DAILY HEALTH SCREEN 1) Do you have a fever? Have you taken any fever reducers in the past 24 hours? 2) Do you have a new cough, shortness of breath or any difficulty breathing? 3) Do you have body aches, chills or feel sick today? 4) Have you developed a loss of your senses of taste or smell? 5) Have you had diarrhea, nausea or vomiting in the last 24 hours? If you answer YES to any of the questions above, please stay home and contact your doctor. OR If you have been in contact in the past two weeks with anyone diagnosed with COVID-19, or with anyone suspected of having COVID-19, you must stay home and contact your doctor.
