A History of Beaumont Hospital Beaumont Hospital has played a pivital role in Irish healthcare since its inception almost 30 years ago. It continues to lead the way in its current role heading up the Dublin North East Hospital Group, an extensive medical network that will help shape the future of patient care in Ireland.
B
eaumont Hospital was commissioned and built between 1978 and 1984 and has become a landmark on Dublin’s Northside since it opened in 1987. The hospital was formed as a result of the amalgamation of two city-based hospitals, the Charitable Infirmary in Jervis Street and St Laurence’s, known as the Richmond. Both had a long and proud tradition of excellence in patient care and progressive medics such as Sir Dominic Corrigan. These values and traditions remain central to Beaumont Hospital’s mission today. One of the most significant developments since Beaumont opened was the acquisition of St Joseph’s Hospital, Raheny, in 2004. This hospital has 70 beds and serves as both a rehabilitation facility for elderly patients and for non-urgent elective surgery. In 2007, Beaumont Hospital, along with seven other hospitals, was designated as a Centre of Excellence for Cancer Services. The past year saw us embark on another significant chapter in the history of Beaumont with the announcement of the new hospital groups by Minister James Reilly in May 2013. This puts Beaumont at the centre of an extensive medical network serving the community in Dublin and the north east. Beaumont leads the group that includes Our Lady of Lourdes Hospital, Drogheda; Connolly Hospital, Dublin; Cavan General Hospital; the Rotunda Hospital, Dublin; Louth County Hospital and Monaghan Hospital. The Royal College of Surgeons of Ireland are the academic partner of the group. Beaumont will continue to enjoy close links with Dublin City University and other academic institutions in respect of training and research.
Beaumont Hospital currently employs approximately 3,000 staff and has 820 beds. It is the national centre for neurosurgery, renal and pancreatic transplantation and cochlear implantation. In addition, it is the regional treatment centre for ear, nose and throat, and gastroenterology. The hospital also provides a 24-hour emergency service for its own catchment area of approximately 250,000 people. An average 60 patients per day are admitted for trauma or elective treatment, making it one of the busiest general hospitals in the country.

Patient Safety is Paramount in Beaumont Quality and safety of patient care or ‘clinical governance’ is a major focus in Beaumont Hospital. Consultant microbiologist Prof Edmond Smyth, Chair of Clinical Governance and a member of the hospital’s senior management team, discusses the level of its importance to the hospital.
T
he safe care of patients is the primary priority for Beaumont Hospital and this ethos is present at every level and within every department. Prof Edmond Smyth represents the voice of safe patient care on the management team. “Patient safety is top of the agenda. Over the last five years, the hospital has put considerable time, effort and resource into ensuring there are systems in place to make the hospital as safe as possible for patients,” explains Prof Smyth. The Integrated Quality and Safety (IQS) department includes staff dedicated to quality, clinical governance and audit, health and safety, risk and legal, and patient complaints. Reviewing and Improving “We believe it is vital to have a ‘just and fair culture’ for reporting and reviewing incidents of harm or nearharm to patients to ensure such incidents are dealt with appropriately. It is essential that the hospital learns from failures and that practice is changed, where necessary, to ensure the same errors do not occur in the future,” says Prof Smyth. A particular high-risk area in all hospitals is the use of medicines. Drugs have a tremendous potential for good but errors, adverse events and drug-interactions are not uncommon and can cause significant harm. For that reason, there needs to be a particular emphasis on the safe use of medicines. Policies and Procedures Hospital-wide policies, procedures and guidelines are audited to ensure
compliance with good practice. Regular feedback is provided to management and the Board and in the last year there has also been a focus on patient engagement and learning from the experience of patients and their families. In 2014 we held the Third Annual Clinical Audit and Patient Safety meeting. Directorates, departments and services presented audit results and data on quality improvement initiatives undertaken across the hospital. This provided an opportunity to share learnings and focus on other areas for improvement. Minimising Infection Prof Smyth is himself a consultant microbiologist whose role is in the prevention, diagnosis and treatment of infection and of course, in recent years, a spotlight has been shone on how hospitals prevent health-care associated infections (HCAIs) like MRSA, C. difficile and VRE. “Patients coming into hospital should not be at risk of acquiring preventable HCAIs, which cause significant morbidity and indeed mortality. Patients with transmissible infections or those colonised with multi-drug resistant organisms must be identified in a timely fashion; managed appropriately from a nursing and medical perspective and the correct procedures implemented to prevent onward transmission. Hospital hygiene, good hand hygiene practice and the rigorous implementation of standard precautions are of paramount importance,” concludes Prof Smyth.

Neurology Services As the national centre for Neurology in Ireland, Beaumont hospital proivdes treatment for comprehensive range of neurological disorders. The department is also home to high level clinical reasearch and committed to the ongoing training of clinicians in this area.

Beaumont Hospital provides regional, supra-regional and some national neurology services, as well as specialist services for stroke, epilepsy, multiple sclerosis, motor neurone disease, post-polio syndrome, migraine, Huntingdon’s disease and movement disorders. The hospital has a dedicated ward with specialist neurology nursing, an epilepsy monitoring unit, a stroke unit, a neuro-intensive care unit and a new neurology day unit, which has just been opened.
Neurology services
Because of its strong legacy in neurology [in the 1990s the service was heavily centralised there], Beaumont has traditionally had a larger proportion of consultant neurologists compared to other parts of the country. This has allowed it to build up regional, supra-regional and some national neurology services. Prof Orla Hardiman, Consultant Neurologist at Beaumont and head of the new Academic Neurology

Unit at Trinity’s Biomedical Sciences Institute says it had been tradition in Beaumont to encourage very extensive sub-specialisation and she thinks that it worked well: “Our vision is to provide a very good and high-level general service to our local community which includes 24/7 on-call for our local patients and those presenting at ED. We are the only hospital in Dublin to provide this on-call service. Along with our clinical professional colleagues, we also provide sub-specialist care in a range of disciplines and this also allows us to build a clinical research domain into our practice. There is a lot of evidence to demonstrate that clinical research improves the overall quality of patient care.”
Stroke Services
Beaumont consultant neurologists contribute 50 per cent to the
hospital’s stroke service and, without any additional resources, also contribute to the more high-level stroke services required for other parts of the country. Consultant Neurologist Dr Joan Moroney has a special interest in stroke care and this is now a very high volume service. In 2003, Comhairle na nOspidéal recommended that excluding stroke services, there should be one consultant neurologist per 100,000 population. Ireland currently has one consultant neurologist for every 130,000 people and Irish neurologists provide both neurology and stroke services, indicating that there is still a severe shortage of consultants within the specialty.
Epilepsy Services
Beaumont provides a specialist consultant-led and specialist nursingled epilepsy service with a nurse-led Beaumont Hospital

telephone-based advice service. Beaumont is one of the main centres for epilepsy under the Clinical Leads programme and has an active nurseled service with nurse prescribing. The hospital operates an awardwinning electronic patient record system for epilepsy. The service has grown from that started by Dr Hugh Staunton in the 1980s. Under the direction of Prof Norman Delanty, it provides both a medical and surgical programme, including a state-ofthe-art epilepsy monitoring unit and specialist surgical management of intractable seizures. Prof Delanty has built a successful research programme with the Royal College of Surgeons in Ireland including pharmacogenomics, neuroimaging and health services research.
Motor Neurone Disease
Beaumont has built up a national motor neurone service and provides care for 80 per cent of motor neurone patients in the country. Prof Hardiman says: “Our clinical professionals have considerable expertise in management of motor neurone disease; we have a specialist nurse in motor neurone and we work very closely with the palliative care services which have accepted that this is part of their service. We provide direct care for 80 per cent of the country’s motor neurone patients. We work closely with the Irish Motor Neurone Disease Association and provide an outreach service for all Irish patients. We also have links with services in Northern Ireland. We have a big clinical programme, provide a very high level of multidisciplinary care and we have a large research programme on motor neurone disease that is based in the Academic Unit of Neurology in Trinity College.”
Muscular Dystrophy Service
Beaumont provides a supra-regional muscular dystrophy service. The service is allied to Cork University Hospital and services in Northern Ireland. The two southern Irish centres share a research registrar who is supported in part by Muscular Dystrophy Ireland. 12

