Firm Overview & Experience The following is in response to proposal questions 1,2&3 contained in the RFP.

1. Firm Overview LBA Interiors, a component of Lantz-Boggio Architects, specializes in the design of Senior Living and Care communities. LBA Interiors with a 18 year history and numerous award wining projects utilizes its unique expertise in Senior Living and hospitality to create Senior Environments that maximize personal approach to care and opportunities for life experiences LBA Interiors is recognized as a National Leader for the design of Senior Living and Care Communities. The firm has completed projects in 30 states, encompassing the full continuum of Senior Living and Care as well as special environments for memory impaired. With a staff of 10, including 8 Interior Design Specialists, the firm is procedurally organized to perform services anywhere in the country. The staff and associates of LBA Interiors are among the most talented and experienced designers in the Senior Living and Care Industry. Team members have a broad depth and breath of experience and all work synergistically in order to meet the scheduling objectives of its clients.

Compliance with Regulatory & Accessibility Requirements With completed projects in 30 states, LBA Interiors has extensive knowledge of Federal and State Licensing and Accessibility Regulations for Skilled Nursing and Assisted Living communities in Illinois and throughout the United States. The firm has completed 4 Senior Living and Care projects in the state of Illinois including a 156 unit Assisted Living and Memory Care community in September of 2014.

Our Approach At the outset of the Presbyterian Homes Arlington Heights Project, we will conduct a regulation update study to determine all current licensing requirements of the Illinois Department of Health (IDPH), that are applicable to our scope of work for this project. We will also conduct a standards and regulations updates study of the Federal and State Fair Housing and ADA Regulations. Further, we will research all applicable building codes and amendments adopted by the local Building Department for permitting and occupancy of the project. We will then conduct a Building Code Study that comprehensively outlines applicable compliance criteria for inclusion and coordination of our entire scope of work. All research, reviews and studies referenced above will be compiled into an inhouse compliance manual for this project. We will work closely with RLPS as the Architect to assure that all compliance issues relating to licensing, regulations and codes are coordinated with the work of the Architect.

North Shore Place Northbrook, Illinois Intended to serve a range of acuity levels, the building’s interior design incorporated best practices for aging in place. Subtle techniques for achieving this include color selections and patterns that create contrast of furniture and fabric to the flooring, an abundance of low energy lighting, strategically placed finished materials, local artwork for way-finding and Universal Design for furniture placement so residents and guests with physical challenges can maneuver throughout the community while enjoying its amenities. These non-institutional design elements are especially appropriate for those with visibility and cognitive impairments. The common areas are akin to 5-star hotels. The main lobby is colorful, spacious, and feels rich with deep red accents, white marble floors, black granite accents, and comfortable contemporary furniture.

Sunny Vista Living Center Colorado Springs, Colorado This new senior community features 116 beds of skilled nursing and rehabilitation along with a state-of-the-art town center. The building is positioned on the site to take advantage of the commanding view of Pikes Peak to the west. The Colorado lodge-style architecture enriches the resident experience with natural building materials and amenities including a predominance of private suites, living room and bistro, general store, beauty salon and spa, activities, private dining, chapel, sun porches and other social gathering and common areas.

The initial programming for the community indicated the need for 58 beds of skilled nursing and 58 beds of rehabilitation. The project is strategically designed for a portion of the resident rooms to be used as either of these components, thereby, allowing management and staff the ability to respond to market fluctuations and demands. Senior Environments Clermont Park CCRC Denver, Colorado Originally constructed in the 1960’s and 1970’s, this senior living campus is utilizing a multi-phase repositioning program to replace the housing components that are obsolete, upgrade the buildings that remain, and add a new town center with lifestyle features and amenities that will allow the campus to attract current and future generations of seniors. The project will consist of a new 2-story skilled nursing facility, new 4-story independent living facility, new 1-story town center, and upgrades/renovations to the existing tailored living, assisted living, and memory care building. When complete, the campus will offer 60 beds of skilled nursing, 15 units of assisted living, 17 units of memory care, 92 units of tailored living, and 71 units of independent living.

Senior Environments Azura of Lakewood Lakewood, Colorado This project consists of 108 beds of skilled nursing and rehab in a 2-story building. The design is based on a 9-resident household, each centered around an informal morning room. Three households are then organized into neighborhoods that each share living, dining, and activity spaces. The entire resident population also has access to the Town Center that includes a bistro, private dining, salon, and activity spaces. The second floor features an expansive state-of-the-art physical therapy gym to which services are offered to the surrounding community. Size 79,500 SF

Senior Environments St. Paul Health Center Renovation Denver, Colorado The urban setting of this building inspired the contemporary interior design and was intended to feel home-like and encourage a feeling of well-being. Updating this 32,200 SF nursing home included adding a new two-story lobby addition to the front of the building and substantially remodeling three skilled nursing floors. The remodel included converting 22 beds from skilled nursing to rehab and the remodel of the 6th floor. The design goal was to modernize both the exterior and interior of this 40-year-old building and to provide a much more livable environment. Semi-private resident rooms were greatly improved with new finishes, built-in furniture, and a windowed privacy wall. Private showers and baths were also added to resident rooms. Senior Environments Mill Vista Lodge at Wind Crest Highlands Ranch, Colorado Lantz-Boggio Architects was commissioned by Erickson Living to design a new prototype care center for their existing CCRC in Colorado. The objectives of the prototype were to house all levels of care within the Continuum to complement the existing independent living units which already existed on the campus. Care components in the building include 56 units of Assisted Living, 34 units for the Memory Impaired, 42 Skilled Nursing beds including Long-Term Care and Short-Term Skilled Nursing Rehabilitation. The facility known as the Mill Vista Lodge also contains in-patient and out-patient skilled therapy gyms with accommodations for physical therapy, speech therapy and ADL therapy.

