BY DAWN KAHLNestled within a small shopping center in downtown Portland lies a 6-operatory dental office that has served the community for over four decades.
The practice, owned and managed by Dr. Martha Rich, has been a consistent presence in the area, and her dedication to her craft is evident in the generations of loyal patients she has cared for and the staff members who have worked alongside her for almost 30 years.
Dr. Rich has owned her practice since 1982, and for her, it has always been about one thing: the patients.
“We’ve been through a process of learning, listening skills, learning to ask questions about the patients. And I think it’s those relationships that have that have made us successful.”
From 1984 to 1991, Dr. Rich served as an associate professor at the OHSU School of Dentistry, specializing in Fixed Prosthodontics.
In addition, she treated complex occlusion cases for her colleagues and newer generations. It was here that Dr. Rich’s interest in orthodontics began. As she dove deeper into the world of temporomandibular joint (TMJ) disorders and the science of occlusion, she was intrigued by the possibility that adjusting a patient’s bite could improve the functionality of the entire system.
This spurred her to embark on her next adventure – several hundred hours of study in functional orthodontics, specifically TMD, TMJ, and other jaw disorders.
In addition to her memberships in the ADA and ODC, Dr. Rich is highly active in AGD and the Oregon Academy of General Dentistry. She also belongs to the American Academy of Craniofacial Pain, the American Academy of Oral Systemic Health, and the American Academy of Physiological Medicine and Dentistry. She belongs to several study clubs and is a past member of the International College of Craniomandibular Orthopedics, International Association of Orthodontics, and the American Academy of Dental Sleep Medicine.
Dr. Rich is interested in preventing a variety of dental diseases and performs airway screenings for kids in order to prevent systemic diseases and sleep apnea issues later in life. She often fabricates sleep apnea appliances for kids if necessary.
“The interesting thing now that we know about sleep apnea is a lot of times, it starts in childhood. So we’re trying to pick out kids that are having breathing problems early on and try to nip that in the bud and get them to breathing well. ”
“And now we’re working with a whole new field of therapists called myofunctional therapists that are teaching people to swallow correctly, chew correctly, breathe correctly,” she explains.
Dr. Rich became interested in TMD in dental school but felt like she didn’t get any information on it. She said the patients were coming in and being told to just “stop chewing’, which she thought was terrible advice.
After her years of research, she diagnoses and treats TMD with a multi-disciplinary approach, starting with a thorough evaluation in the office and the careful use of splints for accessing and healing the joints, if necessary. “We participate in care with physical therapists, acupuncturists, chiropractors, massage therapists, naturopaths, myofunctional therapists, and craniosacral therapists. We use TENS units in the office to promote relaxation, along with infrared therapy, heat, ice, and lasers.”

She notes that this does sometimes lead to functional orthodontics or occlusal reconstruction.
Burkhart Account Manager Matt Milligan has worked with Dr. Rich and her practice since 2016 and recognizes that dentistry is more than just a job for her. “I think that’s the difference in someone like her… it’s not a skill that she found out she was good at… it’s a passion for taking care of people and being involved.”
Her commitment to learning and providing the best service to her patients has earned her the respect and admiration of her staff and patients alike. Dr. Rich not only invests in her own training but also ensures her staff receives the same Continuing Education credits by training alongside
her. “She has done a great job and is still creating a great atmosphere where they feel like they are part of the team,” Milligan says.

Rich was recently awarded the Oregon AGD General Dentist of the Year, which is given annually to individuals who exemplify qualities such as service to dentistry and the community, excellence in clinical skills, and contributions to and participation in continuing education. She credits her involvement in the recent opening of a new Continuing Education Center at the bottom of the iHeart Radio building downtown as a big part of the reason for the award.
“Dentistry is a fabulous profession. We have the ability to spend a lot of time with people and get to know them better than their physicians typically know them. And I think that’s an honor, but it’s also a challenge to us to keep learning and keep trying to help them.”
Recently, Dr. Rich has taken on a partner in the practice, Dr. Sam Wells, who shares her values and will help allow her to shape the future of her practice. “I am stepping back from clinical dentistry a little bit. But I also do a lot of TMD, chronic pain, patient treatment, and treatment for sleep apnea with oral appliances. And so, I will keep on with that as long as I can.”

