the technology was devised by the UK SME, Sofmat Ltd, and has been developed in collaboration with engineers from the University of Bradford. The 3D barcode is made up of tiny indentations in the surface of the product, created by pins which are integrated into its mould. Using micro actuators, the pins can be set at different heights, each step corresponding to either a letter (A-Z) or a number (0-9). The prototype - developed with funding from Innovate UK - works with a four pin array, enabling over 1.7 million different configurations. “The system enables very small displacements to be made in each pin each step being just 0.4microns, 100th of the width of a human hair,” explains Dr Ben Whiteside, from the University of Bradford. “These have to be set with a very high accuracy, and with sufficient force so their position is maintained during the manufacturing process. While our system has been developed initially for products made from plastics or composites through injection moulding, it could also be used to stamp or emboss the code onto a product.” Estimates for the total value of fakes sold worldwide each year reach as high as $1.8 trillion. Most anti-counterfeit devices are stuck onto the product or its packaging after manufacture, making them easy to copy. Because the 3D barcode is an integral part of the product itself it is very difficult to reproduce. “A 3D barcode allows much more complexity than existing anti-counterfeit systems,” explains Sofmat Director Dr Phil Harrison.

High-volume facilities better for nursing hip fractures A potentially useful quality indicator about nursing facilities is the number of hip fracture patients during the prior year There is not a lot of information available to help family caregivers choose the best skilled nursing facility for an elderly loved one who breaks a hip, but a new study suggests a potentially useful quality indicator: the facility’s number of hip fracture patients during the prior year. The sweeping new analysis shows that the most experienced skilled nursing facilities were more than twice as likely as the least experienced to successfully discharge patients back to the community within 30 days of breaking a hip. The results, led by Brown University researchers, appear in the Journal of the American Geriatrics Society. “While volume is not a direct measure of post-acute care quality, it appears that it is a good proxy that captures hard-to-measure aspects of quality, like the expertise of the staff, that are clearly associated with an outcome that patients care about: returning home soon and in a condition that allows them to remain at home without further institutionalization in a hospital or a nursing home,” said lead author Pedro Gozalo, associate professor (research) of health services, policy and practice in the Brown University School of Public Health. Gozalo and his colleagues reviewed records of more than 512,000 patients age 75 and older who sought care at more than 15,400 skilled nursing facilities between 2000 and 2007. The researchers measured the volume of cases at each of the facilities in the 12 months leading up to each admission and noted whether the patient was discharged to the community within 30 days. In all, about 31 percent of people were able to return home within a month, but that rate varied widely among facilities of different experience. The rate of successful discharge from facilities with more than 24 cases in the prior year was 43.7 percent, but it was only 18.8 percent at facilities with one to six cases a year. To check whether this could be for some reason other than volume, Gozalo and his colleagues statistically accounted for a large number of attributes of the patients and of the facilities, such a nurse and therapist staffing levels. “Even adjusting for a long list of patient risk factors and for important facility characteristics, facilities that had cared for more than two dozen hip fracture patients in the last 12 months were more than twice as likely to successfully discharge patients in a timely manner compared to facilities that had three or less hip fracture admissions,” Gozalo said. The study also revealed that many patients are not going to higher-volume facilities, even though they could. According to the records, more than 70 percent of people who went to a lower-volume facility could have found a higher-volume facility within 10 miles. That finding, Gozalo said, suggests that people do not have much information about quality. The Centers for Medicare and Medicaid Services make some information available, but that can be hard for many families to access and interpret. Hospital discharge planners also vary widely in what information they provide to families. “Based on our results, it would seem that in addition to any other sources of information and recommendations the family can get, they should definitely ask each facility they are considering what was the number of hip fractures they cared for in the last year,” Gozalo said. “It is a basic but very informative predictor of how well their loved one may fare at that facility.”

Is the Russia crisis also a crisis for medical technology? Interview with Jennifer Goldenstede, head of department Foreign Trade and Export Promotion at SPECTARIS e.V., Association for Medical Technology The Ukrainian political turmoil has been keeping the world in suspense since 2013. It has also caused growing tensions between Russia on the one hand and the U.S. and EU on the other hand. Both sides try to pressure each other with sanctions against individuals, international financial transactions and whole industry branches. In the interview with MEDICAtradefair.com, Jennifer Goldenstede talks about exports of medical technology to Russia and explains the trade sanctions’ impact on the industry. Miss Goldenstede, how important are medical technology imports from the U.S. and the EU for Russia?

