Hello Summer! Did you know that almost a quarter of you don’t apply suncream during the summer or when on holiday? And men are twice as likely as women to forgo protection! Before I started working in the medical aesthetics industry seven years ago, I lived in Australia where I was, to be fair, religious with suncream application in the summertime to avoid burns Editor and, of course, skin cancer. But what I didn’t know Shannon Kilgariff until I had met and interviewed countless medical aesthetic professionals here in the UK is how much the sun impacts how we age – and that you need to put it on all year round! From sun spots and pigmentation to wrinkles and saggy skin, the sun’s rays have a huge impact on our appearance. “Using toothpaste on a spot will make it disappear quicker” DR LAND SAYS: Putting toothpaste onto a spot has been an at-home remedy for too long! The idea that it can dry out your spot and kill off the bacteria has led to its continued popularity, but it could be doing more harm than good. Toothpaste tends to contain some harsh ingredients – great for scrubbing food and plaque from your teeth, however, not so good for your skin health. Calcium carbonate (chalk), glycerine, fluoride and zinc are not great ingredients for your skin. Dry your skin out with these and it’s going to produce more oils to protect itself which exacerbates the problem. Some toothpastes contain antibacterial components that have been shown to reduce the number of bacteria in the mouth, but there is no evidence it will work on skin so stick to the products specifically made for helping spots!

“I can apply makeup straight after having an injectable treatment” NURSE HENSHAW SAYS: You must not apply makeup straight after injectable procedures like dermal fillers or anti-wrinkle toxin treatments to prevent bacteria from getting into the place of infection. The heavy consistency of makeup such as foundation and concealer can also clog the injection site, leading to small pimples post-treatment. It is best to wait 24 hours after the procedure, to ensure that the skin has fully healed. Pure mineral makeup can be applied after an injectable treatment, however it is still best to wait approximately four to six hours. Care should be taken to ensure that your hands and utensils being used to apply the makeup are clean prior to usage – dirty makeup brushes have a lot of bacteria on them and can also cause infection when applied to healing skin. 6

“Dermaplaning will give me a beard” DR LAND SAYS: Dermaplaning uses a surgical scalpel blade to thoroughly exfoliate the skin and remove the fine, vellus hair that covers your face. Persistent rumours believe that this will cause the hair to grow back thicker or darker, causing the appearance of a beard – but this is entirely false! The treatment can give the appearance that your hair has changed though. Natural hair grows with a finely tapered point and feels soft to the touch, however, when the hair has been sliced straight across it can grow back feeling sharper and rougher and gives the impression of being thicker. Each individual hair is the same diameter as it was previously, it’s just easier to feel. If you do have areas of darker, coarser hair then you can look at other treatments such as laser, IPL hair reduction or electrolysis.

“Eyelid surgery is just used to turn back the clock” MR SCAWN SAYS: People often think that upper eyelid surgery is solely a cosmetic procedure, but it can actually help you see better! As they age, many people become aware that the heaviness, puffiness and drooping of their upper eyelids negatively impacts upon their vision. Sometimes this heaviness can even cause slight pain or aching due to the forehead muscles working overtime to lift the eyelids. Upper eyelid surgery includes the well-known blepharoplasty surgery to remove excess skin and fat, but also ptosis surgery to tighten the muscles internally within the eyelid. So, while successful upper eyelid surgery should refresh one’s external appearance, it can also brighten vision.

NURSE HENSHAW SAYS: To really fight against the sun’s harmful rays and prevent ageing and skin cancers, you need to apply sun protection all year round. When it comes to your face, you should apply in the morning after cleansing and before other skincare products. For an SPF to be most effective, continue to reapply it every two hours throughout the day as it will break down due to sweat, sebum (oily and waxy substance found on the skin) and daily activities. I know this can be inconvenient and will also mean continuous cleansing and wastage of other skincare products and makeup, so to get around this you can use a loose makeup that has a SPF of 30 or higher. This can be dusted over the face every two hours throughout the day with a brush and will not require you to cleanse first and will not cause your skin to feel heavy with product build-up. You can also purchase a setting spray with SPF which can sprayed over your makeup too!

Defining Skin Boosters The injectable treatment that can give your skin the hydration it deserves

Over recent years there’s been a real shift in beauty trends – no more are people after an overdone, obvious look. Instead, many are leaning towards subtle enhancements and tweaks – and above all else, this means taking care of your skin! One aesthetic practitioner who has really noticed the difference of treatment trends in her clinic is Dr Yusra Al-Mukhtar. “I call it the year of the skin! Or ‘skinimilism’, if you will,” she laughs. “People are really looking to restore their natural glow – they don’t want to be fake; they want to be fresh. In a recent survey I conducted – Beautified Britain Index: The Skin Report – we found that in the last year there has been a 309% increase in searches for skincare online. There was also a huge increase in searches for treatments that can help with acne scarring and a rise in #dolphinskin on social media platforms – this refers to skin that has a glowy sheen,” she says. While there are many skincare products and ranges out there that can help to achieve this look, there’s a lesser known solution to revitalising the skin quality which comes in the form of an injectable treatment. Now, we know what you’re thinking – aren’t injectables used to plump up your lips and freeze your forehead? We aren’t talking about fillers or botulinum toxin here, we’re talking about something else – skin revitalisers and boosters!

What are skin boosters? Not as well-known as its dermal filler and toxin counterparts, skin boosters are used for very different reasons. Dr Al-Mukhtar explains, “Fillers are usually used for creating volume and are injected deep into the skin. Skin boosters, on the other hand, are injected superficially meaning they are inserted into the uppermost layers of the skin. They are used for improving skin quality, moisture, texture, elasticity, and fine lines.” According to Dr Alexis Granite, skin boosters are used to help increase skin glow. “Think of them as an injectable moisturiser,” she says. “Boosters and revitalisers also help to stimulate collagen production over time, so your skin ends up in a better place than 8

where it started with consistent treatments.” This is because your collagen levels decline as you age, resulting in thinning skin and wrinkles. She adds that while most boosters are comprised of hyaluronic acid – a substance naturally produced by the body which helps with moisture – some may include ingredients such as glycerol, amino acids and nucleotides, which help with skin structure.

Who can benefit from skin boosters? If you’re wondering whether this treatment might benefit you, you’re in luck! According to Dr Granite, nearly everyone can benefit from a skin booster, no matter what skin type or colour. She says, “They are one of my favourite treatments as the risk profile is low and the upside is high.” Dr Al-Mukhtar adds that the treatment can benefit women (and even men) of all ages over 18, noting, “My younger patients typically come in to get treatment for acne scarring as it smooths out the texture of the skin, whereas those in their 30s tend to come in for the treatment of fine lines. Those who are 40 and above use the treatment to get their glow back, because as we age we lose our collagen and elastin meaning we have less smoothness to our skin. So, pretty much anyone can benefit – but of course, that doesn’t mean everyone needs it. People come in for their own individual specific concerns and

should have their own bespoke medical consultation, so it really does depend!” Dr Granite notes that as well as being a good standalone option, skin boosters can also work well as part of a combination rejuvenation treatment programme in older patients. To find out more about combination treatments, turn to p.46. People who aren’t suitable are those who may have active areas of inflammation, for example acne and spots, explains Dr Al-Mukhtar. She notes, “We wouldn’t inject into these areas because it can actually worsen the condition and may cause infection. For these patients, I would firstly focus on treating the skin condition, then we can use skin boosters to smooth the final surface, improve the condition of the skin and we can treat the scarring.” Other people who may not be suitable include those who are pregnant and breastfeeding, as with all injectables. While the most common use for skin boosters is facial rejuvenation, they aren’t just for the face and can also be used to rejuvenate the skin on other body areas like the neck, décolletage, and backs of the hands.

What are the risks or side effects? Dr Al-Mukhtar notes that as with any other injectable treatment, there is a risk of bleeding, bruising and temporary swelling. Dr Granite notes that a more serious potential complication would be a blockage beautyuncovered.com

As with any treatment in aesthetics, there are a variety of brands from different companies available. “For me, there isn’t one that’s necessarily better than any other, as they all have a unique technology and can achieve different things,” says Dr Al-Mukhtar. “So, as a practitioner it’s about finding the right one for your patient. This is why it’s important to go to a medical practitioner who is fully qualified and experienced, so that they can choose a product that will give you optimal results and treat you holistically, without a one size fits all approach." When it comes to product choice, Dr Granite chooses Belotero Revive for her patients. In her clinic, Dr Al-Mukhtar uses Restylane Vital and Vital Light Skinboosters, BELOTERO Revive, Profhilo or Teosyal PureSense REDENSITY 1, depending on what area she is treating.

