Carmel Clay Parks & Recreation awarded the Darkness to Light Partner in Prevention distinction for 2014-2015 by Chaucie’s Place This is a prestigious certification showing CCPR’s commitment to keeping the children they serve safe and healthy. The guidelines for distinction include CCPR meeting the following criteria: • • • • •

At least 90% of employees have went through the Stewards of Children training Trained new employees in a timely manner Maintained record of training to ensure staff renews training every 3 years Completed background checks on all employees prior to employment Ensured that one adult-one child situations are in settings that are observable and interruptible

The Darkness to Light Partner in Prevention distinction shows parents/guardians, school personnel and the community that the organization is dedicated to fostering an environment that better protects children from sexual abuse. CCPR is committed to prevention through training and having implemented appropriate policies designed to deter potential perpetrators. Staff training increases awareness and teaches how to recognize opportunities for abuse to occur as well as help recognize when abuse has occurred. Staff is also trained on how to react if they recognize signs of abuse or if a child discloses abuse.

Summer Camp series Policies Full Summer Rate (must pay in full to receive 10% discount) Discount Available thru In-Person or Phone Registration (not available online) Parents/guardians receive a 10% discount by registering their respective camper for each of the nine weeks of summer camp (June 1 thru July 31, 2015). Parents/guardians may register their respective camper for individual weeks at different CCPR summer camps, and still receive the fantastic advantage of receiving the 10% discount for paying in full for the full summer rate (either inperson or phone registration). The parent/guardian must specify which weeks and camp name the camper will attend. Partial Payment Plan (not available for full summer rate) Parents/guardians may reserve a spot with a 50% non-refundable down payment for each summer day camp week. Payment in full is due by May 25 (Memorial Day). If payment in full has not been received by May 25, the reservation(s) will be cancelled. Weekly rates may not be transferred into a full summer rate. Transfer Policy A transfer from one day camp to another day camp is contingent upon the availability of spots within the desired day camp. If the day camp is at maximum capacity, a transfer will be not allowed. Waitlist Policy There is no fee to be placed on the waitlist. If a spot becomes available in the desired camp the camper’s parent/guardian that is prioritized at the top of waitlist will be contacted via telephone

and/or e-mail. The parent/guardian will have 24 hours from the time of the call/e-mail to make payment in full for the desired camp. If payment has not been received in full, the camper’s parent/ guardian next on the waitlist will be contacted via telephone and/ or e-mail. Each respective camp week has a maximum of 10 waitlist enrollments. Age Policy The cutoff date for a camper’s age is September 1, 2015 or a camper’s age on the first day of the camp week. Camp Cancellation & Refund Policy • The department will issue a full refund if the department cancels a camp. • Cancellations for a camp week must be requested a minimum of 1 week from the respective camp week start date to receive a 100% refund, less a $7 service charge for cancellation per camp week, per child. • All refunds are subject to the State Board of Accounts claim procedure and may take three to four weeks. Field Trips All field trips are weather permitted. Camper money and electronics are not allowed.

Does your child require an ADA modification? • We will conduct an assessment on your child to determine if additional support is needed to assist your child in the 2015 Summer Camp Series. • If it is determined based on your child’s assessment that additional support is required we will assign a qualified staff member(s) within 30 days from the assessment date to work directly with your child. • Participation in the 2015 Summer Camp Series may not begin until we assign your child a staff member. Play ON | East 5-9 YEARS

Welcome to our playtastic camp just for 5-9 year olds. We will focus on having the best summer we can have in Carmel. We will start every day with morning assembly to get to know everyone and learn new songs. Be prepared to get a few of our songs stuck in your head! We will be here to have a great time all throughout the day with sports, crafts and everything in between. Before snack, we will have a down time for kids to read and just relax from the day. We will then have snack and closing assembly to end our day so every camper knows what to look forward to the next day. Campers are responsible for bringing their own sack lunch and snack. 5-9 YEARS

Welcome to our funtastic camp just for 5-9 year olds! We will focus on having kids being kids and having as much fun as they possibly can. Campers and parents are greeted each morning with a welcoming smile. The camp will start every day with morning color groups, which are divided by age and personalities, so the campers get to know each other and their awesome camp leaders. Each day is filled with fun and exciting games and activities for the campers. Each day before the end-of-day activities, camp leaders nominate “Campers of the Day” for each color group. Campers receive their certificate in front of their peers and are one step closer to having their name placed on the VIP camper wall. At Play On-West, we strive to keep parents in the know with letters from the campers at least a few times each week, as well as our information wall located near the parent table. Campers are responsible for bringing their own sack lunch and snack. Campers must be able to take care of their respective restroom needs. Explore the world of art with us in a hands-on setting! We believe that creative expression is an essential part of a child’s development. Participants will have plenty of opportunities to get creative in this camp as they use fun and varied art materials to create their masterpieces, which will be available to take home. Use your imagination to paint, create and build to your heart’s desire using traditional and non-traditional materials. The creative mind does what the creative mind wants, so we must warn you that things may get a little messy! Campers are responsible for bringing their own sack lunch and snack. We’ll conduct experiments, watch demonstrations, and make take home projects that reinforce the week’s focus. Success on Stage | Acting 8-14 YEARS