Post-polio Syndrome
Beaumont provides a national service for post-polio syndrome, which includes specialised physiotherapy services and works closely with the Irish Post Polio Support Group.
Multiple Sclerosis
The MS service provides a widely ranging multidisciplinary clinical care programme including an outreach service provided by two specialist nurses. The service works closely with MS Ireland and is actively involved in Phase III clinical trials of new MS compounds.
Migraine Service
Beaumont’s migraine service was established by Prof Orla Hardiman in 1999 in association with the Migraine Association of Ireland. Under the stewardship of Consultant Neurologist and specialist in migraine, Dr Martin Ruttledge, the service has grown into a large programme offering state-of-theart treatments for intractable migraine and other headache syndromes. The migraine service has also developed clinics, provided by Nurse Prescriber Esther Tomkins, and works closely with the Migraine Association of Ireland.
Movement Disorders
Beaumont has two specialists in movement disorders: Prof Dan
Healy, who also has specialist training in neurogenetics, and Dr Fiona Molloy, a jointly certified neurologist/neurophysiologist with specialist training in dystonia. Dr Molloy provides a Botox clinic and a recent neurogenetics/movement disorder clinic has been developed by Prof Healy. A new multidisciplinary service for Huntington’s disease will begin later this year under the direction of Dr Niall Pender, Director of Neuropsychology, in collaboration with the departments of Neurology and Psychiatry. Prof Hardiman says: “We have a high number of clinical research fellows and quite a lot of clinical research that is deeply embedded within our clinical service. Our commitment is to multidisciplinary care. We provide dedicated training in neurosciences for doctors and nurses and have a very strong commitment to excellence. “We have superb neurorehabilitation services with specialists in neurological physiotherapy, occupational therapy, speech and language therapy and dietetics. There is a very strong commitment to the development of partnerships with colleagues in other allied clinical professions, particularly in the areas of neuropsychology, neuroscience nursing and neurorehabilitation.”

Cochlear
The National Cochlear Implant Programme THE NATIONAL COCHLEAR Implant Programme at Beaumont Hospital has been providing highquality care for adults and children with severe to profound hearing loss since its foundation in 1995. Hearing aids work by amplifying sound, which may be all that is needed for people with mild or moderate-severe hearing losses. For people with more severe or profound losses, however, making the sounds louder may not help, because the damaged hair cells cannot pick them up. For these people, a cochlear implant, which sends an electrical signal directly to the auditory nerve, may be more effective. Cochlear implants are highly sophisticated devices, which provide access to sound for people with severe to profound hearing loss. The surgically implanted device, when coupled to an external processor, can provide access to speech and everyday sounds to aid or improve communication abilities. The cochlear implant sends an electrical message through a wire called an electrode directly to the auditory nerve, bypassing the damaged or absent hair cells. This means that, provided that the auditory nerve is still working, profoundly deaf people can hear sound. The cochlear implant consists of two parts, a surgically implanted internal part and an externally worn part called a processor. During the surgery, the surgeon implants the internal parts of the cochlear implant underneath the 14
skin. The receiver/stimulator sits in the bone just behind the ear and the electrode array is inserted into the cochlea. The electrode array has been especially designed to be thin, tapered and flexible, so that it can slide into the cochlea without causing damage. Consultant ENT Surgeon, Ms Laura Viani, who heads up the programme, said assessments for cochlear implant suitability are carried out across all age groups, ranging from babies of a few months to those aged over 80, and includes those with complex needs. A detailed assessment is needed to find out whether someone is likely to benefit from a cochlear implant. The assessments are carried out by the cochlear implant team. As a general guideline, to be considered for an implant you need to meet the following criteria: • Severe to profound sensorineural hearing loss in both ears • Receive only marginal benefit from hearing aids • Medically suitable for surgery • Strongly motivated • Having a good support network from family, friends, or other professionals. The implant team assesses each case individually and will look at these issues and at other factors which will affect whether or not a patient
is likely to benefit from a cochlear implant. If, following assessment, an individual is deemed to be a cochlear implant candidate and cochlear implant surgical intervention is chosen as the management option by the parent/guardian or patient, the CI Programme provides lifelong management for CI recipients residing in the Republic of Ireland. This includes individuals transferred from other programmes worldwide. The programme is completely publically funded and includes assessment, hearing aid fittings, surgical intervention, and all postoperative follow-up and management. Currently, the programme has over 700 cochlear implant recipients. The Cochlear Implant (CI) Programme is delivered by a multidiciplinary team of highly trained and qualified professionals specialising in deafness, including ENT surgeons, audiological scientists, speech and language therapists, teachers of the deaf, psychologists, and administrators. The CI Programme also liaises closely with other specialists including radiologists, geneticists, ophthalmologists, interpreters, and social workers to provide a comprehensive patient-centred service during the assessment and

Critical Care Service The Critical Care service plays an essential role in the treatment of high-risk patients identified by the early warning score and in the care offered to patients after major surgery and other medical interventions.
trends towards minimally invasive surgery and increased numbers of medical patients, as the general population gets older. The hospital hopes to expand its critical care capacity with the addition of high-dependency beds in the near future. This will lead to increased ability to treat high-risk patients identified by the Early Warning Score and to improve the quality of care offered to patients after major surgery. Beaumont has a large population of patients with complex disorders, including those from its national specialties of neurosurgery and renal transplantation. The unit is unique nationally in participating in a comprehensive audit programme where outcomes are measured and benchmarked against 250 ICUs across the United Kingdom. Outcomes in Beaumont’s ICU are comparable to, and in some cases better than, these units. The audit has provided ongoing data on which areas the hospital should also focus on to improve quality of care. Beaumont Hospital is one of the largest intensive care units (ICUs) in the country, with a potential total of 22 beds divided between general ICU and Richmond Neurosurgical ICU. These units care for patients from the hospital’s catchment area and patients referred from across the country. The hospital is the National Neurosurgical Centre and many referrals are for specialist neurosurgical care. Over the last 10 years, the relative proportion of surgical patients has decreased and medical patients have increased, reflecting general

Transplant, Urology and Nephrology
Major Expansion of
Urology and Kidney Services at Beaumont
The hospital provides a regional service for medical kidney disease, including patients with renal impairment, proteinuria, haematuria, kidney stones, general urology, urooncology, a rapid access prostate cancer assessment, dialysis and a national service for kidney transplantation. Beaumont and Connolly Hospital are in the process of establishing a shared Department of Urology. Three new consultant urologists are being recruited bringing the total in the department to 11. Nine of the consultants are based predominantly in Beaumont, with the remainder based predominantly in Connolly. Eight of the surgeons are combined urologist and transplant surgeons and we are re-organising our urology and transplant work