Dr Al-Mukhtar recommends three treatments – one month apart – and says the results will last for 15 months until a follow-up treatment will be needed, however this depends on the brand of skin booster

Under the skin, cellulite occurs when fat cells push up, while fibrous tissue bands surrounding them pull down; which is why we see bumpiness in the areas affected.

“Contributing factors include genetics, hormones, age and lifestyle,” explains Dr Chantrey, noting that she sees people who live very healthy lifestyles, with a good body mass index (BMI) rating, but who still feel self-conscious in swimwear or summer clothes because of their cellulite.

So, the big question then, can it be treated? A cellulite severity scale, created by medics, suggests that there are three grades: • Grade 1 (mild): an ‘orange peel’ appearance with between one and four depressions that are close to the surface and a slightly sagging appearance of the skin • Grade 2 (moderate): a ‘cottage cheese’ appearance with between five and nine medium-depth depressions and the skin appears moderately draped • Grade 3 (severe): a ‘mattress’ appearance with 10 or more deep depressions and the skin is severely draped

Regardless of location or severity, the appearance of cellulite is a frustration for many. Multi-award winning surgeon and aesthetic practitioner Dr Jonquille Chantrey has seen numerous patients whose mental health has really suffered as a result of the condition. She highlights that research suggests 60% of women think it’s their fault they have cellulite, while 57% feel judged for having it. Yet, Dr Chantrey explains, it is very normal for most women to have some degree of cellulite and it’s not

“Yes!” says Dr Chantrey, emphasising, “It remains one of the most challenging concerns to treat, but it can be done.” Before deciding upon a treatment approach, Dr Chantrey believes it’s important to learn more about the patient to select the most beneficial strategy for them. “It’s essential to understand your diet and exercise regime, as well as any hormonal issues and/or medication to provide you with the most relevant advice,” she says, highlighting that addressing these factors may provide satisfactory improvement without having any cosmetic treatments.

If lifestyle advice doesn’t help, however, there are other options available. In Dr Chantrey’s clinic, her top treatments include hyaluronic acid, subcision and radiofrequency.

Hyaluronic acid (HA) How it works: injected below the skin to create a smooth layer that helps disguise the cellulite’s lumps and bumps. Results: HA offers the added benefit of hydrating and strengthening the skin. Depending on the severity of the cellulite and the brand of HA used, you could see results lasting up to 18 months. Dr Chantrey says: “Most HA products will include anaesthetic, so there’s not much pain associated with treatment and very little downtime – you’ll just need to keep the area clean and avoid alcohol and exercise for a couple of days. I use the Juvéderm range of dermal fillers and I’ve found this to be a very successful method!”

How it works: heat from the radiofrequency device is used to stimulate collagen production and breakdown fat cells, which will lead to a reduction in the appearance of cellulite. Results: multiple treatments are needed for best results – Dr Chantrey recommends one a week for six weeks, followed by ongoing maintenance every four to six weeks. With the NuEra Tight device she uses, Dr Chantrey has seen excellent results. Dr Chantrey says: “There are very few risks if the device is used properly and in safe hands. The NuEra Tight device is suitable for all skin types and BMIs, although results may not be as successful in those with a higher BMI. So far, I’ve seen really good results considering the treatment is non-invasive and very comfortable for patients – it almost feels like a heated massage!”

Alternative treatments

Subcision How it works: a needle or device is used to break the connective tissue bands below the skin to release the tension that causes cellulite. One device that utilises this technology is Cellfina; available in a number of clinics across the UK. Results: one study suggests that 99% of the 232 patients treated with subcision were satisfied with results, which were found to last up to two years. Dr Chantrey says: “I would usually do this as an add-on to liposuction and it can have a really good effect! Lipo alone won’t treat cellulite and, if you do go down this route for body contouring and cellulite treatment, be prepared for the post-op side effects of lipo; usually swelling and bruising.” Before

As well as the treatments Dr Chantrey offers, you could explore other methods to address your cellulite concerns. Carboxytherapy – whereby carbon dioxide is inserted under the skin – can be used, as well as acoustic wave therapy that aims to flush out fat cells, and laser treatment to melt fat cells away. There are lots of lotions and potions on the market that claim to tackle cellulite, but Dr Chantrey urges that you shouldn’t be fooled by big marketing claims. Skincare ingredients that have been suggested to work include caffeine – which aims to dehydrate fat cells to make them less visible – and retinol, which may reduce the cellulite’s appearance by thickening the skin. Note that at least six month’s treatment is needed to see results.

So, if you’re considering treatment, what’s the one thing to bear in mind? “Results are unlikely to be permanent,” says Dr Chantrey, emphasising the importance of exploring all options and, ultimately, recognising that cellulite is extremely common and nothing to be ashamed of. Finally, remember that you might not be suitable for every type of treatment so do your research and always see a qualified and experienced practitioner.

Like Mother Like Daughter The journey of a mother and daughter who both tried radiofrequency microneedling treatment for the first time to improve their skin quality

No matter your age, you probably want to look the best version of yourself. That’s what 57-year-old Jackie Brown and her 32-year-old daughter Sinead Burrell desired. The mother and daughter duo both had their own insecurities when it came to their appearance. For Jackie, it was the impact of years of insomnia – a lack of sleep can have serious consequences on our ageing and appearance with affects such as paler skin, more wrinkling, dark circles and more skin sag, according to research. “I wanted a physical and emotional boost and for illnesses not to be so evident on my face. Ultimately I was hoping to look brighter, not as fatigued and tried looking,” she explains. As an acne sufferer, Sinead was conscious of the repercussions of years of damage and scarring to her skin, noting, “I wanted a more even complexion without having to wear makeup and for my skin to look tighter and more radiant.” They started researching possible treatments and came across Judy Murray’s recent transformation with Morpheus8 – a radiofrequency microneedling procedure which promises to tighten, lift and smooth the skin. The treatment is seriously trending right now, and with results like Judy’s, there is no wonder why it’s popular! Impressed with her new look, Jackie and Sinead found Aspire Clinic in Reading which was promoting this treatment online and met with aesthetic nurse prescriber and clinic founder and director Jenny O’Neill to see if the treatment would be right for them – it was a journey they wished to embark on together so they had each other for support.

What is radiofrequency microneedling? Experiencing radiofrequency microneedling A thorough consultation is always the first step to a treatment like radiofrequency microneedling, because it isn’t for everyone. Nurse Jenny asked about their reasons for coming in to see her, about their medical and treatment history, and explained their different options. “We felt really at ease,” Jackie reflects, “The process of the Morpheus8 treatment was thoroughly and clearly explained to us and how it would help both of our different concerns.” After having two weeks to reflect on the consultation and treatment, Jackie and Sinead came back to the clinic for their treatment. First, the skin on the face and neck was cleaned and numbing cream was added because the treatment can cause some slight discomfort. During the treatment, nurse Jenny gently placed the Morpheus8 device onto the skin across the face and the neck, which released the needles and radiofrequency heating energy. Jackie says, “It did hurt a bit, but for an extremely short time – nurse Jenny was amazing at keeping me distracted and kept me updated throughout the procedure which made it more bearable.” After the Morpheus8, Jackie and Sinead experienced a Dermalux LED light therapy treatment using the Tri-Wave programme, which combines three lights to trigger your body’s natural cell processes to speed up skin repair and rejuvenate. Jackie and Sinead were in the clinic for a total of two hours for the treatment. but Jenny picked up on this and she took time to talk to me about it, adapting the treatment to suit,” Sinead reflects. After each treatment, the pair were given a soothing cream to calm the heat and redness of the skin, which they kept on for 24 hours. It’s common to experience redness after this treatment for a few days. They were also advised to avoid the sun to prevent hyperpigmentation (see p.26) and makeup to prevent infections. “I left the clinic with a big smile because I was glowing!” exclaims Jackie.

Things to Know About Hyperpigmentation The causes and solutions to that problem pigmentation

A wrinkle-free, well-structured face is great, but if you’ve got an uneven skin tone with dark patches all over it, you may not feel confident to go makeup free and get frustrated finding appropriate coverage. But why do you have dark patches? And how can you improve them? Known collectively as hyperpigmentation, there are three main forms: post-inflammatory hyperpigmentation (PIH), melasma and solar lentigines. Here, consultant dermatologist Dr Hiba Injibar lists 10 things to know about hyperpigmentation to help you achieve even and glowing skin!