Be a star this summer in your very own musical! Do you love to act, sing, and dance? Whether you’re experienced on the stage or this is your first time trying it out, the Acting side of Success on Stage has a spot just for you. Starting day one with auditions, you’ll be matched with a role that only you can bring to life, whether it’s a unique part of the chorus or one of the singing soloists of the show. The first half of the summer we will be putting on the musical Alice @ Wonderland, where the Mad Hatter, the Cheshire Cat and other favorites encounter a contemporary Alice hooked on Facebook and Twitter. The week of June 29-July 3, will be spent in acting and singing workshops geared specifically for our next play. The last half of summer you’ll be transported by flying carpet as you help create the magical musical version of Aladdin. Campers are responsible for bringing their own sack lunch and snack. Campers must be able to take care of their respective restroom needs.

Join us for a boy’s only evening! Make this the ultimate night for you and your buddies. Participants should bring pillows or blankets to lay on while watching the movie. Join us week by week as we put together the various parts of our stage production. Campers in the Stage Design crew create the backdrop, furniture, props, and costumes, as well as putting together the program and taking photos and video. Field trips will be centered on research and inspiration for set creation. Please note the week of performance most of the set work will be finished. The Stage Design crew will be assisting the actors’ performance week with makeup and hair, as well as setting up and working during both the dress rehearsal and the show.

We all love some friendly competition! What about boys vs. girls?! Come and see what exciting things we find out about ourselves as we take some time to have fun and explore who we are as boys and girls! We’ll be getting dirty on the field and primping like princesses. Let’s see how strong we can be when we come together! Join in the fun as we celebrate how awesome we all are and how everybody rocks and rules.

Recharge your batteries and become one with nature. Get your sunscreen, walking shoes and sun visors! Enjoy the outdoors as we explore nature. Each camp day, Outdoor Explorers will hike an average of one-to-two miles with breaks on various camping trails. So, if you don’t mind getting dirty and LOVE adventure this is the camp for you!

Do you have a passion for a particular sport but have never played it or want to work more on fundamentals? If so, then this is the camp for you! Football, Soccer, and Volleyball are just a few of the sports that we will be playing each week. This camp is not for the advanced player, but is for those who are just starting out in a particular sport or who want to develop skills that are essential to succeed in athletics. Move to Improve will challenge you every week with a different sport in a setting that is designed to help you reach your goals. The coaches are ready and the field is set. All we need now is you! Are you a 12-15 year old who wants to serve as a positive role model? The I.M.P.A.C.T. (Initiative, Motivation, Professionalism, Achievement, Compassion, Teamwork) program will teach essential life skills in a job-like setting. Members of I.M.P.A.C.T. will attend a weekly job training session that will focus on each of the six elements of the program. Interested individuals must be 12 by June 1, 2015 and will interview to become a member of I.M.P.A.C.T. Carmel Clay Parks & Recreation 2014-15 Extended School Enrichment & 2015 Summer Camp Series Registration Form PARENT AUTHORIZATION, WAIVER and RELEASE

