As you know, our CEO position has been vacant for a few months. The Board has spent considerable time determining our best road forward as we know that CEO turnover has been a chronic issue. It came up several times in our survey results which confirmed that it is an issue that concerns our membership as well. After research and consideration by a task force of the Board, a recommendation was made to engage an Association Management Company. Request For Proposals were sent out and the responses were analyzed for the best fit for AVA.

After careful consideration of the proposals and interviews, the board selected Talley Management Group (TMG). They manage 35 associations, primarily in the healthcare space. They have over 35 years of experience in association management and have clients who have been with their company from their beginning. The partnership with Talley will begin officially on March 1st and will provide us with staff and services that we currently must outsource. We look forward to working with them to implement our strategic plan and help us move AVA forward.

Tonya Hutchison, Judy Thompson, and Blake Hotchkiss will be employed by Talley and continue to serve AVA as they have in the past. We anticipate that TMG will provide them valuable support in their roles and will help us move AVA forward as an even stronger organization. We appreciate the work that Kayce Maisel and Jennifer Finlayson have done for AVA and wish them well as they move to their new endeavors.

CALL FOR 2023 PEDIATRIC/NEONATAL GRAND ROUNDS SUBMISSIONS

Vascular access specialists face and solve numerous care challenges every day. As you read this, likely a recent challenge with a patient and your solution may come to mind. You may have wondered if any of your colleagues have encountered this type of situation and what worked for their patient. Wouldn’t this be important to share? This is exactly the collaborative magic that vascular access specialists thrive on.

The Pediatric/Neonatal Grand Rounds will now be in its 8th year this fall. This session was conceived to provide a national and now international forum incorporating real cases representing

• A unique presentation of a vascular accessrelated complication

• An unusual vascular access challenge or outcome

• An unusual vascular access complication

• An ethical challenge or controversy related to vascular access

• A patient-safety related concern or case

Since its inception, participation in the Pediatric/Neonatal Grand Rounds has provided a welcoming venue for those new or experienced in presenting to grow in national engagement. This event has consistently shown its value in building the community of pediatric/ neonatal vascular access clinicians. At the Annual Scientific Meeting, this session has grown in breadth and popularity each year and has consistently reached “standing room only” each time.

This year, the AVA Foundation presents its Premier National Vascular Access Grand Rounds. Similar to the Pediatric/Neonatal Grand Rounds criteria, clinicians are encouraged to consider submitting their cases of vascular access challenges in patients over 18 years of age through the lifespan. These include a unique presentation of a vascular accessrelated complication, an unusual complication, challenge or outcome, an ethical challenge or controversy or a patient-safety related case. The submissions will be peer-reviewed and accepted presenters will be recognized for participation in the first Association for Vascular Access (AVAF) National Grand Rounds. Your work is immeasurably important to your patients every day. Please consider sharing unique situations meeting the above criteria to help your professional colleagues grow to ensure more patients benefit from optimal vascular access practices. The Call for Submissions for the Pediatric/Neonatal Grand Rounds and the AVAF Ground Rounds will be forthcoming shortly. One of our greatest benefits of AVA is learning from each other to advance vascular access care. We look forward to learning from you!

AVA 2022 CONFERENCE HIGHLIGHTS

Another

conference has come and gone!

The energy was surreal as we all gathered in Minneapolis for yet another successful conference. Henry Ford states “Coming together is a beginning; keeping together is progress; working together is Success”. We would like to share some highlights of this year’s conference.

We kicked off conference with our Pediatric and Neonatal Hand’s on workshop. This pre-conference had the most registered attendees to date! The stations included, ultrasound training, advanced insertion techniques including MST and needle over the sheath in pediatric and neonatal PICC insertion, setting up a sterile field/donning sterile gloves and gown. The session concluded with a mock PICC insertion. We ended the evening with our annual PediNEOSIG reception. We were surrounded by good company and cocktails courtesy of our generous vendor sponsors.

The outstanding presentation touched on how far we have come in vascular access since sharpening and sterilizing our own needles to what the future holds for vascular access to thrive in the healthcare system into the next decade.

Congratulations to Mary Beth Hovda Davis, MSN, RN, VA-BCTM Janet Pettit Scholar. AVA and the Pediatric Neonatal Special Interest Group recognized Mary Beth as the 2022 Dr. Janet Pettit Scholar. Mary Beth has provided visionary leadership for neonatal and pediatric clinicians. She has served on the PediNEOSIG ELC in various roles and has been an active member for 14 years!

Mary Beth highlighted key components of vascular access care of the pediatric patient including family

centered care, the unique challenges that the pediatric population pose and discussed how vascular access programs can navigate these challenges and advance practice. Thank you for your dedication to our most vulnerable population.

The Executive leadership council would like to continue to embrace our current and future members. This allows our community to stay connected throughout the year as we continue to provide educational experiences for our neonatal and pediatric clinicians. Looking ahead, we are planning to offer a webinar in the spring, details to come.

