TABLE CLINICS
Thursday, May 28 | 11:30 AM - 1:00 PM | Convention Center
JBLM TABLE CLINICS Post-graduate residents in the Joint Base Lewis-McChord (JBLM) Advanced Education in General Dentistry program will present table clinics on Thursday, May 28 from 11:30 AM – 1:00 PM at the Convention Center One CE credit available.
Dr. Jesse Adamson Minimally Invasive Endodontics — Teeth Treated to Last Endodontically treated teeth lose their structural integrity during the process of caries removal, access preparation, and shaping. But what if we could limit the extent of tooth structure removed during endodontic treatment? After all, the more tooth structure remaining, the better chance of survival of the tooth. The goal of minimally invasive endodontics is to create smaller access preparations, smaller apical preparations, and as a result, conserve as much tooth structure as possible. Minimally invasive principles in endodontics may increase the longevity of teeth, and may change the way we evaluate and restore endodontically treated teeth as practitioners.
Dr. Ryan Beetham Sticky Bone: A Guided Bone Regeneration Technique to Combat Alveolar Ridge Deficiencies Alveolar ridge deficiencies compromise a dentist’s ability to place dental implants surrounded by an adequate volume of bone. Traditional treatment modalities to increase alveolar ridge defects include block grafts and ridge splits. Sticky bone is a graft material that can lead to a similar amount of bone generation compared to the aforementioned treatments, but is less invasive and offers accelerated tissue healing. Sticky bone utilizes growth factors in platelet rich fibrin (PRF) to do all this and more. Indications for use, the preparation technique, and case studies will all be discussed at this table clinic.
Dr. Allison Bertoni Bioclear Method — A New Technique for Restorative Dentistry Exploring the Bioclear technique and its differences to traditional composite resin bonding. What are its indications for use and its advantages and disadvantages over the traditional restorative method? Dr. Alan George Investigating the Effects of “Vaping” and Oral Health Vaping has become a popular alternative to smoking and is very common among adolescents. It has become a public health concern and even caught the attention of congress. There have been hospitalizations connected to vaping in 46 states, in fact, 10 deaths have been linked to E- Cigarettes via autopsy. There is a trend of states banning these products for good reason. What are the short and long term effects of vaping on the oral tissues? How do these effects compare to the mountains of evidence stacked against smoking as a risk factor for periodontitis. How should we be educating our patients? Ultimately, what factors should we consider to diagnose disease processes and how does this new habit affect case selection.
Dr. Peter Hwang Successful Restoration After Selective Removal of Carious Lesions The traditional method of successful restoration suggests complete removal of cariogenic biomass. However, some studies indicate that even with a remaining infected tissue, cariogenic process can be arrested if restoration is done correctly under the right condition. This conservative approach, depending on cases, will offer patients more favorable treatment plan as to endodontic treatment and fixed prosthesis restoration, which are very likely to occur when complete caries removal leads to pulpal exposure. This table clinic will discuss the surprising favorable prognosis of restoration after selective removal of carious lesions. It will also explore the most effective method for tooth preparation, liner application, and direct restoration that will arrest remaining caries, therefore avoiding the potential involvement for endodontic and prosthodontic treatments.
Dr. Samuel Lo Orthodontic Extrusion: Methods and Case Selection For Immediate Dental Implant Placement In The Esthetic Zone Immediate dental implant placement can be beneficial for both clinicians and patients by shortening treatment duration. However, it is only indicated in cases where soft and hard tissue conditions are considered ideal at the time of implant placement. The lack of these tissue conditions can be due to factors such as natural anatomical limitations — one
limiting factor being thin buccal plate for anterior cases. Therefore, immediate dental implant placement in the esthetic zone may prove to be challenging and require pre- or post-implant site enhancement procedures. An alternative treatment method to the traditional extraction and bone graft procedure is orthodontic extrusion to improve soft and hard tissue conditions prior to implant placement. This table clinic will review recommended orthodontic methods and clinical guidelines for case selection for orthodontic extrusion for pre-implant site development.
