MESSAGE FROM THE CHAIR OF THE BOARD OF TRUSTEES

On behalf of the Nantucket Cottage Hospital (NCH) Board of Trustees I would like to express our sincere gratitude to our 2022 donors for your continuing generosity, commitment and support of the mission of NCH in preserving and enhancing the wellness of the Nantucket Community. Because of your generosity, NCH is able to provide world-class care to the entire community.

In addition, we would like to express our appreciation and gratitude to all of the NCH employees, volunteers and community partners that provide the high-quality and safe patient care and range of services to our entire community throughout the year for their dedication and tireless service.

The NCH Board of Trustees is a diverse group of yearround and seasonal residents of Nantucket with a wide range of experience and expertise in the community and the healthcare industry. The role of the Board is to serve as the governing body for NCH with the fiduciary responsibility to ensure that the hospital has a vision and a mission that is right for Nantucket and that it executes on that vision and mission both today and sustainably for the future.

As the primary provider of healthcare in a geographically isolated community, NCH is faced with a wide range of unique opportunities and challenges both today and tomorrow. We all know that the quality of life we enjoy on Nantucket is dependent on having accessible, resilient and timely high-quality healthcare available, close to home, for our community throughout the year.

To deliver this care, our greatest strength is our talented team of dedicated healthcare professionals, who when needed, can leverage the talent, technology and extraordinary capabilities of Mass General Brigham through our over 16-year affiliation with MGH and now the MGB Community Division.

As the past three years have shown, the island community depends on NCH to have the resilience

and resources to serve a wide range of patient needs. As we emerge from the crisis of the COVID pandemic, we find the healthcare industry is now experiencing unprecedented staffing, operational and financial challenges throughout the state of Massachusetts and the entire country. NCH is no exception.

For NCH to meet these challenges today and for tomorrow, we must address the dramatic increases in wage costs, inflationary pressures, and the growth and seasonality of our patient volume throughout the hospital.

Now, more than ever, philanthropy is a critical source of funding that enables Nantucket Cottage Hospital to meet our challenges and the needs of the island. The recent construction of the new hospital, funded 100% through philanthropy, has provided a state-of the art platform to deliver a modern standard of highquality care. Additional monies are required for housing, talent, resources and new technologies to support our most formidable challenge of attracting, developing and retaining talent.

Many of you supported the Annual Fund and the relaunch of the annual Boston Pops Concert as well as our annual fundraising events which enables the continuation of the fundamental healthcare services and funds for new offerings that the Community needs to address for its diverse and growing population.

As the NCH Board of Trustees, we are very grateful for your continued support, stewardship and continued financial support of our mission. Introducing Amy E. Lee, President of Nantucket Cottage Hospital

Amy E. Lee, MBA, MBHA, MJ, FACMPE, a seasoned hospital executive who has worked in health care for more than 25 years, was named president of Nantucket Cottage Hospital effective October 17, 2022. An accomplished leader, Lee most recently served as chief operating officer and vice president at Central Maine Medical Group/Central Maine Healthcare, where she is known for her strategic vision and her role as a trusted mentor to colleagues.

Prior to joining Central Maine, Lee served as vice president for Primary Care, Medical Specialties, Physician Residency Programs and Patient Contact Center at Carle Foundation Hospital, a 433-bed hospital in Urbana, Illinois.

Lee holds three master’s degrees: a Master of Jurisprudence in Health Law from Loyola University in Chicago; and a Master of Healthcare Administration and a Master of Business Administration from St. Ambrose University in Davenport, Iowa.

MESSAGE FROM THE PRESIDENT

When I became the President of Nantucket Cottage Hospital, I was very aware that philanthropy plays an important role in supporting the hospital’s mission. You just need to walk through this beautiful new facility to see how much the community values and supports quality healthcare for all who live on and visit the island. What I was not prepared for is the depth and breadth of the philanthropic support and how much of an impact it makes on the hospital’s work.

In the pages that follow, you will see a long list of individuals who support the hospital’s current operations through the annual fund and the Boston Pops on Nantucket fundraiser. These gifts are critical in making year-round, around-the-clock care possible in our isolated, high-cost community. You will also read how an extraordinary contribution from the Hospital Thrift Shop, made possible by a cadre of dedicated volunteers, has funded a critical new MRI scanner at NCH and how the volunteers, swimmers, and donors of Swim Across America on Nantucket continue to make world-class cancer care possible right here on the island.

