CEO welcome The new St Vincent’s Private Hospital (SVPH) – which is located at Merrion Road, Dublin 4 – opened in November 2010, making it the largest acute private hospital in Dublin. The new hospital, serving patients from Dublin and the adjoining catchment area, provides modern, purpose-built inpatient, day care and outpatient accommodation to support its medical and surgical diagnostic and treatment facilities. This facility has enabled us to provide highquality innovative services in a state-of-the-art facility. The hospital offers 236 inpatient beds, 30 day care beds, 22 oncology day care bays and 230 underground car parking spaces. As a member of the St Vincent’s Healthcare Group of hospitals, we employ over 700 staff and maintain our role as a key private acute healthcare provider to the region. The hospital is keen to extend the services available to patients referred by GPs in the region and, to this end, now incorporates 24 specialist consulting rooms, state-ofthe-art interventional cardiology, radiology, endoscopy, surgical and outpatient facilities. Our future plans include opening a Medical Assessment Unit (MAU), incorporating a Chest Pain Clinic before the end of 2011. The MAU will accept patients referred from GPs for assessment and, if necessary, admission. It is our commitment to remain at the forefront of clinical care that has led SVPH to be awarded Joint Commission International (JCI) Accreditation. A new era
in private healthcare Established in 1974 by the Religious Sisters of Charity, SVPH is now the largest private hospital in Dublin, offering state-ofthe-art diagnostic and treatment facilities for patients. From humble beginnings… St Vincent’s Private Hospital (SVPH) originated from two private nursing homes at Leeson Street and Pembroke Street in Dublin city centre, both operated under the care of the Religious Sisters of Charity. With the establishment of St Vincent’s Hospital at Elm Park, the nursing homes were working in isolation from the main hospital and a decision was made to move into new premises closer to St Vincent’s Hospital. The hospital was completed and opened for patients on May 1st 1974 and was, at the time, entirely financed by the Sisters of Charity. The hospital initially consisted of 140 beds, mostly fourbedded and five-bedded rooms, and 24 single rooms, complete with bathrooms. A building project completed in the early 1980s added another 20 single rooms, an X-ray department, an MRI scanner, administration offices, a private consultants’ clinic, physiotherapy and pharmacy. During the 1990s, SVPH began to develop comprehensive radiotherapy services. Also during this time the Hospital began to concentrate on a number of specialities for day care
patients including oncology, gastroenterology, chest investigations, minor surgery, urology and gynaecology. you can trust SVPH has been a fully JCI accredited hospital since 2009, offering patients exceptional quality of care and safety. What is JCI?
SVPH and JCI
Joint Commission International (JCI) is the international arm of the Joint Commission (USA) and is a well recognised international accreditation body (using standards based evaluations), and has been working with healthcare organisations in over 80 countries since 1994. JCI developed a set of international standards and an accreditation programme for hospitals and other healthcare organisations, and provides advisory and educational services, in order to continuously improve the quality and safety of patient care.
SVPH voluntarily applied the JCI standards throughout the hospital and have demonstrated continuous and sustainable improvement in the quality and safety of hospital processes and patient care. In September 2009, SVPH were awarded with JCI accreditation. Due to the significant change with the move to the new hospital in November 2010 (a new facility and a new service), JCI surveyors visited the new SVPH in June 2011 and were hugely impressed with the state-of-the-art facility and the safe processes, and decided that SVPH should retain its JCI accredited status.
Although the standards set uniform, high expectations for the safety and quality of patient care, country-specific considerations related to compliance with those expectations are part of the accreditation process, in order to accommodate legal, religious and/or cultural factors within a country. The JCI evaluation of a healthcare organisation is designed to be valid, reliable and objective, and the accreditation decision is made by an international accreditation committee.
As part of the JCI accreditation programme, every three years JCI will re-evaluate the hospital against the standards – the next JCI survey is due to take place in September 2012 – and will look for evidence of sustained compliance and improvement. This demonstrates SVPH’s long-term commitment to patient safety and continuous improvement in patient care.

In the early stages of the building project of SVPH, the designers and engineers of this multimillion euro facility engineered a complex which contained clean design with a modern flare for technology. The building’s exterior represents a new vision for a medical institution while keeping a distinct separation between the wards, administrative offices and operating theatres. Our modern facility embraces the future and was designed and built to the highest standard, providing our patients with the best medical treatment facilities in the country. Our building encompasses modern operating theatres, open patient rooms and large spacious wards all enhancing the experience of your medical treatment with us. An atrium and courtyard provide means to maximise the use of natural daylight and ventilation where possible.

SVPH services strategy It was important to emphasis a level of calm and comfort which would enhance the experience of patients and visitors to the hospital. Therefore, a design strategy was enacted to include an advanced ventilation system complete with rooftop air handling, chiller plant rooms to ensure room temperature is kept at a comfortable level, and a mechanism which would distribute cold air throughout the building. The building is also served by a sophisticated boosted water distribution system with no hot water storage, temperature monitoring points at sentinel locations and with the addition of a chlorine dioxide dosing unit. The design of the building dictates that sufficient space is made available for our patients. Therefore, our heating, power and IT systems are all based

at basement or rooftop level to reduce any inconvenience to our patients and staff.
Building management system Reducing any inconvenience for our patients is of paramount importance to SVPH. To ensure a smooth operation of the hospital, a building management system (BMS) was installed. The main task of the system is to work as a central control point dealing with alarms, set points, time schedules and operation of the heating, ventilation and air conditoning systems. This is operated from the services manager’s office located in the Services Building. The building is home to the administrative and technical operations of the Hospital and is charged with overseeing building and clinical services are maintained at the highest level.
Environmentally friendly The building also contains lighting controls with automatic presence indicators, which switch off the lighting when a room is not in use. This and other sustainable mechanism ensures SVPH is among the most energy efficient hospital facilities in the country. Weather compensation and boiler optimisation is also used in the hospital’s heating system which is efficiently controlled depending on the external temperature. We utilise a combined heat and power (CHP) plant as the lead boiler. All power plants emit a certain amount of heat during electricity generation, this can be released into the natural environment through cooling towers, flue gas, or by other means. By contrast, CHP captures some or all of the by-product heat for heating purposes. The building has also been insulated beyond the current building regulations while the ventilation system uses variable speed drives to minimise the use of energy. Our sedum roof on the theatre block is one of the many environmentally-friendly features, with a grass roof which increase the biological diversity of the structure and reduces rain running down the external of the building. Our building operates a recycling programme which is designed to minimise our waste to landfill which is in excess of 48 per cent. An ‘e correct’ machine is also used which composts food materials minimising any wastage.
SVPH interior
The theatre block comprises two traditional theatres, two ultra-clean ventilation theatres, a cardiology catheterisation laboratory and a
day theatre. All of the theatres were designed, installed and validated to the Health Technical Memorandum 03 – ‘Specialised ventilation for healthcare premises’. The theatres also utilise an adaptable modular system, which means that the powder-coated wall and ceiling elements can adapt to the most varied building conditions.The rooms are equipped with modern operating facilities providing the most technically accurate clinical experience in the country. Endoscopy, radiotherapy, cardiology and diagnostic imaging facilities are also catered for. In separate rooms, the hospital carries out general radiography, CT, MRI, interventional radiology, mammography, ultrasound and Dexa scanning.

SVPH exterior The external design of the building is also increasingly important to the look and feel of the new hospital. Designed by the awardwinning firm, Scott Tallon Walker Architects, the exterior exuberates the meaning of our hospital through iconography. The quick cuts in building design along with the modern facade, ensures we are one of the most modern medical facilities anywhere in the Ireland. In late 2007, engineers from ARUP and Buro Happold worked with the lead architects at Scott Tallon Walker to ensure the early plans incorporated a stylish design with the demands of a modern hospital. The building was designed to overtake the previous 164 bed SVPH facility established in 1974. Updating an outdated design took hours of work to ensure the facility instilled the same quality of the previous centre while keeping in mind the demands of the modern medical client. John Paul Construction was the main contractor for the hospital.
Medical equipment The new SVPH stands as the final completion of the expansion which began with the opening of phase one a €4.5 million Radiation Oncology Department in 1995 and aims to make a significant contribution to meeting the national demand for radiation oncology services.

of treatment for cancer, the new hospital provides additional facilities such as two additional operating theatres, 12 consulting suites, and satellite laboratories. The ward rooms are large open spaces and contribute nicely to the sleek and modern design of the hospital. The entrance and surrounding areas of the hospital welcome patients and visitors to partake in this modern clinical facility through adequate parking and biking facilities. Once inside the facility, patients and visitors will be able to dwell in the comfort of a modern and sleek design. For patients, we offer large comfortable rooms with 32inch flat-screen televisions, ensuites with showering facilities, locally controllable heating, great views, comfortable seats (which transform into loungers), patient safes, telephones and WiFi access. The building is designed to enhance the stay of patients and visitors.
The new facilities are also home to a number of treatment machines, which were updated significantly in order to be incorporated into the design of new SVPH. The new treatment machines provide a more efficient treatment in a number of ways. For instance, our treatment times are now shorter, thanks to the addition of the Elektra Precise Treatment System. The system has designed an integrated solution to ensure that the performance parameters allow for a wide range of radiotherapy techniques. The system also provides better quality control of treatments and all systems are linked to the department network, which records and verifies accurate treatment details and treatment design to the machines. The new facility also incorporates two new Elekta Linear Accelerator treatment units and a new GE CT scanner with virtual simulation software for treatment design. Here at SVPH we are dedicated to the enhancement of new technology to improve our medical service. The new facility propels us as the leaders in modern medical treatment using a contemporary flare for patient comfort.

