Leading the Field CEO of Mount Carmel, Mr John O’Brien, explains what the ethos of the hospital is and how it will continue to develop its services, staff and technology to deliver the best in private medical care.

M

ount Carmel Hospital (MCH) is a well established and respected, independent private hospital with focused healthcare for maternal care, general adult health and paediatric care. An emphasis on women’s health is one of our core strengths. We have 94 inpatient beds, 29 daycare beds, a four bedded High Dependency Unit and a five bedded Delivery/ Assessment Suite. Mount Carmel Hospital is located ten minutes from the M50 and 15 minutes from the city centre. The hospital is widely known for maternity care and the only private maternity hospital in the country. However this represents only 30 per cent of the hospitals business. In order to deliver excellence in patient care, MCH has extremely low infection rates, high nurse-to-patient staff ratios, no waiting lists and superb clinical outcomes. The hospital holds the prestigious Joint Commission International (JCI) accreditation standard. This internationally recognised standard reflects Mount Carmel’s commitment to providing the highest quality patient care.

A re-development programme commenced in 2011 with several departments upgraded to date including: • Refurbishments in theatres • A third external theatre was developed for side room procedures • A new patient services centre in the main reception • A new dedicated Women’s Health Unit with ten consultation rooms and a Pregnancy Ultrasound Suite • Outpatient facilities expanded for consultant assessment/diagnosis • Upgraded car parking facilities and increased capacity for parking • New lifts As MCH is the only private maternity hospital in Ireland, we have an international reputation for the delivery of high quality services including maternity and paediatric care as demonstrated through our JCI accreditation. This reputation is vitally important in order to further recruit high quality staff as demonstrated through the recruitment of two additional specialist Consultants, Prof. Walter Prendiville and Mr. Peter Lenehan. Mount Carmel Hospital has also developed some

clinical links with national teaching hospitals to further enhance the delivery of services. This reputation has led to the development of some new exciting international links with the UK, USA, Middle East and Spain. The other major part of our inpatient business is the dedicated Orthopaedic Unit where we conduct 500 joint replacements (approx) each year. We have a multi disciplinary orthopaedic service including physiotherapy rehabilitation. Other areas of significant activity include specialist clinics, some tailored treatment plans such as direct access endoscopy and direct access colposcopy, women’s health, paediatric surgery services and general surgery. Prof Walter Prendiville, the leading colposcopy specialist in the country, is

based at Mount Carmel and he provides some of the best clinical outcomes in Europe which are instrumental in detecting pre cancer cells. In addition, Mr Peter Lenehan, Ms Mona Joyce and Ms Andrea Nugent provide a colposcopy service as part of the women’s health team. At Mount Carmel Hospital, we have also established ourselves as leaders in the field of endoscopy care, with dedicated facilities guaranteeing a smooth, swift process. Quality and Accreditation

Setting the Standard Head of Hospital Quality and Risk, Pearl Crosby, explains her role at the hospital and why Mount Carmel is internationally recognised as consistently achieving high standards.

“I

’ve worked here at the hospital for eight years, working at first in the Pathology Department as the Pathology Quality Manager and Head of Biochemistry, before taking on my current role as Head of Quality and Risk, which is to ensure that the highest standards are met and maintained here at Mount Carmel Hospital,” she explains. “In the Quality and Risk Department, we manage and monitor all quality and risk issues and incidents reported by staff in the hospital and co-ordinate, in conjunction with a PALS (Patient Advice Liaison Service) officer, Ms Pamela O’Neill, which addresses any complaints or concerns which patients may have,” Pearl adds. At Mount Carmel, feedback is solicited from all patients who avail of services or treatments at the hospital and Pearl indicates that: ‘We are returned a high number of patient survey forms on an ongoing basis, which we then enter into our Quality Database. This information is then acted upon to generate a report for follow up in the various departments.’ The

patient survey feedback, in conjunction with incident reports and standards, helps to co-ordinate and drive departmental audits in terms of quality practices. The hospital is accredited under the most exacting standards by the Joint Commission International (JCI) and is also a member of the European Framework for Quality Management (EFQM), which has its own excellence awards and allows Mount Carmel to measure its standards against those of its European peers. JCI is an independent international organisation focused on improving the safety of patient care. Achieving and maintaining accreditation indicates a hospital’s commitment to providing the highest standards and best practices for patient care. Accreditation promotes patient confidence by assisting an organisation to ensure a safe and effective environment for the delivery of health care. “We are also involved in setting up the quality systems here in line with the National Draft HIQA standards, and maintaining and continuously improving the systems we have in place. It’s a process of

continuous improvement and maintenance of standards at such a level that we are, in effect, ‘audit ready’ at all times.” Alongside her colleagues Pamela O’Neill and Roisín Featherstone, Pearl uses the Q-Pulse IT based quality management system, which makes it easier and more streamlined to manage the oversight of quality within the hospital. “Quality and the maintenance of standards are vital for both Patient and Staff Safety here at Mount Carmel and that’s what we work hard to maintain,” adds Pearl.