Services & Fees Scope of Work LBA Interiors will provide interior design services for the Presbyterian Homes, Arlington Heights Campus to include: Project 1 Community Expansion This project includes a 70 unit new assisted living residence, a 20 unit new memory care residence and a 6,000 square foot chapel addition. Project 2 Community Renovation The project includes the renovation of approximately 5,000 square feet of space in the wellness areas of the commons (adjacent to the new construction in Project 1). All work will include coordination with the Owner and the Owner’s A&E design team.

I. Schematic Design Phase LBA Interiors will work with the Owner, Architect and Project Team in conceptualizing a design that works well to represent a thoroughly developed character and program for the building. We will assure that we have a thorough understanding of your expectations for the outcome of the project with regard to function, quality and the image for the project. At the onset of this assignment, we will work closely to understand the overall goals of the project. We will work with you and the Architect to establish the design, function and budget estimates. This budget will be utilized by the design team as the gross maximum cost for Interior Design FF&E selections. All decisions and conclusions with regard to budget and concept that the Project Team jointly establishes will be summarized in a final design/budget approval meeting. Upon approval, we will begin the Interior Design FF&E phase of the project. We will provide conceptual space plans and furnishing options with decorative lighting layouts and preliminary FF&E budgets to review with the Project Team.

Services & Fees II. Design Development Phase LBA Interiors will meet with the Project Team to develop detailed design criteria for the operation and function of all areas. We will meet all established deadlines set forth by the Owner, Architect and Project Team. We will prepare interior design contract drawings as needed for the required scope of work. Furnishings and finishes will be selected based upon our understanding of the intended uses for each space. We will consider the operational program, along with the functional intentions of each space in the design concept. We will utilize the best industry practices in the development of all design and product selections. We will then conduct presentation meetings at which we will present to the Project Team the initial finishes, layout and color concepts along with FF&E for the project. Upon receiving input, we will make revisions to the selections as needed. When these materials have been approved and the budget is confirmed, we will begin the FF&E selection process. We will conduct a final design presentation meeting including all finishes and FF&E. This will include floor finish, wall finish, furniture, fabrics, artwork and accessories. LBA Interiors will provide written specification for all finishes and FF&E. FF&E will be purchased after complete pricing is approved.

III. Construction Documents Phase LBA Interiors will provide construction documents and written specifications to the Project Team for bidding and construction. Construction documents are to include reflected ceiling plans, decorative light fixture selection, floor and wall finish plans, furniture plans, interior elevations, sections and details, artwork plans and design fabrication drawings as required. Construction documents to be delivered to meet the Architects SD/DD/CD schedules. All documentation will be directed to the Architect for distribution unless otherwise requested.

Services & Fees We will provide marketing boards for presentation use for displaying materials, finishes and furniture. In addition, we will provide comprehensive finish and FF&E specification binders with all applicable information for purchasing. LBA Interiors will prepare updated FF&E estimates of installed costs including products, services, freight and taxes along with coordination and review of all documentation.

IV. FF&E/Construction Administration Phase LBA Interiors will manage the overall procurement process for an organized and timely execution that meets the installation schedule. We will ensure that all products purchased are only those that have been pre-approved, and will not be substituted for products of lesser quality. Re-selections will be provided as needed if those specified do not meet the budget and/or delivery schedule. LBA Interiors will confirm that all selections for the FF&E meet the final budget. We will obtain and review vendor pricing and notify Presbyterian Homes of deposit requirements and payment schedules. We will serve as a liaison, coordinating the construction schedule and procurement, delivery and installation of all FF&E items. LBA will respond to all applicable interior ASI’s, RFI’s and Submittals. All responses will be issued to the Architect for distribution. We will ensure appropriate electrical location on site for all furniture placements. LBA will observe construction quality for casework, millwork, interior floor and wall finish and provide on-site construction administration review as required.

V. FF&E Installation • We will coordinate regularly with you to maintain the FF&E installation schedule for all products provided in the FF&E procurement package; including all upholstery, case goods, artwork, accessories, lamps, foliage, patio furniture, window treatments and interior signage (as required). • We will work directly with the project installation crew to assure furnishings, artwork, drapery and accessories are placed in accordance with the design intent. • LBA’s Interior Design Team will be at the installation when the FF&E is delivered into the building. We will supervise the installation crew to ensure all installation and/or warranty issues are documented and that the installation is completed thoroughly and effectively. Services & Fees Schedule & Meetings LBA will provide communication, meetings and Construction Administration Services for this project as needed. We will coordinate directly with the Owner or Owner’s representative, the Architect, Consultants, and General Contractor (as required). Based upon a schedule mutually established with the Project Team we will submit a Schedule of Performance to conform to the anticipated completion schedule.
