Summer Camp series Policies Full Summer Rate (must pay $1,400 in full) Parents/guardians receive a discount of $140 by registering their respective camper for the full summer rate. Payment must be made in full to receive the discount. Weekly rates may not be transferred into a full summer rate.
Full Summer Rate - A La Carte (must pay $1,400 in full) Parents/guardians may register their respective camper for individual weeks, and still receive the fantastic advantage of paying the full summer rate. The parent/guardian must sign their camper up for the full summer rate to receive the discounted rate and must specify which weeks and camp name the camper will attend. Weekly rates may not be transferred into a full summer rate.
Partial Payment Plan (not available for full summer rate) Parents/guardians may reserve a spot with a 50% non-refundable down payment for each summer day camp week. Payment in full is due by May 26 (Memorial Day). If payment in full has not been received by May 26, the reservation(s) will be cancelled. Weekly rates may not be transferred into a full summer rate.
Transfer Policy A transfer from one day camp to another day camp is contingent upon the availability of spots within the desired day camp. If the day camp is at maximum capacity, a transfer will be not allowed.
Waitlist Policy There is no fee to be placed on the waitlist. If a spot becomes available in the desired camp the camper’s parent/guardian that is prioritized at the top of waitlist will be contacted via telephone and/or e-mail. The parent/guardian will have 24 hours from the time of the call/e-mail to make payment in full for the desired camp. If payment has not been received in full, the camper’s parent/guardian next on the waitlist will be contacted via telephone and/or e-mail. Each respective camp week has a maximum of 10 waitlist enrollments.
Age Policy The cutoff date for a camper’s age is June 2, 2014 or a camper’s age on the first day of the camp week.
Camp Cancellation & Refund Policy • The department will issue a full refund if the department cancels a camp. • Cancellations for a camp week must be requested a minimum of 1 week from the respective camp week start date to receive a 100% refund, less a $7 service charge for cancellation. • All refunds are subject to the State Board of Accounts claim procedure and may take three to four weeks.

PLAY ON | EAST 5-9 YEARS Welcome to our playtastic camp just for 5-9 year olds. We will focus on having the best summer we can have in Carmel. We will start every day with a morning assembly to get to know everyone and learn new songs. We will be here to have a great time all throughout the day. Before snack, we will have a down time for kids to read and just relax from the day. We will then have snack and closing assembly to end our day. Tuesdays and Thursdays will be swim days. Wednesdays will be the field trip day to take us off site. We will end the week with a vendor to entertain us. All campers must be able to take care of their respective bathroom needs. The ratio of staff-to-camper is a minimum of 1-to-12. Campers are responsible for bringing their own sack lunch and snack.

Let’s get this summer going with some moovin’ and groovin’. We are ready to start the summer off right with games and activities that will keep you going throughout the day. Being a kid wouldn’t be the same without getting a little messy. We might be wet messy or just art messy, nobody really knows. We suggest packing extra play clothes this week. On Wednesday, we are going to the Anderson Center for the Arts where we will create different works of art from around the world. We hope to have a good, messy time. It is hot playing outside, so let’s get wet and have a great time! We hope to incorporate wet and fun into each and every day. On top of swimming on Tuesday and Thursday, we will also go to Morse Beach to swim on Wednesday. Imagination is a very fun tool that anyone can use at any time. We hope to use it all week long. We are going to the movies on Wednesday to see How to Train Your Pet Dragon 2. What is more imaginative than training a dragon? We will treat everyone on Friday with some Cotton Candy. It really is a stretch of the imagination to see how sugar turns to cotton! Insects are all around us, and we intend to have fun with the creepy crawlies. We will crawl, buzz, and inch our way towards fun of all kinds. We will truly feel like we are in flight when we visit Hunter’s Honey Farm. We will see how the honey is produced, make our own beeswax candle, and pour our own honey into a jar to take home. Kids are wild like animals, so let’s be wild. We are going to act like animals all week with our games and crafts. We are going to the Indianapolis Zoo on Wednesday for some awesome animal sightings. SUCCESS ON STAGE 8-14 years Have a transformational summer under the big lights! Success on Stage is the place for campers drawn to the magic of theatre. Choose the Performance side, where you’ll get the chance to act, sing, and dance on stage, or the Production side, where you’ll create backdrops, costumes, and props, and learn how to work lights and sound for a real production. Due to popular demand, this summer we are focusing solely on musicals. The first half of the summer we’ll dig up some comedy in Young Dracula (aka The Singing Bat), and the second half we’ll create the Enchantment of Beauty and the Beast. Each production will take place on a Thursday afternoon at 2 p.m. and host a reception afterwards. Spend the summer with peers who enjoy doing some of the same things as you. Each week presents an opportunity to make choices and spend time doing whatever sounds fun. Want to go on a hike, play cards, or maybe just relax and talk with friends? This summer camp will have plenty of time for that! Amidst all the fun, we will have a chance to cool down at The Waterpark at the Monon Community Center every Monday, Wednesday, and Friday and travel off-site for a field trip every Tuesday and Thursday. Some field trips are within walking distance, so we will have a chance to get plenty of exercise! All campers must be able to take care of their respective bathroom needs. The ratio of staff-to-camper is a minimum of 1-to-12. Campers are responsible for bringing their own sack lunch and snack. Camper money and electronics are not allowed. Please respect this adjustment from 2013.

Are you a 12-15 year old who wants to serve as a positive role model? The I.M.P.A.C.T. (Initiative, Motivation, Professionalism, Achievement, Compassion, Teamwork) program will teach essential life skills in a job-like setting. Members of I.M.P.A.C.T. will attend a weekly job training session that will focus on each of the six elements of the program. Interested individuals must be 12 by June 2, 2014 and will interview to become a member of I.M.P.A.C.T. If selected, each member will be assigned to one of the following summer day camps: Play On, The Summer Experience, Science of Summer, or Adventures in Art.

