Fall in Texas is a funny thing. Not like “haha” funny, but rather an odd, quirky kind of funny. You see, after a long, hot, and oh so dry summer we are all so ready for a change that the first time the temperature dips below 90 degrees we convince ourselves that fall has arrived. While it may be slightly cool in the morning, by the end of the day we all just walk around sweating from the heat of our pumpkin spice latte and our flannel shirts. Oh well, at

Should you stick with index-based investments?
Y
You may have heard that you can simplify your investment strategy just by owning index-based or passive investments. But is this a good idea? You’ll want to consider the different aspects of this type of investment style.
To begin with, an index-based investment is a vehicle such as a mutual fund or an exchange-traded fund (ETF) that mimics the performance of a market benchmark, or index — the Dow Jones Industrial Average, the S&P 500, and so on. (An ETF is similar to a mutual fund in that it holds a variety of investments but differs in that it is traded like a common stock.) You can also invest in index funds that track the bond market.
Index investing does offer some benefits. Most notably, it’s a buy-and-hold strategy, which is typically more effective than a market-timing approach, in which individuals try to buy invest ments when their prices are down and sell them when the prices rise. Attempts to time the market this way are usually futile because nobody can really predict when high and low points will be reached. Plus, the very act of constantly buying and selling invest ments can generate commissions and fees, which can lower your overall rate of return. Thus, index investing generally involves lower fees and is considered more tax efficient than a more active investing style.
Also, when the financial markets are soaring, which happened for several years until this year’s downturn, index-based invest ments can certainly look pretty good — after all, when the major indexes go up, index funds will do the same.
Conversely, during a correction, when the market drops at least 10% from recent highs, or during a bear market, when prices fall 20% or more, index-based investments will likely follow the same downward path.
And there are also other issues to consider with index-based investments. For one thing, if you’re investing with the objective of matching an index, you may be overlooking the key factors that

Compounding Customized Medication for Over 15 Years
by Lauren Stumberg | photos by Ron PritchettDDr. Leigh Ann Greenberg, owner and pharmacist of Annie’s Apothecary, is passionate about solving problems. In a world of one-size-fits-all massproduced medicine, Annie’s Apothecary focuses on being the best resource to achieve optimal health and wellness through the art of com pounding, nutritional supplements, and patient education.
At its origins, pharmacists prepared all medicines. During the 1950s and 60s, with the advent of manufacturing, compounding declined. The pharmacist’s role shifted to a
dispenser of manufactured medication. More recently however, physicians and patients once again realize the benefits of preparing cus tomized medications to meet specific patient needs. The success and growth of Annie’s Apothecary over the last 15 years suggests there is definitely a demand for the special ized medications provided by a compounding pharmacy.
Dr. Greenberg earned her Doctorate degree from the University of Texas at Austin Pharmacy School in 2004 and opened Annie’s
Apothecary in August of 2007. According to Leigh Ann, “Prior to opening my pharmacy, I managed a hospice pharmacy where compounding proved to be very rewarding in controlling end of life symptoms. I learned how to manage people and developed my skills as a compounding pharmacist.”
Compounding is a collaboration between the patient, physician and pharmacist, a process that is regulated by state boards of pharmacy. There are countless reasons why a prescription might need to be compounded, but according to Leigh Ann, the most important one is to optimize treatment. “For example, a patient may need a dose of thyroid medication that is not available from a drug manu facturer. We can buy the bulk powder and make any strength the prescriber wants the patient to take. Not all commercially available medications are the right fit for every patient, and that is where we come in. It is our job to specialize the medications to help the physician and patient achieve a more positive therapeutic outcome.”
In addition, Annie’s Apothecary prepares medi cine using unique delivery systems, such as a sublingual troche or lozenge, a topical cream or even a lollipop. Often times, certain drugs are taken off the market due to low sales volume; however, there are patients that still need those particular medications. A compounding pharmacy can recreate that medication for those patients. A caveat to compounding is that exact replications of drugs on the market cannot be compounded due to FDA regulations.
When it comes to kids and pets, getting them
Annie’s Apothecary is the only PCAB (Pharmacy Compounding Accreditation Board) compounding pharmacy in Kendall County. While all pharmacies must meet licensure requirements, PCAB allows pharmacies to distinguish themselves as a high-quality pharmacy, committed to the highest compounding standards. PCAB accredited pharmacies have gone the extra mile to demonstrate that they comply with nationally accepted quality control, quality assurance and quality improvement standards. Every three years after the original accredita tion date, PCAB sends out a surveyor for another on-site evaluation and each year they must send off a full report of their practices to the PCAB review team to ensure they are continuing to meet their high standards. With this in mind, you can trust that you are getting the best quality possible.
continued from page 17
to take their medication is not always easy! The staff at Annie’s Apothecary works with the physician and patient to select a flavor, such as tanger ine or marshmallow, that makes the taste more pleasant. According to Dr. Greenberg, “We actually do a lot of work with veterinarians. That area of our business has really expanded over the years as pet owners are looking for an easier way to give medication to their furry friends. For example, vets can prescribe a topical gel to rub into a cat’s ear flap instead of owners fight ing to get a pill into the cat.”
The pharmacists at Annie’s Apothecary also offer consultation ser vices. This is a one-on-one discussion where they evaluate your medication list, clinical symptoms, lifestyle, diet, and lab values, if available. The goal for every appointment is to create a customized and achievable plan to help you feel better. After the con sultation, the pharmacist continues to partner with you throughout your jour ney to whole body wellness. Annie’s Apothecary offers testing services for hormones, food inflammation and neu rotransmitters. “With these tests we help people of all ages target certain things that are causing them problems. We’ve started seeing lots of children come in for the food sensitivity tests and it is very rewarding to see the positive impact that these changes in diet or supplementation can have on their lives, even with conditions such as ADHD and autism,” said Leigh Ann. “Upon the results of the tests, we offer guidance on diet and supplements and work with their physician for any prescriptions they may need. If they don’t have a relationship with a prac titioner, we have several in Boerne we routinely work with.”
In addition to Leigh Ann, Annie’s Apothecary is made up of a phenom enal team. Pharmacists Dr. Rachel Krajewski, RPh and Lisa Stokes, RPh are joined by Compounding Technicians Martin Sunter, Sandy Ramirez, Jenna Leyva, Jon Cuellar and Nutritional Advisor Shawn Stakes. With the business in good hands and her
staff running the day-to-day operations, Dr. Greenberg found herself reaching out within the community and becoming more involved in the Greater Boerne Chamber of Commerce, specifically the Healthcare Connection Committee. According to Leigh Ann, “While becoming involved in these organizations, I noticed a shortage of attorneys present that could help with the formation of nonprofit organizations, pro bono services, etc. I went with my Leadership Boerne class to the Supreme Court of Texas and I was overwhelmed with the feeling that this was where I needed to be! Next thing I know, I started St. Mary’s Law School in the Fall of 2020 and will soon graduate.” While Dr. Greenberg is not quite sure where her law degree will take her, she knows that it is all falling into place with her love and desire to fix problems.
“Our business is really all about problem solving. Helping people through the art of customized medicine is something that I’ve always known I wanted to do. Being able to do just that for these past 15 years has been a dream come true!”