A full range of urology, nephrology and renal transplant services are provided in Beaumont Hospital and a €14.5 million expansion of these facilities is currently underway.
so our urology model of care will be that urological cancer and transplant urology will be carried out in Beaumont and benign urology procedures will be predominantly performed in Connolly. Beaumont Hospital’s consultant nephrologists manage the care for about 350 patients undergoing various modalities of renal replacement therapy in the hospital itself and in its satellite haemodialysis units at the Northern Cross and the Beacon Hospital. There are 270 patients on in-centre haemodialysis, 20 patients on home haemodialysis and 55 patients on peritoneal dialysis. The directorate has recently established satellite nephrology clinics in both Connolly and Drogheda hospitals to meet the needs of patients and will be opening a new satellite haemodialysis unit in Drogheda in the coming months. Beaumont Hospital is also developing a new dialysis unit on its campus, which will be opened next year. The National Renal Transplant Programme at Beaumont Hospital is one of the busiest in the world, performing between 160 and 190 kidney transplants per year. The aim is to increase this to 250 transplants per annum. Up to the middle of this year, the hospital had carried out over 4,000 transplants. An important part of the hospital’s work is assessing donors for transplant suitability. Both live and deceased donor transplants are carried out. This summer, the Government announced an investment of e14.5m as part of a phased expansion of the transplant and urology facilities at Beaumont and Connolly hospitals.

The scope of radiology has widened significantly, with interventional radiology becoming increasingly routine in the investigation and treatment of many conditions that formerly required surgery. In addition, Beaumont Hospital has stateof-the-art imaging equipment providing highly detailed imagery for clinicians. Beaumont Hospital has a wide range of diagnostic and interventional radiology. GPs can directly order a wide range of plain film X-rays for their patients at Beaumont Hospital. These X-rays are carried out by appointment. Beaumont also has a walk-in chest service, where patients can come without an appointment for a chest X-ray. In addition, there is a separate X-ray unit attached to the Emergency Department (ED) for plain film X-rays, which means that patients in the ED do not have to go to the main Radiology Department. More sophisticated radiology services such as ultrasound, CT and MRI are ordered by hospital clinicians. A significant amount of

Beaumont’s radiology work is done on foot of the hospital’s national specialties, such as neurosurgery. As Beaumont is one of the designated cancer centres in the country, this results in a high demand for CT and MRI examinations. Interventional radiology is now a growing part of the work of the Radiology Department. According to Donal Rorke, the service’s business manager: “Interventional radiology uses minimally invasive imageguided procedures to diagnose diseases. Conditions that formerly
required surgery are now treatable by interventional radiologists. By minimising the physical trauma to the patient, peripheral interventions can reduce infection rates and recovery time, as well as shorten hospital stays. “Much of the work is neurointerventional, specialising in stroke treatment, where time is of the essence. When stroke patients come into the Emergency Department, in appropriate patients, consultant neuroradiologists can stabilise the patient, giving them a good chance of a full recovery.”

The Clinical Directorate of Laboratory Medicine provides an extensive clinical and diagnostic laboratory service to a range of hospitals (HSE, voluntary and private), other healthcare-providing facilities both nationally and regionally, and to general practitioners in Beaumont Hospital’s catchment area. The range of testing provided includes both routine and national specialised esoteric tests. In 2012, more than 4.56 million tests were performed, with a total repertoire of over 800 different test types being offered. The Clinical Directorate of Laboratory Medicine is committed to the provision of laboratory services compliant with best international standards. All laboratories have a fully accredited status following inspection. Beaumont is an important centre
for surgical specialties A large range of surgery, from plastic surgery to vascular surgery, is available at Beaumont Hospital. Its talented team includes Clinical Director Prof. Arnie Hill, along with upper GI surgeons, vascular surgeons, gynaecologists and consultant orthopaedic surgeons. Beaumont Hospital provides breast surgery, colorectal surgery, upper gastrointestinal surgery, lower gastrointestinal surgery, gynaecology surgery, orthopaedic surgery, plastic surgery and vascular surgery, according to Prof. Arnie Hill, Clinical Director in the Surgical Directorate. The Surgical Directorate has 134 beds in three wards and is also responsible for the day service and the Outpatients’ Departments for each of the specialties. Over 172,000 patients are seen annually in these OPDs.
Beaumont Breast Clinics
Prof. Hill says that about 9,000 new cases are seen each year in the hospital’s breast clinics, of which
approximately 300 new cancers are diagnosed each year. Beaumont is one of the eight designated breast cancer services in Ireland and, as such, has fulfilled all the HIQA guidelines in terms of standards of delivery of symptomatic breast cancer. A new feature of the breast service this year is the development of a family history service. Prof. Hill said this had been introduced in view of the increasing awareness of the importance of family history in breast cancer (particularly following the Angelina Jolie publicity). It is expected that about 1,000 patients will be seen in this service this year. Beaumont is the largest centre for

Beaumont is one of the designated upper GI centres for oesophageal and gastric cancer for patients, providing a comprehensive oesophagectomy and gastrectomy service for the region. It fulfils all the quality parameters laid down by the National Cancer Control Programme. The hospital has three designated Upper GI Surgeons, Prof. Paddy Broe, Prof. Tom Walsh and Mr. Mayilone Arumugasamy.

Beaumont Vascular Surgery Service
Beaumont has the largest programme for endovascular treatment of abdominal aortic aneurisms - over 70 such procedures were carried out last year. It is also the only centre in Ireland that provides a service for endovascular treatment for ruptured abdominal aortic aneurysms. The unit also provides a very significant minimally invasive service for the treatment of varicose veins, using radio frequency ablation. The vascular surgical service is run by four consultant surgeons, Prof.

The orthopaedic service is run by specialist consultant orthopaedic surgeons. They provide a trauma service for the hospital. Mr. Hannan Mullet has a national reputation in shoulder surgery, Mr. Denis Collins provides a national referral service on revision arthroplasty and Mr. Mitchell Donnelly, with expertise in joint replacement, has recently joined the team.

The gynaecology service, which has expanded recently to service a large increase in the benign gynaecology workload, is run by consultant gynaecologists Mr. Paul Byrne and Mr. Barry Gaughan.

Acute Psychiatric Care
New Acute
Psychiatric Service The Ashlin Centre, a new multi-million euro psychiatric unit which opened recently at Beaumont Hospital provides adult general and psychiatric of old age admission beds. A new a13 million acute adult psychiatric unit has recently been opened in Beaumont Hospital. Located opposite the main entrance hall and beside the multi-storey car park, the opening of the unit sees 30 years of planning finally come to fruition and represents the first phase of a major psychiatric development on the hospital campus. The Ashlin Centre, which is a new purpose built adult psychiatric facility, provides 38 general adult beds and six Psychiatry of Old Age acute admission beds. Developed by the HSE in cooperation with Beaumont Hospital, the unit is managed by the HSE North Dublin Mental Health Services and replaces services historically provided in St. Ita’s Hospital, Portrane.
The new unit provides consultant led, multidisciplinary acute inpatient mental health care in an open setting for adults and the elderly. There are five internal gardens as well as a courtyard, all of which have been designed to enable patients to avail of different activities and spaces. Marie Keane, Deputy CEO of Beaumont, said that the Ashlin Centre provides 44 single ensuite bedrooms. These include 38 acute psychiatric admission beds for adults. All of the patients in these admission beds have access to all the Beaumont clinical care services, including diagnostic and pharmacy services. Eight are high observation beds Six acute psychiatry of old age beds are also provided in the centre Phase two of the development will include a mental health/intellectual
disability specialist acute admission unit which will share space with a new renal dialysis unit. The two storey building is managed and staffed by the HSE Dublin North Mental Health Service. The unit is integrated with Beaumont Hospital and run in collaboration with Beaumont. Beaumont Hospital in conjunction with the HSE now has a 24-hour emergency psychiatric consultation service from the Emergency Department in Beaumont. The new centre has replaced the Joyce Rooms in Fairview Community Nursing Unit. HSE staff have relocated to the Ashlin Centre. These services were previously provided in St Ita’s Hospital, Portrane.

