bestbites Fresh French Onion Soup Days slowly grow shorter and leaves begin to crinkle as the calendar flips over to autumn. A crisp night calls for a warm bowl of this fresh, homemade French onion soup made from a recipe courtesy of John Szymanski, executive chef and food safety manager of The Kroger Co. INGREDIENTS: 1 Tbsp butter 2 lbs Vidalia or sweet onions, sliced julienne ½ cup sherry 2 qts chicken stock ¼ tsp fresh thyme, chopped Sliced baguette – ½” thick Swiss or Gruyere cheese as garnish
Sauté onions in butter in large stockpot until browned. Deglaze with sherry and let reduce. Add stock and thyme and simmer one hour. Season to taste with salt and pepper.
Make croutons by placing baguette slices on a sheet pan, then baking in 350oF oven for 10 minutes or until golden brown. Ladle soup into serving bowls or crocks. Top with two croutons (optional) and sprinkled cheese; place under broiler two additional minutes until melted. Sprinkle fresh thyme leaves over top as garnish.

This person can help record and recall conversations with providers.” Maybe someone in the support network could assume a designated administrative role, managing notes, reports and schedules. “Staying organized seems to help patients cope,” comments Sara Owens, BSN, RN, OCN, breast health nurse navigator with WellStar Kennestone Hospital. “They tend to feel more in control, can better communicate with their treatment team and are better able to advocate for themselves.” Owens explains that when patients are first diagnosed, they are in the acute
Hearing that diagnosis, watching the physician’s lips move as they pronounce the C-word, lingers as a surreal moment, with time standing still. There’s no way to accurately anticipate the reaction to such news. Will the words be heard? Will the brain even process the information? When it’s time to get the test results from the doctor, don’t go at it alone. “The impact is big from day one,” acknowledges Anita Johnson, MD, FACS, Breast Surgical Oncologist at Cancer Treatment Centers of America® (CTCA) in Newnan, and she addresses that impact at her initial meeting with the patient after their diagnosis. “The first thing I ask is: Who helps take care of you? A Hint of Tint Allow time to absorb information. Don’t hesitate to get a second opinion or third— whatever necessary to fully understand what’s happening in the body and what the road to recovery might look like. Trust that the best decisions will be made by evaluating all the different opinions. Go ahead and shop it around. This is hard. Talk about it. 30

Risky Business? Dana felt fortunate when, in 2013, her breast cancer was discovered at an early stage. As she puts it, she “got off easy with a lumpectomy, lymph node removal, radiation and the lovely drug Tamoxifen.” She wasn’t entirely surprised when her surgeon and oncologists suggested genetic testing (for the reported breast cancer genes, specifically BRCA1 and BRCA2), Dana Barrett since her mother Host of “The Dana Barrett Show” on biz1190 AM had also had breast cancer. But she hadn’t realized that she fell into a category of patients that raised an eyebrow, given a combination of risk factors: • • • •
She’s of Ashkenazi Jewish descent. She was under age 50 when diagnosed. She has a daughter to consider. Her mother had breast cancer.
While she waited for results, she confesses to anxiety over how she’d handle testing positive. She’d have to consider other surgeries—maybe a double mastectomy or oophorectomy—and then worried about the implications for her daughter. “The whole thing is about risk and chance and percentages and odds,” Dana laments. She tested negative for the three most common mutations, but was encouraged to retest at the five-year mark as “they keep learning more.” Meanwhile, she empathizes with women who get a positive result on those tests, knowing that it’s “one of the most confusing, stressful, scary things a woman can hear.”

One night in 2012, while shifting to get comfortable on the sofa, Jennifer felt “something weird” on the side of her chest. Within a few days, she’d seen doctors and received a breast cancer diagnosis at the young age of 31. “I went into full research mode. I met every possible doctor.” As Jennifer gathered her strength for the fight ahead of her, she began talking Jennifer Lingvall Founder of Wigwam Wellness Festival to friends and asking questions. She’d spread so much joy and love to others over the years that now her community rallied around, ready to support her. From doctors and therapists to gifted healers and nutritionists, her friends helped her build a network of resources, which she now shares with others. “I kept Google docs of my experiences and notes as I was making decisions. I pushed for as much information as possible. I wanted answers. I think it’s important to take the reins for yourself. It’s okay to challenge your doctors; empower yourself!”