Jennifer Goldenstede: In recent years, they had a high significance, but this has changed now. One example is the German exports to Russia. From 2010-2012 there has been a considerable increase of more than 30 percent each year, reaching almost 70 percent in 2012. In 2013 and 2014 those figures decreased with a decline of around 25 to 30 percent each year. In the first half of 2014 the export volume was still at 292 million Euros, in the first half of 2015 the volume was at 226 million Euros. This means a decline of 23 percent. There currently are economic as well as travel sanctions between the U.S. and the EU on one side and Russia on the other side. What influence do these have on the manufacturers of medical technology?

Goldenstede: The economic sanctions are not the foremost problem for the industry – this would be the import substitutions. The Russian government has decided that those manufacturers of medical technology should be preferred in public government tenders, which really provide added value within the Eurasian Economic Union. There are lists which indicate for which goods and expendable materials the Russian import quota should be lowered more and more. Of course Russian companies are the ones which profit most from this. Foreign companies first have to create added value locally to be able to participate in those tenders.

Goldenstede: We are strongly committed to ensuring that foreign trade fairs such as the ZDRAVOOKHRANENIYE in Moscow can continue to take place. There already were some cancellations out of political reasons in the foreign trade fair program. However, these have not yet affected the medical technology industry. Other instruments that we can use are for example the market development program of the Federal Ministry of Economics, which includes visits of delegations to the country. We have proposed, inter alia, Russia as a destination country. However, it is still unclear whether this will be implemented. Lastly, the information transfer to the member companies is very important. They need to know what is happening in Russia, so they can be prepared and make informed decisions about their business there.

Thus, these sanctions are only consequently an obstacle for companies that want to export to Russia: If a national procurement is not possible, the Russian authorities first check if they can purchase the products from countries that do not sanction Russia. Building bridges: Industry associations can work to maintain and to promote international

Transasia buys a UK based molecular technology company for Rs. 250 crores

What is Living-donor liver transplant? Living-donor liver transplantation offers an extremely successful alternative to deceased-donor organ transplantation. In living-donor liver transplant, surgeons remove a portion of a living donor's liver. Another surgical team then removes the recipient's entire diseased liver and replaces it with a portion of the living donor's healthy liver. The donor's liver and the portion of the donor's liver given to the recipient grow back to their full size within a few weeks. What is donor eligibility and information? Living donors can include: Family members – first and second degree Spouses The following are not allowed to donate: Distant relatives Friends Neighbors Co-workers Staff Living-Donor Liver Surgery Eligibility Potential living liver donors should: Be between the ages of 18 and 55 Not be overweight Have a matching blood group (the same or blood group O; Rh + or – status is not important) Be in good general health and have no history of: Liver disease, including cirrhosis and hepatitis B or C Heart disease Uncontrolled diabetes HIV Cancers which could have or

other diseases that could either spread to the liver or complicate the surgery What are the benefits to the recipient of having a living liver donor versus a cadaver donor? There are several advantages of living donor transplants. First, the success rate of live donor transplants is a little higher than deceased donor transplants. Second, the rejection rates of liver grafts are lower than those from deceased donors. Third, patients needing urgent transplant can get their family member to donate within 1-2 days, whereas getting deceased donor liver grafts that quickly is usually never possible. By the same logic, patients can have an early, planned living donor transplant when they are in the best physical and psychological condition to receive it. However, in case of a deceased donor, it is always an emergency transplant, and involves waiting for several months with patients often deteriorating during the waiting period.

Myth: The donor will lose the donated part of the liver which will sooner or later result in unhealthy liver.

The recipient will need to stay in the area for 2-3 weeks following hospital discharge for weekly blood work and visits to the transplant clinic. The donor will need to stay in the area for approximately 1 week following discharge. How long are donors followed?