Cancer Care
Beaumont Hospital, as a designated cancer centre, provides a wide range of surveillance, diagnosis, treatment and post-treatment care for patients with cancer.
Beaumont Hospital patients come from its catchment area of north Dublin and the north east of the country. Beaumont Hospital also cares for patients from all over the country for certain specific cancers such as neurological cancers, for which it is a national centre. “There is a high-quality, integrated approach to the provision of cancer services at Beaumont Hospital. From diagnosis, including rapid access clinics, to surgical treatment and/or medical and radiation oncology treatments, patients receive a high level of care,” says Helen Shortt, executive in charge of developing cancer services in the hospital. 26

A Multidisciplinary Approach
All patients referred to the hospital are diagnosed and treated by a multidisciplinary team including consultant surgeons, consultant oncologists and consultant radiation oncologists. This team, along with radiologists and pathologists, makes the decision as to whether patients require surgery, chemotherapy, radiotherapy or a combination of these treatments. Following surgery,
most treatment is provided as a daycase including radiation oncology. The development of radiation oncology facilities at Beaumont Hospital, as part of the St Luke’s radiation oncology network, means that many patients spend less time travelling for their therapy. It also means that for many patients, all of their cancer treatment can take place in Beaumont Hospital. The Radiation Oncology Centre on the hospital campus has four linear accelerators.

Cancer Care cancer MDT; this is a very large increase on the 127 discussed in the previous year. The Rapid Access Prostate Clinic offers patients an appointment within 20 working days following referral. The service strives to provide a clinic appointment and, if considered necessary by the consultant urologist, a prostate biopsy on the same day. Referral criteria, including results from two PSA tests, have been agreed by the National Cancer Control Programme. The numbers of prostate cancers diagnosed through this service is rising; 99 patients were diagnosed through the rapid access service in 2011 and there were 137 diagnosed through the service in 2012. Patients attending general urology clinics are also diagnosed with prostate cancer but are not included in these numbers. The symptomatic breast cancer service aims to provide an appointment within 10 working days for all urgent referrals and within 12 weeks for non-urgent referrals. Around 300 patients are diagnosed with breast cancer in Beaumont Hospital each year and over 8,000 patients attend the symptomatic breast cancer service each year.
Increase in Instances of Cancer Such a facility has greatly improved access to radiation oncology facilities for patients in the region.
Rapid Access Cancer Clinics
Beaumont provides a number of rapid access clinics for certain cancers although all consultants will ensure that patients with a possible cancer diagnosis are seen as quickly as possible. The Rapid Access Lung Cancer service, an initiative of the National Cancer Control Programme, provides an appointment for patients within ten working days of receipt of referral. Beaumont Hospital meets it 100 per cent. There were 235 patients with lung cancer which were discussed at the rapid access lung
Beaumont is the larger of the two centres which make up the country’s neuroncology service (the other is in Cork) and many of the patients for this specialty treatment are hospital-to-hospital transfer. Over 660 patients were discussed by the Beaumont Hospital neuroncology MDT during 2012; of these, 441 patients had neurosurgery. Other cancers in which Beaumont specialises are cancers of the oesophagus and stomach and cancers of the colon, rectum and anus. The haematology service at Beaumont has seen an increase of 18 per cent in the numbers of haematological cancers diagnosed and treated. The numbers of patients diagnosed with skin cancer have
increased by 21 per cent between 2011 and 2012. Medical oncology treated 13 per cent more patients in 2013 than it did in 2011. Palliative radiotherapy and chemotherapy are available and a consultant in palliative care medicine has sessions in Beaumont. Surgery for lung and pancreatic cancer is not carried out in the hospital and patients requiring this surgery are referred to national centres. Dr. Susan O’Reilly, Director of the HSE National Cancer Control Programme, has said that the number of invasive cancers in Ireland is projected to increase by 46 per cent between 2010 and 2020 and by 108 per cent between 2010 and 2030. According to Dr O’Reilly, critical success factors in cancer outcomes include population-based screening, early diagnosis/stage shift, multidisciplinary teams, high volume/expert centres and national standards/guidelines/protocols/ policies/processes. In combination with the excellent medical care provided by doctors and specialist nurses along with allied health professionals, appropriate diagnostic and treatment facilities, ambulatory/inpatient resources, LINACS and drugs are all essential components to providing the necessary standard of cancer care for patients.

Beaumont Leads the Way in
Cystic Fibrosis Treatment and Research
Beaumont Hospital is one of the largest adult cystic fibrosis (CF) centres in Ireland, providing specialist treatment and services to adults with CF.
Two consultants with rotating NCHDs, run the CF services at Beaumont along with a team comprised of three nurses, a physiotherapist, dietician, psychologist and a social worker. The service also provides a national referral centre and clinic for alpha 1 antitrypsin deficiency (A1ATD), a form of inherited emphysema, along with a general pulmonary clinic. GPs can refer directly by letter to the CF service and there is a walk-in service for existing CF patients of the hospital. Prof Gerry McElvaney, joint director of the CF service with Dr Cedric Gunaratnam, says that while CF was traditionally a disease of children, improvements in diagnosis and treatment meant that today over 50 per cent of people in Ireland with CF are adults and this proportion is set to rise in the future. Ireland has the highest incidence of CF in the world. One in 19 Irish people carry a CF gene and there are approximately 1,200 people with CF in Ireland. Beaumont has a new outpatients’ unit specifically for the treatment of patients with CF. The 2,500 square foot unit was built at a cost of e3.5 million, with funds provided under a capital allocation from the HSE. The dedicated centre facilitates the delivery of a comprehensive range of services by multidisciplinary teams and allows more patients to be seen 28

during the day. The services provided include consultations with doctors and nurses, provision of treatments which might otherwise have required hospitalisation, assessment of pulmonary function, physiotherapy, dietetics and psychology. The multidisciplinary team treats over 150 patients. With increasing numbers, this has resulted in an in-house development of a dedicated electronic record for Beaumont’s CF patients by Dr Gunaratnam, the only such IT facility specifically for CF patients in Ireland. The CF unit is an OPD unit Monday to Friday and does not contain overnight beds. Patients can be started on home IV therapies and can meet with physiotherapists, dieticians and psychologists. “Infection control is a priority. The new building includes consultation and therapy rooms with air filtration units that allow for 12 air changes per hour – approximately three times the average rate in a normal room. This has significant benefits in reducing the time needed between patients for all important infection control purposes,” says Prof McElvaney, “While all appointments have to be scheduled, the new ambulatory day care facility improves patient access to essential services. Along with priority access to a limited number of en-suite inpatient rooms, it means that for
Beaumont’s CF patients, admission as an inpatient via the Emergency Department will be avoided as much as possible, although with increasing patient numbers this may soon no longer be the case. “The new centre also contributes to the research activities in CF already well established in Beaumont, principally in collaboration with the Royal College of Surgeons in Ireland. The CF unit in Beaumont is recognised worldwide as a centre of excellence for CF research,” according to Prof McElvaney. Beaumont also provides the national alpha 1 antitrypsin deficiency (A1ATD) clinic. Consultants or GPs can refer patients directly to the clinic, where they will be tested free of charge. If they are diagnosed with the condition they will be treated in the Beaumont A1ATD clinic. Prof McElvaney says the A1ATD clinic has rapid access to new and exciting therapies for A1ATD. Beaumont is the national referral centre for A1ATD and for patients with other forms of inherited emphysema. The Irish incidence of A1ATD is high, with one in 24 Irish people carrying the gene. This is much higher than in the US or elsewhere in the EU, so it is not surprising that Ireland is at the forefront of research into this life-threatening and lifeshortening inherited condition.