“Build clear and consistent communication with your healthcare team,” Gamblin says. “Know who you can call and when … the healthcare team is there for you; you are not a bother!” Developing a partnership is a crucial part of the approach, as Torres describes. “Having honest conversations about prognosis, the potential benefits and risks of treatment, helps patients make informed decisions about their care. It also helps patients to align their priorities so that they and their families are prepared for any outcome.” “Cancer research continues to progress on a day-to-day basis,” says Shefali Shah, MD, chief of medical oncology and hematology at Kaiser Permanente Georgia. “Therapy is becoming increasingly individualized.” Part of that individualization begins with the team assembled for each case. “Developing a personalized plan always involves a multidisciplinary approach,” confirms April L. Speed, MD, oncological breast surgeon and member of the Komen Atlanta Board of Directors, requiring an entire care team to collaborate and form a plan. For some patients, this can mean combining a traditional medical approach with a variety of alternative therapies, and their treatment teams will have resources for all the options. At CTCA, each patient receives a plan that includes a variety of modalities with oncologists, radiologists and surgeons, joined by specialists in physical therapy, mind/body therapy, naturopathy, chiropractic care, acupuncture and nutrition. Asaf Yalif, MD, FACS, of Y Plastic and Reconstructive Surgery, agrees, “By having open communication and transparency, we can make sure that we put the patient’s personal journey at the forefront. Finding the Right Help Karen confesses to being that classic patient who noticed something but waited. She had a mammogram and the doctor saw something, but Karen didn’t act on it. She went back six months later for thermal imaging. Then she started her search for the right physician. “I was frustrated with Karen Reynolds Corporate Trainer the first doctor. “With CTCA, my experience was different from the first phone call. Once I got there, each step of the process was set out for me and, after a threeday assessment period, I had a full treatment plan. After eight rounds of chemo, a lumpectomy and six weeks of radiation, I rang the bell for my last treatment on November 18, 2014. “Chemo brain surprised me, I didn’t know about the impact on short-term memory and organizational skills. I wrote everything down, but then I lost the notebook. I’d take notes in strange ways and later forget what they meant. Visual clutter could bring on a meltdown. But, a cognitive therapist at CTCA helped me learn new strategies and suggested brain games. The CTCA survivorship program helped me resume my normal routine. When you’re so focused on getting well, you neglect other things. For me, some things have finally clicked. I’ve finally cut sugar out of my diet. I’m working on portion control and eating from the rainbow. “Be patient and understanding [of those in treatment]. Sometimes, we don’t even know what we are feeling, much less the right way to express it!”

Also, there are amazing computer applications specifically geared toward keeping track of medical information and cancer treatments, many of which are free. Chemo Brain Doc Notes is one that can help patients keep track of information, appointments and side effects related to their chemo treatments.” When it comes to emotional support, some feel that’s harder to quantify. Owens sees this as a pivotal role, influencing the patient’s ability to cope emotionally. “Having a support person who can act as a ‘safe place’ for the patient to express their worries and as a sounding board for decisions is important to their outcomes.” Shah suggests that patients and caregivers consider reaching beyond immediate friends and family, “Some find that joining a support group where they can talk to others going through a similar experience is very comforting. If the idea of speaking to strangers does not appeal to you, social media and other online forums are also a great way to build a network of support and connect with others.”
A Rose by Any Other Name The doctors, nurses, technicians, therapists and experts that are part of the treatment team have dedicated their lives to this battle. But they also count on patients to teach them new things along the way. Knowledge about the disease is constantly developing. Communicate with your team every step of the way. BEST SELF ATLANTA