At SVPH, we are dedicated to the prevention of infections and diseases for all patients and staff. Here at SVPH, we are setting the standard for the prevention of infections and infectious diseases through our infection prevention and control (IPC) programme, which aims to reduce the risk of infections through education, surveillance and action. The infection control programme encompasses all areas of the hospital to ensure quality of care and patient safety and is in line with national guidelines published by Health Protection and Surveillance Centre (HPSC). The prevention of infection is monitored directly by senior management, which oversees the IPC team and ensures project development and the inclusion of adequate infection control mechanisms in all clinical areas.
Adhering to healthcare guidelines The CEO and the staff of SVPH correspond daily in regard to infection prevention and control. There is a yearly hygiene audit conducted by an external auditor to ensure our facilities and processes are compliant to national standards. Any breaches in standards are addressed within the risk management framework of SVPH. All new staff receive education on infection prevention and control procedures at induction training. During the education process, staff will learn the basics of hand hygiene, standard and transmission based precautions, prevention of needle stick injury, decontamination of reusable medical equipment and waste management. Healthcare-associated infections are also discussed. The education programme helps us to communicate directly with ward staff on how best to prevent the spread of infection with a strong emphasis on hand hygiene.
SVPH and hand hygiene Hand hygiene is the single most important element to prevent the spread of infection. In our new hospital we have excellent hand hygiene facilities. Hand foam dispensers are located at the entrance and in all public areas, to facilitate all staff, visitors and patients entering the hospital to decontaminate their hands. Hand foam dispensers are also located outside each patient bedroom and in all clinical areas. A hand wash sink is located in every patient bedroom and also in clinical areas. All staff members are required to undertake mandatory hand hygiene education on an annual basis. A hand hygiene audit is conducted bi-annually by the IPC team and our compliance rate is currently at 85 per cent. The current national goal, as set out by the HSE for 2011, is 75 per cent.
SVPH: Additional steps to prevent infections The hospital can provide single rooms with ensuite facilities to the vast majority of our inpatients, this greatly assists in the prevention and control of infection. There are 14 HEPA filtered neutral pressure isolation rooms available to facilitate the management of patients requiring airborne precautions. For example, patients with pulmonary TB or H1N1 virus. These rooms can also be used for protective isolation of immuno-compromised and neutropenic patients.
The central decontamination unit The Central Decontamination Unit (CDU) at SVPH is responsible for supplying medical devices and equipment required in a sterile condition for wards, departments and operating theatres. It is essential that those items required, have been sterilised under properly controlled conditions and by suitably qualified staff. Steam and hydrogen peroxide gas sterilisation are the methods used at SVPH. The departmental manager is an international speaker on decontamination. She is a member of the WHO Decontamination Project Group and other European committees. The CDU works closely with the IPC team.
A microbiologist is available to advise the medical team in relation to infection prevention and control issues which may occur.

Shared knowledge The rise of new technology has given birth to a new form of file sharing at SVPH. RiS/PAC is a modern file sharing system that is enhancing our patients’ clinical experience. What is RIS/PACS? Radiology information systems (RIS) and picture archiving and communications systems (PACS) are a fast developing aspect of modern medicine. These systems offer more efficient data management for data and images and are becoming increasingly relied upon for clinicians. SVPH’s RIS/PACS project is part of a group scheme which includes St Vincent’s University Hospital and St Michael’s Hospital in Dún Laoghaire. RIS/PACS ensures timely diagnosis for a patient, better use of radiologist time, and availability of critical clinical information across the organisation through increased access to reports outside the Radiology Department. The benefits of RIS/PACS include the electronic storing of all patients’ prior and current images from the Radiology Department, outpatient clinic and hospital ward. Our service results in fewer repeat exams (thus reducing a patient’s exposure to unnecessary radiation). It provides a faster turnaround for reports (introduction of speech recognition) a rapid transfer of images between clinicians for consultation or remote referral. Our high-tech security mechanisms ensure the security of patient data with controlled and audited access and allows for the full availability of prior examinations and reports, along with the current images. The programme also reduces the costs for the Radiology Department, as it eliminates the need for the production of film – the transport of which can delay a patient’s examinations. For the first time, the system allows for the introduction of electronic ordering/requesting for radiology examinations – in place for the emergency department (ED) – with plans to roll out across the three sites. An electronic report to GPs via HealthLink is used which ensures patient information is made instantly available to all St Vincent’s Hospital Group members.

Model of care The key driver for a group wide RIS/PACS is the continuity of care for our patients’ attending any one of the three sites. Accessibility at point of care to a patient’s entire radiology history images and reports are essential to improve the patient’s journey through the care pathway. Patient safety remains the highest priority for all in the delivery of the radiology service which is fundamentally linked with the quality of care (JCI patient safety goal). The use of integrated RIS/PACS improves the aims of the Faculty of Radiologists, RSCI (2010) report entitled ‘Guidelines for the Implementation of a National QA Programme in Radiology’.
Impact to the target population
The introduction of integrated RIS/PACS has been shown to contribute directly to more efficient patient care. The use of voice recognition application reduces radiology report turnaround times, making verified reports available to referring clinicians sooner. Referring clinicians can then plan intervention/treatment based on the verified report and images. This also allows for a quick and early intervention which increases a positive prognosis for patients.
The catchment population for SVPH is in excess of 350,000. St Vincent’s University Hospital is also the national referral centre for cystic fibrosis, adult liver transplant, breast, prostate, colorectal and prostate cancer services. The radiology departments across the three sites perform in