Laboratory Services At Mount Carmel

The range of laboratory services available at Mount Carmel continues to keep pace with the broad range of services which the hospital provides. Pathology Services Manager, Nuala Merrin, explains.

Nuala Merrin heads a team of 14 scientific and admin staff at the lab. “We conduct a broad range of testing here, with our histology, endocrinology and blood transfusion capabilities particularly in demand. We also provide blood testing for the GP community.” “My role here is to manage workflow at the laboratory and to ensure that everything is done to the highest possible standards. The laboratory itself is partially accredited by the Irish National Accreditation Board

(INAB) and the hospital is Joint Commission International (JCI) accredited. INAB is the gold standard when it comes to laboratory accreditation and it provides evidence of a commitment to quality and standards at all levels.” In the laboratory at Mount Carmel, as with all other departments in the hospital, the focus is always on the patient. The guiding principal for any modern laboratory is to add a valued service to the hospital and improve the services available to the patients. Patient satisfaction is the ultimate measure of performance. “We work with a very good team here at Mount Carmel,” adds Nuala. “I continuously analyse the workflow and test requesting patterns of the clinicians both internal and external, ensuring that policies and practices are in place and also identifying

what improvements, modifications or new equipment may be necessary to keep abreast of current best practice.” The laboratory has recently invested in an automated blood grouper to assist in driving efficiencies and service and, more importantly, to guarantee a safe blood transfusion service. “We have a strong working relationship with other departments within the hospital and I think that is one of the strongest parts of the dynamic of Mount Carmel hospital,” adds Nuala. “We have increased our histopathology and endocrinology services in line with the caseload at the hospital and look forward to continuing to develop laboratory services in line with the development of procedures at Mount Carmel Hospital, to assist our medical staff in delivering the best possible care to our patients.”

at Mount Carmel

The highest standards of infection control are strictly observed in Mount Carmel to ensure patient and staff safety, explains Anne Marie Ward, Infection Control Officer.

A comprehensive infection control programme is in place at the hospital, with protocols reviewed on a regular basis and adapted to adhere to national and international best practice guidelines. “In order to maintain and improve our standards in reducing the transmission of infection, we focus on hand hygiene practice and environmental hygiene,” says Anne Marie Ward. Monthly audits enable the team to ascertain the overall cleanliness in all areas together with hand hygiene audits and training and education ensuring strict adherence to proper hand washing technique. “To date, our infection rates are below average

for a hospital our size with an overall infection rate of 1.15 per cent in 2012,” adds Anne Marie. “Our standards are very robust, with consistent training and updating for staff. We adhere to the standards set by JCI and also HIQA, across the board. We also have a full Clinical Sterile Supply Department (CSSD), with full traceability of all items used.” Together with environmental and hand hygiene awareness, robust surveillance is in place in order to monitor surgical site infection rates and these include, but are not limited to, orthopaedic and obstetric surgery. In 2012 the hospital saw belowbenchmark results for their hip prosthesis surgery and a marked improvement in obstetric post operative surgical site infection rates. Ongoing education both at department level and in the classroom ensures that all staff are updated on current trends within the infection control arena. The successful team guaranteed the hospital was not exposed to a ‘winter vomiting’ outbreak due to the measures they implemented during the norovirus season.

Resources are allocated to maintain the highest of standards in the infection control area. The Hospital Infection Control Committee includes members of the multidisciplinary team and is very active in addressing any issues that should arise along with advising, supporting and providing expertise to the Infection Control Team. “My role is very varied and constantly changing with developments at Mount Carmel. Hygiene is everyone’s responsibility and we are committed to continuing to ensure patient and staff safety by improving our practices to maintain our very high standards.”

Pharmacy Services at Mount Carmel Claire McSherry, Pharmacy Services Manager, explains the role of her department at the hospital and how the pharmacy is involved in ensuring optimum care for patients.