Setting Priorities “I’m fourth generation on my dad’s side—my great-grandmother, grandmother, greataunt and dad had breast cancer in their 70s, and my mom had breast cancer at 42 like I did. When you get sick, no matter the diagnosis, wellmeaning friends, family and even strangers have opinions about what you should and shouldn’t do, which is often more overwhelming than the Dominique Love diagnosis. There is no CEO, Atlanta Food & Wine Festival and IWSC Group North America silver bullet saying, ‘If you do this, you will never get cancer again.’ And, I did get cancer again. Six months after the breast cancer diagnosis, I was diagnosed with thyroid cancer. Both cancers were highly treatable and surgically removed from my body, but there was a bigger issue here: I had cancer twice in six months. Something needed to change. I needed to tune out all the ‘what you should do’s’ and develop my own health plan. “Years ago, a business coach taught me how to prioritize actions to achieve goals through an exercise called a priority star. My overall goal was better health, so applying the logic of the priority star, I mapped out how sleep, nutrition, exercise, family time and me time worked together toward that goal. Going further, I recognized sleep as the number one priority—I make better food choices, my workouts are more productive and I’m more focused and engaged with family when I’m not exhausted … then, I don’t feel bad about taking me time. When sleep became my priority, I gained energy, focus and increased stamina. My small changes are having a ripple effect.”

Torres looks to her patients for cues on how she can enhance their care. She says, “My research on quality of life and side effects of breast cancer treatment has been largely driven by conversations with my patients. I like being asked, ‘Why?’ by my patients because, if I don’t know the answer, I view it as an opportunity to conduct research to figure it out.” For Johnson, constant learning fuels her passion for patient care. “For me, through my training, breast was the specialty that was always evolving,” she remarks. “I love the science behind it, and I love taking care of other women.” Which is why Johnson focuses heavily on reconstruction, asserting, “I want to help all my patients as they get over the thought that they had breast cancer. I’m a woman, too. It makes me feel good when you are looking good!”
In Bloom Mindset plays a crucial part in the cancer battle. Mind and body work in tandem. If there’s ever a time where the axiom “attitude is everything” holds true, it’s when that attitude guards the arsenal of a cancer warrior. Torres says, “I try to get a sense of how enthused a patient is to pursue traditional treatments, how aggressive they want to be in their treatment plan, what are their competing responsibilities and how much they value their quality of life.” Allowing for every decision to be intensely personal gives a patient an active role in this ongoing cancer drama and also gives them hope to achieve the life they want in recovery. “One particular lady that I’ve been working with,” reports Owens, “has every barrier you can think of when it comes to getting care—no family support, BEST SELF ATLANTA

CARLETTA CUNNINGHAM Carletta’s devoted to her active lifestyle and, since finishing treatment, has completed four triathlons, two 2-day walks of 30 miles each and now prepares for a 120-mile kayaking and biking trek from Key Largo to Key West. “I have a strong family history of cancer on both sides and my maternal grandmother died of breast cancer. So, I had regular mammograms and a normal screening in October 2012. However, for a few months I just didn’t feel well and couldn’t shake it. When I went to

my primary care physician (PCP) office in March of 2013, they scheduled me for an ultrasound. That’s when it showed up. I had a biopsy and, by June 2013, a bilateral mastectomy. Then I found out I needed 16 rounds of chemo and that scared me more than the surgery. I was scared of what I didn’t know and what I didn’t understand. I thought: ‘HOW will this attack my active body? What will it do to me?’ “I decided that I wanted to live a life WITH and ON purpose. I bought a calendar for the next year and made plans for each week—specific plans and