ABRIDGED PRESCRIBING INFORMATION (For full prescribing information, refer to the Summary of Product Characteristics [SmPC])RoActemra® (Tocilizumab) 20mg/ml Concentrate for Solution for Infusion Indications: RoActemra, in combination with methotrexate (MTX), for the treatment of adult patients with moderate to severe active rheumatoid arthritis (RA) who have had an inadequate response or intolerance to one or more DMARDs or TNF antagonists. In these patients, RoActemra can be given as monotherapy in case of intolerance to MTX or where continued treatment with MTX is inappropriate. RoActemra has been shown to reduce the rate of progression of joint damage as measured by X-ray and to improve physical function when given in combination with methotrexate. Dosage and Administration: Recommended posology is 8mg/kg diluted to a final volume of 100ml, given once every 4 weeks by iv infusion over one hour. For patients weighing more than 100kg, doses exceeding 800mg per infusion are not recommended. No data on doses above 1.2g. Treatment should be initiated by healthcare professionals experienced in the diagnosis and treatment of rheumatoid arthritis and patients should be given the Patient Alert Card. Dose adjustments: Dose modification, interruption or in some cases discontinuation of RoActemra, is recommended in the event of raised liver enzymes, low absolute neutrophil count (ANC) or low platelet count (see SmPC for details). In patients not previously treated with RoActemra, initiation not recommended in patients with an ANC below 2 x 109/l. Not recommended in children under 18 years of age due to a lack of data. Closely monitor renal function in patients with moderate to severe renal impairment as RoActemra has not been studied in these patients. No data in patients with hepatic impairment. Contraindications: Hypersensitivity to any component of the product; active, severe infections. Warnings and Precautions: Serious (sometimes fatal) infections reported in patients receiving immunosuppressive agents including RoActemra. Do not initiate in patients with active infection. If serious infection develops interrupt therapy until infection controlled. Caution in patients with history of recurring/chronic infections, or other underlying conditions which may predispose patients to infection. Vigilance for the timely detection of serious infection recommended. Advise patients to contact their healthcare professional when symptoms suggestive of an infection appear. Screen for latent TB prior to starting therapy. Treat latent TB with standard anti-mycobacterial therapy before initiating RoActemra. Viral reactivation (e.g. hepatitis B) reported with biologic therapies for RA. Patients screening positive for hepatitis excluded from clinical trials. Events of diverticular perforations as complications of diverticulitis reported uncommonly with RoActemra. Use with caution in patients with a history of intestinal ulceration or diverticulitis. Patients with symptoms of complicated diverticulitis should be evaluated promptly. Serious hypersensitivity reactions reported-may be more severe and potentially fatal in patients who have experienced hypersensitivity reactions during previous infusions even if they have received premedication with steroids and antihistamines. Appropriate treatment should be available for immediate use in the event of an anaphylactic reaction during treatment with RoActemra. If an anaphylactic reaction or other serious hypersensitivity/serious infusion related reaction occurs, stop administration of RoActemra immediately and discontinue RoActemra therapy permanently. Use with caution in patients with active hepatic disease or hepatic impairment. Not recommended in patients with baseline ALT or AST > 5 x ULN; use with caution in patients with ALT or AST > 1.5 x ULN. Monitor ALT and AST levels according to SmPC – other liver function tests including bilirubin should be considered where indicated. If raised, follow recommendations for dose modification/interruption. Risk of neutropenia may be increased in patients previously treated with a TNF antagonist. Continued therapy not recommended in patients who develop an ANC < 0.5 x 109/l or platelet count < 50 x 103/μl. In patients not previously treated with RoActemra, initiation not recommended where ANC is below 2 x 109l. Caution in patients with low platelet count; monitor neutrophils and platelets according to SmPC. If reduced, follow recommendations for dose modification. Elevations in lipid parameters seen - refer to SmPC. Assess lipid parameters according to SmPC, if elevated, manage patients according to local guidelines for hyperlipidaemia. Potential for central demyelination with RoActemra currently unknown; physicians should be vigilant for symptoms of new onset disease. Immunomodulatory medicines may increase malignancy risk. Live and live attenuated vaccines should not be given concurrently with RoActemra as safety not established. RA patients should have CV risk factors managed as part of usual standard of care. Not recommended for use with other biological agents. Advise patients experiencing dizziness not to drive or use machines until dizziness resolved. Product contains 26.55mg sodium per 1200mg. Drug Interactions: In RA patients, levels of simvastatin (CYP3A4) were decreased by 57% one week following a single dose of tocilizumab to levels similar to or slightly higher than those observed in healthy subjects. Monitor patients taking medicines which are individually adjusted and metabolised via CYP450 3A4, 1A2 or 2C9 when starting or stopping RoActemra, as doses may need to be increased to maintain therapeutic effect. The effect of tocilizumab on CYP450 enzyme activity may persist for several weeks after stopping therapy. Refer to SmPC for further details on the effects of RoActemra on cytochrome CYP450. Fertility, Pregnancy and Lactation: No adequate data from use in pregnant women. Animal study showed an increased risk of spontaneous abortion/embryo-foetal death at high dose. RoActemra should not be used during pregnancy unless clearly necessary. Women of childbearing potential should use effective contraception during and up to 3 months after treatment. No lactation data in humans. A decision on whether to continue/discontinue breastfeeding or RoActemra therapy should be made taking into account the relative benefits to the child and mother. Side Effects and Adverse Reactions: ADRs occurring in patients with RA receiving tocilizumab as monotherapy or in combination with MTX or other DMARDs: Very Common (> 1/10): upper respiratory tract infections and hypercholesterolaemia. Common (> 1/100 - <1/10) - cellulitis, pneumonia, oral herpes simplex, herpes zoster, abdominal pain, mouth ulceration, gastritis, rash, pruritus, urticaria, headache, dizziness, hepatic transaminases increased, weight increased, total bilirubin increased, hypertension, leucopenia, neutropenia, peripheral oedema, hypersensitivity reactions, conjunctivitis, cough and dyspnoea. One-stop-shop for radiology
The Radiology Department offers a quality diagnostic and therapeutic service in SVPH. The new Radiology Department at SVPH offers a full range of X-ray, CT, MRI, ultrasound and interventional radiology imaging and treatment capability. A specialist group of diagnostic and interventional radiologists offer a comprehensive radiological assessment and consultation service. Interventional radiological procedures include cancer therapies such as chemo-embolisation and radiofrequency ablation. We also offer extensive abdominal imaging including CT, ultrasound, advanced MR liver imaging and extensive musculoskeletal imaging, including ultrasound, MR, CT and fluoroscopic guided therapeutic injections. SVPH offers cutting-edge technology to provide optimum patient diagnostic and treatment services. The new department is well equipped to deal with all radiological requirements.
CT scan services As part of the CT advanced diagnostic facilities, the hospital has installed two high-tech CT scanners from GE Healthcare, including Ireland’s first high-definition CT scanner. The new scanner uses a breakthrough garnet gemstone CT detector, the first new CT detector technology in the industry in over 20 years. The gemstone detector can improve image clarity by up to 33 per cent for routine body imaging and by up to 47 per cent for cardiac imaging, helping to improve doctors’ ability to distinguish different types of tissue. The exciting technological development of this high-definition CT helps improve doctors’ ability to see fine anatomical detail in what can be difficult diseases to image. However, the new scanner does not just improve image clarity, but is designed to reduce the amount of X-ray dose patients receive by up to 50 per cent for full body scans, and up to 83 per cent for heart scans.
MRI services Magnetic resonance imaging (MRI), is a method of imaging the organs inside the body. It uses the
very latest technology and has no known harmful effects. MRI utilises a large circular magnet, which is open at both ends, radio waves and a computer to form pictures of the body. The patient lies in the bore of the magnet for the examination. As our bodies contain a lot of hydrogen, much of it in the form of water, and since certain parts of the hydrogen atom are magnetic, we are able to make them transmit radio signals. Using these signals and a computer, the collected information is converted into pictures called ‘images’. At SVPH, we have a 1.5 Tesla GE scanner that provides high-quality images at reduced scan times which is very important for patients who may be claustrophobic or find it difficult to remain still for long periods of time. All systems of the body can be imaged. These include neurological scans, abdominal and pelvic scans, MRCP, MRI prostate, cardiovascular system and musculoskeletal system.

Just like with all scans carried out by the Radiology Department at SVPH, the results of the scan are stored on a computer database and the information is made accessible to doctors and specialists dealing with the patient.
Interventional radiology Interventional radiology is a specialty dedicated to the practice of imaging guided procedures for diagnostic and therapeutic purposes. Interventional radiology plays a major role in quality healthcare. All interventional procedures are performed by specially trained radiologists. COMMON INTERVENTIONAL PROCEDURES Angiography: An X-ray exam of the arteries and veins to diagnose narrowing, blockages and other blood vessel problems; uses a catheter (tube) to enter the blood vessel and a contrast agent (X-ray dye) to make the artery or vein visible on the X-ray. small balloon into the vessel and inflating it. Used by interventional radiologists to unblock clogged arteries in the legs or arms (called peripheral vascular disease or PVD), kidneys, brain or elsewhere in the body. Biliary drainage and stenting: Uses a stent (small mesh tube) to open up blocked ducts and allow bile to drain from the liver. Central venous access: Insertion of a tube from the skin into the blood vessels so that patients can receive medication or nutrients directly into the blood stream, or so blood can be drawn. Chemoembolisation: Delivery of cancerfighting chemotherapy agents directly to the site of a cancer tumor; currently mostly being used to treat cancers of the endocrine system, including melanoma and liver cancers. Embolisation: Delivery of clotting agents (coils, plastic particles, gel, foam, etc) directly to an area that is bleeding or to block blood flow to a problem area, such as an aneurysm or a fibroid in the uterus. Fallopian tube catheterisation: Uses a catheter to open blocked fallopian tubes without surgery; a treatment for infertility. Gastrostomy tube: Feeding tube inserted into the stomach for patients who are unable to take sufficient food by mouth.
Thrombolysis: Dissolves blood clots by injecting clot-busting drugs at the site of the clot, often used for treating deep vein thrombosis and stroke. TIPS (transjugular intrahepatic portosystemic shunt): A life-saving procedure to improve blood flow and prevent hemorrhage in patients with severe liver dysfunction. Uterine artery embolisation: An embolisation procedure of uterine arteries to stop lifethreatening postpartum bleeding, potentially preventing surgical hysterectomy. The same procedure is used to treat fibroid tumors and is then called UFE (uterine fibroid embolisation). Uterine fibroid embolisation: An embolisation procedure of uterine arteries to shrink painful, enlarged, benign tumors in the uterus, also called UAE (uterine artery embolisation).