The tangible impact of philanthropy is apparent across all hospital’s operations. Last summer, NCH launched Urgent Access. Not only did Urgent Access provide convenient and affordable access to acute care for visitors and residents alike, but, as you will read, it contributed to shorter Emergency Department wait times as well. The Emergency Department patient volumes continue to rise, and we are no longer experiencing the seasonal lulls of the past. Urgent Access allows the Emergency Department team to focus on our patients who are truly in need. We are also very

proud of the number of patients we appropriately keep on-island rather than transferring off-island. In 2023, thanks to strong philanthropic support, NCH will be able to increase the hours and number of appointments in our Urgent Access clinic.

The power of philanthropy on the health of the island extends beyond the walls of the hospital and into the community. NCH is a leader in the Nantucket Behavioral Health Initiative (NBHI), a community-wide effort to redesign and transform mental healthcare on the island. Much of the initial funding for the initiative came from designated community impact funds through the hospital’s capital campaign. NBHI began to make a meaningful difference this year with the roll-out of a completely redesigned and integrated mental health crisis response system.

Finally, no conversation of hospital finances would be complete without mentioning employee housing. Thanks to funds raised for that purpose in the capital campaign, NCH added 22 bedrooms across 7 brand new housing units to its inventory this year. Additional housing projects are in various stages of planning that will use campaign funds as well as additional major new donations to address the housing crisis on this island. The construction of employee housing will continue to be the hospital’s greatest capital need for years to come.

Pops is Back with a Bang

The 25th Anniversary Boston Pops on Nantucket, presented by Jill and Stephen Karp and White Elephant Resorts, delivered on its promise as Nantucket’s biggest and most beloved event and the most important annual fundraiser for Nantucket Cottage Hospital. The concert was back at Jetties Beach for the first time since 2019 due to the pandemic.

Event co-chairs, Bruce and Elisabeth Percelay, joined on stage by their sons, William and Charlie, pulled out all the stops to make sure that the 25th Boston Pops on Nantucket was, as described by many, “the best Pops on Nantucket ever.” Pops host and broadcasting legend, Natalie Jacobson, welcomed the crowd on a beautiful Nantucket August evening. Recording artist and actress, Skyler Wright, sang the National Anthem, which concluded with a fly-over of vintage warplanes. Nantucket Cottage Hospital interim President, Jeanette Ives Erickson, was on hand to thank the crowd for their generous support of the hospital.

The special guest for the 25th Pops was Marc Martel and his band who had the whole beach dancing to the hits of Queen. At one point, Martel asked the crowd, “have the Boston Pops ever rocked this hard?” The highlight of the evening for many was a surprise high-tech drone lightshow during the 1812 Overture. The drone show, which was the first of its kind on Nantucket, featured 150 illuminated, synchronized, and choreographed drones that arranged themselves into stunning aerial formations above Jetties Beach.

Most importantly, the 2022 Boston Pops on Nantucket raised a record net $2.5 million for the operations of Nantucket Cottage Hospital. These funds go directly into delivering patient care on Nantucket.

Growing Primary Care at NCH

When you live on an island, you look out for each other. No one feels the weight of this responsibility more than Nantucket Cottage Hospital’s primary care providers. They are the orchestra leaders of your health and well-being. They’ll take care of all your routine needs such as annual physicals, vaccines and the care of common or chronic health issues. They will also guide you to a specialist if needed, on- or off-island.

Nantucket Community Behavioral Health Initiative

In late 2020, the leaders of NCH and Fairwinds, Nantucket’s counseling center, began conversations to address the under-resourced and fragmented system of mental healthcare on Nantucket. The two were soon joined by the Community Foundation for Nantucket, which provided a grant for initial funding, and NAMI of Cape Cod and the Islands. This group formed the nucleus of what has come to be known as the Nantucket Behavioral Health Initiative (NBHI).

NBHI’s first action was to conduct a comprehensive assessment of the island’s mental healthcare delivery system. A steering committee was formed that added leaders from across the island community, including town government, the schools, police, EMS, health and human services organizations, the spiritual community, independent clinicians, and community members, to the NBHI effort.

Government Performance Systems, a Colorado-based consulting firm that participated in a successful similar initiative in Vail, was engaged to conduct the assessment. The resulting report, which can be found at nantuckethospital.org/communityoutreach, confirmed that Nantucket was one of the most under-resourced communities in the Commonwealth for mental health services. The report goes on to identify 16 specific gaps in the continuum of mental health care on the island.