goals. I look at this new normal, what has been given to me, and I want to use the platform to help. I’m intentionally putting myself into situations that keep me in the present and out of my head. This diagnosis made me adventurous. I call it my ‘Keep It Moving Mission!’”
YVONNE EDWARDS Yvonne is entering her second battle with breast cancer, after first digging into the trenches almost 20 years ago. “Actually, I’d had a lump in the one breast since age 14 and doctors had always watched it. I had no family history of cancer. When I was 30, I had some discharge from that breast. I immediately had a biopsy and was diagnosed. I had a lumpectomy, radiation and chemotherapy. Back then, I had a ‘superwoman’ attitude and wanted to keep this to myself as much as I could. I pushed away help. I powered through, tried to keep working and wore myself out. When I finished treatments, I tore up all my paperwork. That part of my life was over. “Earlier this summer, I felt something again. Now, 20 years later, there’s another lump. I already had an appointment scheduled with my PCP and then scheduled a mammogram, following up with an ultrasound. This time I’m telling everybody. Everybody. We gotta talk about it. I still feel the damage I did to myself the last time, trying to keep it all to myself. Now, I see that this is a sisterhood. A sorority.”
TRELLIS USHER Trellis admits to postponing her first mammogram for far too long and thinking that breast cancer was something that would never happen to her. “My message is about early detection. Yes, YES, you DO want to know!” In 2014, while Trellis was out with some girlfriends, their conversation turned to mammograms. When Trellis, then age 44, confessed she hadn’t yet had one, her friends vowed to nag her until she did. First, Trellis had a checkup with her OB/GYN, who found a lump. Soon, she learned she had triple negative breast cancer. “In my case, I did the chemo before the lumpectomy and then the radiation. And, I wanted to deal with it. I kept working as I could. When you tell people … everyone has a point of view. There’s an overwhelming amount of information.” Trellis used closed discussion groups on social media to keep friends and family updated and treasures the support she received. She also heralds the invaluable support from her breast cancer community: “This is the best group I never wanted to join!”

dangerous living situation, language barrier, cultural differences, inconsistent employment. I was able to work with my team, including the doctor’s office, a social worker and a case manager with the YWCA of Northwest Georgia, to get her into a safe environment and started with treatment. She’s been amazingly strong and resilient!”
Glowing Forward Many patients find the transition from oncology to general medical care with a primary care physician (PCP) confusing— especially if no prior relationship with a PCP had been established prior to their cancer journey. Kim Randolph, MSN, FNP-BC, spearheads the survivorship program at CTCA and recognizes the need to help patients find a new routine for regular medical care. “Our goal for survivorship is to be that bridge between active treatment and surveillance,” Randolph explains. “We help with any side effects or residual problems patients may have, but we also examine the new things someone may be dealing with. There can be some emotional and psychological issues at that time.” A huge part of Randolph’s mission is monitoring patients’ overall health. “Some become so focused on the breast cancer that they ignore comorbidities,” she reveals. “I strongly encourage them to find someone in their community for follow-ups.” Once patients establish a relationship with that PCP, Randolph and her team send over their survivorship care plan, called Journey Forward, documenting all their providers and a brief summary of treatment. “I am big on primary care,” Randolph says. “I want everyone to have that PCP; someone who knows them when they are sick and when they are well!” BEST SELF ATLANTA

About 10 years ago, while managing a salon, Rebecca covered the wig studio for an absent employee. Gradually, Rebecca became more involved in the studio and with the clients served; soon, her career took a powerful new direction. Now, she specializes in hair replacement for women losing their locks due to medical conditions. “Being able to provide hope and happiness to clients struggling through a difficult health crisis is an honor I cherish. It gives me a purpose greater than who ‘Rebecca’ is on my own. For some reason, I feel like I am supposed to be doing this.” Three of Rebecca’s clients share part of their journey with us:
TRACY STARR | ATTORNEY “My annual mammogram was clear, but a few months later, I felt a lump near my clavicle. I was diagnosed the day before Thanksgiving, started treatment on New Year’s Eve and had a mastectomy on April 15 of this year. At this point, I’m still in it. My port comes out in December. I gained traction by having a plan broken down into steps. I wanted [doctors] to really talk to me—to treat me, not just my diagnosis. I interviewed several [different physicians] and eventually put together my team. Having all the information up front can be a great overview, but it can also get scary. At one point, I put down the books and started reaching out to individual people on a stage-by-stage, question-by-question basis. Look for those people. Look for the mentoring and advocacy groups.”
TRACY SCHWARTZ | MOTHER OF THREE “I’d finished breastfeeding the baby and hopped into the shower; I found a lump. Though a mammogram appeared clear, the lump was so palpable I had an ultrasound. I went into survival mode. I am so lucky that my whole family lives in Atlanta for support. At first, I assumed the doctors would just tell me what to do. But, really, most of this was up to me making my own educated decisions. I learned how to ask hard questions. Advocate for yourself, even with the best doctors. There’s always new data, studies and information. However, I don’t have the luxury of time for a long-term study. So I push the envelope. I’ve worked with traditional doctors and tried integrative methods. Some medical professionals are more open to that than others; don’t be afraid to ask.”