STORIES FROM THE FRONTLINE: AN INTERVIEW WITH GEORGIE ELSON

I believe interviews of patients with acute and chronic conditions will best spotlight what life is like on the frontline when they are dependent on vascular access. The patient’s perspective can and should inform what we do and how we do it. For this issue of IV Quarterly, I interviewed Georgie Elson.

Georgie depends on central vascular access for hydration and multiple intravenous (IV) medications. Mx. Elson is a nonbinary patient with a complicated medical history that thwarts standard-of-care and underscores the need for vascular access specialists. A specialist needs a deep understanding of all options available for the nonstandard patient.

Georgie suffered a traumatic brain injury in 2016. As a result, their Ehlers-Danllos Syndrome (EDS) and Postural Orthostatic Tachycardia Syndrome (POTS) were exacerbated. EDS is a group of inherited disorders that affect connective tissue. This syndrome results in hyper-flexibility, fragile skin, and poor wound healing.1 POTS causes the heart to beat faster as an inadequate response to transitioning from sitting to standing.2

In addition, Mx. Elson has Mast Cell Activation Syndrome. This syndrome releases substances that produce signs and symptoms similar to an allergic reaction and severe inflammation.3 A daily necessity for hydration and other IV medications escalated their vascular access needs from an occasional short peripheral IV to long-term venous access.

According to Georgie, “My PICCs [peripherally inserted central catheters] would last 7 months at the longest before my skin would blister and weep from the adhesive stabilization or there would be an infection. Ultimately, my doctors advised that I switch to a Port [implanted vascular access device] to avoid adhesive issues, but they had to remove it when skin would not heal”.

Due to their EDS, the daily needle insertions and removals into the port eventually caused the skin to erode, exposing the silicone septum. Georgie then had a pediatric port placed in her chest near the right axillary fossa; unfortunately, poor placement, a right-handed person performing their access, and slow wound healing led to a port-pocket infection.

Mx. Elson explained what happened next, “I was back to a PICC after the ports, and the first PICC only survived 3 months. Because I am a full-time wheelchair user until I have bilateral hip surgery

and my CSF [cerebral spinal fluid] leak has yet to heal, extra trips to the hospital are extremely hard for me, even when necessary. So although my PICC was working great, the blistered skin from adhesive securement was causing yet another catheter replacement and unnecessary trip to the hospital”.

Following the last PICC failure, Georgie consulted friends and social media support groups to find an answer to their securement problems. Even though Mx. Elson utilized multiple brands of adhesive securement throughout their life with a PICC, the same skin reaction impacted them every time. With the next PICC, they wanted to try something completely different.

Describing their discovery and use, Georgie said, “On social media, I found SecurAcath [subcutaneous anchored securement system] discussed by others with adhesive reactions. I researched the company’s website further and tried to figure out how I could get the device placed on a new PICC. The hospital I normally go to does not have this device and was unable to get it just for me. When they placed my current PICC on January 19th, 2022, they left 3cm out so I could get a Securacath placed later. After just 1 week with the standard adhesive securement, my skin was itchy, sore, and inflamed. I got the Securacath placed at home during a routine dressing change saving me an additional trip to the hospital. My Securacath held my PICC in place with no adhesive, no movement, and my skin reaction healed quickly. My home care nurse loves being able to clean all around the site without the usual adhesive gunk left behind.” 4

When asked about the difference between adhesive securement devices and subcutaneous anchored securement systems, they answered, “For me, there is no question that eliminating the adhesive was critical for me to maintain my PICC and avoid complications. With adhesive securement, I had constant itching, burning, and soreness under my dressing; with Securacath, there’s none. My home care nurse has gotten used to the Securacath holding the PICC in place while she cleans and dresses the site. She says she has never seen a better PICC site.”

“When I was dependent on various adhesive securement, my PICC would often back out a little with every dressing change, and sometimes my line would get shoved back into my arm accidentally. With Securacath, there is none of that because it stays with the PICC all the time. I have had my PICC for over a year and had the Securacath replaced once. Unfortunately, because of my unique connective tissue problem and the original placement near my elbow, the repeated bending eventually caused skin irritation after almost a year in place. Sadly, I had to make a 2-hour trip to the nearest hospital with Securacath as an option. It was quickly and painlessly replaced with a new angle to avoid a future issue.”

When asked what words of wisdom they have for the vascular access specialists who may read this interview, Georgie said, “I think Securacath should be at every hospital as a securement option. On social media, there are support groups for people dependent on central lines and many posts about reactions to adhesive securement. So if you are the vascular access specialist taking care of us and our lines, you should know that an alternative to adhesive can help save our skin and our lines.”

We invite you to submit original manuscripts in the field of Vascular Access. We are interested in receiving manuscripts on clinical practice, education and research related to vascular access including articles on vascular access manufacturing and technology, and vascular access care and maintenance issues in hospitals, home settings, hospice, and alternative care facilities.

We also invite submissions to our Patient/Consumer Perspective column where we ask you to share personal stories or “lessons learned” about caring for, living with or having a vascular access device. In about 1000 words and in conversational style, present your story. You can submit on behalf of someone or encourage them to write it themselves.
