Dear Family Medicine Resident,

We are excited to present the Heartland Health Alliance (HHA) Family Medicine Reinvestment Program (FMRP). The program was established in 2018 through a collaboration between the Lincoln Medical Education Partnership (LMEP), Bryan Health, and the HHA.

The program’s purpose is to invest in rural family medicine physicians by supporting residents during their three-year LMEP residency. Twenty-six (26) member hospitals from the HHA have come together to fund stipends for participating residents. In these rural communities, you will leverage your diverse family medicine skills and develop deep rooted relationships, all while making a significant impact on rural medicine.

In exchange for recruitment to one of the 26 participating HHA hospitals, residents can expect:

• A $1,000 monthly stipend distributed during your three (3) years of residency with the signing of a promissory note. Two (2) years of practice at a participating hospital is required to satisfy the promissory note.

• If you execute the Agreement to participate in the program within the first thirteen (13) months of your residency training at LMEP, the Monthly Stipend will be retroactive to the beginning of your residency training.

• Dr. Sabrina Cerny, LMEP Program Director and Dr. Doug States, LMEP Faculty Physician are available to discuss this program.

• Moonlighting opportunities are available in several hospitals within the program.

• A Physician champion is available at each participating hospital to provide you with firsthand experiences and to answer your questions about practicing medicine in their community.

• Previous program participants are available to share their experience. Eight (8) residents from LMEP have joined and signed with a participating hospital.

Beatrice Community Hospital and Health Center (BCH) is a community Critical Access Hospital (CAH) in Beatrice, Nebraska, located about 30 miles south of the state’s capital, Lincoln, Nebraska.

BCH is the largest CAH in Southeast Nebraska and Northeast Kansas and has become a hub for healthcare services outside of the Lincoln metropolitan area. A new hospital was built on the northern edge of the community in 2012, and a new primary care provider clinic was opened in 2023.

A full-service hospital, BCH offers inpatient acute care, labor and delivery, emergency services, express care services, cardiopulmonary services, a sleep lab, diagnostic imaging, general surgery (including robotic surgery), rehabilitation services, laboratory services, orthopedic services, pain services, wound care, specialty provider clinics, women’s and children’s services, infusion services, behavioral he alth services and aesthetic services.

The Boone County Health Center, located in Albion, Nebraska is a recognized leader in providing a continuum of healthcare to the 12,000 rural residents in Boone, Antelope, Greeley, western Madison, Nance, western Platte and Wheeler counties in north central Nebraska for over seventy years. The Health Center and medical clinics are the singular and primary source of healthcare for the rural communities it serves. The hospital offers 22 all private, spacious suites, five nursery layettes and two family -oriented obstetrical suites. The Health Center is the hub for five medical clinics in the towns of Albion, Spalding, Newman Grove, Fullerton, and Elgin.

Seven Family Practice physicians, six physician-assistants, and three nurse practitioners staff the Health Center and satellite medical clinics. Overall, the Center employs over 3 40 medical and support staff, and is the largest employer in the service area. The Health Center also works in tandem with physicians across the state to offer professional outpatient clinics.

Boone County Health Center is an award-winning facility having received a 5-Star rating from CMS for five years. We are the only hospital in Nebraska to be voted top hospital in patient care among hospitals of all

Moonlighting Opportunities: Moonlighting is available in our emergency department and patient floor as hospitalist. Residents will come in and cover for openings in the shifts. Hospitalist shifts run 7-days a week; Tuesday through Monday 7am6pm with tele-hospitalist covering nighttime. Brodstone Family Medical Center-Superior is located on the hospital campus with two, satellite Brodstone rural health clinics located in Edgar and Nelson. With remodeling projects completed in 2005, 2010 and 2017, Brodstone is a beautiful facility with numerous ancillary services available. Brodstone has recently undergoing a 41,000 square foot expansion to the facility with a new Emergency Department, Patient Wing, OB Wing, Infusion Center and room for growth. The project was completed in the fall of 2023 and is seeing patients in the new space. There are currently nine providers and a reputation of a great place for students to learn about healthcare in a rural setting. Nearly 30 specialists hold clinics at Brodstone each month in the Specialty Clinic.

Our community is driven by agriculture. From farming to ranching we have a variety of opportunities. We work hard but also like to play with our local Ainsworth Municipal Golf Course, hunting and fishing opportunities, camping and the Cowboy Trail. The Cowboy Trail offers biking, hiking, or horseback riding opportunities and runs right through Ainsworth.

Ainsworth has a population of over 1,600 people according to the 2020 census and we take pride in our local community.