Beaumont
Pain Clinic Pain management is an important field of medicine, and the 750 patients who visit Beaumont Pain Management Clinic every year know. Cases seen include chronic neck and back pain, and pain following surgery.
Beaumont Pain Management Clinic accepts direct referrals from GPs and hospital consultants in its immediate area and from most of the north east. It also accepts some national referrals for problems such as complicated facial pain and spasticity disorders. The service is provided by Dr Josh Keaveny and Dr Valerie Pollard, both of whom are consultant anaesthetists with a special interest in pain management. They provide two outpatients’ clinics and several interventional sessions each week. Clinical Director Dr Josh Keaveny says the majority of their patients have chronic neck and back pain, persistent pain following complex spinal surgery, complex regional pain and facial pains, e.g trigeminal neuralgia.
What We Do
The Pain Clinic carries out a variety of pain-relieving interventions and is one of the few pain centres in the country to offer neuromodulation, trigeminal ablation and intrathecal baclofen therapy. It has a large throughput of outpatients – about 750 annually. Dr Keaveny says that there is a high demand for pain services at present: “Pain is now accepted as a chronic disease in its own right and patients are entitled to access to specialist treatment as a basic human right.
We are hoping to expand the service and secure a third consultant in pain medicine. Currently the waiting list for new patients is nine to 12 months and similar for certain procedures.” The Medical Council is expected to give pain medicine specialty status in the near future. This milestone
will increase awareness of pain management, education, funding and treatment. Pain management is under-resourced in Ireland although 14 per cent of patients complain of pain at any one time and the cost to the exchequer is estimated at e4 billion. Beaumont Hospital

Rehabilitation And Elective Services At
St. Joseph’s Hospital
St. Joseph’s Hospital, Raheny caters for elective surgery and has a rehabilitation unit. New initiatives such as a haematomachrosis clinic and the transfer of the Beaumont day hospital make St. Joseph’s a vital part of the local community.
St. Joseph’s Hospital, Raheny, which is managed by Beaumont Hospital, provides a wide range of elective and rehabilitation services. Formerly a private hospital, St. Joseph’s – which was taken over by Beaumont in 2004 – is now a Level Two hospital.
Elective Surgery
St Joseph’s operates a five-day surgical ward with 14 overnight beds for surgical patients. It is consultant-
led and used for elective surgical procedures including general surgery, vascular surgery, ENT surgery, urological surgery, plastic surgery and dermatological surgery. Pain control services are also provided. Last year, St. Joseph’s treated 4,262 elective surgical patients in its fiveday surgical unit.
Rehabilitation Unit
The hospital also has a 20 bed rehabilitation unit for geriatric

The rehabilitation ward is staffed by a multidisciplinary team led by a clinical nurse manager, Elaine Butler. The multidisciplinary team includes a physiotherapist, an occupational therapist, a social worker, a speech and language therapist and a dietician. The staff provide comprehensive multidisciplinary rehabilitation to older people with a wide range of often complex rehabilitation needs. Mary Keogh, Head of Services in St. Joseph’s, explains that the rehabilitation generally took four to six weeks and the unit treated over 200 patients a year.

History Edenmore House, formerly Violet Hill, dates back to the first half of the 18th century. Samuel Dick, a wealthy Dublin merchant, acquired it in 1787. He built the crescent cottages in the village for his workers as well as the charity school beside the graveyard. In 1825 the house became the home of John Maconchy, who renamed it ‘Edenmore’ after his family estates in Longford. The railway company bought the house and lands in 1842 in order to build the railway but later sold the property. The house continued as a private residence until it was acquired in 1957 by the Sisters of St. Joseph of Chambery, who incorporated the old house into a nursing home and named it St. Joseph’s Nursing Home. It gained hospital status in 1968 and it became known as St. Joseph’s Private Hospital. The Sisters of Chambery sold the hospital to Dr. Abdullah in 1997 and it continued to run as a private hospital until 2001. It was brought into public ownership for continued use as a hospital by the HSE (Eastern Regional Health Authority) on September 24, 2001 and was managed by the Northern Area Health Board. The operational responsibility, including maintenance of the site, was transferred from the NAHB to Beaumont Hospital on August 6, 2004.

Outpatients
The Process Outpatients Department Our Outpatients Department is a critical gateway to Beaumont Hospital, where last year, more than 143,000 patients were seen in the 156 clinics, covering numerous specialties. Hours The Outpatients Department operates between 8.00am-6.00pm between Monday and Friday. Each day there are three clinic sessions: 8.00am-11.00am 11.30am-2.30pm 3.00pm-6.00pm You can find specific clinic hours and times on our website, www. beaumont.ie/clinic
Location Most clinics are located off of the main corridor near the hospital’s front entrance.
Outpatient Appointments Each patient is given an individually timed appointment. This helps to improve patients’ waiting times and allows for a more co-ordinated approach to patient care. Patients are referred to the Outpatients Department from various sources such as GPs and internal or external consultants or clinical specialists. Referrals can be sent to a named consultant or consultant specialities. Those that are not addressed specifically will be referred to the most appropriate consultant. Referrals received are all reviewed by the relevant consultant and triaged as either urgent or routine. In accordance with national waiting list policy, patients should expect to receive a letter acknowledging receipt of the referral 34
within two or three weeks. A copy of this letter will be issued to their GP if contact details are provided along with the referral. Accordingly, external referrals from a source other than a GP, for patients who have not attended Beaumont Hospital before, should include essential information e.g. patient’s date of birth and GP’s name and contact details. Urgent cases are issued with an appointment to attend the Outpatient Department and wait times will reflect the urgency of their referral. Routine patients will be offered an appointment to attend the Outpatient Department in chronological order. For a limited number of treatments or procedures carried out under local anaesthetic (e.g. pain management), a consultant may elect to refer a patient directly to a treatment waiting list without the need for review in the Outpatient Department. In order to minimise triage delay and to avoid the issue of unnecessary outpatient appointments, it is imperative that GP/external referrals provide a comprehensive patient history.
Routine Outpatient Department Wait Time Targets Beaumont Hospital aims to provide all referred patients with an outpatient appointment within 12 months unless referred directly to a treatment waiting list without the need for attendance at the Outpatient Department.
Booking Appointments To book an outpatient appointment, you will need a referral letter from a GP, a Beaumont Hospital consultant, a consultant from another hospital, or the Emergency Department. You can post or fax your letter or deliver it by hand. Mail Mail your letter to: Secretary to [Consultant & Speciality], Beaumont Hospital, Dublin 9 Fax Address the fax to: Secretary to [Consultant & Speciality]. Fax numbers are provided opposite. Deliver by hand Please drop it at the outpatient registrations desk. The consultant team will review your letter and decide on the most appropriate appointment time. You will be informed in writing of the details. Please note that waiting times vary per speciality. Once a referral has been triaged, patients will be assigned to an outpatient clinic waiting list. Patients will be scheduled to attend an Outpatient clinic where their medical history will be taken and treatment needs/options will be discussed. If surgery is required, a waiting list card will be completed by the consultant at the clinic and forwarded to the relevant inpatient or day case admissions office. For efficient management of the waiting list, patients will be contacted approximately six weeks ahead of time with an offer of a suitable date to attend the Outpatient Department.