With the assessment in hand, NBHI moved into an operational phase. The leadership group expanded to

include the town, the schools, and others. The hospital provided further funding with a major grant from its Community Health Initiative grant program. Joan Quinlan and Nancy Lane, two of the state’s leading experts on community and mental health, were brought in to coordinate the effort.

Collectively, the NBHI leadership group identified addressing the fragmented system of mental health crisis response on the island to be the highest priority. With the support of NCH and the NBHI, Fairwinds applied for and was chosen to be the state designated Community Behavioral Health Center for Nantucket. NCH and Fairwinds then created a partnership that has resulted in a seamless system of crisis response for the island, whether the patient is in the community or at the hospital and regardless of their insurance status. As a result, many patients are seen without needing to go to the Emergency Department while those that do are receiving better care and quicker placement in off-island facilities, if necessary.

In late 2022, the NBHI leadership elected to form a stand-alone 501(c)3 to further its mission to “collaborate to deliver an inclusive and interconnected system of prevention, intervention, treatment and recovery that helps our community get well and stay well.” This will allow for a full-time dedicated director to continue this important work. NCH will be represented on the board and continue to lead and support this important initiative.

LIFE CHANGING CARE FOR A YOUNG FAMILY

In July 2022, Eliza Miller took to Instagram with a post that brought huge relief to family and friends. After four terrifying days, her four-yearold daughter, Emme, was home from Nantucket Cottage Hospital.

“Emme was having a hard time breathing suddenly, She had gone limp in Pat’s arms. We rushed her to the Nantucket Cottage Hospital ER and they immediately took action to get her oxygen, tests galore, hydration, and so much more. Over the course of three days, Emme improved, thanks to the team of amazing doctors, nurses, aids and more at @ackhospital. I’ve never been so impressed.

It was a lot of emotion and stress, but the hospital staff was SO kind, so patient, so reassuring. Nantucket Cottage Hospital really saved Emme’s life with compassion and love and kindness. I will forever hold NCH in my heart. Emme’s too!”

BEINECKE SCHOLARSHIP PROGRAM

NCH awarded 22 Beinecke Scholarships totaling $37,600 to island students and residents pursuing education and training in medical and healthcare fields in 2022. Open to graduates of Nantucket High School, residents of the island, and employees of the hospital, this scholarship fund was created by the late Nantucket businessman and civic leader Walter Beinecke, Jr.

Beinecke, the man who founded Sherburne Associates and is credited with transforming Nantucket into a modern destination resort town, was a life member of Nantucket Cottage Hospital’s Board of Trustees. The Beinecke Scholarship Fund’s origins date back to the early 1950s when his father, Walter Beinecke, Sr., made annual gifts through the hospital to Nantucket High School graduates to help them pay for nursing school. After his father’s death in 1958, Walter Beinecke, Jr. formally established the scholarship fund in his memory.

DAISY AWARD FOR EXTRAORDINARY NURSING

The DAISY Award is an international recognition program that honors and celebrates the skillful, compassionate care of nurses. In 2022, NCH recognized two DAISY recipients, Katie Taylor and Lisa Lacoursiere, for their dedication to compassionate nursing on Nantucket. The DAISY Foundation was established by the family of J. Patrick Barnes after he died from complications of an auto-immune disease in 1999. During his hospitalization, they deeply appreciated the care and compassion shown to Patrick and his entire family. When he died, they felt compelled to say “thank you” to nurses in a very public way.

THE SEINFELD/HARTMANN PRIZE FOR COMPASSIONATE CARE

The Seinfeld/Hartmann Prize for Compassionate Care is an endowed prize with a cash gift that is given each year to a caregiver chosen from a list of nominees submitted by the public. In 2022, the honor was awarded to Bonnie Fitz-Gibbon, the Palliative Care Counselor at Palliative and Supportive Care of Nantucket. Her nominator wrote, “I have NEVER met someone with all the skills that Bonnie has displayed in helping others. It is a rare person that can do this. In this world that we live in today, there has been so much loss. Bonnie, and what she represents, signals to me that there is hope for the future and hope that we can help each other and have well-being.” The Seinfeld/Hartmann Prize was established by Judy Seinfeld and Margot Hartmann to promote a legacy of compassion at NCH.