Hyperpigmentation = dark patches of skin Hyperpigmentation occurs when there is an excess production of melanin. This is pigment produced by your melanocytes, which are the cells that give skin its colour. The opposite is hypopigmentation; whereby you get light patches of skin. Vitiligo – the condition Michael Jackson claimed to have, as well as celebrities like model Winnie Harlow – and albinism are both forms of hypopigmentation. Hyper and hypopigmentation are both harmless, but some people choose to treat them to create a more even complexion.

It can occur anywhere on the body Most people are concerned about the hyperpigmentation on their face, but you can have it on all areas of your body. Common places you will see darkened skin patches are the knees, elbows and backs of the hands. You may also be bothered by hyperpigmentation in the intimate area. It’s perfectly normal to see it in the genital-perineal area, perianal area, on the pubis, inner thighs and groins. It can be treated, so never be ashamed to chat to your aesthetic practitioner about how they can help! Remember to never buy products online without seeking advice first – the intimate area can have very sensitive skin so it’s vital to treat it appropriately.

Hyperpigmentation is more common in darker skin types The darker your skin, the more melanocytes you have. This means that those with darker skin types have a higher risk of hyperpigmentation. That said, lighter-skinned individuals are also affected – particularly those who’ve had excessive sun exposure. Post-inflammatory hyperpigmentation is caused by other skin conditions When skin is irritated or injured, its automatic response is to produce more pigment to protect itself. This means the first type of hyperpigmentation (PIH) can be triggered by things like acne, eczema or burns. This is why you’ll see darkened skin around scars, for example. Pregnancy increases your chance of getting it The second form of hyperpigmentation is called melasma, which is usually caused by hormonal changes. Birth control and hormone replacement medication can have an effect, while research suggests that up to 50% of pregnant women experience melasma. It usually appears on the cheeks, forehead, upper lip, nose and chin, but can also occur on other areas of the body exposed to the sun. Melasma becomes more noticeable in the summer and can improve after giving birth and with hormonal adjustments.

Hyperpigmentation is another side effect of ageing Often referred to as age, sun or liver spots, the medical terminology for the third type of hyperpigmentation is solar lentigines. They are particularly noticeable on the face and backs of the hands; essentially, the areas of the body most exposed to the sun. Older people or those who’ve spent more time in the sun are usually most commonly affected.

You may also be bothered by hyperpigmentation in the intimate area, which can be treated, so never be ashamed to chat to your aesthetic practitioner!

Skincare can help It’s best to see an aesthetic practitioner for a skin consultation before starting a regime for your hyperpigmentation. Ingredients to look out for include: • Vitamin C: blocks tyrosinase, an enzyme the body uses to produce melanin and, as an antioxidant, protects cells from further damage from the sun or pollutants • Kojic acid: similarly to vitamin C, also works by inhibiting tyrosinase enzymes and protects skin thanks to antioxidants • Retinol: encourages cell turnover, bringing healthier skin cells to the surface while exfoliating older, darker ones • Niacinamide: a form of vitamin B3 which can interrupt the process of cell pigmentation and brighten skin, while also being used to treat acne and prevent future hyperpigmentation • Azelaic acid: anti-inflammatory properties and can suppress melanin production – safe to use in pregnancy so beneficial for melasma treatment • Mandelic acid: an alpha-hydroxy acid that helps exfoliate the skin • Arbutin: considered to be a natural form of hydroquinone – a chemical that bleaches the skin – so can be a preferred method Sunscreen is essential to stop hyperpigmentation occurring, so my recommended products are Eucerin Sun Pigment Control and Bioderma Photoderm Mineral sunblock.

You can peel the pigment away If skincare isn’t quite having the desired effect, maybe a peel could help. A peel is created using a mix of ingredients that are usually of a higher strength than those used in skincare. Before having a peel, your aesthetic practitioner should start you on a ‘preparation phase’. This could involve going into clinic every couple of weeks for a very light peel, known as ‘superficial’, or advise to follow a homecare regime whereby you’ll use specific skincare products to prepare the skin in advance of your peel. You’ll usually need about six weeks’ prep before treatment, and aftercare or ‘the maintenance phase’ is just as important for ongoing results. There are different brands of peel formulations available; I use dermamelan from mesoestetic because it can be used on most skin types and uses respected ingredients such as vitamin C, ferulic acid, niacinamide and vitamin E, that address more than the pigmentation.

Sunscreen is the key to prevention The sun is a major culprit in all forms of hyperpigmentation; whether it’s causing or exacerbating the condition. Wearing SPF 50, a broad, wide-brimmed hat, seeking shade and staying out of the sun in peak hours – between 11am and 3pm – are essential tips to reduce risk. Sunbathing and/or using sunbeds should not be part of your beauty routine – while hyperpigmentation is not dangerous, remember that skin cancer is!

Lasers can blast hyperpigmentation Q-switched and pico lasers are commonly used to manage hyperpigmentation. They produce short bursts of light that cause the pigment to shatter into tiny, microscopic pieces under the skin, which are absorbed by the body or peel off externally. You’ll need some topical anaesthetic to ease pain and you may experience some scabbing, but this is completely normal. To avoid infection, your practitioner should keep the area clean while treating, and you should do the same following the procedure. There is always a risk of scarring or further hyperpigmentation if the device is not used correctly, so always research the practitioner and the device they use prior to treatment. I use the Synchro QS4 Q-switched laser device from DEKA and have seen some really successful results. Laser treatment is not an overnight solution – be aware that you may need up to 10 sessions to see results!

Being a Model for Aesthetic Treatments

One patient experiences a slightly different way of receiving an aesthetic treatment

Having previously had botulinum toxin injections, lip fillers and cheek filler, 47-year-old Kate was no stranger to aesthetic treatments. However, this time was a slightly different experience – she was going to be an aesthetic model! We aren’t talking about walking down a catwalk as you might generally associate with models – being an aesthetic model usually involves the education of practitioners. For an aesthetic practitioner to learn how to treat a patient safely and successfully, they need to observe a skilful trainer perform the procedure and also practice the treatments themselves on real-life people. Aesthetic models may also be used for other reasons, such as educating the public. Kate’s injector – E4’s Body Fixers expert Dr Nina Bal – was filming a video for her YouTube channel to educate people on cosmetic treatments! “I was told about the opportunity by a friend who works in the industry. I’d been thinking about getting another treatment because I’d recently started to notice that my lower face was beginning to droop. I also noticed fine lines by my nose, which I’d wanted treated for a while, and thinning lips, so it seemed like the perfect chance for me to get these looked at,” Kate explains. When she arrived at the clinic, Kate noted that Dr Bal was very attentive to all her models and gave her a psychological assessment and wellbeing questionnaire as well as a physical assessment and consultation in which she identified mid-facial volume loss, malar fat pad (fat on the cheek) atrophy, dehydrated skin, lip asymmetry and a gummy smile. As her main concern was volume loss, Dr Bal felt that dermal filler was the best choice for treatment. Dr Bal then explained to Kate what she could expect from the procedure, outlining the potential side effects.

Kate notes that she didn’t feel any pain throughout the procedures. “Dr Bal gave me a stress ball during the treatment, but I didn’t even need to use it!” she says, adding, “I did feel a little uncomfortable when she injected the lips, but as I’ve had them done before I knew this was to be expected as it is a very sensitive area. All in all, I felt super relaxed throughout the whole thing.”

Prior to the treatment, Kate explains that her entire face got covered in numbing cream for about 20 minutes. She was then asked to go into the treatment room where the filming was taking place. “Whilst she was preparing me for the treatment, she was talking first to me and then to the camera to explain what she was going to do, but I knew what I’d signed up to so I didn’t mind at all!” laughs Kate.

Once the treatment was completed, Kate was able to return to her usual working day straight away. “When I got to work, nobody even noticed I’d had the treatment done!” she says, “There were no tell-tale signs on my face except for two visible injection points, but no one even saw these until I pointed them out!”

Dr Bal injected Kate’s cheeks first using a dermal filler called STYLAGE XL. Then she did the jowls using STYLAGE M, and finally gave her lips 32

In the days following the treatment, Kate notes that she didn’t have any side effects other than what is typically to be expected with injectable procedures – some sensitivity and slight bruising.

If, like Kate, you’re interested in becoming an aesthetics model, it’s important to research the company and trainer you choose, so before you sign up to becoming a model there are a few things you should check.

In the days following the treatment, Kate explains that Dr Bal kept in touch with her by sending emails to check that everything was okay, as well as giving her a number to call in case she experienced any issues. “This helped me to feel more comfortable that I would be cared for should anything out of the ordinary happen,” Kate explains. Overall, Kate had an extremely positive experience. She says, “I’m super happy with the results as I feel like all my problem areas were addressed, and I would definitely go back to Dr Bal when the results start to wear off.”