To oversee and provide guidance for the use of pharmaceutical products in Mount Carmel, the hospital has a Drugs and Therapeutics Committee and there is a rigid and documented process for

the request of any new medications, which must first be approved by the Committee. “There are five full-time staff employed in Pharmacy Aervices at Mount Carmel and at all times of day or night there is at least one on-duty with one other on call. Like all departments at the hospital, the pharmacy is constantly busy, dealing with the requirements of each new patient at the hospital.” In addition to catering for the pharmaceutical needs of the patient population, the pharmacists at Mount Carmel are also very much involved in the risk management processes within the hospital. “In the rare case of a problem, we conduct a full adverse incident review and we are also involved in the clinical guidance process in relation

to any patient, where relevant.” The daily duty of the onduty pharmacists would see them monitoring the medication chart for each inpatient, each day. “We are also heavily involved in education and would lecture on relevant procedures and any changes to processes within the hospital relating to medication,” adds Claire. The pharmacy, as an integral part of the hospital, is also very much involved in the continuing achievement of Mount Carmel Hospital in retaining its JCI accreditation. “With the standards required here, the pharmaceutical department is very much part of the JCI process, with particular responsibilities for driving policies and procedures in relation to patient medication management,” explains Claire.

Pastoral Care

A Holistic Approach Emotional support for dealing with difficult situations is another vital aspect of the all-round approach to patient care at Mount Carmel, explains Health Care Chaplain, Helen Miley. “What we do is try to create a calm environment for them, to make them understand that there is no time limit or hurry for dealing with difficult situations. We help guide them through that to a place where they are better informed and able to make difficult decisions to enable them to move forward,” explains Helen.

astoral care services at Mount Carmel Hospital take a holistic approach to each individual that they work with and attempt to ensure that the emotional, as well as the practical, needs of patients are met. Pastoral care support is nonjudgmental and welcoming of all faith backgrounds. It is available to all patients in Mount Carmel. The team offers their time to service users through emotional support, bereavement counselling and reflection services. “My role here is Health Care Chaplain,” explains

Helen Miley, “The majority of our cases would involve support to expectant mothers with complications and support to mothers who have suffered a miscarriage or indeed, stillbirth.” Pastoral care services at the hospital offer a professional counselling service through one-to-one sessions, which helps those who are suffering from the trauma of bereavement or the illness of a child and helps them to create a space in which they can deal with this difficult information and begin a process of understanding, and eventually, healing.

“Emotional support needs to be present throughout the entire process, from the difficult days after a bereavement or bad news, right through to the grieving process and the learning-to-cope phase. We tailor a service that is suitable to each case and all cases are completely unique. Although the pastoral care services at the hospital are rooted in the Christian tradition, there is absolutely no religious dogma of any kind to the services which we provide. We deal with maternity patients, their families and also those who avail of the many other services here at Mount Carmel, such as pre-operative and post-operative patients. We are here with them all the way and can also refer them to an external counselling services for further emotional support should that be necessary,” adds Helen.

Key to ensuring the smooth operating of the surgical facilities at Mount Carmel Hospital is the relationship

between the consultant, nursing and administrative staff. “My own work is evolving on a daily basis, as no two days are the same in terms of the cases which we are seeing in surgery,” adds Breda. “What is of great benefit to the hospital is the level of experience we have in terms of our staff and how they work together. You simply cannot buy the expertise which we have here. We have nursing staff in particular who have worked here for many years and that’s a benefit as they build up a strong working relationship with the consultants.” The surgical facilities themselves have all been significantly modernised at the hospital in the recent past, with new anaesthetic machines and complete refurbishments of the

theatres themselves. “We are consistently trying to take the service we provide to the next level, in terms of facilities, technology and procedures. Technology dictates the pace of progress in patient care and the hospital has invested heavily to ensure that we have the best of what is currently available,” she explains. Breda adds that the ease of access to expert staff, streamlining of treatments and the comfort and security of the hospital makes it an excellent choice for patients. “All patients are monitored along every step of their journey at Mount Carmel. We have excellent systems in place to document all aspects of our patients experiences at the hospital and we hope to be able to further enhance and improve our services in the future,” adds Breda.

reda Fogarty is the Theatre Manager at Mount Carmel Hospital, where there are seven general theatres, two with laminar clean airflow facilities and one minor surgical procedures suite. “My role here is to manage the theatres and their operational capacity and address all clinical management and related issues. The most important role of any department within the hospital is to ensure optimum care for our patients during their time at the hospital. We have a tradition of providing an excellent level of patient care in a safe environment at every level,” she explains.

aving spent 15 years at the hospital, Roisín Phelan is delighted with the new ultrasound facilities in the dedicated Women’s Health Unit at Mount Carmel. “It is a wonderful, bright, modern, welcoming facility. We have two new ultrasound bays here and we are developing a third in the near future. That means a complete suite of new equipment is now in place, amongst the finest available today. My role here, alongside my colleagues, is to provide obstetric scans for our patients at Mount Carmel, throughout the three trimesters of their pregnancy; covering the early scan, the anatomical scan and the foetal wellbeing scan.”