Hemodialysis access maintenance: Use of angioplasty or thrombolysis to open blocked grafts for hemodialysis, which treats kidney failure. Needle biopsy: Diagnostic test for breast, lung and other cancers; an alternative to surgical biopsy. Radiofrequency ablation: Use of radiofrequency (RF) energy to ‘cook’ and kill cancerous tumors. Stent: A small flexible tube made of plastic or wire mesh, used to treat a variety of medical conditions (e.g. to hold open clogged blood vessels or other pathways that have been narrowed or blocked by tumors or obstructions). Stent-graft: Reinforces a ruptured or ballooning section of an artery (an aneurysm) with a fabric-wrapped stent, a small flexible mesh tube used to ‘patch’ the blood vessel. Radiotherapy at SVPH The Radiotherapy Department was established in 1995 with the introduction of radiation oncology to SVPH, and has since been at the forefront of providing radiotherapy services as part of the hospitals comprehensive multi-disciplinary oncology services. The Radiotherapy Department at SVPH has made a significant contribution to meeting the national demand for radiation oncology services in Ireland. In 2005/2006, SVPH invested €4.5 million to upgrade the existing Radiotherapy Department. The development plan comprised of the installation of two new Elekta Linear Accelerator treatment units, new GE CT scanner, with virtual simulation software, which offers up-to-date tumour and organ at risk localisation.
Technology The new treatment machines have provided more efficient treatments in a number of ways. The Elekta Precise Treatment system has designed an integrated solution to ensure that the performance parameters allow for a wide range of radiotherapy techniques. The system also provides a better quality control of treatments and all systems are linked to the department network, which records and verifies accurate treatment details and treatment design to the machines.
The core staff group within the department include radiation oncologists, radiation therapists, medical physics and dosimetry, engineering and administration.
Service capacity At present 70 patients can be treated on both machines between 9am and 5pm. The department provides approximately 13,000 treatments annually. In the event of a machine breakdown all remaining patients are seamlessly transferred onto the other treatment unit as both machines are matched. We have no waiting list with the majority of radical patients starting treatment the following week and all palliative patients start treatment the following day if not the day of CT-simulation.

In 2009, the department upgraded its patient management system to MOSAIQ®. MOSAIQ® is a dedicated oncology information system (OIS) that streamlines the entire medical

The system also includes iView GT, an online treatment imaging device that provides additional ability to take patient positioning to the next level of accuracy, through the instant delivery of high-quality images using solid state detector technology. In addition the department is equipped with a HDR brachytherapy unit. This is extensively used for the treatment of gynaecological malignancies and superficial disease. oncology or radiation oncology workflow, from the first diagnosis and staging, through planning, treatment, follow-up and long-term survival. MOSAIQ® is designed specifically to handle the complexities of radiation oncology. MOSAIQ® has the ability to interface with CT-simulation and treatment machines (linear accelerators) so that treatments can be prescribed, delivered and recorded electronically. This also facilitated the introduction of intensity modulated radiation therapy (IMRT), which is considered the most advanced radiation available to destroy cancer cells without damaging normal tissue. IMRT uses varying beam intensities to send radiation to the tumor site from the most favorable paths. This method of radiation therapy allows higher doses of radiation to be delivered to the tumor while minimising damage to surrounding vital organs. We are currently using this exciting new technology to treat head and neck, lung, prostate and breast patients. The department has also introduced the use of gold seed fiducial markers within the daily treatment of prostate cancer patients to verify target organ position. During a course of radiotherapy the prostate can undergo significant displacement over the seven-week course of treatment. The fiducial markers act as a surrogate of actual prostate position and has improved treatment targeting with daily
imaging and correction therefore minimising the effect of interfractional prostate motion.
Training and education The department has strong academic links with Trinity College Dublin and participates in the training of radiation therapy undergraduates with two dedicated mentors for students inhouse. We actively participate and run courses through the School of Radiation Therapy with an exceptionally high-level of attendance of all staff members on these courses and study days. In addition, the department is very active in research and development. The department has publications in peer review journals and participates in both radiation therapy and medical physics conferences at national and international level.

Medical physics have academic links with the main Dublin teaching hospitals with physics lectures provided to St Vincent’s and Galway University Hospital. The Department participates in the training of medical physicists in collaboration with St James’ and St Luke’s hospitals. At SVPH, we are proud of our extensive Pharmacy Department. Our pharmacy services are provided by highly skilled pharmacists and pharmaceutical technicians. The Pharmacy Department in SVPH is comprised of three service functions. The services are provided by an enthusiastic, dedicated team of pharmacists and pharmaceutical technicians. 1) The dispensary service procures, stores and dispenses medications for areas throughout the hospital, and also dispenses medications to outpatients and staff. 2) The aseptic compounding unit is a new specially designed unit for the sterile compounding of chemotherapy products for patients of the hospital. 3) The ward pharmacy service is provided by a team of experienced clinical pharmacists and pharmaceutical technicians. The role of the clinical pharmacist is outlined below. The pharmaceutical technicians have responsibility for co-ordination of the supply of all medications to all areas of the hospital.

Clinical ward pharmacists SVPH has invested in a team of five skilled and experienced clinical pharmacists (CPs) to work on the wards with patients, nurses, doctors and other allied healthcare professionals. The CPs aim is to meet each inpatient of the hospital as soon as possible after admission and review all prescriptions for appropriateness for individual patients. Medication reconciliation Medication reconciliation is the term used for the process of reconciling the medications prescribed at admission with the medications the patient was on before admission. The CP does this by reviewing the prescriptions in the St Vincent’s Private Hospital / ANNUAL

Clinical review Clinical review is the process of examining each medication individually and collectively and evaluating the following: The appropriateness of the drug, dose, frequency, and route of administration; Therapeutic duplication; Real or potential allergies or sensitivities; Real or potential interactions between the medication and other medications or food; Variation from organisation criteria for use; A patient’s weight and other physiological information; and Other contraindications. The CPs clinically review each prescription and endorse and date the drug kardex to show that this process has taken place.
Education is key Education of patients on medications is provided when a new drug is started and when the patient expresses uncertainty
as to the reason for a medication or the best way to take a medication. Lack of information on medications can often lead to non-compliance with drugs which can lead to patients being prescribed more medications than are necessary. Non-compliance is routinely identified by the CPs and they investigate reasons for, and solutions for, non-compliance to improve patient compliance. Discharge counselling on medications prescribed is also available if requested by the patient
Patient monitoring The CPs routinely provide therapeutic monitoring for effects of medications which includes: Monitoring effects of drugs on patient through conversations with patients and reviewing observations. Recommending and monitoring blood tests. Performing renal function calculations to ensure treatments and doses are appropriate to individual patients.

Monitoring renal function for patients receiving drugs which may adversely affect the kidneys. Monitoring liver function for patients receiving drugs which may adversely affect liver function. Monitoring choice of, and duration of, antibiotic therapy to ensure antibiotic use is in line with hospital policy and appropriate to patient needs and responses. Therapeutic dosage monitoring for drugs with a narrow therapeutic window. The CP is best placed in the hospital to review patients from a broad pharmacological perspective. This broad pharmacological perspective means that the pharmacist may, for example, recommend that the patient has a consult from another speciality to the one they are admitted to. So, if a patient is admitted for a surgical procedure, and the CP has identified that the patient has a poorly controlled respiratory condition, the CP may discuss this with the patient’s admitting consultant and ward ward manager, and recommend a review by a respiratory consultant.
pharmacy initiatives Since the launch of this service in 2008, the CPs have proved themselves as a dedicated and caring team within the pharmacy team at SVPH. Each CP takes on additional support functions in the hospital such as SARI pharmacist duties to assist the hospital with compliance with national antibiotic monitoring and surveillance. The CPs also play a key role with medication safety monitoring and reporting. The hospital supports the CPs with ongoing training and education to ensure that they have the most up-to-date knowledge and skills. There is also a weekly in-house pharmacy meeting where medical journals are reviewed and experiences are shared with the wider pharmacy team. The CPs have introduced many process improvements, one example of which was the installation of individual drug safes for each patient adjacent to the bed of each patient. They have also delivered three major research pieces at the cutting edge of clinical pharmacy service delivery, including: 1. Reducing Medication Errors at Admission and Discharge (Sinead Murray, 2010) This work was awarded runner-up position in the category of innovation at the Hospital Pharmacists Association of Ireland annual conference in 2011; it was also nominated and short-listed in the Healthcare Initiative Awards 2011 in the category of Patient Safety Initiatives. Sinead will be presenting her work at the UK Clinical Pharmacy Association Conference in November. 2. A review of the Medication Incident Reporting System (Ruth Ryan, 2011) This work has overhauled the medication incident reporting culture in SVPH and has introduced a new MIR form and database to allow for analysis and trending of medication incidents. Better reporting and analysis of MIRs now allows for identification and implementation of improved medication systems thus providing an even safer environment in SVPH. 3. The Impact of a Pharmacist in a Multidisciplinary Team on an Oncology Ward in SVPH (Nuno Silva, 2011) The CPs and the wider pharmacy team in the SVPH multidisciplinary team, provide internationally recommended critical services to ensure that SVPH provides a safe environment for our very important patients.