Stephen Murphy

Sandy Craig Leadership Award

Fire Chief Stephen Murphy retired in 2022 after a 35-year career as a fire fighter and EMT – the last five as Chief. In his time as Chief, Steve has made a major impact on the health and safety of the community by adding Advance Life Support capabilities to Nantucket’s EMT program. Most recently, the community witnessed first-hand how well-prepared the Nantucket Fire Department was when they miraculously contained the Veranda House fire.

Sandy Craig Leadership Award

If ever there were a lifetime achievement award for Leadership at NCH, Steve Karp would certainly qualify. Over the last decade, Steve led the planning, permitting and construction of the new hospital building and also helped raise funding for the project. As the Chair of the NCH Real Estate Committee, Steve has not only volunteered his own time and expertise, but he has also volunteered the expertise of his company, New England Development. Most recently, Steve has participated as a member of the New President Search Committee.

Myrt Eldridge Volunteer of the Year Award

Former and returning Trustee, Jeanine Borthwick, developed ACKs of LOVE, an employee appreciation program to boost hospital morale during the winter. Along with a team of volunteers, Jeanine arranged for more than 15 local business to provide hospital employees with free and discounted services on food, shopping and activities. The program was fully funded by Jeanine and the other volunteers, allowing local business owners to benefit as well. The program was turnkey, free to the hospital, and the staff loved it.

Kim and Alan Hartman co-chaired the NCH Annual Fund in 2021 and 2022. With the 2020 cancellation of the Boston Pops on Nantucket, 2021 was a critical year for finances of the hospital. Kim and Alan’s energy, creativity and insights helped drive the Annual Fund to record levels, continuing today.

Planned Gift Enables Employee Housing Expansion

Over the past year NCH increased its employee housing inventory by 20 bedrooms in seven new housing units. In early December, NCH broke ground on three new stand-alone homes on Meadowview Drive that are expected to be complete in May of 2023.

The homes are being constructed on a lot that was created by dividing an existing lot that the hospital received in an estate gift from island native James R. Dennis. Mr. Dennis gifted the original property to NCH as a Life Estate in 2002. The hospital, in turn, rented the existing three-unit property from Mr. Dennis from 2002 until his passing in 2018, providing him income from the property during his lifetime.

The new homes, which include a total of ten bedrooms and ten baths, were manufactured offisland by Dreamline Modular Homes of Nantucket and transported to the island in partially finished form via barge in January. The construction cost of one of the homes was underwritten by a bequest from the Patricia P. Griffin 2019 Trust, and the home will be named the Pat Griffin House in Mrs. Griffin’s honor.

Previously in 2022, NCH purchased two brand new six-bedroom duplexes in the Richmond Development area off Old South Road. Along with the Meadowview

Drive homes, these bring the hospital’s total owned inventory of employee housing to 77 bedrooms across 34 housing units. The purchase of the new duplexes and the construction of the new homes was funded through donor gifts dedicated to that purpose from the hospital’s capital campaign.

Nantucket Cottage Hospital Sees Record Summer Emergency Room Visits

Nantucket Cottage Hospital saw a record number of emergency room patients over the summer of 2022, outpacing the previous highs set just last year.

From June through August, the hospital recorded 5,266 emergency room visits. That number is even more significant considering the hospital reopened its urgent access clinic, which saw another 3,230 patients over the summer.

“In 2021 we had huge volume – we were slammed – and we didn’t have urgent access, so there were challenging times for us and for our patients,” said Chris Glowacki, Nantucket Cottage Hospital’s vice president for strategy, community health, and its chief development officer. “This summer, emergency department volume was still up, and acuity went up.”

And yet, thanks to a number of changes –including better staffing, a revamped triage system, and new nurse-driven protocols – a number of key metrics actually improved compared to the summer of 2021, hospital officials said.

Internal data shared by the hospital with the Current showed that the average patients’ length of stay was down sharply, as was the average time from arrival to be triaged and seen by a provider. The number of patients who left the emergency department without being seen also decreased significantly compared to 2021.

“I’m proud, I think we did a really good job,” said Amy Beaton, the nurse director of the emergency department who joined Nantucket Cottage Hospital in February. Beaton came to NCH from a much larger institution - Reading Hospital in Pennsylvania – where she worked in a 110-bed emergency department with 16 pediatric beds, and 10 psychiatric beds. But from her first day she said she heard what a challenge the summer of 2021 had been for the emergency department team at NCH.

Beaton said she lobbied for additional seasonal staff – a request that was granted – and listened to the emergency department’s nurses about what had worked and what hadn’t in the past. She set out to revamp the NCH emergency department’s triage system, to get patients triaged faster and more efficiently.