regarding each case and that can only be of benefit to our patients.” In addition to obstetric ultrasounds, Roisín and her colleagues also provide fertility scanning as part of Mount Carmel’s fertility treatment services. While the clinic is busiest during the hours of the working day, there is always ultrasound cover in place. “We always have personnel available to deal with any situation which may require an ultrasound,” she says. Each scan takes in the region of 20 minutes and for the staff at the ultrasound department, the personal touch is very important. “The ultrasound is an immensely important part of the pregnancy for

the mother, and indeed the father, it is part of the process of bonding with the baby and we ensure that this is done in a caring, sensitive way which clearly explains the scan to the parent or parents. We factor enough time into each scan to talk to the patients and to get to know them, it is all part of what attending a hospital like Mount Carmel is about,” says Roisín.

At the pre-assessment clinic, a comprehensive medical assessment takes place to ensure patients are fit for surgery thereby minimising risk, ensuring a speedy recovery and ensuring the patient is fully informed and prepared for the surgery.

Orthopaedic Surgical Equipment The dedicated orthopaedic operating theatre at Mount Carmel Hospital uses laminar flow ventilation, a proven method of minimising post operative infections which can impact on clinical outcomes. The

Heart of the Matter Consultant Cardiologist, Dr Vincent Maher, explains how Mount Carmel is developing and expanding a comprehensive facility for the diagnosis and treatment of many cardiac conditions.

“I

n conjunction with other consultant colleagues, we are expanding Cardiology services at Mount Carmel. Our involvement includes management of patients with varying cardiac disorders, such as heart in-efficiencies, refractory hypertension, arrhythmias, premature vascular events and multiple comorbidities in previously stable cardiac patients. Our non-invasive diagnostic service has expanded to provide trans-thoracic echocardiography, stress testing, ambulatory blood pressure and holter monitoring, event monitoring in addition to other procedures.

Other non-invasive vascular measurements such as Carotid Duplex sonography, abdominal aortic assessment by ultrasonography, peripheral vascular assessment and CT pulmonary arteriography compliment the cardiological assessment to help risk stratify our patients. For practical reasons, invasive cardiology procedures are not carried out at present at Mount Carmel. However, the current level of service will help provide a strong general cardiology service at the hospital,” says Dr Maher. “It is hoped to expand the outpatient service to provide better access for GP referrals needing nonemergent but prioritised cardiological investigation. Ideally, we hope to provide a comprehensive cardiac risk assessment for at risk patients in conjunction with our laboratory and radiological services. Our aim is to expand cardiac services at the hospital to include management of high-risk cardiac patients and those with other vascular disease,” added Dr Maher.

Future developments include the establishment of a weight optimisation service with a clinical team including physiotherapy, dietetics and nutrition, life coaching, cardiology and endocrinology. “The aim will be to tackle cases of moderate, manageable obesity within a multidisciplinary environment,” explains Dr Maher. “One of the strongest aspects of Mount Carmel is the very high-level of staff expertise at all levels in the hospital. I believe that we have some of the best nursing staff in the country working at Mount Carmel. They are complemented by wellinformed, enthusiastic, energetic administrative and managerial staff throughout the hospital,” he added. With an established and expanding range of cardiology procedures in place, cardiac services at Mount Carmel continues to evolve to provide a comprehensive, noninvasive diagnostic service for the management of most patients with cardiac disease.

Special Care Baby Unit

The facilities The Paediatric Department at Mount Carmel Hospital consists of three independent, complimentary units; a Nursery, a Special Care Baby Unit (SCBU) and paediatric outpatient rooms. The SCBU is fully equipped to deal with most eventualities upon birth and has the facilities to care for premature babies who need help breathing or who need to be fed via tube. The SCBU has recently been refurnished and its capacity increased to nine beds, including two high dependency beds which can cater to new born babies needing ventilator support. The new isolation room in the SCBU is a welcome additional service. It provides a nursing facility for infants up to six weeks of age who need to be readmitted to the hospital for jaundice or feeding problems.

Emergency care There is a myth that Mount Carmel is unable to deal with emergency situations, however this is far from true. Our Special Care Baby Unit ensures that we are fully equipped to deal with babies born as premature as pre 30 weeks. We also have an adult High Dependency Unit should complications arise with the mum during or after birth.

Recent developments We have recently created a clinical governance system which has established approved treatment pathways. This means that everyone on the team,

doctors and nurses alike, will be on the same page when choosing a treatment method. This creates uniformity in approach and ensures that the best practice is always adhered to.