You should always be treated in a clinical environment (not someone’s house or a hotel!), and the trainer and trainee should be a medical practitioner who is regulated by their professional medical body such as the NMC/GDC/GMC. There are also aesthetic associations which assess training providers to ensure they are safe, so visit the JCCP & Me website and check whether they have been approved. Don’t be afraid to ask the training provider questions, such as how they would support you if you experienced a complication, and make sure you do your research!

Practitioner perspective

For Kate’s treatment, we started with augmentation of the cheeks using 2ml of dermal filler to provide more facial structure and reduce nose to mouth lines by contouring the mid-face, then 1ml of dermal filler for adding volumisation to the body of the lips and restructuring the outline of the lips. I then addressed deep smile lines to correct the wrinkles and loss of elasticity and augmented the mouth to chin lines to reduce risk of swollen patches of the mouth in the prominent skin folds, as well as loss of vertical dimension. This used 4ml of filler. I was very happy with the results achieved, as volume has been restored in a very natural way. The cost of this treatment in my clinic would typically be £2,500.

I decided on Kate’s treatment plan by first discussing with her what her main concerns were and what she wanted to achieve, then by clinically assessing her face and having a very honest conversation of results, possible risks and benefits. Proposed alternative treatments were discussed, for example skin products, botulinum toxin or no treatment at all, but due to the main problem being volume loss we decided together that dermal filler was the best option. It’s important to note that I do not treat any patient seeking to improve their appearance until after an assessment of suitability and excluding Body Dysmorphic Disorder.

Content creator, campaigner and Love Island star Sharon Gaffka opens up about her experiences getting cosmetic treatments, why she’s recently had her fillers dissolved, and her aspirations to challenge difficult conversations

Tweakment Talks and Campaign Conversations Whether you’re a young woman in your 20s hooked on the show, or know someone who is, chances are you’ve heard of the popular reality TV programme Love Island. The contestants are renowned for having a tweak or two, with many openly speaking out about the cosmetic treatments they have had done – these range from non-surgical dermal fillers and veneers to surgical boob jobs and everything in-between! One Love Island contestant who’s been especially open about her cosmetic enhancements is 26-year-old Sharon Gaffka, who appeared on the seventh season of the show. Sharon has been very honest and sometimes vocal about her cosmetic treatments, having a renowned row with fellow Love Islander Hugo Hammond about whether tweakments make you ‘fake’. Following her talk at Beauty Uncovered LIVE in London (see p.22), where she opened up about her aesthetic treatment experiences, Sharon explains what motivated her to seek cosmetic enhancements and what she’s learnt along the way. Being open and transparent about tweakments…

You’ve been very honest about the treatments you’ve had, why do you take this approach? As a 26-year-old content creator with a large social media following, I know that 90% of my followers are young women and I am very aware that what I post can have an impact. When I was younger, Kylie Jenner, who is renowned for her lips, finally admitted to having had dermal fillers, and prior to this people just thought she was using lip liner, which I think is a bit unfair. I believe it’s important to be open and transparent with your followers. Saying that, I never advocate that people need or should go and get treatments and it should always be down to the individual to decide what’s right for them.

Do you ever feel pressured to have cosmetic enhancements? I think a lot of people feel pressured to look a certain way, but I choose to get treatments for myself and no one else – certainly not for likes on my social media! I do feel good about myself and I feel safe and happy with the person I am. I feel empowered that I have control over myself and what I decide to do.

So what cosmetic treatments have you had done? I love all things beauty and makeup like many young women, so I have been very attracted to trying aesthetic treatments! I’ve had dermal filler in my lip, chin and jawline and I’ve also tried microblading for my eyebrows, medical-grade skincare, microcurrent CACI facials and AlumierMD chemical peels. I’ve also had the wrinkle relaxing treatment botulinum toxin (known commonly by the brand name Botox) and I’m currently having Invisalign to improve my teeth. Oh and I’ve had breast implants!

How did you start your dermal filler journey?

You seem young to be having anti-wrinkle injections, why do you get toxin treatments?

I was 23 when I had my first cosmetic procedure. At the time, I had very narrow lips and I felt they were disproportionate to the rest of my face. I used to use a lot of lip liner, and it only goes so far. I kind of wanted to widen my mouth rather than make a large pout and I really liked the results. After this, I tried chin and jawline filler. I think a lot of young women who try fillers go through a phase of having too much or having treatments they don’t need because the results can be so amazing. Other Love Island contestants like Molly-Mae Hague have openly admitted that they’ve fallen into that trap and I can definitely relate. Now, I have gone full circle and at 26 I’ve have had all my fillers removed!

Why did you get your fillers removed? Well, to be honest, I didn’t like the look of the fillers in my chin and jawline because I didn't feel like they worked for my face so I got them removed a while ago. Recently, I had my lip fillers dissolved because the product that was injected moved and had started getting lumpy and it didn’t look very nice. I’ve been told that sometimes this is a side effect that can happen with dermal fillers. They were dissolved by injecting a product called hyaluronidase, which breaks down the hyaluronic acid in the dermal filler. However, it also removes the natural hyaluronic acid in your lips, so it’s not a procedure that you should take lightly and you should really consider the risks beforehand. It takes about two weeks for your lips to heal and for the final result to appear.

What have you learnt about getting fillers? When I had my first lip filler treatment, I went to a renowned aesthetic doctor in London who injected Juvéderm fillers. The results were amazing and I was really happy. However, I was on a graduate salary back then and needed a price that was more viable for me. So, for my next treatment I went to someone far less qualified and got a really bad result with lots of lumps. Since this experience, I’ve learnt that if you’re going to have these treatments they need to be done properly so you need to do your research, see a registered medical practitioner who has a lot of experience and of course, you also need to be in the financial position to get them done!

I know! So many people ask why I have toxin at my age, but it’s not actually for wrinkles! I get it for medical purposes. I suffer from migraines and after learning that it can really improve headaches I had treatment with dental surgeon Dr Zara Nortley and found that injections in my forehead work really well to ease the pain. I also get quite a lot of tension in my jaw and clench it and grind my teeth, which can cause jaw aches, so I’ve also started having it injected into my masseter muscles, which have really helped ease this pressure.

A lot of people feel pressured to look a certain way, but I choose to get treatments for myself and no one else What treatments did you have before appearing on Love Island? On TV, everything is amplified and very obvious so I know many Love Island contestants feel they need to have a bit of a beauty transformation before appearing on the show. I did have treatments before appearing on the show, but I didn’t have them specifically FOR Love Island. I do remember wanting my teeth done, so this is why I’ve now started my journey with Invisalign. Before Love Island, I actually focused more on my skincare. I went for a skin consultation with Dr Nortley and started using medical-grade cosmeceuticals. What have your experiences been like with Invisalign? It can make your teeth feel quite tender and painful, but it’s nice because I can go to events and I don’t have to worry about having braces and getting food stuck in them! My treatments have been really good and I am getting fast results. I’m planning on getting my teeth whitened after.

Now to cosmetic surgery… why did you choose to have breast implants? I really liked how my boobs looked naturally in terms of nipple and shape, but I just felt like they were a bit too small in comparison to the rest of my frame. I did debate having them done when I was younger, but I ended up waiting until I was 24. I think that people only ‘see’ the after results of an augmentation and no one really talks about the healing process! The not being able to sit up without help and not being able to lift your arms up. I mentally struggled with the lack of freedom, and remember crying for the first week, so it’s definitely something to consider before having the operation. Now, I'm so happy with them and I have no regrets.

What type of implants did you have and why? I had Motiva implants and I really liked them because they came in four profiles. My natural breasts don’t stick out from my chest wall a lot and I didn’t want to change that. I also didn’t want them to look ‘done’ so having a mid-profile implant was the best choice for me.

What tweakments do you want to try next? I’m interested in trying platelet-rich plasma vampire facials at some point as I’ve heard they can make your skin look amazing! I think the treatments I have in the future really depend on how I age and what my medical aesthetic practitioner believes are suitable for me.

What’s your advice to young women who might be thinking of getting cosmetic enhancements?

abnormal lumps on my breast which was really scary, but thankfully I had them checked and they were non-cancerous. Because of these experiences, I am very passionate about promoting cancer awareness. Recent campaigns I’ve been a part of include a public health initiative with Superdrug on breast cancer awareness, and the Cervical Screening Saves Lives campaign with the Department of Health and Social Care and NHS. Outside of cancer, I’m also an ambassador for Young Women’s Trust, a feminist organisation working to achieve economic justice for young women, and I’m a huge advocate for anti-spiking!