The following is an overview of information for patients of Beaumont Hospital and their visitors. More detailed information regarding admissions is provided on page 46. For up to date information on visitor restrictions please visit our website, www.beaumont.ie. Patient information
Full details of the hospital’s services and facilities are included in a number of key languages on the hospital website, www.beaumont.ie. Patients are supported throughout their time at Beaumont Hospital by a multidisciplinary team appropriate to their needs. This is further supported by an interdisciplinary chaplaincy team and facilities such as a shop, restaurant and banking facilities. Patients are asked to provide their own personal care items such as night wear and toiletries and to make their own transport arrangements to and from the hospital. Our admissions and discharge procedures are detailed on page 46.
Getting to and from Beaumont Hospital
By car: Exit the M1 at the Coolock exit and turn left onto Coolock Lane. Then turn right onto Kilmore Road (opposite the Northside Shopping Centre) and right again on to Kilbarron Road. Make a left onto Trim Road, which takes you to the hospital’s rear entrance. Keep right to proceed to the multi-storey car park. Please be advised that clamping is in operation on the site. A map is available on our website www.beaumont.ie. By bus: Beaumont Hospital is serviced by the 17A from Howth Junction train station/Kilbarrack Road, the 27B from Connolly train station and the 104 from Clontarf Conquer Hill to Vernon
Avenue, Harmstown, Edenmore, Donaghmede SC, Clarehall SC and Northside SC, then to Beaumont Hospital. By Taxi: Taxis can be called from the hospital’s main reception desk. Taxis can drop off and pick up close to the main entrance to the hospital.
Visitor Information
The standard visiting hours in place as of July 1, 2013 are: Monday – Sunday
(including bank holidays) 3pm to 4pm and 6pm to 8pm. Exceptions In exceptional circumstances the clinical nurse manager or their deputy may issue a visitor’s pass for visiting outside of these times. Special arrangements are in place in the General Intensive Therapy Unit, Richmond Intensive Care Unit, St. Raphael’s Children’s Ward, Emergency Department and St. Michael’s Ward.
Restrictions Further restrictions may
be imposed during an emergency or urgent situation, such as an infection outbreak. Admissions
In Patient Medical Admissions and Discharge The admissions and discharge process is an integral part of care and treatment. Our goal is to work with you and with your family and friends to understand your needs and to help you get settled as quickly as possible.
Admission Arrangements
Patients are admitted to Beaumont Hospital either by elective admission (pre-arranged) or emergency admission through our emergency or outpatients departments. If you are an elective patient, you must contact the hospital before you leave home to ensure all arrangements are in place and to confirm bed availability. A daily government levy is charged for the first 10 days of your admission in any 12-month period. A bill will be issued on your discharge. Medicalcard holders are exempt from this levy. All patients, public and private, will be requested to complete and sign the relevant form/s. This enables the hospital to directly bill the medical insurers, if applicable, on your behalf.

Charges & Billing
Beaumont Hospital
Private patients may request private or semi-private accommodation through the admissions or registration office, or by asking the ward manager. A private patient temporarily placed in a public bed will be transferred to the appropriate accommodation as soon as possible. Private accommodation cannot be guaranteed.
Upon arrival in a ward, a nurse will help to settle you in and a doctor will work with you to complete your medical history. You will also receive an information leaflet that contains information about your care in your specific ward. You should feel comfortable asking your doctor or nurse about your condition.
Admissions
Your Healthcare Team
During your stay, your core healthcare team may consist of: n The nurse on duty, who provides you with your nursing care. n The nursing manager, who is responsible for ward management and clinical management of care. n Allied health professionals – physiotherapists, occupational therapists, social workers and speech therapists, who provide you with their specialist service as necessary. n Your consultant, who is the doctor responsible for your treatment while you are in the hospital. Your consultant will supervise your treatment and determine when you are ready for discharge. n Your medical team, who treat you under the supervision of the consultant.
Diet
On admission, you will be asked about your dietary needs. Once in your ward, you will be offered a daily menu. Some patients, however, may be on a special diet. In general, breakfast is served at 8.00am, lunch at 12.30pm, and tea/ dinner from 4.30pm–5.00pm. For health, safety and hygiene reasons, please do not bring food into the hospital.
Hygiene
Please help us maintain a clean, healthy environment for patients and staff: n Please clean your hands with soap and water OR with the hand gels located throughout the hospital. It is especially important to clean your hands after you cough or sneeze, before meals, and after going to toilet. n For your own safety, please make sure your hands are clean n Do not place towels or clothing on radiators n Do not store belongings (including suitcases) on the floor n Remove toiletries from the bathroom after use n Do not store food in or on lockers unless it is in airtight containers

Laundry
As the hospital cannot provide a personal laundry service, please make arrangements with relatives or friends.
Phone Calls & Mobile Phone Usage
If someone needs to call you, they should call the ward directly. Please note, though, that calls to the wards are fielded at nursing stations and may call away a nurse from attending a patient. For public telephone information, please see the Services section of this handbook. The use of mobile phones can interfere with life support or diagnostic equipment; therefore mobile phones are only permitted in designated areas of the hospital, which are signposted.
Smoking Policy
In line with current legislation, smoking is prohibited in Beaumont Hospital. There is a gazebo outside the front-entrance for patients and visitors who wish to smoke. For more information, please refer to the smoke free information leaflet available on every ward.
Support
Our Health Promotion Department provides a Quit Smoking Support Service. Contact (01) 809 2941. The National Smokers’ Quitline (1850 201 203) provides telephone advice, support and follow-up for smokers who wish to quit.
Patient Representatives
Patient representatives are available to investigate complaints and to offer information and support. They will help to resolve difficulties that may arise in relation to hospital services. Phone: (01) 809 3234. Office hours: Monday–Friday: 8.00am–8.00pm Saturday & Sunday: 10.00am– 2.00pm.
Comments and Concerns
We hope that your stay is comfortable. Your feedback is important to us, be it praise, suggestions, or concerns. If you have a concern or complaint, please talk to a member of staff in the clinic, ward or department where the problem arose. Every effort will be made to deal with your concerns or complaint and put things right. If your concern cannot be resolved to your satisfaction, you will be connected with the patient representative’s office.
Your Right to Information
Personal information is contained on your personal health record. You have a right to access and review that information. Please call Routine Access at (01) 809 2873 for details. If for some reason your request cannot be granted, you will be notified and can contact the Freedom of Information Department at (01) 809 2866.
Your Right to Confidentiality
While you are in the hospital, your data will be accessed by a variety of medical, nursing, allied health, and administrative staff. All members of Beaumont Hospital are required by hospital policy to respect the confidentiality of your data. If you have any concerns in this regard, you may contact the patient representatives office at (01) 809 3234.
Discharge Arrangements
Your healthcare team will advise you at least 24 hours before you are ready to leave the hospital. Once you have been informed of your discharge please ensure that arrangements have been made for going home on the morning of discharge before 11.00am. Please understand that your bed may be required for a patient waiting in the Emergency Department. Any valuables being kept for safekeeping are returned to you. A medical certificate is issued if required. You will have prescriptions for any medication your doctors might have prescribed for you on discharge. Day Patient Admissions St. Finbar’s Day Ward
Referrals to St. Finbar’s Day Ward for treatment are completed by consultants following patient review in the Out-Patient Department (OPD).
Urgent Referrals
These patient’s will be referred to the day ward admissions office by a member of the medical or surgical team with instructions in regard to the necessary time-frame for admission. The patient will be added to the treatment waiting list with an ‘urgent priority’ flag. The admissions clerk will contact patients by telephone, usually one week prior to the proposed date for admission.
Routine Referrals
Following clinic, the completed treatment waiting list card is sent to the day ward admissions office. Patients are added to the treatment waiting list in chronological order with a ‘routine priority’ flag. Patients are then contacted with a proposed date for admission, usually one week ahead of this date. Occasionally patients may be offered dates at short notice in the event of cancellations.
Nurse Led Pre-Assessment Service
Patients being referred for daycase treatment under general anaesthetic will require medical preassessment before their admission date. These patients are referred to St. Finbar’s day ward directly from the out-patient clinic to make an appointment to attend with the preassessment nurse. Pre-assessment can occasionally be carried out on the same day. In particular, patients who have long distances to travel should highlight this and every effort will be made to minimise 48