We are a 19 bed Critical Access Hospital (CAH) in the sandhills of north central Nebraska. We have our local Rural Health Clinic (RHC) - Ainsworth Family Clinic as well as an active Specialty Clinic hosting specialists from a variety of areas that benefit our patients. Our employees take an active role volunteering in the county in many ways; from our facility grown garden, to highway trash cleanup and volunteering at our community movie theater.

Decatur Health is a Critical Access Hospital and RHC Family Practice clinic in Oberlin Kansas. We provide healthcare across most of the life span. The hospital does not do OB. Our Family Practice clinic sees all ages and we have access to specialist monthly. Hospital coverage includes ER call, 1 weekend a month and 1 to 4 additional days per month. Hospitalized patients are the admitting provider unless they are out then the on-call person will cover them.

The City of Benkelman, the largest Community in Dundy County, is located in southwest Nebraska, approximately 50 miles west of McCook, Nebraska. The city is served by U.S. Highway 34, and State Highway 61, which bypass the Community to the north and west. Benkelman is the County-Seat of Dundy County, with an estimated 2022 population of 815.

Downtown Benkelman is in the south-central portion of the Community, along Chief Street between West 8th Avenue and Railroad Street. The Downtown consists of professional offices, specialty retail stores, banks, restaurants, the Dundy County.

Library, Post Office and various other public facilities. The City Offices are located at the intersection of East 7th and “A” Street, just east of the Downtown core. The Community of Benkelman offers a variety of amenities including public schools, restaurants, governmental services, grocery and convenience stores, senior center, hospital, etc.

Most of the major employers in Benkelman are scattered throughout the Community and are traditional in nature when compared to other rural Nebraska communities. Major employers include Dundy County- Stratton Public Schools, the Dundy County Courthouse, Dundy County Hospital and Viterra USA Grain, LLC. Blackshirt Feeders, LP is in the process of building a 150,000 head beef cattle feedlot that will operate in Dundy County close to Haigler, NE. According to the U.S. Census Center for Economic Studies, in 2019, an estimated 431 persons were employed in the City of Benkelman. This accounts for an estimated 76.8 percent of all persons employed in Dundy County, which recorded 561 employed persons in 2019.

Dundy County Hospital

Dundy County Hospital is located in Benkelman, Nebraska and was built in 1968. The Hospital serves the southwestern corner of Nebraska, northeastern corner of Colorado, and portions of Kansas. Dundy County Hospital was initially constructed to accommodate 30 acute and swing beds. The Hospital is a Medicarecertified facility and in December 2000, it lowered its bed size to 14 to meet the conversion requirements of the "Critical Access Hospital" designation. In 2020 the bed count was lowered to 12. A priority of the Hospital is to continually update its equipment to ensure it is state -of-the-art. In 2020 the Hospital built on a new Physical Therapy Department with a therapy pool. With the community support the 3.5 -million-dollar project was built debt free. The Hospital also has access to helicopter transport to increase its ability to offer more s ervices.

OGH, though a relatively small facility, has remained current with modern technologies. OGH utilizes the Cerner EMR for the recording of all patient encounters. Within the building are housed a 64-slice CT, Digital Radiography (both fixed and mobile), Siemens Ultrasound, C-Arm, and modern, up-to-date laboratory testing equipment.

The Sullivan County Memorial Hospital, located in the county seat of Milan, has been an integral part of Sullivan County since it was established in 1953. This thriving rural area has experienced significant growth and diversification since the opening of a Smithfield pork processing plant in the mid 1990’s. Milan offers safe and affordable housing, excellent schools and friendly neighborhoods. Larger communities of Kirksville and Trenton are 30 minutes away; however, Milan offers grocery and retail shopping along with restaurants, a public swimming pool, a golf course and the development of a 2300-acre reservoir to open in 2026, which will provide a water source and recreational hub.

Sullivan County Memorial Hospital

SCMH has been certified as a Critical Access Hospital since 2000 and follows its mission to provide quality health care to the community through services such as Acute Care, an Emergency Department, three Rural Health Clinics, in-house Cardiac Rehabilitation, Speech, Occupational and Physical Therapy, and Skilled Care provided within the hospital. Rheumatology, Urology, Podiatry, Orthopedics, Wound Care, OB/GYN, and Pain Management are offered as visiting specialties. Services are provided and overseen by a team of three Physicians, collaborating closely with five Advanced Nursing Practitioners.

SCMH is currently seeking licensure for a retail pharmacy which is anticipated to open in fall 2024. Even more exciting are the preliminary discussions that are underway regarding construction of a new hospital, with even greater health care opportunities for residents. While nearly 200 rural US hospitals have closed in the past two decades, SCMH remains a vital, growing asset to residents of Sullivan and surrounding counties and has a bright, promising future.