Why is anti-drink spiking a particular focus for you?

I’ve had so many young girls message me about lip fillers – some younger than 18! It’s now illegal for someone under 18 to get treatments (which I was shocked to learn wasn’t the case until recently), but before this I always said to wait until they are at least 18 (preferably older) and also to ensure they are financially stable before getting these treatments. Take a lot of time to think about if this is what you really want, consider the side effects, and be mindful of Instagram trends, for example, don’t just go and get a lip filler treatment because a certain look is trending at the time. Finally, research your practitioner well and always go to a registered medical professional!

I myself have been a victim of drink spiking and understand the shame and guilt that many victims face after experiencing this. You wonder if it’s your fault, if you did something wrong or if there was something you could have done to prevent it – but victims need to remember that it’s never their fault and it’s the fault of the perpetrator. I’ve been raising awareness by speaking openly about this on TV and in the media and have launched my #ProactiveNotReactive campaign. I gained more than 1,500 testimonials from other victims from the ages of 14-65 and approached my local MP, David Johnston OBE and met with him and shared my experiences and the experiences of others. I want to see drink spiking have its own laws, being taught in schools to improve education and understanding in this area.

An ambassador for young women…

You’ve also spoken extensively about your experiences with social media trolling… what’s your advice to others who might be experiencing this?

What motivated you to become involved in campaigning? I’ve come from a family of public servants and I think because of this, I’ve always wanted to do something that’s bigger than me. I’m very passionate about being a spokesperson and ambassador for other young women, and normalising hard conversations. These range from having cosmetic treatments and social media trolling, to the gender pay gap, racial abuse and drink spiking. I have a habit of picking difficult tasks, but someone’s got to do it to raise awareness!

I’ve had a lot of experience in trolling, particularly on TikTok, from remarks about my appearance to hate from speaking out about drink spiking! Cyberbullying and trolling isn’t an issue that just affects people in the public eye, it can impact any social media user. The important thing to remember is that often there is an issue with the person doing the trolling, and it’s not about you. I think people forget that – I know I have! My advice is to have a support network and share the burden with those you trust around you. Similarly, if you know someone who is being trolled, reach out to them because chances are, they need your support.

What campaigns have you been a part of recently? I only ever take part in something I truly believe in and a lot of the campaigns I am associated with are to do with cancer. I’ve been very open about the fact that I’ve had four members of my family suffer with cancer, and three have passed away because of it. I myself have had beautyuncovered.com

hey say the eyes are the window to the soul. But sometimes, the appearance of the eyes and surrounding areas can belie what’s behind them, giving a false impression of the individual. According to Mr Daniel Ezra, these types of misconceptions can drive male patients to undergo aesthetic treatments to the eye area. He explains, “People can make assumptions about how others look. If you have large bags under the eyes, you might look like you’ve been up all night and not keeping up with your work, and people who have very low eyebrows and frown lines can look angry and unapproachable,” he says. “So, we’re often influenced by certain emotional and behavioural assumptions others make about us, based on how we look.” This is a significant motivating factor for men, whom Dr Ezra says comprise as many as 30% of his clientele seeking eye rejuvenation treatment. Aesthetic procedures in general are 10 times more common among women than men. Yet demand from the male population has increased by up to 50% in the last few years, not least because of the so-called ‘Zoom boom’. People see a somewhat distorted picture of themselves on a video screen, leading many to zero in on their perceived imperfections. And men are arguably more susceptible, being less used to examining their mirror image in such forensic detail as women.

Age-related changes around the eyes Alongside tweaks to the jawline and hair loss treatment, the eye area – known by medics as the periocular/periorbital region – is among the top three areas of focus for men. And it’s not surprising, since the eyes bear many of the hallmarks of facial ageing – hooded eyelids, crow’s feet, frown lines, bags, and dark circles, for instance. As we get older, collagen production declines and the skin loses elasticity, resulting in lines, wrinkles and loose skin. This tends to be more marked around the eyes, according to Mr Ezra, where fluid builds up due to poor lymphatic drainage in the tear trough area, and where excess skin can create a sagging effect around the eyelids. “The eye socket itself is also full of fat,” Mr Ezra explains. “With age, the membranes holding that fat backwards weaken, and the fat begins to bulge forwards causing under-eye bags.” The skin around the eye is around four times thinner than elsewhere, so it’s often the first area to show age-related changes, giving that tired, sullen appearance that no amount of sleep will fix.

Types of treatment Whatever the concern, there are several tools of the trade that can help combat them and rejuvenate the male eye. The more invasive options include blepharoplasty and surgical brow lifts, explains Miss Jennifer Doyle, in which fat or excess skin is ablated from the eyelids and the brow area respectively. “Surgery can take decades off the right candidate and may be suitable for patients seeking 44

dramatic results,” she says. But these procedures entail considerable downtime and carry the tell-tale signs of cosmetic surgery, like stitches and prolonged swelling and bruising. For men who want subtler rejuvenation, to simply look more refreshed, the payoff isn’t worth the price. With fewer men choosing to go under the knife, non-surgical alternatives are rising in popularity and, as Mr Ezra says, it’s prudent to consider them as a first step before going down the surgery route. Skincare As a first line of defence and to proactively halt periorbital ageing, many practitioners recommend topical skincare solutions containing retinol. A derivative of vitamin A, retinol slows collagen degradation, plumping saggy skin and smoothing wrinkles. According to Miss Doyle, patients often see positive results. “Getting someone on a really good retinol designed for the periorbital area, such as the AlumierMD Retinol Eye Gel, is an excellent place to start,” she explains, “because they should get improvement from the retinol’s thickening effect in the skin.” Thickening the skin not only irons out lines and wrinkles, but also strengthens the surrounding tissue’s structure, which helps to hold back accumulating fluid and periorbital fat, reducing bulging and minimising the appearance of bags. With any retinol product, the key is patience – results won’t magically appear overnight – and starting on a low concentration to avoid irritation. Miss Doyle says it can be increased incrementally as the skin adapts, with gradual improvements seen over a period of several months. Injectables for the eye area Men wanting a quicker fix to reverse signs of ageing can turn to injectables. “Dermal fillers can be helpful for localised hollowing, particularly tear trough hollows, where you can get fantastic results in selected patients,”

Mr Ezra comments, adding that botulinum toxin (known by the common brand names Botox, Bocouture, Juvenus and Azzalure) can be helpful in smoothing out crow’s feet and frown lines. For the delicate under-eye area, practitioners use dermal fillers containing hyaluronic acid (HA). The naturally occurring ingredient supports collagen production and boosts hydration, and because it’s a clear gel, it’s easy to smooth, and gives a natural aesthetic to tear trough hollows while also tackling dark circles. Only a handful of dermal fillers are licenced for under-eye use in the UK, and the Teosyal Puresense Redensity 2 product by Teoxane is favoured by both Miss Doyle and Mr Ezra. Miss Doyle also uses Juvéderm Ultra by Allergan, while Mr Ezra adds Galderma’s new Restylane Eyelight to his product list – turn to p.25 to find out more about this product! It should be noted that injectable treatments aren’t suitable for everyone and those interested should seek a consultation with a registered medical professional. Other options to rejuvenate the eye area If dermal fillers conjure an image of celebrities sporting the puffy ‘pillow face’ look, you’re not alone. Although the right practitioner will be able to achieve a natural look, men in particular sometimes shy away from injectables, associating them with a plump, rounded and more feminine facial aesthetic. The male bone structure may also rule out under-eye fillers. “A lot of men have what we call a negative vector, so instead of the cheeks projecting forward they’re more concave,” Miss Doyle explains. “Those men are quite difficult to treat with tear trough fillers because they don’t have support from the mid-face to hold up the area under the eye.” Don’t worry though, there are alternatives! Skin tightening treatments are often more suitable for these individuals, Miss Doyle adds, noting, “We offer a laser procedure called SkinTyte from Sciton, which uses infrared to heat the tissue to about 40-42 degrees and that stimulates collagen, so you get a nice tightening effect on the lax skin around the eyes.” Another option is platelet-rich plasma (PRP) therapy, in which plasma is extracted from the patient’s own blood and injected under the eyes. In high concentrations, plasma contains growth factors and that all-important collagen, so it regenerates skin cells and improves