Providing Quality, Affordable Healthcare in Kendall County
IIn 2002, Ms. Joyce Clair, a local Nurse Practitioner, had a burning desire to provide affordable healthcare to the uninsured individuals of our local community. Joyce described her inspiration as being a call ing from God. As a result, Clair was able to bring together a group of volunteers, and along with the help and support of many others, Hill Country Mission for Health was established. This past May, the orga nization celebrated their 20th year!
In the beginning, HCMH operated as a mobile medical clinic that was purchased by funds from private donations and an initial grant from the Kronkosky Charitable Foundation. For the first four years, the mobile unit operated in the Boerne, Comfort, and Bandera areas. Then, in 2006, the Boerne Rotary Club completed their Century Project by building and donating a brick-and-mortar structure to the Mission on real property located at 122 Commerce Avenue in Boerne, then owned by the Rotary Club. The building provides real and expanded space to include two exam rooms utilized to see more patients, eliminating the need for the mobile clinic, which up to that point was still being used for medical exams. A small amount of administrative office space and a garage to house the mobile clinic were also part of the new structure. In 2016, the Rotary Club sold the real property to HCMH where the medical staff now provides primary healthcare to over 1,000 patients annually.
“Over the years we have been blessed to be able to provide primary medical care for eligible, low-income residents of our community,” said Connie Padgett, Executive Director of Hill Country Mission for Health. With a current staff of eight on payroll, HCMH employs an Advanced Practice Nurse Practitioner and a Registered Nurse to see and treat patients, and a phlebotomist/lab technician for in-house lab work. The clinic also enjoys the service of several volunteers, with and without medical training, including its Medical Director, an orthopedic doctor, and a dermatologist. The clinic’s goal is to collaborate with specialty physicians, especially those whose expertise includes endocri nology, cardiology, pulmonology, ophthalmology, and dentistry, who are understanding of the healthcare needs of the demographic served by HCMC.
Hill Country Mission for Health has never been a free clinic, but rather a 501(c)3 charitable health care clinic that provides primary healthcare for eligible adults between 18 & 64 years of age, who are low-income, are uninsured or under-insured, and who reside in Kendall County communities within the Boerne Independent School District. Accordingly, the services of this clinic include assessment, diagnosis, medical management, and counseling regarding lifestyle choices that impact overall health and wellness, as well as disease management.
According to Padgett, “Each new patient must be interviewed to establish qualification for our services. Their application packet is then submitted to our Nurse Practitioner, Sarah Martinez, APRN, FMP-C,
who then determines whether the patient’s healthcare needs fall within our scope of practice. If they do not qualify, we will then refer them to other facilities that will fit their needs. We do everything we can to help our patients, and when we can’t, we try to help them find services or specialists that will work with them or offer discounted services.”
“A lot of our time spent with patients is educating them on how to better take care of their health. We have two volunteer licensed RN’s who devote their time at the clinic to just this.”
Something new for Hill Country Mission for Health is that they recently received their Class D Pharmacy license from the Texas State Board of Pharmacy. “This will allow us to receive donated medica tions from pharmaceutical companies and other charitable humanitar ian organizations that we will be able to provide to our patients at no charge, and is something we are very excited about,” said Padgett.
Just as it did for everyone else, Covid proved challenging for HCMH too. “We had to screen patients at the door and follow all the mandates in place at the time, and as a result were only able to see one patient per hour. Finally in mid-2021 we were able to unlock the door and start to get back to normal. We stayed open the whole time though, and by adjusting our protocols made it through. We are so grateful that a few of our funding partners generously donated funding to help us keep going as well.”
“Every day some kind of blessing comes through the door. It is wonderful to see what or who shows up,” added Padgett. The kind ness and generosity of this community is truly spectacular. Through the unselfish donation of time and effort from our wonderful staff, volunteers, and treasured donors, we can keep our costs low, maximize our resources, and provide the much-needed care for our patients that they otherwise would not be able to receive. Our organization has been tremendously blessed by our funding partners, as well as so many wonderful individuals, businesses, and agencies in our community. We constantly pray for their continued support.”