Oncology at SVPH includes both day care and inpatient services and offers the very best in patient care and treatment. Current statistical trends, advancement in investigative technology and our aging population suggest that the number of cancers diagnosed, in the next 20 years will double. Consequently, in the new SVPH, we have significantly expanded our oncology services and consolidated our wealth of knowledge and expertise along with improving the caring environment. The oncology multidisciplinary team and service at SVPH is one of the most progressive, comprehensive, highly-trained and respected in Irish private medicine. It has led the way, since the mid 1970s, embracing new advances in pharmacology, medicine and nursing even in times of financial constraint. Patient care and best practice is at the forefront of decision-making.
The oncology team From a medical perspective, the team now has 12 consultants and two designated oncology registrars. The team comprises of four medical oncologists, three haematologists, three radiation oncologists, a psychiatrist and a palliative care consultant, all of whom are national and international leaders in their particular field. They hold weekly multidisciplinary case review meetings with consultant radiologists and pathologists to ensure optimal treatments and diagnosis. The oncology registrars participate on a rotating educational scheme within St Vincentâ&#x20AC;&#x2122;s Healthcare Group and alternate between oncology day care and the designated inpatient areas. From a nursing perspective, there is a very integrated, comprehensive, clinically expert team not replicated in any other private hospital. Two oncology nurse co-ordinators, for example, play a critical role in the preparation and in-depth patient education of all new patients at the outset of treatment, while nurse specialists in haemoviligence, infection control
and prevention, diabetes, breast care, colorectal and palliative care all contribute their expertise to patient care. The nurses working in oncology are compassionate, experienced professionals; many of whom are educated to Masters level. They are under the clinical direction of a nurse in an advanced practitioner role, who is committed to upholding best practice. Strong links with the department of nursing at UCD have developed over the years. Each year, nurses are supported to undertake the Graduate Diploma in Oncology Nursing and students come on clinical placement to work and learn from our expert staff.

Referral/appointment procedure Referral for treatment is via a consultant/ service within St Vincent’s Healthcare Group or via a consultant from another healthcare facility. Appointments to commence chemotherapy treatment are arranged, after a consultant review, by one of the oncology nurse coordinators, who will liaise with the relevant consultant and clinical nurse manager from the appropriate service. The team would not be complete without the dedicated, experienced chemotherapy pharmacists who work closely with the doctors and nurses to prepare and oversee treatments. At SVPH, all inpatient areas have a designated clinical pharmacist as part of the ward team. Other members are from the areas of administration, pastoral care, social work, dietetics and physiotherapy.
Day care oncology Day care oncology was established in SVPH as far back as 1984. At that time nurses prepared and administered the chemotherapy. Today, chemotherapy is prepared by specially trained pharmacists and technicians in a designated sterile compounding suite. The day care facility is managed by a clinical expert with a Masters in Oncology Nursing. Unlike many other facilities, nurses who work in day care at SVPH all have a Graduate Diploma in Oncology

P or are currently undertaking the academic programme, which creates a very dynamic environment were the latest treatment options are discussed and administered. Notwithstanding the level of knowledge and expertise, the staff are friendly and welcoming. The custom-designed and spacious unit, which overlooks Dublin Bay, is equipped with comfortable reclining treatment chairs, each with individual flatscreen televisions and audio visual extras. Private consultation rooms are available along with isolation rooms, and patients are reviewed by their consultant at each treatment session. A 24-hour contact service is also available to patients for advice and symptom support during their treatment trajectory and nursing staff then liaise with the consultant as required. On a lighter note, tea, coffee, sweets and cakes are also available, with a light lunch being served daily at noon.
Inpatient oncology The inpatient services, overlooking the Dublin Mountains, now offer additional private rooms, some of which are hepa-filter, neutral pressure isolation rooms, especially designed to safeguard immuno-compromised patients. Inpatient chemotherapy is again administered 36
by nurses with a Graduate Diploma in Oncology and managed by three expert oncology clinical nurse managers. As survival rates increase and many learn how to live with cancer, aspects of the disease are perceived more as a chronic illness. Therefore, the trusting therapeutic relationship that we aim to develop between the team and our patients’ is one that we hope will support and sustain both patients and their loved ones throughout their individual journeys. cardiology care at SVPH AT THE NEW SVPH WE PROVIDE A COMPREHENSIVE CARDIOLOGICAL INVESTIGATION AND TREATMENT FOR BOTH INPATIENTS AND OUTPATIENTS. SVPH provides consultant-led inpatient, day care and outpatient cardiology services for patients referred from GPs and consultants. Our facilities at SVPH are fully-equipped to provide the technical knowledge needed to undertake advanced cardiac procedures. “The new SVPH development has facilitated a significant upgrade in cardiology services which had not been previously provided in the old SVPH. We now have a comprehensive range of non-invasive and invasive/ interventional diagnostic and therapeutic modalities encompassing all aspects of adult cardiological conditions,” says Dr Peter J. Quigley, Consultant Cardiologist. The extensive service is one of the most modern in the country, offering fully-equipped ward rooms, a cardiac catheterisation laboratory and a comprehensive Cardiology Department.
Catheterisation Laboratory Our new catheterisation laboratory, situated on the second floor, benefits from state-of-art (Phillips) imaging system. It is fully-equipped and staffed to provide diagnostic coronary angiography, coronary angioplasty/stenting, valvuloplasty/pacemaker and ICD implantation and electrophysiology/ablation procedures. This facility offers an excellent clinical experience for our patients and is staffed by a highly-trained and multidisciplinary team, including a consultant cardiologist, CNM2 and specialist nurses, a clinical specialist radiographer and a cardiac physiologist. A fulltime nurse/manager coordinates procedures on a daily basis with the six-bedded day ward, cardiology inpatient beds and consultant cardiologists/cardiology registrar. Direct urgent referrals are also catered for when necessary.

SVPH // Cardiology services SVPH // Cardiology Department The catheterisation laboratory at SVPH has a Phillips cardiovascular system including the first fully integrated electrophysiology laboratory in the country. The flat detector enables superior imaging with reduced radiation dose. Tools such as stent boost and 3DCA enable accurate diagnosis of artery disease. The lab is equipped with a wide selection of stents allowing for immediate treatment with PCI. Intravascular ultrasound (IVUS) and fractional flow reserve (pressure wire) are also integrated into the lab further allowing for specific cardiovascular treatment. Intra-aortic balloon pump (IABP) support and both transvenous and transcutaneous temporary pacing are onsite.
OUTPATIENT SERVICES The non-invasive department, situated on the first floor of the hospital, is staffed with six fully trained cardiac technicians skilled in transthoracic/transoesophageal echocardiography, treadmill exercise testing, 24-hour Holter monitoring/event monitoring and electrocardiography. A same day ‘walk in’ facility is available for ECG and echocardiography while exercise treadmill tests and Holter/event monitors are available by telephone booking. More advanced cardiac imaging including CT coronary angiography and cardiac magnetic resonance imaging is readily available through our close liaison with the Radiology Department.
Picture archiving and communication system In recent years, the Cardiology Department at SVPH has worked with St Vincent’s University Hospital to ensure our patients’ information is readily available across the greater hospital network. The Phillips Cardiovascular Information System (CVIS), facilitates this by enabling image archiving and sharing. File handling on CVIS links the patient’s report with images of the procedure that then can be viewed throughout both the private and university hospital sites. This is convenient if a patient is admitted into one hospital after having examination carried out in the other. It also allows a more efficient service in clinics and on the wards, as the information is available at the click of a mouse as opposed to searching though paperwork. “The new chest pain clinic due to open in November will facilitate immediate consultant evaluation of patients with suspected ischaemic or other heart disease. We trust that these developments will beneficially impact on delivery of optimal medical care to our patient population,” says Dr Quigley. 40