The team There is a slightly different structure in the Mount Carmel Paediatric Department than most other hospitals. There are no SHOs or registrars on the team which means a consultant will be respoinsible for the baby from the very beginning. There is a consultant paediatrician on attendance 24 hours a day, making the neonatal service consultant led and delivered at all times. This is extremely beneficial to high risk pregnancy patients. We also have experienced neonatal nurses on staff who are constantly upskilling and evolving with the field. We have an excellent working relationship with the Coombe Hospital and our neonatal nurses regularly attend training there to ensure they keep their qualifications up to the highest level. This year they completed a training programme in operating the neonatal Continuous Positive Airway Pressure (CPAP) driver which is needed by premature babies whose lungs have not fully developed. The CPAP prevents the lungs from collapsing. The next step in development will be intensive care training which they will undergo next year.

Personal approach to care At Mount Carmel, we provide an excellent professional service to our patients and we believe that building good personal relationships is just as important. On a human level we are there to offer support and advice to all of our new parents. We aim to send our mums and babies home well-rested and well-cared for and we have the facilities to ensure this is possible. If the mum needs recovery time after a Cesarean section, then she can spend as much time as she needs in the post-op recovery room. If she is in a position to care for her baby then the baby will stay with her in her room. However if she needs rest, then the baby will be cared for in the nursery. Premature babies are slow to establish feeding and must be tube-fed for some time until they are more developed. During this time if the mother has been discharged, she is free to spend as much time as possible with her baby and the breastfeeding co-ordinator will help the mum to start feeding as soon as the baby is ready. If the baby needs to spend some time in the SCBU, parents have free access to visit as frequently as they wish. We have a double check system whereby the baby is examined by the consultant paediatrician once they are born and examined again before they are discharged.

Nutrition & Dietetics at Mount Carmel T

he Dietetics and Nutrition department at the hospital is extremely busy, utilising four dieticians to cater for the requirements of the patient population. “We perform a variety of services here,” says Ann. “We deal with the maternity cases and those admitted to the hospital for other procedures. In terms of maternity cases, we examine the nutritional needs for expectant mothers and help deal with issues such as gestational diabetes. We also perform a glucose tolerance test at the 26 week stage of the pregnancy. For maternity cases, it’s vital that a healthy, balanced and nourishing diet is in place and we consult with expectant mothers on an individual basis.” “Our role extends to those undergoing other procedures at the hospital

and what their particular requirements may be,” adds Niamh Kelly. “In addition, we would deal with patients who may be suffering from obesity and also patients who may have malnutrition; there is a very broad spectrum of cases,” she says, adding that patients today are a lot more informed about various diets and methods. One of the diets which they can prescribe at Mount Carmel is the low-FODMAP diet, a diet low in certain carbohydrates that have been identified as causing gastrointestinal problems. “While the diet is not for everyone, it is proving to be very effective with an 85 per cent upwards success rate to date,” says Niamh. Ann says that the outpatients department would account for up to 50 per cent of the cases they deal with. “Patients can also

self-refer to the dietetics and nutrition clinics we provide, increasing the amount of patients we can see,” says Ann. “For a small department, we are very proud of the services which we provide. We also deal with the ethnic and religious dietary concerns of our increasingly diverse patient population; catering for Kosher, Chinese, Halal, West Indian and eastern European dietary requirements, to name but a few. We are very sensitive and culturally aware in this area,” she adds. They provide an educational resource too, providing information to patients and also liaising with transition year students to provide them with an introduction to this area. “We also work with the Hospital’s Health Promotion Committee to promote awareness in areas such as diet, blood pressure and cholesterol, for example,” she adds.

Medical Inpatients Patients are referred to us with a wide variety of medical conditions such as respiratory, neurological, musculoskeletal and general balance and mobility issues.We see these people, assess them and prescribe a treatment programme to improve their physical health. Where necessary, these patients can continue to be seen in our outpatient physiotherapy department or referred to our colleagues in the community.

Outpatients: “At the hospital, we provide a general outpatient physiotherapy service, specialising in a broad range of musculoskeletal issues. Patients who have attended the hospital can continue their rehabilitative treatment. Classes are provided for both hip and knee replacement placement patients, where they attend on a weekly basis to progress their rehabilitation and are monitored in small group sessions. Unique to this department is the Phototherapy TLO1 unit, in which narrow band UVB treatment is carried out for the psoriasis. Patients attend three times per week for a period of time. Dr Geraldine Morrow is

Biomechanical assessments are carried out utilising our Gait scanning equipment in the prescription of orthotics. “Unique to our service is a dedicated women’s health and continence outpatient service. Patients who are experiencing problems both during and after their pregnancy are treated on an individual basis. Common conditions include pelvic girdle pain, carpal tunnel syndrome, postural fatigue and exercise advice. at Mount Carmel Hospital Ophthalmology is a constantly advancing speciality, with new technologies and methodologies being continuously developed and implemented. At Mount Carmel Hospital, the ophthalmic team strive to bring these latest developments safely to their patients to achieve the best clinical outcomes, writes Professor Lorraine Cassidy, Consultant Ophthalmic Surgeon.