Patient and treatment selection When rejuvenating the eye area, choosing the most appropriate treatment options for each patient is key to achieving optimal results. That’s especially true for men, because the desired aesthetic is usually very different than for women, and therefore requires a different approach. For example, female patients typically want a higher, arched brow while men tend to prefer the more masculine shape of a flatter and lower brow – although everyone is different and the desired outcome depends on individual preference. “Women like to have a lot of their eyelids showing, but in a man it can sometimes look odd. It’s very important that your practitioner understands the male aesthetic,” says Mr Ezra. Treatments for hooded eyes or drooping brows should be aimed at eliminating excess skin without lifting the brow too high, revealing much of the upper eyelid and giving an overly feminised look. “It’s only surgery that will do the job,” he adds. Likewise, Miss Doyle points out, a good skincare regime or a course of injectables can only achieve so much. “Those non-surgical options will improve and subtly rejuvenate the area, but they won’t remove large under-eye bags,” she explains, adding that in such cases, again, you’ll need to consider surgery. With men often wanting a less obvious result – cosmetic treatments are “still seen as quite a feminine thing to do” Miss Doyle comments – it’s important to manage expectations. “I always gauge what people want to achieve and what downtime they are willing to go through to get it,” she says. There, the patient’s age is a consideration. Older, retired men, according to Miss Doyle, are often more open to surgical options and the associated after effects (visible stitches, bruising etc) and recovery time. “But the younger cohort of men who are still working, in their 30s and 40s, don’t really want people to know,” she notes.

You’ll find someone offering aesthetic treatments on every high street, but for anyone thinking of eye rejuvenation the message is simple: choose your practitioner wisely. “Often the complications you see are because practitioners don’t understand how to match the right patient with the right treatment,” says Mr Ezra. “You need someone who is highly skilled in the eye area to advise on that.” Miss Doyle concurs, pointing out that with the delicate and unforgiving eye area it’s always best to err on the side of conservative caution, and consider non-surgical treatments first. “However, if you’re going for surgery, I recommend seeing an oculoplastic surgeon rather than a generalist as they are specialised in this area.”

Find Your Perfect Surgeon When someone tells you they are a doctor, you probably trust you are in safe hands. While this is true in most instances, unfortunately this isn’t always the case. Earlier this year, it was reported that a 79-year-old man from Birmingham experienced botched plastic surgery, leaving him unable to close his eyes for Mr Naveen Cavale, nearly three years! consultant plastic and reconstructive surgeon, But how do you know who to go to? REAL Plastics, Battersea Here, consultant plastic and reconstructive IG: @realplastics surgeon Mr Naveen Cavale puts together a checklist for anyone considering plastic surgery in the UK, to ensure a safe and successful treatment.

Checklist to ask clinics offering plastic surgery

Do your research In the UK, you can search for any doctor on the General Medical Council’s (GMC) website, which regulates all doctors in the UK. The person performing your surgery must be listed on the GMC’s specialist register for plastic surgery. If they’re not, then this is a massive red flag! It’s also a great idea to find out about the experiences of other patients by visiting reputable review websites.

2. Find out their complications protocol

3. Ask how experienced the surgeon is Find out how frequently the surgeon performs the particular procedure you intend to book in for. They should be performing it every month at the very least and have many years of experience.

Insurance is extremely important because if things don’t go to plan, and you decide to seek compensation, the surgeon must have adequate cover that pays out. Your surgeon must be covered for this so make sure you ask. Again, if they’re not, it’s a definite red flag.

5. Know where the operation would take place In England it’s a legal requirement for cosmetic surgery to be performed in a CQC-regulated establishment so be sure to check their register. You can even read the hospital’s latest inspection report! For Wales, check out Care Inspectorate Wales, The Regulation and Quality Improvement Authority (RQIA) in Northern Ireland and in Scotland it’s Healthcare Improvement Scotland.

6. Listen to your gut Does the practitioner seem ‘nice’ and ‘normal’? If you are getting a bad feeling about them, then it’s worth listening to your gut and finding someone who you relate to better! You can check out if they have a social media profile to get a feel for them before your consultation to find out how they come across. You should also check out their before and after images, you can also ask the clinic to show you some results and the surgeon should also be able to show you photographs.

Understand cooling-off periods A cooling-off period is the time a surgeon must give a patient to go away and think about the decision of whether or not to have a procedure. As per the surgeon’s regulatory guidelines, this should be a minimum of two weeks, and they should not be pushing you into having the procedure.

8. Check they are involved with the surgery community In the UK, there are several associations that a plastic surgeon may be a part of. Every surgeon who is a part of these groups have undergone extensive checks, so you can be confident that they have a high level of standards. Forehead lines – they’re something we all have and wanting them improved is one of the most popular reasons for getting an aesthetic treatment. While they can be softened, unfortunately they aren’t something we can completely prevent – the lines are mainly caused by our muscle movement, meaning they form when we raise or burrow our eyebrows. Over time, this can cause grooves to develop in your skin that eventually present as wrinkles. But did you know that not everyone’s forehead lines present in the same way? According to consultant, plastic and reconstructive surgeon Mr Dalvi Humzah, there are four main ways that forehead lines can appear and knowing which type you fall into will affect what type of treatment plan your practitioner should follow. So, which type are you?

WHY DOES YOUR SHAPE MATTER? Mr Humzah explains that knowing your individual forehead anatomy is important for getting optimal results from an aesthetic treatment. He explains, “The most popular procedure for treating forehead lines is botulinum toxin. This product is injected into a muscle and causes it to relax, limiting its movements. For forehead lines, this treatment means the existing lines look smoother. In addition, you can’t move your eyebrows inward anymore, so you don’t make the existing lines deeper.” He adds that when you’re going to a practitioner for toxin, you need someone who is going to treat you as an individual rather than replicate the same injection points for everyone. “If your muscles contract differently and your lines don’t present in the

Mr Humzah calls this the flying bird shape! This is where the muscle covers the entire forehead, including the central part, giving rise to parallel wrinkles that extend continuously throughout the forehead.

This type is when you have straight lines presenting horizontally across the whole forehead, explains Mr Humzah. Like the V-shape, this is a common presentation of forehead lines, and occurs because the muscles are formed by two bands separated by a connective tissue.

Getting the treatment to reflect individual anatomy can help give a more natural result.” When seeking treatment, Mr Humzah recommends ensuring you go to a medically-qualified and regulated injector who is able to assess your face correctly and perform the appropriate injection method.

TREATMENTS Alongside botulinum toxin treatments, Mr Humzah explains that dermal fillers can help lessen the appearance of forehead lines as

they help to create plumpness and volume, and skin boosters can be used to help to smooth out the texture of the skin by providing it with more hydration. He explains, “The forehead is easily exposed, so it is one of the areas which is most prone to sun damage. Sun damage can cause accelerated ageing, so it’s important to always wear an SPF of at least 30 even on non-sunny days. While we can’t stop forehead lines appearing completely, implementing this from a young age means you will help to reduce their appearance in the future. Using retinol in your skincare can also be good, as it stimulates collagen to help the skin resist wrinkling and smooth out its appearance.”

TYPE 4: LATERAL

Mr Humzah explains that these are where your lines only present in the middle of your forehead because the connective tissue is located laterally, and the muscle is in the central part of the forehead, leading to a column of lines in the central part. This means there are no lines around the corners for your eyebrows.

According to Mr Humzah, this type only presents with lines at the sides of the eyebrows, but with nothing showing in the middle of the forehead. This is because the two muscle bands are placed laterally in the forehead with a large portion of connective tissue in the central part.

Take Care of Your Skin Health This Summer With the sunny months upon us, we ask is SPF really that important?

After more than two years living under the cloud of the COVID-19 pandemic, this summer will bring with it a much-anticipated sense of freedom to enjoy some fun in the sun with friends and family. As well as being a great mood-booster, the sun is an important source of vitamin D, which is needed for healthy bones and which the body creates through direct sunlight on the skin. However, sun exposure is not without risk, so protecting the skin from its harmful effects should be top of any sunworshipper’s list of priorities. While most people acknowledge the link between sun exposure and skin cancer, they may not be aware that the sun is directly responsible for around 90% of the visible signs of ageing on our skin. The process is known as photoageing and causes those familiar indications associated with Mr Paul Banwell, consultant plastic surgeon, age: lines and wrinkles, pigmentation The Banwell Clinic, (age spots, liver spots, blotchiness), East Grinstead IG: @paulbanwell loss of elasticity, broken capillaries (spider veins), and uneven skin texture. “The skin is subject to all sorts of damage mainly from what we call the solar constant. That includes ultraviolet radiation that we’ve all heard of – UVA and UVB – but also includes radiation from infrared and visible light,” explains Mr Paul Banwell, consultant plastic surgeon specialising in skin. “Those wavelengths damage the intrinsic structure of the skin, leading to ageing and potentially, skin cancer.”