Dietary advice may be necessary for the following patients, who: (a) Require nutrition support i.e. patients who are malnourished or at risk of malnutrition. (b) Require therapeutic diets e.g. for diabetes, coeliac disease, renal disease. (c) Require preventative diets e.g. for heart disease, weight management. Dietitians are responsible for the nutritional care of patients referred while in the hospital setting. They carry out nutritional assessment of these patients and design care plans to meet nutritional requirements or correct poor nutritional status. SVPHâ&#x20AC;&#x2122;s Diabetes Unit provides a quality, evidence-based service to diabetes patients, on both an inpatient and outpatient basis. According to recently published statistics, there are approximately 180,000 people in Ireland with type 2 diabetes and 16,000 with type 1 diabetes â&#x20AC;&#x201C; a figure that is expected to rise to 233,000 by 2020. There is also 30,000 people with undiagnosed diabetes. Perhaps the most worrying statistics uncovered by a market research company on behaviour and attitudes, on behalf of the Diabetes Federation of Ireland and the healthcare company Sanofi, is that there are approximately 146,000 people in Ireland with undetected pre-diabetes who will have type 2 diabetes within five years unless they take action. At SVPH, we have provided a multidisciplinary, patient centred diabetes service for 30 years. Our Diabetes Unit is staffed by a highly trained team of endocrinologists and specialist nurses, both of whom are educated to Masters level and one is in an advanced nurse practitioner role. The team work tirelessly to ensure that national and international standards of diabetes care are adhered to.
Accessing the Unit Although patients are predominately in charge of self-managing their diabetes once diagnosed, the team at SVPH are on hand on an outpatient basis, to answer any questions that patients might have and offer guidance to ensure patients are coping appropriately. Following receipt of written referrals from a GP or consultant, patients can attend the unit for diabetes education and a full health history review. Patients are shown how to test their blood sugar regularly; record the readings and make notes in their dairy; look for patterns of blood glucose readings they would like to improve and prepare questions for the team. It is important to take their diary to the unit when attending.

At the unit, the patient’s overall condition will be checked. Test results and blood glucose readings will be discussed, along with other information in the patient’s diary. The patient’s blood will also be tested for HbA1c, liver, kidney and thyroid function and cholesterol. HbA1c shows how well the patient’s diabetes has been controlled over a 12 week period. The patient’s blood pressure is also measured and his or her urine may be tested for albumin. Regular examinations of the patient’s heart, eyes, feet, and nervous system are also important and the patient may also need to see the dietitian while at the unit. Medications, if required are discussed.

outpatient eduCation Outpatient education is provided for both type 2 and type 1 patients. The DAFNE (Dose Adjustment For Normal Eating) education programme, which is used to manage type 1 diabetes, provides people with the skills necessary to estimate the carbohydrate in each meal and to inject the right dose of insulin. This, in theory, allows patients to eat according to their lifestyle and preferred food. DAFNE was first developed in Germany, and has been refined continuously throughout the UK and Ireland. The aim of the programme is to enhance flexibility from a patient’s perspective. The freedoms that DAFNE promotes, in terms of diet, are proving it extremely popular. The patient information leaflets serve as an educational tool for patients and staff alike. They also ensure that patient safety is the central priority of diabetes care throughout the healthcare group. It was considered imperative that staff realised while diabetes was not the

As a result of this view the necessity for change was acknowledged and addressed at SVPH. In 2008, improvements to the diabetes services provided were considered paramount, with the need to standardise and improve the care of all diabetes patients, to provide consistent diabetes policies across the whole of the healthcare group. primary cause for hospital admission; the prevalence of diabetes in hospitals is at least approximately 12 to 25 per cent. The importance of ensuring optimal diabetes control for patient benefit was reiterated. Staff are more aware of the impact of hyperglycaemia and its associated increased morbidity/mortality, increased length of stay and cost and unfavourable post discharge outcomes, for example, wounds. Change in practice was instigated in the day care Oncology Department. It was recognised that oncology day care patients who were receiving high dose steroid treatment were at high risk of hyperglycaemia. Guidelines for the management of steroid/glucocorticoid-induced hyperglycaemia which also included a patient information leaflet on steroid induced diabetes, changed the care of the oncology patient, with over 50 referrals to the diabetes nurse specialist in just three months. The group share resources and compliment each other for diabetes service provision. Specialised areas within the group are accessed if considered in the best interest of the patient. In relation to type 1 diabetes education, St Vincent’s University Hospital and SVPH are currently providing the DAFNE week of structured education to patients within the group, with SVPH and St Michael’s Hospital also providing DAFNE advisory programme support to DAFNE graduates. Foot care services are also available to be accessed by the group if required.
The Insulin Working Group Insulin was recognised as one of the top five high alert medications on the list from the Institute for Safe Medication Practices. Insulin as a high alert medication carries a high risk of causing patient harm. The Insulin Working Group set about improving current practice to ensure patient safety and reduce the likelihood of errors in relation to insulin. It was considered paramount that the right drug is given to the right patient at the right time via the right route.Redesign of the insulin medication record was a priority of the Insulin Working Group and the Multidisciplinary Diabetes Team. The High Alert Medication Policy and the Hypoglycaemia/Glucagon Policy also addressed the need for clear, concise guidelines for staff. The insulin prescription medication chart has been updated as a result of the work of the Insulin Working Group. Pilot and clinical audit of the new insulin prescription was carried out from August to October 2009 in SVPH. 48

Both areas will have a huge impact on ensuring patient safety at ward level. The fixed dose prescribing section was considered an improvement. The pilot and audit were presented in poster format at the clinical audit study day for the whole of the healthcare group. The pilot and clinical audit in SVPH resulted in further improved changes. Hypoglycaemia treatment has now been added which allows for clear and easy implementation at ward level.
A collaborative effort In 2010, St Vincent’s Healthcare Group (SVPH, St Vincent’s University Hospital and St Michael’s Hospital) and St Columcille’s Hospital in Loughlinstown, were finalists in the Aramark Healthcare Innovation Awards, for this public-private healthcare collaboration. The strong partnership approach among the diabetes teams in the St Vincent’s Healthcare Group and St Columcille’s was highly commended by the judging panel. The work of the Diabetes Policy Development Group set up by Patricia Harkin (2009) and Insulin Working Group were highlighted and acknowledged as a major contributory factor in the projects success and once again setting the standard for best practice.

St Vincent’s Hospital Rationalises Support Suppliers and Delivers Enhanced IT Services for Less One of the country’s busiest hospitals, St. Vincent’s University Hospital has rationalised technical support vendors and enhanced IT service delivery at lower cost, with the help of leading IT services company MJ Flood Technology. By bundling a reactive support package with pro-active professional services, the company has helped the hospital’s IT department to deliver more reliable, more flexible and enhanced IT services for less. SVUH has an extensive mission-critical IT infrastructure with 4,000 PC users, 1,900 PCs, 160 servers, 500 printers and more than 100TB of production data. The hospital’s ICT team is under constant pressure to deliver 100% network uptime and deploy new technologies to support the highest quality patient care.

By integrating high value professional services into a single contract, SVUH have much better visibility of what they are paying for and what they are getting in return.” “We know from week to week how the contract is performing,” says Neal Mullen, deputy ICT director, SVUH. “We can clearly demonstrate to management how the contract is adding value to ICT operations and in turn helping us to focus on high value tasks such as IT strategy development rather than fire-fighting on a day to day basis.” Enhanced IT Service Delivery Under this service delivery model, the MJ Flood Technology team have assisted in a number of key ICT projects under the contract such as an Active Directory design and migration and a Microsoft Exchange deployment for 3,800 users. “It’s very much a team effort,” according to Neal Mullen. “Through MJ Flood Technology, we have technical skills and resources on-tap. While the incidence of breast cancer is unfortunately increasing amongst Irish women, breast cancer is currently ranked as the most commonly diagnosed invasive cancer in Ireland at 32.3 per cent, a trend that can certainly be traced back to increased awareness and improved services in diagnostic techniques. The Maple Centre at SVPH is a purpose-built breast imaging centre on the first floor of the hospital. The centre has been designed to facilitate breast imaging in a private and comfortable setting. The equipment at the centre is modern digital mammography and high frequency ultrasound machines and all patients can be assured they will receive the very best treatment.
triple assessment CliniC Women, over 35 years of age, with a breast lump, bloody nipple discharge, nipple retraction, skin tethering or ulceration are referred to the Triple Assessment Clinic. The Triple Assessment Clinic was established at SVPH in 2005 to provide a rapid diagnostic service to patients. At the Triple Assessment Clinic, the patient’s breast will be examined by a doctor and/or advance nurse practitioner. Then patients will have a full imaging workup together with a clinical examination. If there is a focal imagining or clinical abnormality, the patients will have a biopsy. The clinic is held in SVPH on Monday mornings. On arrival to the hospital patients are asked to check in at the X-ray department. There, they will be met by one of our administration team who will direct them to the radiographer to perform a mammogram and/or ultrasound. The choice between mammography or ultrasound for imaging depends largely on the women’s age, although women over 35 usually have both tests.
A mammogram is a special low-dose X-ray of the breast and generally takes less than 20 minutes to complete. It is capable of detecting breast cancer at a very early stage by picking up small changes in breast tissue – mammograms can identify breast cancer as early as two years before a lump is large enough to be felt. The images are obtained by placing the breasts in a special mammography machine. In this machine, the X-ray beam can pass through the breast and an image is formed on an x-ray film. The breasts must be compressed during the x-ray, this is vital as it reduces the radiation necessary for good quality images.