What are the risks to the donor from the surgery? With increasing expertise and in high volume centers, the risks of living donor surgery are minimal. Like in any surgery, they can rarely have wound infections, bleeding and the need for blood transfusions in the operating room, excessive abdominal drainage, bile leakage and minor chest infections. However, most donors are discharged within a week, and their postoperative problems are sorted out within 2-3 weeks after which they do not need any medical attention or medicines. We know that donors have lots of questions, so they are counseled by surgeons, hepatologists, psycholo-

Donor follow-ups are at 1, 3 and 12 months. Myths and Facts Myth: The donor is at risk of a lengthy and potentially dangerous surgical procedure without accruing any health benefit. Fact: The mortality risk for Living Donor Hepatectomy (liver segment removal) is extremely small in large volume transplant centers like the Medanta Liver Institute where more than 2000 such operations have been performed successfully by some of the most experienced experts in the field.

Fact: The transplanted liver in a recipient and the remnant liver in a donor, grow back to their normal size Liver regeneration is more than half completed by 2 weeks and takes 68 weeks to get back to the size of a normal healthy liver. Myth: It may not be safe to be a livingdonor for liver transplant. Fact: In any live donor liver transplant, ensuring safety of the donor is of paramount importance for the team. In fact, a detailed pre-operative evaluation is done for the donor for two reasons: first to assess if the operation will be safe for them, and second, to assess the suitability of their liver for the recipient. Hence donating just a part of the liver will make not make any difference to the donor’s health in the medium and long-term (after the initial 2-3 weeks), but would definitely give a new lease of life to the recipient.

MEDICA CONNECTED HEALTHCARE FORUM: NETWORKING IN THE FIELD OF HEALTH HAS MANY FACETS

N

etwork connection from hospitals to schools: The topics at the MEDICA CONNECTED HEALTHCARE FORUM go much further beyond what one spontaneously thinks of when the word network is mentioned. Running on a new set of days from Monday to Thursday, the forum, which is being held in trade hall 15 as part of the world’s largest medical trade fair, MEDICA 2015 (from 16 to 19 November) with around 4,800 exhibitors, covers a wide spectrum of subject-matter, ranging from networked healthcare systems to the Internet of Things, “Wearable Technologies”, all the way to “Medical Apps”. Right off the bat, two projects deal with young long-term patients, who do not want to, and should not, lose contact to school and their school friends. One of them is

entitled “Avatar Kids”. Within the scope of this Swiss project, the avatar robot “Nao” is the connecting element between school and home and the long-term patient staying in hospital. The connection is established over the Internet. The avatar robot represents the child at school and makes it possible for him or her to be “present” in realtime and participate “live” in school lessons. “Nao is very popular,” explained Jean Christophe Gotanian, CEO of Avatarion Technology. The school children perceive that the absent fellow classmate is present within the robot – and think this is cool most of the time. They can see their fellow classmate via a tablet PC (by Samsung) that has been attached to the head of the movable robot. Thereby, the Avatar robot is not a toy. He is given to the best school friend of the young patient, although it is the teacher that

controls the robot with his own tablet PC. Here, he also sees for example what the young patient is writing in hospital. Four university hospitals and twenty wards are already taking part in the project. According to Christophe Gostanian, contact has already been made with Italy, Belgium, Holland and France. In Germany, the project will be presented to the professional public for the first time at the MEDICA CONNECTED HEALTHCARE FORUM. Use of the Avatar robot “Nao” should not merely be limited to classrooms. On the one hand, for example, the robots should help to provide the children with information regarding pending interventions. However, using the device on the robot’s head, the young patient can be “beamed” to the schoolyard via

an Internet connection or be present during a class trip. Furthermore, by means of this, a connection between the family and the child in hospital can naturally become a reality. In particular, this can be important during the evening hours when young patients are alone. In the future, use at retirement homes is also planned – where, for example, “Nao” will ask questions, demonstrate yoga or movement exercises or answer questions about the weather, but would also be able to draw bingo numbers. In any case, the experience made with Avatar Kids seems to be encouraging. Patients between the ages of six and 17 are pleased about the new possibilities and the doctors decide on how much their patients can achieve. “Smart Cities” project in Hamburg “Some patients get a bad diagnosis