How the sun damages the skin Ultraviolet rays, UVA and UVB, are the commonly known wavelengths of light produced by the sun. When they hit the skin, they penetrate both superficially and to the deeper layers of the dermis, breaking down the skin’s DNA. According to Mr Banwell, the impact is seen straight away as you begin to develop a suntan. That deepening pigmentation is caused by increased production of melanin, a process kickstarted by the skin’s cells to safeguard against further harm. In other words, even if you don’t burn, any change in your skin’s colour is a sign of injury and means your body is in damage-limitation mode. Over time, radiation degrades the collagen and elastin fibres that form the skin’s structure and give youthful skin its plump, taut appearance. That loss of elasticity gradually yields the hallmarks of age – fine lines, wrinkles and sagginess. More dangerously, if the skin’s DNA sustains enough long-term damage, the cells multiply and grow uncontrollably and can lead to cancer. The damaging effects of the sun start to unfold long before the tell-tale signs emerge. “If you’re in the sun a lot from a young age, your cumulative sun exposure increases, which means your risk of skin damage is higher,” Mr Banwell says, adding, “Any burning episodes, especially during childhood or when younger, are highly predictive of future skin cancer formation.” In fact, Mr Banwell reiterates that just one episode of blistering sunburn as a child, or burning in the sun five or more times at any age, doubles your risk of developing melanoma, an often-deadly form of skin cancer that’s

most prevalent in older age groups. The most common type of skin cancer – basal cell carcinoma – is more often diagnosed from middle age, although according to Mr Banwell, “We see those increasingly in younger people as well, through cumulative exposure to the sun or sunbeds.” Even individuals with darker skin tones are at risk Mr Banwell says, “Many people believe that those with dark skin are not susceptible to skin cancer, but this absolutely isn’t the case and when it comes to the signs of ageing, these people are at higher risk to suffer from pigmentation as they age.”

Nowadays, as well as safeguarding against UVA and UVB rays, some high-quality, over-the-counter sunscreens also protect against infrared radiation (IR) and visible light. Research shows that although those wavelengths cause minimal harm on their own, they are significantly more potent when combined with UV radiation – such as when sunlight is the source. IR and visible light do not cause sunburn and likely don’t increase the risk of skin cancer, says Mr Banwell. However, they penetrate deep into the dermal layers of the skin, triggering a similar response to UVA rays and, as such, have an ageing effect. Many sunscreens also contain ingredients like antioxidants that prevent the production of free radicals, molecules generated by the solar constant and by environmental pollutants, and which damage the skin. Mr Banwell favours two brands in particular – Rationale and Heliocare. If you’re tempted to trade the sun for an artificial alternative, the advice is clear. “Sunbeds are an absolute no-no,” Mr Banwell adds. “They are a focused, intense, synthetic UV radiation that can cause cancer and accelerate ageing.” The safest way to ‘fake it till you make it’ also comes in a bottle, with no shortage of self-tanning products available to buy. Some contain SPFs, but while the tan may last, the SPF element wears off, so you still need a good sunscreen as well, emphasises Mr Banwell. Since fake tan can potentially compromise the efficacy of sunscreen, it’s best to apply the tan in the evening and use sunscreen as directed during daylight hours. In a similar vein, if you’re wondering whether the SPF in your everyday foundation will provide all the protection you need, the verdict is a resounding ‘No’. “The SPF in makeup is unfortunately not sufficient, although every little does help. People should wear sunscreen as well,” Mr Banwell notes.

Preventing sun-induced skin damage and ageing The best way to prevent photoageing and skin cancer is, unsurprisingly, to limit your sun exposure as much as possible, especially when the sun is at its strongest – in the UK, that’s between 11am and 3pm. However, there may be times when you can’t avoid being outdoors, for example if you work outside or do a lot of sports. In that case, covering up and wearing a sun hat and large-framed sunglasses will offer some protection. As Mr Banwell says, the best solution comes in a bottle! “Standard skin cancer protection – i.e. using sunscreen – is also the most powerful antiageing strategy,” he points out. “SPF, or sun protection factor, still forms the cornerstone of sun protection strategies, but in recent years they have increased dramatically in their complexity and diversity, which is exciting.”

Know your sunscreen The very best sunscreens in the world are worthless if not applied correctly. That means using a generous amount on all exposed body parts, and – essentially – reapplying every few hours, as well as after sweating, swimming, towelling off or getting changed. Mr Banwell explains that sunscreens in the UK bear two ratings: an SPF between 2 and 50+ and a UVA star rating out of five. SPF represents how long it would take for your skin to burn with versus without sunscreen applied. For example, a sunscreen with SPF30 means that if applied correctly, it will take 30 times longer for you to burn than with no SPF. So, if your bare skin would burn in 10 minutes, it would take 300 minutes – or five hours – to burn if you wear SPF30 and apply it as directed. The higher the UVA star rating, the better the protection against the ageing effects of the sun’s radiation. Mr Banwell advises to look for a five-star rating where possible and, again, apply regularly and generously. General advice is to use an SPF of at least 30, but if you’re particularly susceptible go for 50+.

Moles and more – when to seek advice If you have a number of moles, you’ll likely know the importance of being vigilant. Moles that change over time are often indicative of skin cancer. Familiarity with what your skin is normally like can help you keep track of changes. Other types of non-cancerous pigmented skin lesions that are not moles, sometimes called nevi, can also alter with time and therefore warrant close observation. That’s not to say all moles or marks that exhibit changes are necessarily cancerous; so seeking medical advice from your GP in the first instance is crucial to promptly diagnose or rule out anything ominous. “Any new little lumps or bumps that are present for more than four months, that bleed or don’t heal, especially if you have a history of sun exposure, need to be checked,” Mr Banwell advises.

“New little lumps or bumps that are present for more than four months need to be checked” Four seasons of skincare Investing in your skin health, or ‘skinvestment’, is a profitable pursuit not only in the summer, but all year-round. A tailored skincare regime will maximise all the benefits of using a good, full-spectrum (five-star, SPF30 or higher) sunscreen and limiting sun exposure. According to Mr Banwell, there are a few key components to look out for in your topical products. Vitamin B3, known as niacinamide, hydrates the skin, reduces inflammation and thickens the outermost layer of the epidermis. The antioxidant resveratrol, found in red grapes among other plants, is thought to have anticarcinogenic properties so will preserve the skin and potentially guard against cancer. The final must-have, Mr Banwell says, is vitamin D, which has antioxidant and antiageing benefits, improving skin cell renewal and combatting damage from free radicals. “On top of all that, think about oral supplementation and cosmetic treatments,” he comments. “Chemical peels, microdermabrasion and microneedling, alongside a good skincare routine encompassing SPF for example, improves your skin quality and therefore also go some way to protecting it against ageing and skin cancer.”

Time to Battle Ageing Discover how a facelift can have natural results and turn back the clock Visible signs of ageing are a common concern that we all eventually suffer with. Although it is impossible to stop it completely, there are ways to improve the skin sag, wrinkles and other signs that come over the years. There are many non-surgical solutions from injectable treatments to skin tightening devices, however, for those searching for a permanent result, cosmetic surgery may be the answer. For 53-year-old Katherine, having a more youthful but natural appearance was her top priority, so she saw consultant plastic, reconstructive and aesthetic surgeon Mr Muhammad Riaz to find out what surgical options were available.

Finding the perfect surgeon Katherine had always felt that she looked younger than her age, however after reaching 50 she started to become more conscious about her ageing and believed her face was becoming “saggy.” She had received non-surgical ‘facelift’ procedures with aesthetic devices previously, but it was a slow process and she wanted to see results sooner. She says, “I began to think to myself that I should take the plunge and look for a more permanent solution. I did some research and came across an interesting article which listed the top facial surgeons in the UK, which gave me a good starting point. I went quite far down the road with another surgeon and thought they were a great candidate, however, after more research, I found they did more body surgery and I wanted someone who was really focused on the face.”

After checking the General Medical Council register and the surgical associations (see p.67), Katherine chose Mr Riaz. “I had heard about Mr Riaz on the grapevine as a couple of people I knew had been to see him and had said how great he was. I looked at his website for around 18 months and couldn’t find anyone else who had such subtle, natural results. His reviews were amazing, so I booked to go and see him and as soon as I did, I instantly knew that I wanted him to do my surgery!” explains Katherine.