There are currently 11 Ophthalmic surgeons working at Mount Carmel Hospital. These surgeons cover a large range of sub-specialities, including oculoplastics, cataract surgery, strabismus (squint) surgery, glaucoma surgery, paediatric surgery, including paediatric strabismus and oculoplastic surgery, medical retina, vitro-retinal surgery and refractive surgery. Through the consultant ophthalmic surgeons, there are links with the Royal Victoria Eye and Ear Hospital, which is the busiest and largest tertiary referral eye hospital in Ireland, the Adelaide and Meath National Childrens’ Hospital, Tallaght, St James’s University Hospital and Our Lady’s Hospital for Sick Children. These are all teaching hospitals with links to Trinity College Dublin, the Royal College of Surgeons and University College Dublin. There are also links with the Research and Learning Development Centre in the Royal Victoria Eye and

Ear Hospital, where most of our ophthalmic research is carried out. Ophthalmic consultant clinics take place in a variety of hospitals and clinics around Dublin city.

The theatre Mount Carmel Hospital Eye Theatre has all the latest technology and equipment to deliver top-class eye surgery. Most importantly, we have a dedicated nursing team, which includes eyetrained nursing staff. This is essential as eye surgery is highly specialised and requires highly-trained nursing staff to deliver the highest standard of care. It takes one year of extra specialised training after general nursing to become and ophthalmic trained nurse. These nurses are essential to our work.

Links with other specialities Many different systemic conditions, such as heart disease, rheumatoid arthritis, neurological conditions and diabetes, to name but a few, can affect the eye. All consultants are available to provide ophthalmic consultations, examinations and treatments to patients

attending Mount Carmel with medical conditions. In turn, our medical colleagues are also available to attend ophthalmic patients whenever necessary.

Maternity Unit and Eye Care Mount Carmel Hospital has an extremely busy maternity unit. Mothers and babies can develop eye problems associated with pregnancy and premature birth. Babies may also be born with eye problems and need immediate diagnosis and care. Mount Carmel Hospital has two paediatric trained consultant ophthalmologists, who are available to see babies with eye problems and provide treatment if required. Changing Face of Gynaecological Care Professor Walter Prendiville, Consultant Gynaecologist at Mount Carmel, explains the services available at the hospital and how the nature of gynaecological procedures is changing. Laparoscopic hysterectomy is associated with huge advantages, i.e. shorter recovery, less pain, quicker return to work and less expense to the patient.

G

ynaecology, like many other specialities, is moving from the traditional model of consultation and subsequent inpatient procedure to one of consultation and immediate office or outpatient procedures. Traditionally a woman with dysfunctional uterine bleeding would be managed by way of consultation, referral for an ultrasound scan and perhaps admission for hysteroscopy D&C followed by a further consultation and advice about management, followed by a Mirena insertion or endometrial ablation. Today the same patient may be managed by way of consultation, trans-vaginal ultrasonography and office hysteroscopy/endometrial curette and Mirena insertion all at the same visit. Outpatient hysteroscopy is

now firmly established as the investigation of choice in women with dysfunctional uterine bleeding of suspected endometrial origin. With current 3mm or 4mm hysteroscopes, passing a hysteroscope in the office environment without the need for either speculum or tenaculum forceps on the cervix really is a very comfortable procedure and is perceived, by many women, as being less uncomfortable than a speculum examination and a cervical smear. Inpatient treatment is also shortening. Hysterectomy by way of laparotomy or even vaginally was associated with a five-day, or more, inpatient stay. Today laparoscopic or laparoscopic assisted vaginal hysterectomy is usually associated with a 48 or 72 hour inpatient stay.