But it is even worse for them if they are totally ripped from their social environment.” This was also described by Professor Christian Gerlofff, deputy medical director and head of the department for Neurology at the HamburgEppendorf University Hospital, (UKE). In a project promoted by Cisco at the children’s hospital of the UKE, children who are ill over the long term are given the opportunity to continue participating in their school lessons using video technology. This project is taking place within the scope of “Smart Cities”, which Cisco and the City of Hamburg are working on in cooperation with each other. Under the heading “School and Health”, this specifically makes it possible for Thorben, who has been ill over the long term, to actively take part in school lessons along with his fellow classmates and teachers via audio and video transmission. Equipped with an iPad and the video soft client 'JAbber’ installed on it, Thorben can control the camera located on a trolley in his classroom. “We only have to roll the trolley into the classroom in the morning, plug in the power cable

and switch on the power strip. When Thorben turns on, we hear a quiet noise and know that he is here,” explained Christina Iserhot, Thorben’s school teacher. In fact, Thorben actively takes part in the lessons. “It was our goal to take children out of their isolation and make it possible for them to actively take part in school lessons. If someone whispers in the background or something falls down, Thorben becomes aware of it and is capable of directing the camera toward it,” explained Till Osswald, business development manager of the healthcare department for the EMEA region at Cisco. In the meantime, the pilot project was expanded to include another student, a 17-yearold grammar school pupil. “The experience shows that users accept and understand the technology very quickly and immediately feel included again on a social level.” The project has been developed and executed in cooperation with Avodaq, Cisco and the Hamburg-Eppendorf University Hospital. In the meanwhile, there have been fifty registrations on the waiting list from

Hamburg alone. Particular dynamics in the field of wearables The “Internet of Things” and “Wearables” also have the potential of becoming an essential element of networked health in the future. As part of the MEDICA CONNECTED HEALTHCARE FORUM and the “WT Wearable Technologies Show” (also in the MEDICA hall 15, stand A 23), around thirty companies from the entire supply chain will be showing where things will be heading toward in their opinion. Not only devices will be presented, but also the latest technologies that make these devices possible in the first place. Christian Stammel, founder and CEO of the WT Wearable Technologies Group, particularly emphasises the sophisticated options provided by data processing, which have made enormous advancements due to wearable technologies such as sensors. That which had originally been worn privately, ranging from quantified self-movement all the way to self-tracking, is going to also increasingly establish itself in the field of healthcare and replace older technologies. The spectrum of devices ranges from lifestyle

products such as the Apple Watch, to intelligent glasses and hearing devices, so-called “hearables”, all the way to the latest trend – intelligent patches that continually retrieve physical data, but that can also administer medications in a minimally evasive manner. Smart patches on the path to becoming multi-talents In comparison to T-shirts with integrated sensors, the risk of artefacts is, for example, considerably less in the case of smart patches and the products are almost invisible. These “smart patches” could mean a great alleviation in the future, particularly for diabetics. As an example, Stammel mentions the “Diabetes Care’s FreeStyle Libre”, which has been approved by the FDA. European approval is still pending, but: This is a patch for measuring blood sugar that can be worn while showering, swimming and when doing sport and only has to be changed every two weeks. By means of a painless scan, the patient obtains the current glucose value, the glucose data over the course of the last eight hours and trend

information concerning where the glucose value is heading. As another example for the usefulness of smart patches, Christian Stammel mentions “UpRight”, which monitors body posture while it is worn on the back. The product belongs to the generation of “trainables”; it triggers an alarm if body posture is wrong and helps to correct it. This indicates a new trend: The continuous correction of poor posture by means of the devices. At the MEDICA CONNECTED HEALTHCARE FORUM and at the Wearable Technologies joint stand, the company will be presenting products for measuring blood pressure and ECGs, among other things. The devices are supposed to be very user-friendly and, having an appealing design, be able to be perfectly integrated into everyday routine.

What should be done if patients collect data on their own?

Wearable technologies can also provide support in hospitals for optimising processes and organisation. For example, they can help in avoiding decubitus. Using sensors, it is detected if patients have been moved or not and can serve hospital personnel as a memory aid. Christian Stammel emphasises: “Today, many existing technologies already available can be used without taking any major measures with regard to infrastructure.”