CHILD’S PHYSICAL CONDITION AND EXPECTED BEHAVIOR: I hereby declare my child to be physically sound, having medical approval to participate in the activities of the Carmel Clay Parks & Recreation’s Extended School Enrichment/Summer Camp Series. This health history is correct so far as I know, and my child has permission to engage in all prescribed program activities. I understand that it is my responsibility to promptly notify my child’s site supervisor in writing of any changes in my child’s health. I certify that my child is amendable to discipline and free from habits or attitudes which would make him/her an undesirable participant. PHOTO & VIDEO PERMISSION: Photos and video footage are periodically taken of participants in the Extended School Enrichment/Summer Camp Series. Please be aware that these photos and video footage are the property of Carmel Clay Parks & Recreation and will be used in the department’s publications, website and video productions unless you elect not to give permission for the use of your child’s likeness. I consent to such uses and waive any rights of privacy or publicity I may have in connection with those uses. Failure to complete this section will be considered permission. PERMISSION = ______ YES ______ NO TRANSPORTATION AGREEMENT: Your child may be using bus transportation provided by Carmel Clay Parks & Recreation. This might be for an afternoon swim, field trips, or for transportation to and from the program site. By my signature below, I give permission for my child to travel by bus with the Carmel Clay Parks & Recreation’s Extended School Enrichment/Summer Camp Series staff. MEDICATION PERMISSION: I hereby give permission to Carmel Clay Parks & Recreation staff to administer the following medication to my child: Acetaminophen (i.e. tylenol) PERMISSION = ______ YES ______ NO, Diphenhydramine (i.e. benadryl) PERMISSION = ______ YES ______ NO EMERGENCY AUTHORIZATION: I hereby give permission to the medical personnel attending to my child to order X-rays, routine tests and treatment for my child, and, in the event I am not able to communicate or cannot be reached in an emergency, I hereby give permission to the attending physician to hospitalize, secure proper treatment for, and order injection(s) and/or anesthesia and/or surgery for my child as named above. I will be fully responsible for any costs of such treatment, even if not covered by insurance. RELEASE AND WAIVER: In consideration of my child’s participation in the activities of the Carmel Clay Parks & Recreation’s Extended School Enrichment/Summer Camp Series and acknowledging that risk of injury exists, I HEREBY RELEASE, WAIVE, DISCHARGE, COVENANT NOT TO SUE AND AGREE TO HOLD HARMLESS THE CARMEL CLAY BOARD OF PARKS AND RECREATION, THE CARMEL CLAY PARKS & RECREATION DEPARTMENT, CITY OF CARMEL, CLAY TOWNSHIP, ITS AND THEIR OFFICIALS, OFFICERS, MEMBERS, INDEPENDENT CONTRACTORS, EMPLOYEES AND VOLUNTEERS from any and all claims or liability for personal injury or property damage my child or I may suffer directly or indirectly arising out of or relating in any respect to participating in Carmel Clay Parks & Recreation’s Extended School Enrichment/Summer Camp Series. This waiver and release of all claims, demands, action, and liability shall include, without limitations, any injury, damage or loss to person or property which may be (a) caused by any act, or failure to act, by Releasees even if said injury, damage, or loss results from the negligence of any or all of the above-identified Releasees or (b) sustained by my child before, during or after Carmel Clay Parks & Recreation’s Extended School Enrichment Program. I agree to indemnify and hold harmless Releasees from all lawsuits, losses, damages, claims, and expenses, including attorney’s fees and costs arising from or relating in any respect to my child’s participation in Carmel Clay Parks & Recreation’s Extended School Enrichment Program or my breach of all terms and conditions contained in the Extended School Enrichment Program Registration Form. This provision will apply regardless of whether or not the lawsuit, losses, damages, claims, expenses, attorney’s fees and/or costs arises out of the negligence of any of the Releasees. The laws of the State of Indiana will govern any disputes or other matters relating to this Consent and Liability Release.

AGREEMENT

• To abide by Carmel Clay Parks & Recreation’s Extended School Enrichment /Summer Camp Series staff/child relationships. Carmel Clay Parks & Recreation Extended School Enrichment/Summer Camp Series employee may not provide care to children enrolled in Carmel Clay Parks & Recreation’s Extended School Enrichment/Summer Camp Series outside of the approved Carmel Clay Parks & Recreation’s Extended School Enrichment/ Summer Camp Series activities that are conducted in locations operated by Carmel Clay Parks & Recreation. This would include babysitting, outings or trips. I understand that all Carmel Clay Parks & Recreation’s Extended School Enrichment/Summer Camp Series staff have been informed of this policy. Carmel Clay Parks & Recreation Extended School Enrichment/Summer Camp Series staff may not communicate electronically with Extended School Enrichment students/Summer Camp Series campers. This would include phone communication (including texting), as well as social media contact. • To read the Extended School Enrichment parent/guardian guide and the Summer Camp Series guide, so I am familiar with the policies and procedures of Carmel Clay Parks & Recreation’s Extended School Enrichment/Summer Camp Series. • That should my child’s behavior result in suspension (per the Behavior Consequence Rubric) or present a real threat of harm to him/herself or others, Carmel Clay Parks & Recreation may deny care and participation in Extended School Enrichment/Summer Camp Series programs, and this agreement may be terminated with notification. • To walk my child into the program upon drop-off. Failure to do so may result in prohibiting my child from attending the program. Upon pick-up, to check my child out of the program by an authorized person. Failure to pick up my child by the end of program time will result in a $1 per minute, per child late pick-up fee. I understand that, in the event my child is not picked up one hour after the program ends, and all emergency contact attempts have been exhausted, the local police will be notified and my child may be immediately terminated from the program. • To not send a sick and/or contagious child to the program - child must be fever-free (under 100 degrees) for 24 hours without medication. In the event that a child is severely injured, ill and/or contagious to fellow children, parent/guardian will be contacted via phone and is to pick up child immediately. Any medication (prescription and non-prescription) given at the program requires written permission and provided in its original container. • That should my child’s school be closed early due to inclement weather or emergency, Carmel Clay Parks & Recreation’s Extended School Enrichment will be cancelled and my child will be sent home by: Bus Walk Home Parent Pick-Up • That should my child’s school be closed during the before school program due to inclement weather or emergency, the parent/guardian will be contacted.My child must be picked up by an authorized person by 10:00 a.m. I understand that I will be charged a fee of $1 per minute past 10:00 a.m. • To pay the non-refundable Extended School Enrichment registration fee per child and all applicable fees (e.g. monthly, weekly, punch card or drop-in) by the specified due dates per the Extended School Enrichment Parent/Guardian Guide. Failure to pay by the specified due date will result in a $10 late payment fee, per child. My child will be prohibited from attending the Extended School Enrichment/Summer Camp Series until all past due payments have been made in entirety. I certify that I am the parent/legal guardian of this child and that I have authority to make the representations and grant the authorizations contained herein.