“An untriaged patient is the sickest patient, and until you have a set of medical eyes on a person, you don’t know how sick they are,” Beaton said. “So that was my personal goal and one of the things I’m proud of. Everyone was triaged. We had a plan in place, and my goal for my team was to have patients be seen by a clinician with a triage time of 10 minutes.”

Beaton said they established a “fast track” section of the emergency department to address patients with more minor ailments, and got on the same page with other hospital departments like the laboratory and radiology.

“Everyone was on board,” she said. “The structure worked. It was a great, great team.”

Beaton added that they also had buy-in from Bluewater Health, the firm that holds a contract with NCH to staff the emergency department with medical providers, including physicians and physician assistants.

Cynthia Winston, the program director for NCH’s urgent access clinic, said the operation at the Anderson Building was similarly busy over the summer, seeing 55 patients per day. Both Winston and Beaton said the service was invaluable in diverting some of the volume that would have otherwise gone to the emergency department.

Rather than operate the clinic on a walk-in basis as it had operated before the COVID-19 pandemic, the hospital chose to run the urgent access service by appointment only. At midnight, they would open up half of the appointments for that day on the Patient Gateway – the online portal used by Mass General Brigham –while keeping the other half available for people to book by calling the hospital.

While they acknowledged there was significant competition for those appointments and frustration among some patients who would stay up late or set their alarm to wake up and make an appointment at midnight, Winston said “we did everything we could to make sure the people who wanted an appointment got one and got seen.”

The urgent access clinic was typically staffed with two providers on a daily basis. Winston said the hospital plans to keep the clinic open on a year-round basis.

Turning Second Hand Goods into Cutting Edge Technology

Record $600,000 Gift in 2022 Makes New MRI Possible

Last year, the Hospital Thrift Shop made the largest gift in its 94-year history to Nantucket Cottage Hospital to enable the purchase of a new state-of-the-art MRI scanner. The MRI is the most complex and costly piece of equipment in the hospital.

The Hospital Thrift Shop sells merchandise donated by island residents and visitors – anything from used books to furniture to artwork and everything in between – from mid-May through mid-October and operates as a separate non-profit organization. Thanks to a small staff, a dedicated team of volunteers and its Board of Directors, The Thrift Shop remains a favorite for island residents and visitors both in terms of donations toward a worthy cause and as a shopping destination. Since its founding in 1929, the Hospital Thrift Shop have taken the second-hand donations of Nantucketers and turned it into over $6.9 million to support the hospital.

Founded in 2017 by three dedicated volunteers, Meryl Bralower, Melanie Sabelhaus and Jeanine Borthwick, this event has featured both on- and off-island medical professionals sharing resources and information. Hosted in 2022 by Natalie Jacobson, the panelists included John Winkelman, MD, PhD, Chief, Sleep Disorders Clinical Research Program at Massachusetts General Hospital and Janet Wozniak, MD, Associate Professor of Psychiatry at Harvard Medical School and Massachusetts General Hospital. Another Record-Breaking Year for Swim Across America

Swim Across America – Nantucket has raised more than $4 million over the past ten years to support cancer treatment and patient care on Nantucket. Each year, over 400 swimmers, volunteers, spectators, and supporters gather at Jetties Beach for an inspirational morning to raise money for Nantucket Cottage Hospital and Palliative and Supportive Care of Nantucket.

The money raised directly benefits our friends and neighbors. Annually, NCH provides over 1,400 cancer treatments and oncology visits, and PASCON provides nearly 300 palliative care visits, always free of charge.

NCH welcomed our Pet Therapy Program back to campus in 2022. Pet therapy has been shown to improve patient satisfaction, energy levels, self-esteem, and mood, as well as decrease depression. Pets and owners complete a rigorous process to become certified by the Pet Partners Therapy Animal program. Our Pet Therapy Program has proven to be a comforting source of healing for everyone at the hospital, both patients and staff.

Perhaps most importantly, NCH is about the people in our Nantucket community. Through your generosity, we were able to meet our Annual Fund goal, promote community health through Women and Wellness, set a new fundraising record for Pops on Nantucket, and sell out the Sankaty Swing Golf event in a matter of days. Local support also made possible a new MRI machine underwritten by significant donations from the Hospital Thrift Shop and helped Swim Across America raise more than $600,000 for cancer care on the island.