Dr. Schuler Luce Computer Guided Treatment Applications in Endodontics Endodontics can be complicated — from diagnosis to the completion of the treatment itself. There are many variables which can cause potential complications such as: dense buccal bone in the mandible, unknown exact number and location of root canals and apical foramina, pulp chamber and canal calcification, unknown exact apex location for surgical treatment of a non- healing apical pathology — just to name a few. The use of computer-guided endodontic surgical templates is a novel and viable technique to make potentially complicated endodontic procedures more predictable for the Endodontist and General Practitioner alike. This table clinic will review the different applications for computer-guided endodontics. It will review the benefits and drawbacks associated with the use of computer guided techniques in endodontic treatment. This table clinic will also review the methods for the fabrication of a surgical guide to be used in endodontic surgery.
Dr. Zachary White Alternative Anxioslysis — Revisiting Hypnosis in the Dental Office Clinical hypnosis has long been employed in patient care, but seems to be a dying art. Are there evidence-based indications for hypnosis in the dental office, or is it just subjective pseudoscience? Far from the showy Las Vegas stage hypnotists, dentists can employ hypnosis techniques to aid in patient management and facilitate treatment. Although difficult to master, clinical hypnosis can be a valuable tool in the “dentist’s toolbelt.”

Diagnosis & Management of Oropharyngeal Cancer Theresa Gonzales, DMD, MS, MSS Oral Pathology Primary Audience(s): n
n Additional Audience(s): n n Thursday, May 28 | 8:00 AM - 11:00 AM CE Credits: 3 While some think oral cancer is rare, this cancer will be newly diagnosed in about 132 new individuals each day in the U.S. alone, and a person dies from oral cancer every hour of every day. The oropharynx is one of the most common sites of head and neck cancer in the country. It is estimated that 11,000-13,000 new cases of oropharyngeal cancer are diagnosed in the U.S. each year. The vast majority of these tumors are squamous cell carcinoma, a cancer which arises from squamous epithelial cells which line the upper aerodigestive tract. Males are more than four times as likely as females to develop oropharyngeal squamous cell carcinoma (OPSCC), with an overall annual risk of 6.2 cases per 100,000 men compared to 1.4 cases per 100,000 women. Incidence in the U.S. is highest among white and black men, and lower among Hispanics, Native Americans and Asians/Pacific Islanders. Despite decreasing rates of tobacco and alcohol use, the rates of oropharyngeal cancer have trended steadily upward for the last decade. This is primarily due to the increase in cancers related to infection with the human papilloma virus (HPV). The time lag between an oral HPV infection and the development of HPV-related oropharyngeal cancer is estimated to be between 15 and 30 years. Unfortunately, at this time, the majority are found as late-stage cancers, and this accounts for the very high death rate of about 43% at five years from diagnosis (for all stages and etiologies combined at time 16

Thursday, May 28 | 10:00 AM - 10:50 AM CE Credits: 1 When presented with a tooth with a poor or hopeless prognosis, what is the best course of action to take? If the patient is interested in pursuing replacement options, should an immediate implant be considered? Or should the tooth be extracted and ridge preservation be the treatment of choice? An immediate implant placement is a viable and successful strategy when implemented in the right cases. It can reduce treatment time, number of surgeries, cost, and provide patients with an efficient and comfortable treatment compared to conventional modalities. In some cases such as the anterior, it can result in superior esthetics by preventing excess tissue remodeling. However, there are other situations in which it might not be considered, such as maligned teeth, large infections, maxillary molars, and others. 18

Silver Diamine Fluoride + Glass Hybrid = SMART Caries Treatment and Prevention Jeremy Horst, DDS, PhD Dental Materials Primary Audience(s): n