the number of attendances at the hospital. Pre-assessment remains valid for three months. Patients who have not been treated within three months of having been pre-assessed will need to be further reviewed by the preassessment nurse. In most cases this review can be conducted over the telephone. Patients should at all times ensure that they notify the hospital of any change in telephone contact details as failure to do so may impact on their admission wait time.
General Guidelines for all Procedures
n Patients should report to the reception desk of St Finbar’s Day Ward on the ground floor. n Patients should attend 10 minutes before their allocated treatment slot to allow for registration and other preparations. n Patients under 16 years of age require their parents or guardians to sign an operation or anaesthetic consent form. n Please have a bath or shower on the evening prior to your admission. n Please bring dressing gown and slippers. n Patients are requested not to wear make-up or nail varnish. n Patients should leave all valuables such as jewellery and watches at home. Do not bring large sums of money with you. n Please bring all current medication with you on the day of admission.
n If you cannot attend on the day of your appointment, please notify the day ward admissions office as soon as possible so that the theatre space can be allocated to another patient. Failure to do so may result in a delay in offering an alternative treatment date. See contact numbers below.
Guidelines for Patients Undergoing Local Aneasthetic
n Morning admissions take place from 8.30am-10.00am. Afternoon admissions take place between 12.00pm and 12.30pm. n Patients should attend 10 minutes before their assigned treatment slot to allow for registration and other preparations. n Patients having a local anaesthetic may have their normal diet unless otherwise instructed. n Diabetic patients may take normal diet and medication. n Patients for removal of in-grown toenails must be escorted home by relatives or friends by car or taxi. n Patients having treatment under local anaesthetic may go home unescorted with the exception of elderly or very young patients. n Ask the doctor’s advice if you are on warfarin, aspirin or Plavix. n The contact number for all matters or enquiries relating to local anaesthetic procedures is (01) 809 2056. n Lithotripsy (ESWL) patients cannot drive after their procedure and will need to be escorted home.
Admissions
Guidelines for Patients having a Flexible Cystoscopy
A cystoscopy is an examination of the bladder in order to look at parts of the urinary system. The bladder stores urine which originates in the kidneys. When one needs to pass urine, the muscle in the wall of the bladder squeezes the urine into the water-pipe or urethra. When you have a cystoscopy, a miniature telescope is passed down the urethra so that the doctor can examine the inside of the bladder. This procedure will be carried out either in Minor Theatre or the Endoscopy Suite. Anaesthetic jelly will be squeezed up the tube coming from the bladder (urethra). This jelly numbs the urethra and lubricates it. The doctor will then clean the area with a mild disinfectant and the instrument will be passed up the urethra. How do I prepare for my Flexible Cystoscopy? No special
preparation is required. On the day of the investigation you can eat and drink as normal. After registration you will be asked to change into a hospital gown and provide a urine sample. The hospital gown is used because you are being treated in a sterile environment. Following the examination of your bladder in or the Endoscopy Suite, you will return to St. Finbar’s Day Ward where you will be offered a cup of tea. You may leave St. Finbar’s Day Ward immediately afterwards. The contact number for all matters or enquiries relating to a cystoscopy procedure is (01) 809 2056.
What are the After-Effects of a Flexible Cystoscopy? Most
patients have no problems after a flexible cystoscopy. A mild burning on passing urine usually gets better after a day or so. Drinking extra water will help. If a biopsy specimen has been taken, you may
see some blood in your urine. The most common problem after examination of the bladder is an infection in the urine. If you have a temperature, pain, persistent burning or bleeding, you should contact your GP.
Guidelines for Patients before a General Aneasthetic – Day Theatre
n Morning admissions are scheduled for 7.30am n Afternoon admissions are scheduled for 12.00 midday n Patients having a general anaesthetic should NOT eat or drink anything after midnight –this includes water – the night before the procedure. You should not chew gum on the morning of the procedure as this stimulates gastric juices n If you are in any doubt as to what type of anaesthetic you are having you should call the day ward (01) 809 2069 n If you are on the contraceptive pill, hormone replacement therapy, heart tablets, aspirin, warfarin, epilepsy or diabetes drugs, please ask your consultant or GP for advice before attending the day ward for treatment n After a general anaesthetic your co-ordination may be affected and logical thinking may be temporarily interrupted. It is therefore necessary, for safety reasons, that you be collected from the day ward. Time of discharge can be confirmed by ringing the day ward at (01) 809 2894 or (01) 809 2826 n You should remain with a responsible adult for 24 hours post-discharge from the day ward. Patients living alone must make alternative arrangements n You must arrange for transport to collect you from the day ward. Public transport is not suitable. If you use a taxi you must provide your own escort
such as a relative or friend n You should not drive, operate machinery, cook or consume alcoholic drinks for 24 hours following a general anaesthetic n Please bring slippers and dressing gown n You may wish to bring a book or a personal stereo with headphones with you as you may have to wait for your treatment. Registration and Payment
There are two semi-private beds available on St. Finbar’s Day Ward. Patients will be offered access to these beds, subject to availability. Patients must bring all relevant payment documentation with them on the day of admission, including evidence of health insurance or medical card. Failure to do so will result in patients being levied and invoiced with the day bed admission/treatment fees. Details of daily fees are as follows:
n Public patient with medical card: €0.00 n Public patient without medical card: €75.00 n Public patient with health insurance: €75.00 * n Private patient in semi-private bed: €407.00 ** n Patients will be required to provide electronic authorisation in regard to private health insurance, where applicable. * €75.00 fee will be collected direct from health insurer upon verification and authorisation of insurance. ** Private patients will also be liable for all professional and diagnostic fees arising.

