=================================================================== Overview Description of Special Service Project: The primary step in the Employee
Wellness Strategy is to gather data of the target population. We will first implement a mandatory procedure to be completed by all new Horizon Health Network employees, which will come in the form of an employee health profile. The employee health profile will be the foundation and cornerstone for the Employee Wellness Strategy. The results will not only identify the specific needs of the employees but also identify their interests and wishes; therefore it will be used to guide future employee wellness initiatives to ensure timely and relevant programs and services are offered and are region-specific. The employee health profile will focus on physical health, mental health, voluntary health practices, individual perceptions, readiness to change, and employee interests. The second major task included in the employee health profile is to develop a regional immunization and testing tool to be implemented across the Horizon Health Network. This would be used for all employees including full-time, part-time, casual, students and volunteers in all regions under the Horizon Health Network. This tool would be implemented to complement the Horizon Health Network: Policy & Procedure Manual Immunization of Personnel policy, which states, “all employees and nonemployee personnel are required to comply with immunization and communicable disease surveillance requirements”.