The Impact of DUWL-Borne Infections
Recently, the Health Advisory released via the Health Alert Network (HAN) that was prompted by the CDC’s notification in March 2022 of an outbreak of suspected nontuberculous mycobacteria (NTM) infections in children who had undergone dental procedures at a pediatric dental clinic which is currently under investigation. 2
This was following the devastating 2016 contamination at a pediatric clinic in Anaheim, California and 2015 in Atlanta, Georgia which both impacted children ranging from 4-8 years.
The contamination of dental waterlines in Anaheim resulted in the potential exposure of at least 500 pediatric patients. This exposure was followed by 221 ongoing lawsuits and 71 confirmed cases of NTM infections after pulpotomy procedures. The outbreak in Georgia led to 24 instances of mycobacterium abscessus infections in children undergoing pulpotomy treatment.
In both situations, initial tests confirmed that buildup and irregularity or complete lack of testing in the clinic’s DUWLs led to the infections.3 Sadly, these situations were entirely avoidable.
It’s important to note that even for practices that do not perform complex oral surgeries, DUWL-borne infection and illness are still possible in standard dental work.

Dr. Kory Wilson is telling me the story of how he got into the dental industry, and it’s not for the reason you might expect.
His parents ran a dental clinic, so he grew up around it. But Dr. Wilson is an athlete. It’s part of everything he does, and it colors his perspective. When he was 7 years old, he told his parents he wanted to be a horse jockey. Dr. Wilson, who currently stands at 6 foot 5, said his mother looked at her already tall son and told him that unless he was going to jockey that day, he would need to find a different career. As his parents tell it, ever since then, he’s wanted to be a dentist.
Just like that 7-year-old who, once he pivoted and knew from an early age what he was going to do, Dr. Wilson saw early in his dental career that there would be advantages to being part of a larger practice, so he started to explore options to go that route. He wanted to be at the forefront of a changing industry.
He describes how dentistry can be a tough field – often you feel isolated – days can be stressful and full of patients who are not excited to see the dentist. This has created a natural shift towards more of a group model where dentists can commiserate and lean on each other.
Dr. Kory Wilson’s mentor, Dr. James Robson, started Avondale Dental in 1976 with one practice location. When Dr. Wilson joined in 2007, they were already partnered with Burkhart. They’ve added 5 new locations since then.
I asked him what he saw as some of the advantages of being a large dental group. He first mentions pricing discounts on goods and materials. He says it’s difficult to be a single small practice and be able to afford and be a successful practice. More importantly, he says, “I think today, more than ever, dentists really value that shared mentorship and camaraderie, and new dentists coming out of school, I sense they really want to be a part of a group for that reason.”
The first, and biggest advantage Dr. Wilson lists is pricing on goods and materials. “In some ways, I think it’s really difficult being a single small practice and being able to afford and pay for the kinds of technologies and supplies you need in dentistry to be successful.”

Avondale has expanded this idea to include practices that may only have a single doctor, where that doctor can still enjoy the benefits and support of the group mentality.
“I think we’ve developed a bit of a unique model. And in that we allow multiple levels of ownership across our practice group, both at the individual practice level and at the group level, we call it a super group. And so that gives doctors kind of multiple tiers of like, ownership, and responsibility, you know, almost like they would have in an individual practice, but they get the support and expertise of a corporate team. We also run our practices very individualized. So, you know, we’re not cookie-cutter at all. And we really let our doctors make their own decisions and decide how they want their practices to look and run. And we’re just there to support them in any way that we can to help make them successful.”

Prime&Bond active features patented ActiveGuard™ Technology, a significant advancement in adhesive chemistry that actively controls moisture on the prepared surface. Even if dentin is overly wet or dry, Prime&Bond active is simple to use and provides consistent results for a strong, reliable bond.
Most dental adhesives are rather hydrophobic: they separate from water. With too much water on the prepared surface, these rather hydrophobic adhesives fail to cover the prepared surface uniformly. During air-drying, adhesive gaps can form as embedded pockets of water evaporate, increasing the risks of post-operative sensitivity and bond failure.
Active-Guard Technology with active moisture control is exclusively designed to protect against these risks.

Moisture Levels Vary. This Bond Won’t.
The patented Active-Guard Technology balances hydrophobic and hydrophilic features and helps to achieve an optimized surface tension. Thus, Prime&Bond active can overcome the surface tension of water, allowing the adhesive to spread evenly across the dentin and into the dentinal tubules to form a uniform, homogeneous layer. When the adhesive is air-dried, solvent and excess water evaporate uniformly to leave a thin, consistent coating of adhesive across the entire surface. The result is a strong, reliable bond, with virtually no post-operative sensitivity.
Active Moisture Control for Reliable Restorations
In addition to active moisture control, Prime&Bond active provides several benefits over competing universal adhesives, including:
• Active spreading across the prepared surface to minimize the risk of dry spots that can lead to post-operative sensitivity and micro-leakage.
• Low film thickness, minimizing the risks of changing the preparation geometry and compromising crown fit, or leaving adhesive pools that may show up on radiographs and can be misdiagnosed a voids, gaps or secondary decay.
• No need to apply an activator when used with Calibra ® Ceram cement.