Another tendency to be taken seriously is that physicians are increasingly confronted with data that originate from devices that have not been certified for the medical field. Christian Stammel is quite sure that these products provide reliable data. “Physicians should not simply disregard this data as irrelevant.” Data that is collected by patients and brought to the attention of their physicians can provide at least a

ardiovascular diseases such as heart attack, stroke, and coronary heart disease have been among the most common causes of death worldwide for many years now. Advances in medical imaging play a key role in the fight against these diseases by helping achieve sound diagnoses even at earlier stages and enabling efficient interventional therapy. At this year’s Congress of the

European Society of Cardiology (ESC) in London, United Kingdom, Siemens Healthcare is presenting innovative IT solutions and medical imaging systems in the areas of angiography, ultrasound, magnetic resonance tomography, and computed tomography, with the theme of “More cardiology, less heartache.” These are intended to help physicians diagnose cardiovascular diseases at an early stage, structure treatments to best suit the individual patient, and perform interventions safely. New cardiovascular imaging and information system drives care outcomes Growing cost pressure on hospitals increases the need for cardiologists to streamline their work processes. This is why Siemens has refined its tried and tested Syngo Dynamics cardiovascular information

system (CVIS) to not only help diagnose cardiovascular diseases but also to reduce the administrative load on medical staff and provide the best possible support in outcome-focused management decisions. The main focuses for improvement were on reading and reporting capabilities as well as interoperability and integration into other systems, such as the electronic health record (EHR) system. Data exchange between disparate systems makes improved efficiency and care outcomes possible. This interoperability gives various departments within a hospital – and multiple hospitals within an enterprise– a single point of access to relevant cardiovascular information. For example, specific echocardiography data such as ejection fraction heart failure measurements can now be

included more easily and at earlier stages for therapy or medication planning. While an examination is in progress or during interventions, multimodality clinical images and measurement data can be transferred directly into Syngo Dynamics, which reduces the risk of error compared with manual data input. Unusual results are now automatically highlighted, too, to direct attention to potential pathologies. When results or reports are being drawn up, the system also automatically reviews data plausibility1 as a means of drawing attention to any potential missing or erroneous entries. This speeds up the accurate documentation of patient flow within the hospital, which in turn saves time and reduces costs. Real-time 3D echocardiography


helps personalize treatments In order for physicians to optimally plan and perform surgical and minimally invasive cardiac procedures, the information they obtain about the patient’s heart must be as accurate as possible. With the Prime edition of its Acuson SC2000 premium cardiovascular ultrasound system, Siemens combines two state-of-the-art technologies: A transesophegeal echocardiography (TEE) probe, which is guided into the patient’s esophagus, provides detailed real-time 3D color Doppler images of the heart anatomy and blood flow and the eSie Valves analysis software, which automatically measures individual heart valves in just seconds – much faster than any software currently on the market. Imaging the heart in real-time makes it easier to examine and treat patients. Current imaging methods that use 3D TEE require stitching, which means combining multiple heartbeats on the computer and calculating cardiac function and blood flow from consecutive

cardiac cycles. This approach can lead to potentially misleading image artifacts, especially with patients with arrhythmia. The Siemens technology has been available on the European market since June and has proven successful in clinical practice. “It really makes a difference when you can see complete anatomy and blood flow at high volume rates with Siemens realtime 3D TEE. It enables you to perform valve procedures with more accuracy and more confidence, potentially improving patient safety and outcomes. It is truly a step forward in technology,” says cardiology Professor Stéphane Lafitte, Hôpital Cardiologique CHU in Bordeaux, France. Stabilizing heart movements allow stents to be perfectly positioned Many patients cannot undergo open heart surgery because of their age or health issues. That’s why minimally invasive therapies have developed into a valuable treatment alternative and have now become part of the

clinical routine. One example is the use of percutaneous coronary intervention (PCI) in significant coronary stenoses, which involves balloon angioplasty to restore normal coronary blood flow. To keep the revascularized segments open permanently, cardiologists must position coronary stents or scaffolds (bioresorbable coronary stents) during PCI with absolute accuracy despite the movement of the beating heart. The Clearstent Live application, which is now available for all new Siemens angiography systems, virtually reduces heart movements during interventions, stabilizing the area around the balloon catheter. This gives the cardiologist a clear view of the stent, enabling it to be perfectly positioned – even in particularly challenging locations such as bifurcations. To treat longer lesions, cardiologists sometimes have to place several stents in a row with accuracy down

to the last millimeter, because gaps could lead to restenosis or even heart attack. The use of thinner and thinner stents or x-ray transparent scaffolds makes visibility worse. While routine SPECT cardiac acquisitions typically take about 16 minutes and risk patient movement that may result in lower image quality, a field-upgradeable hardware and software combination of Symbia Evo and IQ-SPECT enables a reduction in imaging time to four minutes with a standard dose, or up to 75 per cent lower injected dose for imaging in standard time.