Ireland has eventually established a cervical screening programme, which is hugely welcome. For reasons that are difficult to understand, the screening service for the national programme was based in the US, which has produced some difficulty for colposcopists and their patients, but slowly over time, the cytology services are moving back to Ireland. This will allow much closer collaboration between cytologists, pathologists and colposcopists as well as a hopefully improved abnormality reporting rate. A colposcopy service has been available in Mount Carmel for some time. ere at Mount Carmel we provide a range of biliary, gastrointestinal and colorectal procedures for

our patients,” says Mr Eguare. “We are now able to provide an expanding range of minimally invasive procedures and endoscopic investigations,” he adds. These include advanced laparoscopic procedures such as laparoscopic cholecystectomy, diagnostic laparoscopy and a full range of hernia repairs and ano-rectal procedures such as haemorrhoidectomy and haemorrhoidal dearterialisation (THD). ndoscopy has evolved into a common procedure for patients and Mount Carmel’s Direct Access Endoscopy service reflects this. Mount Carmel offers the option of Direct Access Endoscopy to referring GPs to expedite patient care. Dr Conor O’Brien explains that waiting time

is minimal: “You could make an appointment and the procedure could be carried out within 24 or 48 hours. We can get an urgent result in 24 or 48 hours and a non-urgent result in seven days. So it is a very rapid service.” This service means easy access to patients, meaning more efficient results for both the

patient and their GP. There are two dedicated suites for endoscopy at Mount Carmel which allows a greater number of patients to be seen every day. In order to be seen, patients must have a referral from their GP. At Mount Carmel they specify which consultant they would like

to see or be put on the next available list. Patients come in to the Endoscopy Department as day-patients after being referred by their GP. “Essentially the GP decides they need the procedure done and refers them directly so the patient doesn’t need to pay to see a consultant initially,” adds Dr O’Brien. The main services offered to patients in the Endoscopy Department are gastroscopy, colonoscopy and sigmoidoscopy. They also do other procedures such as removal of polyps and haemorrhoids. The

hospital also offers full and comprehensive follow-on treatment to patients as required with direct access to specific expertise in the fields of surgery and radiology, including CT colonography. Dr O’Brien mentions that there has been an increase of people availing of the endoscopy service in Mount Carmel recently. He believes this is due to increased awareness about colorectal cancer. “There is awareness about the dangers of colorectal cancer and people are becoming more aware of their symptoms, so the numbers have increased dramatically over the last

Endocrinology at Mount Carmel The Endocrinology Department at Mount Carmel Hospital is dedicated to quality treatment for its patients. Dr Kevin Moore discusses. The Team The endocrinology team at Mount Carmel Hospital consists of Consultant Endocrinologist, Kevin Moore, two dieticians and a diabetes liaison nurse. Together they provide a full range of outpatient endocrine services to their patients. Endocrinology is the study of glands and the hormonal control of the body, the regulation of which can be upset by such conditions as diabetes and pituitary malfunction. At Mount Carmel Hospital; the main emphasis is looking after pregnancyrelated endocrine problems such as gestational diabetes.

Gestational Diabetes Gestational diabetes occurs in women who exhibit high blood glucose levels during pregnancy. Hormones produced by the placenta during pregnancy make the body less sensitive to insulin and as a result the hormones climb. This is the most common pregnancyrelated endocrine problem that we treat here at Mount Carmel. We have approximately 100 patients presenting with gestational diabetes every year. Once

they have been diagnosed, they meet with our diabetes liaison nurse who educates them on how to intensively monitor their blood sugar levels. They also meet with our dietician who takes them through the dietary implications of their condition. As many as half of our gestational diabetes patients can control their condition with dietary management. The other fifty per cent require medication as well as dietary management. We provide a very comprehensive service to our patients and are in touch with them two to three times per week for updates on their blood sugar levels and see them frequently during the third trimester of their pregnancy. We work closely with the obstetrician and monitor the baby on a regular basis to ensure that their sugars are controlled and that the baby is growing appropriately. The majority of women’s blood glucose levels go back to normal as soon as they deliver. Only a small number go on to have diabetes. We carry out a blood glucose check six weeks after delivery to see if the blood glucose

levels have normalised. We would advise all women who have had gestational diabetes to have a followup screening blood test for diabetes every one to two years as they carry a higher risk of long term development of Type II diabetes. Women who suffer from gestational diabetes during pregnancy carry a very high risk of diabetes in future pregnancies. We recommend they get in touch with the diabetes services as soon as they get pregnant in order for it to be appropriately monitored and managed. This service has been in place here at Mount Carmel Hospital for the past 18 months.

The Service We provide a full range of outpatient adult endocrine services. We treat pituitary problems and adrenal problems, right through to reproductive hormone problems like testosterone and oestrogen abnormalities. We also treat a large number of patients suffering with thyroid disease in pregnancy. These cases are predominantly treatable with medications but we

do work with our surgical colleagues to treat pituitary or thyroid abnormalities.

Advances in care In terms of diabetes, there has really been an explosion of new treatments available with the onset of new drugs such as DPP4 inhibitors and the GLP1 agonists. These are a very welcome addition to the medication options for patients with Type II diabetes. These classes of medications are showing very good results. In particular, it is very helpful to use the GLP1 agonist medications for those patients who are over weight and are looking for treatments that will deliver weight loss as well as get their blood sugar under control.