“My very first mammogram, ever, and the tech saw something. Ultrasound showed more; biopsy confirmed cancer. I tried not to panic. I sat in that room by myself and I wanted to start running like Forrest Gump. I didn’t want to hear it. My daughter went through cancer treatment when she was two, so my children already know this. They were worried that I’d be away for long periods of time, but so far that hasn’t been a problem. I have a great support network of family, friends and fellow teachers. But, I am still working through this and sometimes it’s hard for me to ask for help. On chemo days, I need someone to take care of me. I try to get through each day. Sometimes I have to focus on getting through hour by hour. I just finished chemo and will soon have surgery and radiation. Rebecca explained what it would feel like when my hair started falling out. It’s very emotional. Once the hair is gone, there’s no hiding it anymore.”

When she discovered that a drug used in her cancer treatment, Herceptin, had been developed in part by Komen funding, she became an active supporter. “My cancer was aggressive and so was my treatment ... Herceptin ultimately saved my life. Thanks to Komen and its many supporters, I survived.” In 2013, Cati became Executive Director of Komen Atlanta, vowing to work tirelessly for the continued efforts of this team. This year, Komen Atlanta celebrates 25 years as a local resource for women who need screening, diagnostics and support services. In that time, Komen Atlanta has raised $44 million, funding local breast cancer programs and global breast cancer research.
TurningPoint JILL BINKLEY | Executive Director In 2003, Jill founded TurningPoint Breast Cancer Rehabilitation after her own breast cancer journey. Her treatment caused a variety of issues, such as lymphedema, and, as she researched solutions, she discovered that: (1) she was not alone and (2) local resources were limited. So, Jill, an experienced physical therapist herself, began developing the mission and assembling the team that would eventually become TurningPoint. “Everyone on our staff specializes in breast cancer rehab,” Jill says. “Each of us has gone through it ourselves.” From cording to chemo brain, neuropathy to nutrition, massage to mental health, TurningPoint addresses the after effects of breast cancer treatment. In addition to offering rehab, the clinic furthers awareness through evidence-based research. TurningPoint continues to grow with community support, grants and private donations.

Yalif greatly values the inspiration he’s received from his patients. “I think the most profound thing I have learned is the importance of remaining positive in times of great duress. Sometimes, even if you can’t see the light at the end of the tunnel, you must believe it’s there.” “Each patient’s journey is unique and their own,” says Gamblin. “There is no right or wrong way to be a cancer patient. Take care of yourself, allow others to help you and know that you are not a statistic. You are you.” And remember that this is only one chapter of an incredible life story.

INTEGRATIVE MEDICAL CARE FOR BREAST CANCER Integrative medicine emphasizes personal empowerment to choose a healthy lifestyle that can help prevent cancer from occurring, support the body through treatment or prevent recurrence. Many factors may not be in our control, but certain lifestyle choices are.

During cancer treatment, food should not only support the body, but also minimize the adverse reactions and even possibly potentiate the treatment effects. Eat a plant-based diet rich in cruciferous vegetables (broccoli, cauliflower, Brussels sprouts, cabbage), avoid trans fats and increase the amount of omega-3 fatty acids (fish, flaxseed, walnuts) consumed. Avoid sugar and processed foods. Buy organic as much as possible. One great website to check for information is Environmental Working Group for their extensive database including food, safe cosmetics or cleaning products, to name a few. Drink 2-3 cups of green tea a day and don’t forget your water. Incorporate mushrooms in your diet, particularly the medicinal varieties such as maitake, reishi and shiitake.

Use dietary supplements cautiously and always check with your healthcare practitioner. Some of the most common supplements used are aloe vera, vitamin D products, melatonin for sleep and milk thistle for liver protection.

Consider testing for intracellular nutrient deficiencies and replenish accordingly to make sure your body has enough resources to detox and heal.
Make healthy lifestyle choices a part of your routine and attend to self-care every day!