If intervention such as a biopsy or aspiration of cyst is required it will be carried out during this examination. During a biopsy, some tissue may be removed from the lump by means of a small needle through the skin (core biopsy). Local anaesthetic is usually used for these needle tests. to support and educate patients and their families who have been diagnosed with breast cancer. It is usual for the nurse to be involved with the patient at initial diagnosis in order to build a rapport and gain the confidence of the patient and their family. To ensure best practice, all of our patients who have had a biopsy are discussed in detail each week, at our weekly comprehensive multi disciplinary team meeting. Patients are kept involved in the decision-making process from the point of first referral and the breast care nurses are available throughout the entire patient journey. Patients with normal benign results are discharged following a multidisciplinary meeting.

Ultrasound initially
Women under the age of 35 who have clinical symptoms are usually referred initially for ultrasound examination, while women over the age of 35 are routinely offered mammography with or without ultrasound depending on the clinical setting but mammography is a more sensitive test for cancer detection in women over the age of 35. An ultrasound is a non invasive, non ionising radiation modality is extensively used in investigating a wide variety of conditions including disease of the thyroid, liver, pancreas, gallbladder, pelvis (ovaries, uterus and prostate) and of course breast. An ultrasound scan will take approximately 30 minutes. To produce an ultrasound image, a hand held probe called a transducer is used to transmit ultrasound waves into the body. The waves are reflected off the various tissues in the body and returned to the transducer as an echo. Using sophisticated computer technology (software and hardware), the ultrasound signals (echoes) are converted into electronic signals that are in turn converted into an image of the structures in the body. Patients who have a mammographic or ultrasound abnormality as a result of a routine referral are called back for further imaging and clinical examination, depending on the clinical finding, a core biopsy or aspiration is performed. Similarly, patients with a very strong family history or selected patients with a clinical indication may be referred for an MRI. 52

The Respiratory Department in SVPH includes the pulmonary function service and Sleep Disorders Unit. It is currently staffed by a team of six respiratory scientists. In the new SVPH, the PFT labs are located in the outpatients area on level one and the Sleep Disorders Unit and bedrooms are located on Rowan Ward on level four. According to the Irish Sleep Society, people spend up to one third of their lives asleep. Understanding sleep and how sleep disorders develop remains a hot topic in the field of sleep research but the ramifications of these
conditions are clear. Patients commonly suffer from disturbed sleep patterns which lead to daytime sleepiness. They become excessively tired and this may cause poor cognitive function, depression, accidents at work and significant risk of road traffic accidents. To differentially diagnose the type of sleep disorder requires a comprehensive history together with a physical exam and an overnight sleep study. SVPH’s Sleep Disorders Unit was specifically designed to diagnose and treat sleep-related breathing conditions and we have developed a stand-alone, purpose-built facility. The unit continues to lead the way in the diagnosis and treatment of sleep apnoea syndrome, which according to ISS, affects 4 per cent of the general adult population. The level of the clinical problem has allowed the hospital to streamline resources accordingly under the leadership of Professor Walter McNicholas, whose research includes a study on the risk factors associated with sleep apnoea and cardiovascular disease.
The Sleep Disorders Unit at SVPH is the most modern medical facility of its kind in the country and is staffed by highly trained and specialised respiratory scientists, which reflects the scale of the service. “Some other sleep laboratories in the country provide a much smaller service, which may make it difficult to provide the same level of expertise,” says Professor McNicholas.
“Here at SVPH, we have developed a state-ofthe-art sleep laboratory for investigating these sleep disorders, which is on a par with some of the best international sleep laboratories, and adheres to all international standards of best practice. The unit presently offers four fully equipped individual bedrooms that are specifically kitted out for sleep studies, feeding back into a central monitoring area which records all of the sleep related variables and, in my mind,

The diagnosis The initial diagnosis involves a specialist clinical assessment followed by overnight sleep studies in the laboratory, which require considerable expertise to perform and analyse. “Once the patient has been diagnosed with sleep apnoea, the treatment

with CPAP (continuous positive air pressure) is also highly specialised,” adds Professor McNicholas. The clinical assessment begins with a comprehensive medical history and a physical exam. Questions relating to daytime features are also relevant and in most cases feedback from the patient’s sleeping partner will give useful information. Patients are then referred for overnight diagnostic studies in the sleep laboratory. In addition to the technical study patients complete assessment questionnaires relating to sleepiness and its effects. The diagnostic study includes detailed monitoring of sleep and respiratory patterns and limb movements (polysomnography). Patient’s heart rate, body position and snoring intensity are also recorded. All studies are continuously supervised by a respiratory scientist, which offers reassurance to the patient and ensures a high standard of overnight recording. Data is gathered using the latest wireless technology and include simultaneous recording of integrated video which can subsequently be viewed during analysis of the study. Expert analysis of sleep and respiratory patterns follows.
Treatment Patients referred for treatment studies by CPAP undergo an education session with a respiratory scientist on the afternoon of their study. This includes an introduction to treatment, mask fitting and access to all the relevant information relating to the treatment,

which is constantly updated by staff. They are shown the different devices and masks currently available in Ireland. They also have a pulmonary function test. The treatment study involves overnight assessment of CPAP on an auto titrating device. The respiratory scientist on duty is constantly monitoring the patient and can intervene to address any issues that may arise during the study. The next morning the patient meets with Professor McNicholas and arrangements are made for provision of CPAP. The patient is provided with ‘hints on getting used to CPAP’ and contact details for the lab staff who offer support and follow-up, once on the treatment. Patients who opt for treatment with a mandibular advancement device (MAD) may be referred for a repeat sleep study while wearing the device, as an assessment of treatment efficacy.
World-class facilities Here at the Sleep Disorders Unit at SVPH, we take pride in providing state-of-the-art medical expertise to help treat patients referred to our service. Our team led by the chief respiratory scientist together with Professor McNicholas planned this unit to be at the cutting edge of new technology and innovation. The laboratory was designed in a fashion that would allow an increase in the number of patients investigated compared to the old hospital, and also with flexibility to expand further to meet future clinical demand.
Access to services Patients with suspected sleep disorders are referred for initial assessment in Professor Walter McNicholas’ clinic in the Consultants’ Private Clinic, following which they will be referred for appropriate sleep studies to the sleep laboratory. On receipt of the referral, a respiratory scientist will contact the patient directly and arrange an overnight admission for a sleep study. Following analysis of the study, the report is forwarded to Professor McNicholas for evaluation and clinical management decision.

SVPH’s comprehensive Day Care service offers a full range of procedures in a number of specialities.
SVPH provides consultant-led investigative and surgical day care services for patients referred by their GPs or consultants. The move to the new SVPH allowed the various day care departments to expand significantly. Located on the second floor of the new facility, we now have two state-of-the-art endoscopy rooms in our Diagnostic Day Care Centre, a fully functional operating theatre, an expanded Urodynamics Department and 30 day case trolley bays, which accommodate an average of 50 patients daily. Our location close to the main theatres facilitates a safe and stress-free transfer of patients to and from surgery in these theatres.
Day Care Ward Our 30 bay day care ward cares for all surgical day case patients admitted to SVPH. The clinical nurse managers lead a team of skilled nursing staff who provide an enhanced experience to the patients while ensuring world-class standards of care in a department with a rapid turnover of patients. Along with the procedures outlined in our various procedure rooms, patients also attend for therapeutic venesection, invasive radiological, urological and gynaecological investigation and treatments.

