Summer Camp series Policies Full Summer Rate (must pay $1,400 in full) Parents/guardians receive a discount of $105 (excluding Skyhawks and I.M.P.A.C.T.) by registering their respective camper for the full summer rate. Payment must be made in full to receive the discount. Weekly rates may not be transferred into a full summer rate.

Full Summer Rate - A La Carte (must pay $1,400 in full) Parents/guardians may register their respective camper for individual weeks (excluding Skyhawks and I.M.P.A.C.T.) and still receive the fantastic advantage of paying the full summer rate. The parent/guardian must sign their camper up for the full summer rate to receive the discounted rate and must specify which weeks and camp name the camper will attend. Weekly rates may not be transferred into a full summer rate.

Partial Payment Plan (not available for full summer rate) Parents/guardians may reserve a spot with a 50% non-refundable down payment for each summer day camp week. Payment in full is due by June 3 (the first day of camp). If payment in full has not been received by June 3, the reservation(s) will be cancelled. Weekly rates may not be transferred into a full summer rate.

Transfer Policy A transfer from one day camp to another day camp is contingent upon the availability of spots within the desired day camp. If the day camp is at maximum capacity, a transfer will be not allowed. In cases in which a camper is not able to transfer into camp as a result of maximum capacity, the camper will be placed on the waiting list for the desired camp. Campers on the waiting list will be prioritized based upon the enrollment date of the transfer.

Waitlist Policy There is no fee to be placed on the waitlist. If a spot becomes available in the desired camp the camper’s parent/guardian that is prioritized at the top of waitlist will be contacted via telephone and/or e-mail. The parent/guardian will have 24 hours from the time of the call/e-mail to make payment in full for the desired camp. If payment has not been received in full, the camper’s parent/guardian next on the waitlist will be contacted via telephone and/or e-mail.

Age Policy The cutoff date for a camper’s age is June 3, 2013 or a camper’s age on the first day of the camp week.

Camp Cancellation & Refund Policy • The department will issue a full refund if the department cancels a camp. • Payments may not be transferred or refunded after the final day of the camp week (e.g. payments made for the camp week of June 10-14, 2013, will not be transferred or refunded after June 10, 2013). • Refunds for the full summer rate or weekly rate may not be prorated. • Cancellations made through May 1, 2013, or within 24 hours of registration will receive a 100% refund, less a $7 service charge for cancellation. • Cancellations made on or after May 2, 2013, will receive a 50% refund (no service charge). • All refunds are subject to the State Board of Accounts claim procedure and may take three to four weeks.

Welcome to our play-tastic camp just for 5-9 year olds! We will focus on having the best summer ever in a safe and caring environment. At Play On, campers are encouraged to explore their imaginations through a variety of crafts, games, physical, and social activities. Each Monday begins with a group project that will focus on teamwork and getting to know one another. On Tuesdays and Thursdays, it’s off to The Waterpark at the Monon Community Center for swimming fun, and a Wednesday field trip that will take us to an offsite adventure. On Friday, a vendor will come to our camp to entertain us. All campers must be able to take care of their respective bathroom needs. The ratio of staff-to-camper is a minimum of 1-to-12. Campers are responsible for bringing their own sack lunch and snack.

We love to play games. We can play thinking games like checkers, physical games like tag, or just for fun games like hop scotch. This week, we will learn new games that you can take home to your family or friends. We are going to Ben & Ari’s to play some video games as well as miniature golf on Wednesday. Imagination is very important to make play time the best time of the day. Just imagine yourself as a fire fighter or a dancer or a chef. In the middle of the week, we are going to pretend we are back in the settlement days of Conner Prairie. We will visit a time long ago in a village that is stuck in time. Let’s monkey around! Everyday this week is going to be a zoo here at Play On. Everyone will be busy as a beaver or maybe even go hog wild. Whatever we are doing, it is sure to be a great time. On Wednesday, you should be bright-eyed and bushy-tailed for our trip to Trader’s Point Creamery. We will tour the farm and end the trip with dairy and ice cream tasting. Everyone has the potential to be a star. Whether you’re a sports star, a movie star or a crafts star, everyone can be awesome. We will have a great time just being the best we can be here at Play On. It’s getting pretty hot at this point in the summer. What better way to cool off then by getting wet? We will have buckets of fun with water balloons and other water activities. Insects are all around us, and we intend to have fun with the creepy crawlies. Let’s crawl, buzz, and inch our way towards fun of all kinds. We will truly feel like we are in flight when we visit Hunter’s Honey Farm. Let’s explore how honey is produced, make our own bees wax candle, and pour honey into a jar to take home. Explore the exciting world of archeology as you never have before! Get your hands dirty as we conduct our own archeological dig! What will you find beneath the soil? Do potatoes really create electricity? Join us this week as we put the scientific method to the test and determine if our predictions will be true. The Junior Scientists will spend the week working on their own project, which will then be displayed in a science fair for the parents/guardians to enjoy. No field trip.