Dermatology at Mount Carmel The Dermatology Department at Mount Carmel offers a number of services to patients, as Dr Geraldine Morrow explains. The Team

Services and Treatments Dr Morrow explains that the department treats everybody from newborn babies and upwards and they treat all skin related issues from standard skin diseases like eczema, acne and psoriasis, to checking for skin cancers. Typical procedures in the department also include biopsy of skin rashes, removal of moles and small skin cancers with the

plastic surgeons dealing with larger skin cancers. Dr Morrow also specialises in occupational dermatology, which are work-related skin diseases, often seen in those who do wet-work like catering staff, hairdressers and health care staff and on those who work in low humidity environments like offices. The department deals with patients on a day-case basis and Dr Morrow explains that most procedures take about an hour with patients under local anaesthetic. Most patients will have seen one of the doctors in the department prior to their appointment or will have been referred by their GP. Most of the treatment is then followed up by creams, ointments or other suitable prescriptions.

Phototherapy One of the most impressive assets of Mount Carmel’s

dermatology department is their Narrow Band Phototherapy Machine. This provides an ultraviolet light treatment for patients and is excellent in the treatment of psoriasis. Mount Carmel is one of the only private hospitals in the country to have invested in this technology, making it a centre of excellence in the treatment of conditions like psoriasis. Dr Morrow explains the principle of phototherapy: “Normal light has different calibres of rays within it, but the burning ones are the rays that can cause skin disease. We remove those rays from the process and the results are very beneficial for patients,” she says. “The process is strictly supervised and we have a specialised area for it. It is very effective for itchy skin conditions like psoriasis, hard to control eczema and skin diseases during pregnancy.”

Patient information Admission Time/ Check In

Insurance Cover/ Shortfalls

Please check your admission time with your consultant’s private secretary. On arrival at the hospital, please check in at the main reception area and you will be directed to the appropriate area for your admission to be processed.

It is important that you bring details of your insurance cover, membership number and plan as you will need to complete a health insurance form on admission. If you have a shortfall on your policy, you will be contacted by our accounts team in advance of admission and can pay your shortfall at this time or on admission.

Visiting Hours For inpatients undergoing general surgery, visiting times are 1-5pm and 6-9pm.

Items to bring for an overnight stay

In the interest of ensuring the highest level of comfort for patients undergoing major surgeries, visiting hours are 2-4pm and 6-8pm. Immediate family may only be admitted, depending on the patient’s level of wellbeing. Children under seven are not permitted.

If your visit to the hospital requires an overnight stay, the following items may be useful: • Pyjamas or night dress • Dressing gown and slippers • Toiletries (soap, toothbrush, toothpaste, brush, tissues etc).

Maternity patients do not have strict visiting times, all immediate family are allowed and duration of visit is at the discretion of the midwives as each case is different.

Allergies

Fasting/Admission Instructions All patients attending for a surgical procedure or endoscopy need to fast completely from midnight the night prior to surgery – no food or drink (including water) allowed after midnight. Patients should remove all jewellery, make up, contact lenses and nail varnish prior to having a surgical procedure. 36

Patients are requested to alert nursing staff to any allergies/reactions to medication they may have.

Medications Patients are requested to bring any medications they are taking and copies of any prescriptions if possible. Please leave your valuables at home. Whilst your room will have a personal safe, Mount Carmel cannot accept responsibility for loss or damage to personal possessions whilst you are in the hospital.

Paediatric Admissions Please note the following for children attending for

surgery: • Fasting instructions need to be checked directly with your consultant. • All minors under the age of 16 must be accompanied by an adult. If your child requires an overnight stay, one parent must stay overnight and accommodation will be provided. • Siblings of children are not permitted to attend the hospital outside of visiting times. • For younger children, please bring a favourite toy or teddy bear on the day. • Patients are requested to bring any medications they are taking and copies of any prescriptions if possible. Day Patients Overview All patients scheduled for day-care procedures must have nothing to eat or drink (including water) from midnight of the previous day. Please check with your consultant if you may take prescribed medication on the day of the procedure. clarification purposes. Bring details of your health insurance cover with completed self-assessment forms and any medication you are taking. Bring night attire and slippers (soft walking shoes or runners for those having an orthopaedic procedure). Remove all jewellery, make up and contact lenses if applicable. Minors or those under 18 may be accompanied by an adult. Siblings are not permitted. All patients having a daycare procedure should not drive or drink alcohol for the first 24 hours after your procedure. We would ask that a responsible adult accompany you home after your procedure.