n Additional Audience(s): n n Thursday, May 28 | 2:00 PM - 5:00 PM CE Credits: 3 The ADA just announced a new paradigm of dentistry: NonRestorative Caries Treatment. Enabled by Silver Diamine Fluoride (SDF) and bolstered by discing, ART/ITR, SMART, and Hall crowns, “leaving caries in the tooth” with or without a restoration is now standard of care. These changes have brought about new opportunities — and new challenges! While SDF and complementary techniques are faster and less invasive, defining a billing philosophy is tricky and monitoring outcomes is fundamentally different than for traditional restorations. Nevertheless, they can be combined: attendees will learn why SDF is Dr. Jeremy’s gateway drug to glass hybrid cements (e.g. HV-GIC). There is also an overwhelming load of marketing hype about preventives, which has caused many to miss the breakthroughs in materials that actually work, such as Xylitol for caregivers and topical Iodine. Most dentists start using these techniques with patients for whom they have no better options; but with experience we all eventually ask ourselves: “Doesn’t every patient deserve modern caries management?” Learning Objectives: • Design billing strategies for minimally invasive techniques that are fair to patients and the practice. • Learn how to monitor and properly respond to outcomes of non-restorative caries treatments. • Learn how to properly place glass ionomer as a sealant, temporary, or definitive restoration. Speaker Bio Jeremy Horst, DDS, PhD is a practicing pediatric dentist and biochemist known for investigating the strengths and limitations of Silver Fluoride therapy and developing SMART fillings. His mission is to reduce suffering from tooth decay by driving the development of better treatments and preventives. In his teaching and practice he focuses on maximally effective, minimally invasive techniques to stop dental caries and creating an easy relationship with dentistry for children and people with special healthcare needs or uncontrolled caries. Thursday, May 28 | 4:00 PM - 4:50 PM CE Credits: 1 Over the past two decades, dentistry has made important progress thanks to advancements in material science, technology and clinical techniques. Dental bonding revolutionized the shape and content of clinical dentistry, presenting a strong and minimally invasive alternative to the traditional materials. Early research in adhesive dentistry was focused on the bond strength of composite resin to tooth, despite the fact that “sealing” is considered more of a critical issue than “retention” of composite restorations. Polymerization shrinkage of contemporary dental composites still threatens the success of all kinds of adhesive restorations, and is perhaps underestimated from simplified bonding tests. We present a systematic approach for simulation of clinical situation in laboratory research, where the bonded restoration is Audience Key:
subject to challenges such as shrinkage, stress, and fatigue loading. In addition, by real-time imaging using Sadr’s OCT imaging and analytical methodologies, we demonstrate that the integrity of a composite restoration can be visually evaluated and monitored. The visual observations lead to recommendations on clinical procedures for bonding and material selection, leading to a key conclusion: the success of restoring teeth with adhesive technology requires not only an optimal bonding approach, but also a strategy to mitigate the stresses and biomechanically protect the bonded interface and the dental tissue. Learning Objectives: • Understand the actual implications of polymerization shrinkage of dental composites. • Understand how to have a better sealed composite through managing shrinkage stresses. • Understand how fiber reinforced composite can help to mitigate the stresses and biomechanically protect the bonded interface and the dental tissue. Speaker Bio Dr. Alireza Sadr is a Clinical Associate Professor at Department of Restorative Dentistry, Director of Operative Dentistry Pre-clinical Training and Associate Director of Biomimetics Biomaterials Biophotonics & Technology (B4T) laboratory at University of Washington School of Dentistry. FRIDAY MORNING LECTURES Eating Disorders: Medical, Dental & Oral Health Considerations Barbara Steinberg, DDS Oral Medicine Primary Audience(s): n
Friday, May 29 | 7:30 AM - 9:00 AM CE Credits: 1.5 Eating disorders represent a complex spectrum of chronic illnesses, with medical and psychological complications that lead to the second highest mortality of all psychiatric disorders. It is a problem that is epidemic on our college campuses. Members of the dental team play an integral role in helping to identify those patients with undiagnosed eating disorders. In fact, because of oro-facial manifestations, they may be the first health care providers to encounter such patients and thus are instrumental in making appropriate referrals for further diagnostic work-up and treatment. This lecture will address the psychological, medical, dental and oral health issues associated with eating disorders as well as dental management considerations. Learning Objectives: • Gain an understanding of the psychological and medical issues associated with eating disorders. • Identify oro-facial manifestations associated with eating disorders. Friday, May 29 | 