Admissions
Elective surgical admissions Patients are added to the waiting list by a consultant or senior registrar. Patients must be fit, ready and able for surgery at the time of booking. They are only added to the waiting list once their application form/card is completed in full, dated and signed by a consultant or senior registrar. All waiting list forms or cards include the minimum standard of information such as patient details, and surgical procedures, medications, pre-op instructions. Once on the list, patients are assigned a clinical priority, either ‘routine’ or ‘urgent’. It is the responsibility of the clinician making the decision to admit, to define the patient as ‘urgent’ or “routine’. The decision as to when a patient is put forward is made by the consultant, based on clinical need.
Patients will be given a minimum of two weeks’ notice where possible. Patients are not scheduled more than six weeks in advance. Patients who decline a date with at least two weeks’ notice are advised that a second offer will be made but should that be declined they will be removed from the waiting list. In this case, both GP and patient will be advised in writing with a copy of the letter filed on the medical record.

Contacting the Patient
Usually patients are contacted by phone to schedule their admission, where no phone number is available or contactable, a written offer will be made. This gives the patient an opportunity to notify any potential issues pertaining to their admission, reducing the risk of cancellations or attendance failures. Primary Teaching Hospital Status is
Central to Beaumont’s New Strategy The importance of the relationship between Beaumont Hospital and its academic partners the Royal College of Surgeons in Ireland (RSCI) and Dublin City University is considerable, and collectively they provide training and professional development for clinicians and other staff throughout their careers according to Kate Costello, Head of Learning and Development in Beaumont Hospital.
Beaumont Hospital is the primary teaching hospital for the Royal College of Surgeons in Ireland (RCSI) and provides a major educational programme for undergraduate medical students and postgraduates in all sub-specialities including GP scheme placements. The hospital provides tutorials for NCHDs studying for the MRCPI part one and two exams. The tutorials are regarded as critical to the continuing development of NCHDs. Doctors who participate in the tutorials enjoy a high rate of success in the exams. A large Education & Research Centre run by the Royal College of Surgeons in Ireland on the Beaumont Hospital campus provides state-ofthe-art core molecular and cell biology facilities, with a large number of clinical and basic science investigators. In addition, the centre hosts a clinical research centre that facilitates high quality clinical research programmes. The current RCSI President, Mr. Paddy Broe, is a Consultant Surgeon in Beaumont Hospital and the current RCSI CEO, Professor Cathal Kelly, is a former Consultant Vascular Surgeon at Beaumont.
Nursing Training with DCU
Beaumont Hospital is also the primary teaching hospital for DCU nursing students. Currently, most of the clinical placements for the undergraduate nursing programme
are based in acute care. A national review of the BSc nursing programme in 2012 has made a recommendation to expand the curriculum to increase undergraduate primary care experience by increasing the number of community placements in future. Beaumont Hospital also has an extensive postgraduate nurse education programme, many of which are open to our community colleagues including GP practice nurses. We recently ran a dementia study day, which provided workshops where community PHN, primary care teams and staff from Beaumont Hospital worked as part of an interdisciplinary team to develop care pathways for a patient’s journey through the system. A positive outcome of the day was the strengthening of the relationships between hospital, primary care and the surrounding nursing homes. This action learning will be repeated in the autumn with another study day on diabetes, where primary care nurses and hospital nurses will meet and develop a better understanding of each other’s role in order to provide a seamless service to patients.

GP Liaison Service
Beaumont hosts an annual GP interactive study day where members of the hospital consultant staff and multidisciplinary colleagues present on the most up-to-date developments in their specialty and the treatments available to patients in Beaumont Hospital. This is followed by a focused workshop where consultants and GPs discuss various clinical developments and research in small groups. The study day provides a two-way opportunity to highlight and address issues of mutual interest relating to patient care and service delivery. The study day in February 2014 was attended by over 150 GPs. A GP liaison service operating out of the Emergency Department acts as a conduit between the hospital and the GP in an effort to streamline the patient journey, following up on patient attendances, results of investigations and further care arrangements.
Integrated Teaching Learning and Research Considerable work has gone in to developing an academic health concept integrating practice, learning and research across academic partners and Connolly and Beaumont hospitals. This will allow clinicians, researchers and academics to work more closely together to deliver clinical and academic excellence with a strong emphasis on innovation and translational research. The background work has acted as a foundation for the evolving hospital groups.

A significant enabling initiative in recent years is Beaumont’s MSc in organisational change and leadership development programme, which was developed in conjunction with the RCSI’s Institute of Leadership and Healthcare Management and DCU’s Business School, targeting doctors, nurses, health and social care professionals and administration staff across the acute and primary care setting. The two year part-time programme is aimed at enhancing leadership capacity and skills and embedding new learning capabilities within an action learning environment, directed at addressing health system priorities e.g. improving patient flow, introducing protocols and care pathways and shifting care to the next appropriate part of delivery. The initial programme (2010-2012) was a resounding success yielding several high-impact ROI outputs, e.g. realisable delivery improvement projects underpinned by hospital and national healthcare priorities. Participants on the current programme (2012-2014) are drawn from the hospital and the community setting and are working together on solving joint hospital-community patient care priorities as their change projects. Studying and working together on change and quality improvement projects creates an ideal setting for exploring how to progress reducing the over dependencies on hospitalbased care and enabling a shift towards prevention and promoting health and well-being throughout the population. This is a fundamental element of the Government Health Strategy “Healthy Ireland”.

Beaumont Hospital Foundation Working to inspire donations to Beaumont Hospital which are used to bring about improvements in the care which the hospital and its staff provide to patients and their families.
For a hospital with the scale and size of Beaumont, the need for new and improved equipment, renovations, services and educational advancement is enormous. The need is so great that it’s often just not possible to accommodate these requirements within the hospital’s operating budget. The role of the Foundation is to inspire donations to the hospital which are used to finance, often essential, additions and improvements to the hospital and its environment. We rely on the generosity of the public –so many of whom have had direct experience of a visit to the hospital, either personally, or to visit a loved one – to help us generate income. We then, plough their generous support back into a range of projects, all of which will result in this medical facility becoming a better place for over 40,000 patients who attend here each year. Our latest major project, launched recently by An Taoiseach, Enda Kelly T.D, was the opening of a new €2.1 million Day Hospital for Care of the Elderly, located at St Joseph’s Hospital in Raheny which is part of the Beaumont Campus. The hospital will assist in detecting early-
stage health issues in older patients, preventing them from needing hospitalisation at a later stage. The services of the new hospital will also allow patients who have been hospitalised to be discharged to their own home earlier, from where they can complete their care programme by accessing the Day Hospital’s services. Funding was provided for the build through a single, major donation of €1.5m to the Foundation matched with €600,000 of individual donations made by thousands of our generous supporters. In addition to our major projects, each year we commit approx. €100K to smaller projects across the hospital - we buy monitors that check vital signs, heart rhythms and detect changes in a patient’s condition. We buy state-of-the art equipment such
as robotic arms to aid recovering stroke patients and cryoscopes for freezing tumours and even simple but vital equipment like wheelchairs for the Portering department. All our projects are listed on our website at www.bhf.ie. All of this is only possible as a result of the huge support we receive from our donors who engage in organising their own events and taking part in marathons and other sponsored events throughout the year. Speaking about support for the Foundation and the work of the hospital, Mr Paddy Delaney, Managing Director, says, “Beaumont Hospital is a great hospital and we in the Foundation are working to raise the funds to make it even better. We do this with the backing and support of thousands of people countrywide whose lives have been touched by Beaumont in one way or another. Their contribution, however big or small, is always so appreciated and we are happy to direct it towards any area of the hospital that they choose”.