A cardiac CT in a quarter of a second Computed tomography of the heart has supported huge advances in the past decade and lends itself very well to the task of rapid diagnostic evaluation of the coronary arteries. The dual-source technology used in the current Somatom Force highend system is what makes the fastest imaging speed on the CT market possible: A cardiac dataset can be acquired in just a quarter of a second. This means that patients require beta blockers less often to slow their heart rate in order to avoid movement artifacts. It is also not necessary for them to hold their breath thanks to the fast acquisition speed of 737 millimeters per second and high temporal resolution of 66 milliseconds.

In addition, a significantly lower x-ray dose is needed compared with existing premium CT systems. CT angiography benefits in particular from the fact that, with the optimized Care kV technology of the Somatom Force, the tube voltage can be flexibly set between 70 and 150 kilovolts (kV) and more patients can now be scanned with lowvoltage values. When examining obese patients at 70 or 80 kV, this can result in a dose reduction of 68 percent over traditional 120-kV protocols. As a result, the system’s high scanning speed combined with a lower radiation dose opens up new possibilities: for example, extending the use of CT scanning to the early detection of coronary heart disease. In the process, the development competence of the suppliers, in part, is often the point of origin for ground-breaking innovations with regard to efficient and effective medical care. For example, this also applies to further increases in the level of miniaturisation. An especially extraordinary example, which currently brings science fiction to mind, entails nano-robots in the bloodstream that autonomously

carry out operations. Corresponding with these ideas, the Max-Planck Institute (MPI) for Intelligent Systems (Stuttgart) have developed two different micro-swimmers. Thereby, on the one hand, it has to do with a type of clam that moves forward by opening and closing as well as a screw that moves forward by means of rotation. Its diameter is only 100 nanometres; its length 400 nanometres. A rotating magnetic field that is applied externally sets the mini-screw into motion. The manufacturing process for the special swimmers is 3D printing, which is increasingly gaining in significance for a wide variety of applications at the COMPAMED. All materials used, such as polydimethylsiloxane, are biocompatible and body compatible. Researchers imagine that, one day, nano-robots will introduce tumour therapeutic agents directly into the tumour. “Theoretically, at the size of our construction, application within the cell would be conceivable,” explained Peer Fischer, head of the working group Micro-, Nano-

and Molecular systems at MPI for Intelligent Systems. In any case, the mini devices should contribute to making interventions minimally invasive, improve their effectiveness and shorten the times spans required for such interventions. However, a series of years may go by until this science fiction becomes a reality. SMALL BUT IMPRESSIVE AND WITH THE HIGHEST LEVEL OF PRECISION In the meantime, many “mini” solutions have now already become a reality since the demand for increasingly smaller systems remains constant in the field of medical technology. “The life-science industry is showing an increased demand for the miniaturisation, micro-structuring and an integration of optical and electrical functions in inexpensive components,” confirmed Peter Kirkegaard, CEO of IMT Masken und Teilungen AG from Switzerland. IMT addresses this need using manufacturing technologies deriving from the semiconduc-

tor industry. Based upon glass, the company manufactures microchannels, clearance holes, electrodes, optical and electrical coatings, waveguides and grating – the smallest structures have tiny dimensions down to only 150 nanometres. Their fields of application include lab-on-a-chip systems, among other things. Micreon GmbH also acts as a contract manufacturer – the company is among the worldrenowned specialists for micro processing using ultrashort pulse lasers within the pico and femto range. The laser is playing an increasingly important role in manufacturing medical implants, instruments or measurement devices in the field of medical technology. Since the highest level of precision and quality is required in the case of medical products, especially for the ultrashort pulse laser technique, an increasing number of application possibilities are arising. An example includes vessel wall supports (stents) made of organic materials. Since the bio-resorbable polymers