7:30 AM - 10:30 AM CE Credits: 3 Are you recording medication histories that include resveratrol, curcumin, probiotics, gingko biloba, fish oil, elderberry, or milk thistle? Do your patients ask you to recommend only “natural” oral health products? The resurgence of herbal medicine has produced confusion and conflict among health care providers and patients alike. Patients mistakenly equate the terms “natural” and “standardized” with safe and effective. Many patients have embraced nontraditional medicine as a way to become “co-therapists” in their disease state management. Dental professionals must avoid making negative generalizations about alternative therapies in order to stay involved in the therapeutic decision-making process. During this program, we will discuss the herbal landscape, which includes laws and regulations as well as the qualifications of naturopaths and herbalists. A list of reliable science-based references will be included to provide ongoing clinical support for rational decisions related to herbal products. The twenty most popular systemic herbs will be evaluated for dental impact including effect on bleeding, wound healing, intraoral effects, and interactions with dental

drugs. We will also take a detailed look at herbal products promoted for treatment of oral diseases including gingivitis, periodontitis, aphthous stomatitis, and herpes simplex gingivostomatitis. Agents such as Coenzyme Q-10, aloe vera, green tea extract, natural essential oils, melaleuca, and goldenseal will be critiqued for safety and effectiveness in the treatment of oral disease. Product-specific information including ingredients, manufacturer claims, and fluoride content will be provided along with product recommendations based on patient characteristics. We will conclude by discussing step-wise strategies for evaluating and managing patients taking herbal medications. A practical and detailed handout will be provided to allow participants to apply this information to their practice. Learning Objectives: • Recognize the dental impact of common systemic herbal medications and dietary supplements. • Select specific herbal oral health products with reasonable claims and safe ingredients. • Discuss the effectiveness of nutraceuticals on caries, gingivitis and periodontitis. Speaker Bio Professor Karen Baker has been on the dental faculty at the University of Iowa for 39 years and occupies a unique role in dental practice and education. Friday, May 29 | 1:00 PM - 4:00 PM CE Credits: 3 The ADA just announced a new paradigm of dentistry: NonRestorative Caries Treatment. Enabled by Silver Diamine Fluoride (SDF) and bolstered by discing, ART/ITR, SMART, and Hall crowns, “leaving caries in the tooth” with or without a restoration is now standard of care. These changes have brought about new opportunities—and new challenges! Most dentists start using these techniques with patients for whom they have no better options; but with 36
experience we all eventually ask ourselves: “Doesn’t every patient deserve modern caries management?”

Friday, May 29 | 1:00 PM - 4:00 PM CE Credits: 3 Suicide is among the most pressing public health issues of our time. Scientific American recently called the U.S. suicide rate “nothing short of an epidemic,” with one life lost every 11.7 minutes. Washington’s suicide rate is 11 percent higher than the national average, with almost 1,300 suicide deaths in 2017, nearly twice the number of lives lost to opioid overdose and more than we lose to traffic crashes. Dental professionals are at higher risk for suicide than other professions and often see patients who may be at risk. This three-hour training has been prepared with the dental professional in mind to help increase awareness of and confidence to help prevent suicide. This lecture has been approved by the Washington State Department of Health and meets the state’s requirement for all health care professionals, including dentists and hygienists, to take a three-hour suicide awareness course. New Worldwide Reclassification of Periodontitis: Practical Application Kenneth Kornman, DDS, PhD Periodontics Primary Audience(s): n
n
Saturday, May 30 | 8:30 AM - 11:30 AM CE Credits: 3 In the two decades since the last classification of periodontitis, substantial new evidence and new understandings of these diseases have emerged which indicate that some of the population develops more severe clinical expression of periodontitis. The future of periodontics is primarily about managing clinically and biologically complex patients, which constitute at least 20 to 25% of our adult population. This session will provide a simple process for applying the 2018 classification of periodontitis in your practice to provide the best prevention and treatment for your patients.

