Medication-related problems are a public health concern in the US, with adverse drug events leading to over 1.3 million ER visits annually. Transitions of care and legacy prescribing oft en compound this problem, particularly in older adults. The integration of clinical pharmacy services improves the safety and eff icacy of medication use, as well as reduces the cost of care. Additionally, MTM provided by a pharmacist can mitigate risk for care providers and improve nursing staff eff iciency.

This session will describe how the new assisted living survey process works, will provide you with examples of how providers can use MDH survey forms for selfauditing for compliance and quality management activities, and review survey trends from the first three months of licensed assisted living survey activity, including those specific to providers who hold a dementia care license.

805 | IDDSI EXPLAINED—WHY IT IMPACTS THE LIVES OF OUR RESIDENTS

International Dysphagia Diet Standardization Initiative (IDDSI)— what is it and why are some hospitals using it? How do we keep our residents safe if we do not have IDDSI at our site? All these questions will be answered during this session. It is imperative that even if IDDSI is not currently in use at your site, that you have the knowledge and resources to safely convert an IDDSI diet to whichever diet model your site uses until an evaluation is completed by a speech language pathologist.

While a focus on infection control remains critical in MDH recertification surveys, the remainder of the CMS Requirements of Participation are back into play as MDH resumes routine recertification surveys. Time will also be spent focusing on immediate jeopardy deficiency trends (from standard surveys and complaint investigations) as well as complaint investigation patterns.

Like many of the people in our care, Mable had a fall and has challenging behaviors.