Carmel Clay Parks & Recreation 2013-14 Extended School Enrichment & 2014 Summer Camp Series Registration Form PARENT AUTHORIZATION, WAIVER and RELEASE CHILD’S PHYSICAL CONDITION AND EXPECTED BEHAVIOR: I hereby declare my child to be physically sound, having medical approval to participate in the activities of the Carmel Clay Parks & Recreation’s Extended School Enrichment/Summer Camp Series. This health history is correct so far as I know, and my child has permission to engage in all prescribed program activities except as noted in the special needs section above. I understand that it is my responsibility to promptly notify my child’s site supervisor in writing of any changes in my child’s health. I certify that my child is amendable to discipline and free from habits or attitudes which would make him/her an undesirable participant. PHOTO & VIDEO PERMISSION: Photos and video footage are periodically taken of participants in the Extended School Enrichment/Summer Camp Series. Please be aware that these photos and video footage are the property of Carmel Clay Parks & Recreation and will be used in the department’s publications, website and video productions unless you elect not to give permission for the use of your child’s likeness. I consent to such uses and waive any rights of privacy or publicity I may have in connection with those uses. Failure to complete this section will be considered permission. RELEASE AND WAIVER: In consideration of my child’s participation in the activities of the Carmel Clay Parks & Recreation’s Extended School Enrichment/Summer Camp Series and acknowledging that risk of injury exists, I HEREBY RELEASE, WAIVE, DISCHARGE, COVENANT NOT TO SUE AND AGREE TO HOLD HARMLESS THE CARMEL CLAY BOARD OF PARKS AND RECREATION, THE CARMEL CLAY PARKS & RECREATION DEPARTMENT, CITY OF CARMEL, CLAY TOWNSHIP, ITS AND THEIR OFFICIALS, OFFICERS, MEMBERS, INDEPENDENT CONTRACTORS, EMPLOYEES AND VOLUNTEERS from any and all claims or liability for personal injury or property damage my child or I may suffer directly or indirectly arising out of or relating in any respect to participating in Carmel Clay Parks & Recreation’s Extended School Enrichment/Summer Camp Series. This waiver and release of all claims, demands, action, and liability shall include, without limitations, any injury, damage or loss to person or property which may be (a) caused by any act, or failure to act, by Releasees even if said injury, damage, or loss results from the negligence of any or all of the above-‐identified Releasees or (b) sustained by my child before, during or after Carmel Clay Parks & Recreation’s Extended School Enrichment Program. I agree to indemnify and hold harmless Releasees from all lawsuits, losses, damages, claims, and expenses, including attorney’s fees and costs arising from or relating in any respect to my child’s participation in Carmel Clay Parks & Recreation’s Extended School Enrichment Program or my breach of all terms and conditions contained in the Extended School Enrichment Program Registration Form. This provision will apply regardless of whether or not the lawsuit, losses, damages, claims, expenses, attorney’s fees and/or costs arises out of the negligence of any of the Releasees. The laws of the State of Indiana will govern any disputes or other matters relating to this Consent and Liability Release.
AGREEMENT To abide by Carmel Clay Parks & Recreation’s Extended School Enrichment /Summer Camp Series staff/child relationships. o Carmel Clay Parks & Recreation Extended School Enrichment/Summer Camp Series staff may not provide care to children enrolled in Carmel Clay Parks & Recreation’s Extended School Enrichment/Summer Camp Series outside of the approved Carmel Clay Parks & Recreation’s Extended School Enrichment/Summer Camp Series locations operated by Carmel Clay Parks & Recreation. This would include babysitting, outings and/or trips. I understand that all Carmel Clay Parks & Recreation’s Extended School Enrichment/Summer Camp Series staff have been informed of this policy. o Carmel Clay Parks & Recreation Extended School Enrichment/Summer Camp Series staff may not communicate electronically with Extended School Enrichment students/Summer Camp Series campers. This would include phone communication (including texting), as well as social media contact. I understand that all Carmel Clay Parks & Recreation’s Extended School Enrichment/Summer Camp Series staff have been informed of this policy. To read the Extended School Enrichment parent/guardian guide and the Summer Camp Series guide, so I am familiar with the policies and procedures of Carmel Clay Parks & Recreation’s Extended School Enrichment/Summer Camp Series. That should my child’s behavior result in suspension (per the Behavior Consequence Rubric) or present a real threat of harm to him/herself or others, Carmel Clay Parks & Recreation may deny care and participation in Extended School Enrichment/Summer Camp Series programs, and this agreement may be terminated with notification. That when my child is picked up or dropped off, by an authorized person, this person will make sure that a staff person is aware of my child’s arrival or departure, and will sign my child in and out. To not send a sick and/or contagious child to the program. In the event that a child is severely injured, ill and/or contagious to fellow children, parents/guardians will be contacted via phone. That should my child’s school be closed early due to inclement weather or emergency, Carmel Clay Parks & Recreation’s Extended School Enrichment will be cancelled and my child will be sent home by (school year only): Bus Walk Home Parent Pick-‐Up That should my child’s school be closed during the before school program due to inclement weather or emergency, the parent/guardian will be contacted. My child must be picked up by an authorized person by 10:00 a.m. I understand that I will be charged a fee of $1 per minute past 10:00 a.m. (school year only) To pay the non-‐refundable Extended School Enrichment registration fee for each child and all applicable fees (e.g. monthly, weekly, punch card or drop-‐in) by the date and time designated by Carmel Clay Parks & Recreation.