Saturday, May 30 | 8:30 AM - 11:30 AM CE Credits: 3 Let’s face it, sitting through a lecture about dental caries or caries risk assessment isn’t appealing (in fact, it sounds downright boring). However, the science of clinical cariology is beginning to have an impact on restorative techniques and technology. Streptococcus mutans has more to fear than a rheostat, now that we’re armed with salivary diagnostics and bioactive materials. Learning Objectives: • Understand how to intervene in the caries process. • Learn how to change caries risk factors with minimal resistance. • Learn how to improve the prognosis of restorative treatment. Saturday, May 30 | 8:30 AM - 11:30 AM CE Credits: 3 The restoration of implants in the aesthetic zone varies significantly from implant treatment in other areas. Successful outcomes require not just osseointegration of the implant fixture itself, but also creation of appropriate gingival architecture, selection and fabrication of an abutment/ crown system that balances aesthetic and functional demands. Special attention will be given to provisional techniques, abutment/ crown selection, impression techniques and gingival shaping. The techniques and materials demonstrated will balance efficient and practical techniques with clinical results and scientific evidence. Learning Objectives: • Make provisional restorations that enhance aesthetic outcome and maximize aesthetic predictability. • Predictably manage and create soft tissue aesthetics around anterior implants. • Select the appropriate abutment/crown in the aesthetic zone to maximize longterm aesthetics and durability. Speaker Bio Todd Schoenbaum is a full-time Associate Clinical Professor at UCLA, Director of UCLA Continuing Dental Education, and instructs residents and students at UCLA for their implant restorations. He has published over 40 papers, 8 chapters, and one textbook. He is the recipient of the scientific writing award from the JPD. n n
Additional Audience(s): n
Saturday, May 30 | 8:30 - 11:30 AM CE Credits: 3 Suicide is among the most pressing public health issues of our time. Learning Objectives: • Understand suicide’s impact in the dental profession and community. • Explore the suicidal mindset and the role of dental professional teams in preventing suicide. • Learn suicide prevention skills for use in your professional and personal life. • Integrate prevention approaches into staff and patient care. Speaker Bio Dr. Stuber was trained as a public health epidemiologist at Yale University and came to the UW to become a professor in the School of Social Work. As an assistant professor, she learned about the impact of suicide personally. With her background in public health and her platform as a professor at the leading school of social work in the nation, she has dedicated her professional life to the cause of suicide prevention. She co-founded Forefront Suicide Prevention in 2013. Forefront is an innovator in state policy and systems-focused efforts to increase attention and concrete action in the prevention of suicide. Forefront helps empower individuals and communities to take sustainable action in suicide prevention. It restores hope for those deeply concerned about the trends we are seeing in mental health and suicide by building a grass-roots effort focused on giving these issues the attention they deserve. Eat Candy & Avoid Caries: Exploring Caries Management by Risk Assessment (CAMBRA)
The Future of Periodontics: Be Prepared
Continuing Care
Kenneth Kornman, DDS, PhD Periodontics Primary Audience(s): n
n
Saturday, May 30 | 1:00 PM - 4:00 PM CE Credits: 3 Dental practices have changed dramatically because our patients and their oral diseases have changed. The majority of patients who seek regular dental exams and preventive care have minimal treatment needs, and 18 to 29% of individuals with good dental insurance do not see the dentist except for emergency needs. From 1965 to 1985, new information allowed periodontists, dentists and hygienists to predictably control periodontitis in the majority of patients. Today, approximately 20% of patients who see the dentist regularly have complex cases with multiple medical considerations and often have the need for interdisciplinary care. These individuals have the greatest need for periodontal and restorative care, yet also may be the most challenging cases. This lecture will discuss primary changes occurring in dentistry and how we may start preparing. Learning Objectives: • Describe two ways dental practices have changed in recent years that have/will change your practice. • Describe how to identify periodontitis patients that are likely to respond predictably to standard principles for treating periodontitis. • Describe three ways that medical advances are and will be influencing dentistry. Speaker Bio Dr. Kornman is a research professor in the Department of Periodontics at the University of Michigan, editor of the Journal of Periodontology, and co-founder of Sitokine Ltd, a genetics company focused on cardiovascular disease. If you’re still telling patients to “brush, floss, and use fluoride toothpaste,” you’re going to be surprised by the myriad of treatment options available today. Streptococcus mutans has more to fear than stronger fluoride and more of it, as dentistry embraces oral healthcare. Learning Objectives: • Implement disease management in practice. • Improve the prognosis of restorative treatment. • Minimize the dangers of consuming sugar-sweetened foods and beverages.