An extensive consultation After contacting Mr Riaz, Katherine had a video call with him before an in-person consultation. Katherine reflects, “The in-person consultation was around an hour and a half. Mr Riaz spends a lot of time looking at you and analyses every aspect of your face. I know many women who are very specific about what they don’t like about their faces, but I asked Mr Riaz ‘What would you recommend?’. I thought the surgery would focus more on the bottom half of my face, but Mr Riaz said I would benefit from surgery focusing around my eyes as well the upper part of my face.” Mr Riaz recommended Katherine underwent upper and lower blepharoplasties (a procedure to remove excess skin to enhance or create an eyelid crease), cheek and temple lift, extended ‘R’ face/ neck lift (Mr Riaz’s signature technique which has limited surgical steps, leading to minimised downtime, swelling and bruising) and neck liposuction. She notes, “I still wanted to look like me, but just

I relaxed straight away and didn’t even think it looked like me. My partner picked me up and I left the hospital at 11am. They sent me home with some pain relief medication (even though I had virtually no pain) and a post-operative mask to wear for two weeks, which I decided to wear for a month,” she says.

A steady recovery

Successful surgery Katherine went away and thought about the surgery for two weeks to ensure it was something she really wanted. She wasn’t able to eat from 12pm the previous night and arrived at St Hugh’s Hospital in Grimsby for her surgery at 7:30am. Katherine was checked-in to a private room where a nurse took her blood pressure and prepared her for surgery. Dr Riaz came to see Katherine to mark up her face with a marker pen to highlight the areas he was going to operate and then she had a visit from the anaesthetist who talked her through the surgical process again and ensured she was ready. Prior to the surgery, Katherine’s main concern was a fear of cannulas, a thin tube inserted into a vein to drain fluid and provide oxygen, as when she was younger, one snapped in her wrist and had to be delved out. She reflects, “I don’t mind having injections but if you stick a needle in me and it stays there, I’m terrified. I explained my concerns to the anaesthetist who said they could use gas and then put the cannula in but one of the side effects was that you can become disorientated. I just said I would do it! The team were incredible, and a nurse spoke to me to distract me from the needle.” Overall, the surgery took six hours to perform. Katherine explains, “I woke up and had bandages on my eyes which I had to keep

Katherine was told to take two weeks off work, drink lots of water, eat healthy food and have no alcohol. She comments, “As I had my surgery during lockdown, I wasn’t working anyway. You must sleep on your back and keep your face as straight as possible for two weeks, but I continued for a month. It’s just general common sense, you have stitches which are a mixture of dissolvable and removable, but these were painless when removed. I’m a year down the line since my surgery and the hospital still ring me up and check in on me.” Mr Riaz assessed Katherine one week, two weeks and one-month post-surgery to check for any complications and ensure that the scars were healing appropriately. Katherine’s scar was around the sideburn and in front and behind the ear but were maturing correctly and are not noticeable.

Award-winning results Speaking to Katherine one year after her surgery, she is in love with the results. She reflects, “I can’t tell you how perfect the results are because they are so natural. People think I look around 35-years-old! I hardly need any makeup and my under-eye bags have completely gone.” For anyone interested in getting surgery themselves, Katherine advises, “As long as it’s right for you and you’re doing it for yourself, then don’t hesitate. They used my face to nominate Mr Riaz for the Best Surgical Result at the Aesthetics Awards this year and it won! I can’t praise him highly enough.”

Each person experiences a different ageing process, and this is dependant on the type of treatment they might need. Some will require treatment of just the upper face, lower face, or both. Review: I have loved using this sunscreen throughout my day-to-day life. With the summer months here, it’s great to have my skin protected and using a brush applicator has made my life so much easier, whilst ensuring you have covered your face sufficiently. Review: I love this product, especially the way you can easily keep applying it throughout the day and over makeup. It is easy to use and fits nicely in your handbag so convenient to reach and to take travelling. My only concern is that it isn’t obvious that you have covered the entire face, so I probably applied a bit too much, but too much sunscreen is better than not enough and all the benefits outweigh this issue! BEAT THE HEAT AND HYDRATE Nutritionist Simone Thomas tells us what to eat to rehydrate our bodies after excessive summer sweating Sweating or hidrosis is a normal bodily process that regulates body temperature and helps us stay cool. It involves the release of fluid from different parts of the body, with common areas including under the arms, the feet, below the chest, and the palms of the hands. While little to no body sweat is called anhidrosis or hypohidrosis, excessive body sweat is referred to as hyperhidrosis. In addition to keeping us cool, sweating also helps the skin retain moisture by containing lactate, sodium potassium, amino acids, and urea, which all act as humectants (substances that attract water from the air or deeper layers in the skin). What we need to remember after we sweat is to replenish our liquids and stay hydrated, but did you know that foods can also help hydration?

Foods that hydrate The kidneys do a wonderful job of maintaining our water balance. However, after we sweat, we need to replenish what was lost by consuming extra fluids. Water is the most obvious choice, and according to the NHS Eatwell Guide, we should be drinking six to eight glasses of fluids a day. However, if you’re bored with plain old Adam’s ale, why not boost its appeal with slices of cucumber, lemon, lime, grapefruit, or adding mint, berries or ginger? Contrary to popular belief, other liquids do count towards our daily intake, such as vegetable broths, sports drinks, and herbal teas. Caffeinated beverages like tea and coffee also help to hydrate you but they also make you wee more! So, they shouldn’t be your primary source of hydration. Fortunately for us, many foods, especially fruits and vegetables, are packed with water. As a bonus, they also have many health-promoting nutrients and compounds. Some of my top picks include: melon, courgette, strawberries and radishes. Melon Approximately 90.2% of melon is water. This low-calorie fruit (roughly 34 kilocalories per 100 grams) can be enjoyed in salads, juices, sorbets, and soups. For a summery treat, you can have it in a gazpacho or caprese salad, with prosciutto or feta cheese, or on its own with a bit of chili, salt and lime. It’s also a great breakfast food or snack! Watermelon is a great low-calorie way to get more water into your system. Courgette This low-calorie food is only 17 kilocalories per 100 grams. Packed with water (almost 95%) courgettes have more potassium than a banana 62

and are fat and cholesterol-free, as well as being low in sodium and rich in manganese, which helps our metabolism and brain function. Consider replacing baked potatoes with courgettes and adding them to your pasta, frittata, chili, and omelettes. Strawberries Strawberries are a delicious and healthy sweet treat and are surprisingly high in water for such a little fruit (91%)! The fibre content satisfies our hunger and helps us eat less, and they’re also a great source of manganese. Chuck them on your cereal, eat them neat, or put them in a summer salad! Radishes I bet you didn’t know that these tiny peppery vegetables contain 95.3 grams of water and only 16 kilocalories! You probably also didn’t know that this surprisingly versatile vegetable can be grilled, sautéed, roasted and pickled. You can add them cooked or raw to pasta, salads, dips, fried rice and sandwiches or put them in your summer salads for a delicious taste and colour pop!

Don’t sweat it! Sweating is a normal physiological process. If you sweat very little or a lot, you should consult your primary healthcare provider. Many medical aesthetic professionals are also well equipped to help reduce the impact of sweating. They may consider topical antiperspirants and medications that can either be placed onto the skin or taken orally, as well as botulinum toxin and heating devices such as miraDry. To restore lost fluids after sweating, remember we do not have to resort to only liquids. Several hydrating foods, mainly fruits and vegetables, count towards our daily fluid quota.

Creamy Avocado Smoothie This green, energy boosting, hydrating smoothie is great for breakfast, pre work-out or as a meal on a busy day. Avocados contain potassium, healthy fats and fibre, which help your body hold on to water. If you are on the busy schedule you can plan ahead and throw all the ingredients into the blender the night before. All you will need to do in the morning is to blend all ingredients and you are ready to go!

(Serves 2) 2 medium-size ripe avocados 1 frozen banana 1 handful of spinach 2tbsp peanut butter 200ml of milk (or any plant-based milk) • 2tbsp of honey (or maple syrup for the vegan replacement) • For an extra health boost, add 2 Simone Thomas Super Greens Powder capsules

Method

1. Put the avocados, frozen banana, spinach, peanut butter, milk, and honey into a blender. 2. If the smoothie is too thick for your liking, add some more milk. 3. If you want an extra boost, empty 2 Simone Thomas Super Green capsules into the blender. 4. Blitz until creamy and smooth, then divide between two large serving glasses and enjoy. It tastes the best when fresh, although you can keep it in the fridge for up to 24 hours.