Diagnostic Day Care Centre The aim of the Diagnostic Day Care Centre is to provide a patient-centred clinical experience in an efficient, effective, and competent manner in order to produce a superior quality of service. The unit is staffed by specialists nurses and nurse managers who, along with the nursing staff on the day care ward, coordinate procedures on a daily basis. The investigation available in our Diagnostic Day Care Centre include: Colonoscopy Gastroscopy Bronchoscopy Flexible cystoscopy ERCP All procedures are carried out by our consultant staff and are arranged in advance. Direct urgent referrals are also arranged when necessary.
Day Surgery Our Day Surgery Department provides a wide range of surgical procedures in our fully equipped theatre. Our consultant surgeons and anaesthetists perform surgeries ranging from vascular procedure, ophthalmic injections, excisional biopsies, dermatology investigations and central line insertion to rhizotomies, nerve blocks and joint injections for patients attending with chronic pain conditions. This busy department is coordinated by our experienced CNM2 and her team of surgical nursing staff. This department also coordinates the dressing clinic which provides an outpatient service 64
for post surgical patients and patients with a variety of needs such as peripheral ulcers, central venous catheter care and pre-chemo phlebotomy.
Urodynamics Department The Urodynamics Department is managed by our CNM2 and staffed by specially trained nursing staff and provides a comprehensive service that is unmatched in other private hospitals in the area. We provide a full range of investigations and services including: Uroflow and bladder scan for post void residuals Cystometrogram/pressure flow studies Urethral pressure profile Self-catheterisation instruction Continence training
Infusion Suite Our Infusion Suite, which is located within the Day Care Department at SVPH, treats patients with chronic conditions such as rheumatology diseases, inflammatory bowel disease and multiple sclerosis. The service, which is currently accessible through consultant referral only, is growing rapidly and this has facilitated the appointment this year of a clinical nurse specialist in rheumatology and a clinical nurse specialist in neurology which will expand and enhance the care we provide to our patients. Please note: All patients need a GP/consultant referral to day care at SVPH.

Chest pain investigations The Chest Pain Clinic at SVPH is a consultant led service and has been developed to provide rapid access to our Cardiology Department and consultant opinion for your patients. The aim of the Chest Pain Clinic is to reduce waiting times for patients whose symptoms require urgent evaluation but do not necessitate a referral to an emergency department. The opening of the MAU will revolutionise the way patients receive rapid access to assessment and treatment at SVPH. The Medical Assessment Unit (MAU) at SVPH is being developed to provide rapid access to a range of investigations and consultant opinions for patients. The aim is to reduce waiting time for patients whose acute symptoms require urgent evaluation, but do not necessitate a referral to an emergency department (ED).
Accessing the MAU All patients must be referred to the MAU by their GP following a phone call to the nursein-charge. The MAU is not available for selfreferrals by patients.
Pregnant women Children Those suffering from an acute mental illness Trauma patients A clearly defined condition that requires subspecialist care, e.g. acute MI (heart attack), febrile neutropenia or CVA (stroke) Social or family circumstances that is affecting the discharge of the patient to his or her own home.

On arrival in the MAU a detailed history will be taken from the patient. Following review by the Consultant-on-call, tests may be requested and a plan agreed for the patient. However, if admission is necessary any charges paid that are reimbursable to the hospital by the health insurer will be refunded to the patient.
Patient suitability The MAU is available to all adult patients who are considered to be a medical admission and require a period of assessment and a diagnostic workup, following examination by their GP. Patients must be: Non-critical Stable and do not require resuscitation or stabilisation Eligible for admission to SVPH, in other words have private health insurance or the ability to self pay. Surgical Pre-Assessment Pre-op assessment is a very important step for any patient due to undergo a surgical procedure. At SVPH, we aim to make pre-op assessment as efficient and effective as possible. Traditionally patients were admitted to hospital a few days prior to surgery for their pre-operative (pre-op) workup and, pending the results, other clinicians/disciplines input was often required. This often led to delays or cancellations of operations. Pre-op assessment aims to have the patient as fit as possible for surgery by having their complete workup done as an outpatient, this means patients are less likely to have their surgery delayed or cancelled on the day, as any problems discovered at pre-op stage can be dealt with prior to admission.
The core aims of pre-op assessment include: To enhance the efficiency and clinical excellence of patient care. To reduce and prevent cancellations by having the patients pre-assessed. Providing an opportunity to identify other health issues and facilitating prompt treatment of any medical problems. To reduce bed occupancy and length of hospital stay, thus reducing costs. To reduce the number of patients who fail to attend for surgery. To increase the efficient use of operating theatre time. To facilitate informed consent. To improve safety by assessing and quantifying risk, and allaying fear and anxiety. To improve patient education.
Pre-op at SVPH At present, pre-op assessments are performed for patients at SVPH who are due to undergo orthopaedic joint replacements. In the coming months we hope to roll out assessments across all major surgeries.
a pre-op nurse to arrange an appointment prior to surgery. Patients are urged to bring any medication they are taking along to the appointment. The assessment includes a nursing and allied health review. The pre-op nurse may carry out some or all of the following: C omplete a detailed nursing assessment including medical and surgical history C heck blood pressure, pulse, temperature, weight and calculate the patient’s BMI S end the patient for an ECG (heart tracing) I f necessary, may send the patient for an X-ray I f necessary, may take some blood samples. After the nursing review, a physiotherapist will see the patient and advise him or her how to prepare for surgery and explain the physiotherapy treatments after the procedure. All assessments are different and the length of time may vary, but normally the patient will need to attend for approximately one to two hours.
What happens next? Following a successful assessment, the patient is passed fit for surgery and their doctor is informed. However, if during the assessment, a problem is detected, the patient may require some further investigations or another specialist referral, e.g. cardiology. This is co-ordinated by the pre-op nurse. Surgery may be delayed for a time until all investigations are completed. After any investigation, the pre-op nurse will followup the patient’s results with the orthopaedic consultant.
What does the assessment entail?
If a patient is suffering from undiagnosed high blood pressure, they will be referred back to their GP to manage and treat the condition further.
All patients due for pre-op assessment will have been referred by their surgeon following their outpatient appointment. The patient will then receive a telephone call or letter from

Patient information Admissions Patients are required to telephone the hospital on (01) 260 9300 on the morning of admission to confirm their bed. If you are due for admission on the morning of a surgical procedure please confirm with the Admissions Department the previous day. If you are unable to attend for your appointment, please notify the Admissions Department as early as possible. Admissions are normally between 2pm and 4pm. You should bring your own nightwear, dressing gown, slippers and toiletries. Hand and bath towels are supplied.
Medication If you are currently on medication, please bring it, and your current prescription, with you. All medication should be given to the nursing staff on admission. You may choose to have your own medication administered or medication provided for you by the ward or unit. If you are to take medication post discharge, please ensure that you have your prescription and that you understand clearly when your medication should be taken.
Health insurance Many insurance companies operate a direct payment scheme. Please refer to your insurers website for further details or contact our Finance Office on (01) 263 8919. Also please check with your insurance company if you will be required to pay any excess or shortfall for your hospital stay. On admission, patients should bring with them details of their health insurance cover, membership number and insurance plan as direct payment requires insurance form completion on each admission.
Discharge Families are asked to participate with staff in discharge plans for patients. We request patients to vacate their beds on the general wards by 10am and short stay patients by 9am. Please check that you have all of your belongings, including items left in safekeeping. Security Please do not bring valuables with you, such as jewellery or large sums of money. Where this is unavoidable, please use the security box in your room. Insert your own code and share it with members of your family as back up.
Patient safety For your safety we wish to point out the following: Use the call bell if you need assistance. Electrical appliances cannot be used without consultation with the Maintenance Department. Should you wish to leave the hospital building for any purpose, permission is required from your consultant. Following that please inform the nursing staff where you will be. In the event of a fire, follow the instructions of our staff and stay calm. Please note that in the interest of patient safety, it is the practice of the hospital to test the fire alarm at regular intervals.
Oratory An Oratory is available on the first floor for quiet prayer and daily Mass. A Roman Catholic priest is available from 9am to 5pm daily. Clerical representatives from other religious denominations also visit the hospital daily.

Location SVPH is located on the Merrion Road, adajacent to St Vincent’s University Hospital. You can get to SVPH By car: SVPH is located beside St Vincent’s University Hospital on the Merrion Road, the main Dublin to Dún Laoghaire road. Although underground car parking is available, space is limited and subject to charges. We therefore suggest that where possible intending patients and visitors use public transport as outlined below. A multi-storey car park is also available in the adjoining St Vincent’s University Hospital and can be used by patients but charges will apply. By Dublin Bus: The numbers 4, 4A, 5, 7 or 7A from the city centre service the hospital, and the number 18 travels from Rathmines and Crumlin close to the hospital. By DART: Sydney Parade DART Station is just a short walk (approximately eight minutes) from the hospital.

Sheridan’s shop is located on the ground floor near Reception. Opening hours are: Monday to Friday, 8am to 8pm. Saturday and Sunday, 9am to 6pm.
Pastoral care
Visiting times Visiting hours are 2pm to 9pm daily. Children must be supervised at all times. In the interest of infection control we ask visitors to refrain from bringing flowers into the hospital.
Connectivity All inpatient beds have a direct dial telephone number. You will be advised of your phone number on admission. Your telephone account should be paid at Reception prior to discharge.
Taxis
Mobile phones/walkie-talkies can be used in the hospital except where specifically excluded (please follow signs). A free WiFi service is also available to patients. Please register at Reception to connect to this service.