The Reading List
review by Adrianne Carnesa favorite. This book is about books,

with his wife, shopping and going to the temple. He worries greatly about his granddaughter who spends most of her time reading, and not enough time living.
Aleisha is a smart teenage girl with her own worries and reasons for escap ing the realities of her family. Aleisha works at the local library where she finds, what looks like a forgotten piece of paper, in the back of “To Kill a Mockingbird.” The paper holds a list of novels that she hasn’t heard of before. Curious about why the paper was left and about the books listed, she begins to read each novel. As she reads the books, she meets people in the community who are new to her.
Mukesh arrives at the library hoping to find a way to connect with his grand daughter, and he meets Aleisha. She wonders if the list of novels will also help Mukesh, so she shares it with him. The list forms an unlikely bond between two lonely people, extending the love throughout the community.
This story is about how books con nect us to other people and how their transformations are more powerful than we imagined.

Four Phantoms In Concert to Appear in Boerne Performing Arts 2023 Season
WWhile Boerne Performing Arts is making plans to welcome artists from Tbilisi, Georgia to Peking, China for its’ eleventh season, there has been one adjustment in the anticipated lineup. Artists and touring groups world-wide are facing challenges in resuming their performance schedules as audiences and venues transition to a post-covid environment. As a result, The British Invasion, one of the groups in this season’s lineup, had to cancel their entire tour due to start-up production and touring costs. Fortunately, Boerne Performing Arts was able to secure “The Four Phantoms In Concert” celebrating the unforgettable music of Broadway and more, featuring an extraordinary quar tet of performers, all of whom have performed the leading role as the Phantom (a combined total of 6,000 performances) in the Tony Award winning musical The Phantom of the Opera. This thrilling evening of entertainment will be performed by Brent Barrett, John Cudia, Franc D’Ambrosia and Ciarán Sheehan under the musical supervision of two-time Grammy Award winner David Caddick and music director Ryan Shirar. Joining
the Four Phantoms is special guest Kaley Ann Voorhees, one of the youngest sopranos to play the role of Christine on Broadway. It promises to be a fan tastic show celebrating the legendary work of the many composers and artists who have made the Broadway musical an American art form.
The performance will take place on the same date as the prior engage ment, on January 26, 2023. If season tickets have already been purchased, they will reflect the Four Phantoms In Concert performance when tickets are mailed out in early November.
There is still time to purchase season tickets before individual tick ets are made available for all shows in the 2023 season. By visiting BoernePerformingArts.com, you can view video clips for all artists in the upcoming season including The Four Phantoms In Concert on January 26, 2023, Royal National Dance Company of the country of Georgia on February 24, 2023, and The Peking Acrobats on March 24, 2023. In addition, season ticket holders may purchase tickets for the holiday special event December 15, 2022, by Canadian Brass before individual tickets are released to the public on November 1, 2022.