Exercise! Combine aerobic activity, such as walking, with strength training and also consider more meditative movement like yoga or tai chi. Many studies actually show improved strength, immunity, well-being and survival rates.

Try alternative treatments for the various chemo adverse reactions: acupuncture is great for fatigue, nausea, postmenopausal symptoms, pain or lymphedema. Massage therapy is great for anxiety, insomnia or pain; look for a massage therapist who is trained to work with cancer patients. Any form of stress reduction therapy also helps: meditation, art therapy, music or mindfulness-based stress reduction.

partners in health Georgia Reproductive Specialists Fertility Preservation: Building Families One Step at a Time Today’s woman really can have it all, it’s just a matter of careful planning. While no medical therapy can halt the ticking clock that is associated with fertility, oocyte cryopreservation (“egg freezing”) can offer women time for career, education, personal goals or even cancer treatment before building a family. “Many women don’t know anyone personally who has gone through fertility treatments, including egg freezing, so they don’t consider that egg freezing is for them. They think that egg freezing is unaffordable, but many insurance companies and employers are now offering fertility treatment benefits,” says Dr. Desireé McCarthyKeith of Georgia Reproductive Specialists (GRS). The technique for egg freezing is the same as that used for in-vitro fertilization and laboratory methods have improved considerably in the last 10 years. Pregnancy rates are not affected by the woman’s age when she returns to use her frozen eggs or embryos and success rates with frozen eggs may be comparable to those with fresh eggs. “At GRS, we offer sperm freezing, egg freezing and embryo freezing when sperm and eggs are available at the same time. We work with couples, single men and women and LGBT patients to help build their families,” says Dr. McCarthy-Keith. It is best to freeze your eggs when you are in your mid-thirties or younger. If you are considering egg freezing, for medical reasons or otherwise, you should consult with a fertility specialist without delay. A simple ultrasound and blood work can determine if the egg supply is normal and if you would be a good candidate for egg freezing. Once eggs are frozen, they can be safely stored for years until you are ready for pregnancy.

Varicose Veins Are Only Cosmetic The presence of varicose veins and often spider veins indicates that a patient may have underlying venous disease. They may be a sign of an underlying circulatory problem that should be treated.
Only Women Get Varicose Veins Although women are more likely than men to develop venous disease due to the hormonal effects of estrogen and progesterone on the vein walls, many men suffer from venous disease as well. Because venous disease is hereditary, if a family member has this disease, you are more likely to develop it, whether you are male or female.
Crossing Your Legs Too Much Causes Varicose and Spider Veins This is not true. Venous disease is hereditary and worsened by pregnancy, but is not related to crossing your legs frequently.
Varicose and Spider Veins Aren’t Painful or Symptomatic Most patients with venous disease do have symptoms, although they may not attribute them to venous disease. Symptoms such as burning, itching or throbbing over the veins are common, as well as fatigue, heaviness, aching and restlessness. Severe cases can develop hardened, thickened and discolored skin, which may lead to open sores called ulcers.
Varicose Veins Are Always Visible Patients can have varicose veins without necessarily seeing the large ropey veins bulging from the legs. Sometimes the veins may be deeper but can still cause symptoms such as swelling, throbbing leg cramps and fatigue. Often these veins can be felt as lumpy areas under the skin. A venous ultrasound can determine if varicose veins are present.
Women Should Wait Until They Are Finished Having Children to Deal with Spider and Varicose Veins This is one of the most common misconceptions about the treatment of venous disease and it is often physicians themselves that propagate this myth to their patients. Older treatments for venous disease were often inadequate and led to frequent recurrences, so many physicians recommended their patients wait. New treatments, however, are safe and much more effective, so treating the disease before or between pregnancies can prevent patients from having the pain and discomfort of venous disease during their pregnancies. Founded in 1983, Vision Rehabilitation Services (VRS), a 501(C)(3) nonprofit, helps visually impaired, blind and deaf-blind Georgians live independently. The organization works in more than 30 counties to offer comprehensive vision evaluations, provide orientation and mobility classes, facilitate independent living skills, and give access to technology training, career guidance, diabetes workshops and counseling services.