are very sensitive to temperature, the femtosecond laser is the only tool used for manufacturing refined and structured components without any damage. RECORD PARTICIPATION AT THE IVAM JOINT STAND IMT und Micreon are being represented along with around another 50 exhibitors at the joint stand of the IVAM professional association for micro-technology, which will once again be forming a focus for microsystem technology, nanotechnologies, production technology and process control in hall 8a. “This is a new record; our floorspace comprises almost 700 square metres,” explained Mona Okroy-Hellweg, speaker for the IVAM. The COMPAMED SUPPLIERS FORUM is taking place in parallel again in hall 8b, which is traditionally being organised by the trade magazine DeviceMed. The focus of numerous presentations by specialist from internationally leading companies entails current development along the entire process chain. “On all four days of the trade fair, exhibitors will be providing information on technical innovations and further topics within the scope of the interplay between manufacturers, suppliers and physicians or users. When miniaturising the design, increasing the precision of the OCT method can only be achieved at the same time by implementing integrated piezoelectric sensors and an application specific integrated regulation circuit. By means of this, it is possible to integrate highprecision coherence tomographic images into an endoscope and obtain non-invasive three-dimensional images of tissues structures. OCT is used in a variety of medical fields, such as ophthalmology for example.

A “never-ending hot topic” at the COMPAMED entails coatings, especially those with antimicrobial action. Biofilms on catheters can lead to infection in patients. Therefore, in the USA, already two thirds of all catheters on the market have antimicrobial coatings and/or antithrombogenic coatings. Even if different legislation prevails in Europe, in the meanwhile, such catheters are also used here. Using the so-called “non-leaching method” the supplier, Cikautxo, located in northern Spain, has developed catheters with a surface treated with a substance that kills off bacteria at the moment the bacteria come into proximity of it. Once again, the upcoming COMPAMED will give an overview of the entire range of medical technology suppliers. The range of offers for visitors spans from tiny sensors all the way to packaging machines that fill entire rooms, from innovative materials to refined microsystems, from mobile diagnostic devices all the way to electronic manufacturing services (EMS). In the future, 3D printing should also become a focus at the COMPAMED. According to a survey carried out by DeviceMed, 31 percent of the companies questioned already rely on the innovative method; 35 percent are planning to use it in the foreseeable future. Only one third of the approximately 80 companies recorded up until this point do not currently see any possibilities for its application. Also from this point of view, a visit to halls 8a and 8b are certainly worthwhile this year, being the first time for exciting discussions and business to take place for a day longer. Today, imports of such products are negligible in finished form.” He further added that, “Today, the diagnostic business is mainly based on technology. The drive or thrust for smaller, faster and easily accessible devices is increasing. Emerging markets have different needs with respect to test menus, technologies and operating procedures. Thus customized solutions need to be developed depending on the need. Indian

companies with their R&D facilities have developed a range of good quality products for the local market amidst fierce competition.” On speaking of steps to be taken to further boost innovation, he said “Though the Government has introduced a lot of policies for inhouse production, there is still scope for more. The government and private players need to join hands to promote innovation at reasonable prices. The Government needs to initiate policies to encourage indigenous manufacturing. This can be done through availability of funds, land demarked for setting up manufacturing units, tax relaxation, encouraging research & development, etc. Encouraging foreign players to set up manufacturing units in India can help further.” Mr. Vazirani alongwith the other honchos of the industry offered useful insights to place India as a world leader on the global healthcare map. Mr. Sudhanshu Pandey, Joint Secretary, Ministry of Commerce, Government of India, graced the occasion and underscored the

need for enhancing the medical device industry. He referred to the new legislation that is being brought about to regulate and harmonize the segment, which is presently covered by the Drugs and Cosmetics Act. Mr. Pandey said, “Since the legislation might take some time, certain short term measures also can be bought to address the immediate problems of the industry such as labeling requirements, etc.” He called the transfer of technology to give a leg up to the medical device industry by taking up contract designing, research leading to manufacturing. He also referred to better brand building of Indian medical device products to command higher prices in the international market. Products sourced from India at cheaper prices are sold at higher prices abroad, denying the Indian industry the benefits. On a closing note, Mr. Pandey exhorted industry to evolve a pragmatic pricing policy for equipment. Product pricing should be transparent, predictable and economical to make the medical treatment affordable to the common man.