Under the sea, there are so many wonderful things to see! We will become marine biologists by learning about ocean animals and plant life! Experience a fun-filled summer full of adventure! This exciting new camp will be broken down into six smaller groups with age appropriate activities for each group. Tuesdays and Thursdays, we will make our way to The Waterpark at the Monon Community Center for some fun in the sun as we go swimming. Wednesdays, we will bring the fun to us with various on site vendors each week. There couldn’t be a more perfect way to end the week as we will take a field trip each Friday. Join the fun and learn the fundamentals of one of America’s favorite sports, Flag Football! Each day you will master a new skill and develop more confidence on the gridiron. The week will begin with training camp. Each camper will be weighed, measured, and given a speed test. Next, there will be a punt, pass, and catch evaluation. The remainder of the week will be learning the rules, positions, and routes. Friday will culminate in the “Game of the Week” where the skills our young athletes have acquired will be displayed in the Summer Camp Series (SCS) Bowl Classic tournament. This camp will focus on taking the game of basketball and breaking it down to the basic skills that all athletes need to know to be successful on the court. If you have a passion for running, jumping, and throwing, but need to harness the skills necessary to succeed, then this is the camp for you! The focus of the camp will be skill development in track events such as distance running, sprints, soft hurdles, and relays. Field events include: standing long jump, triple jump, high jump, shot put and discus. First up, we will learn the fundamentals of body positioning, breathing, stride, proper stretching, and cool down techniques. We’ll finish our week Friday with a track and field meet. America’s favorite past-time is waiting for you! Baseball! Let’s start at the beginning with fielding, catching, and batting. For our younger athletes, batting tees will be available. Get to know these terms and more as you take to the court for a fastpaced game of volleyball! The focus of the week will be on developing skills in setting, bumping, spiking, and serving. Each camp day, Outdoor Explorers will hike an average of one to two miles with breaks on various camping trails as we explore nature. This week, students will fish for ideas on clothing and props for joggers, Hollywood stars and pirates to name a few. Continue working on the stage design of a gunboat turned cruise ship. The comic elements are in full swing this week. No field trip.

Students will be working within various groups to continue work on the production side of The Wizard of Oz. Help sew Dorothy’s iconic dress, create Tin Man’s oil can, or design the hot air balloon that the wizard escapes into. Watch out for flying monkeys in this last full week before our performance! Performance week! CHILLVILLE 9-13 years Spend the summer with peers who enjoy doing some of the same things as you. Each week presents an opportunity to make choices and spend time doing whatever sounds fun. Want to go on a hike, play your video games, or maybe just relax and talk with friends? This summer camp will have plenty of time for that! Amidst all the fun, we will have a chance to cool down at The Waterpark at the Monon Community Center every Monday, Wednesday, and Friday and travel off-site for a field trip every Tuesday and Thursday. Some field trips are within walking distance, so we will have a chance to get plenty of exercise. Feel free to bring extra spending money in case you get thirsty or want to buy a souvenir! Are you a young adult seeking responsibility? Are you a 12-15 year old who wants to serve as a positive role model? The I.M.P.A.C.T. (Initiative, Motivation, Professionalism, Achievement, Compassion, Teamwork) program will teach essential life skills in a job-like setting. Members of I.M.P.A.C.T. will attend a weekly job training session that will focus on each of the six elements of the program. Interested individuals must be 12 by June 3, 2013 and will interview to become a member of I.M.P.A.C.T. If selected, each member will be assigned to one of the following summer day camps: Play On, The Summer Experience, Science of Summer, or Adventures in Art.

Skyhawks Sports 4-10 years Skyhawks provides a wide variety of fun, safe, and positive programs that emphasize critical lessons in sports and life, such as teamwork, respect, and sportsmanship. Skyhawks has adopted the SNAG (Starting New At Golf) system as its curriculum. SNAG is specifically designed for the entry-level player; SNAG simplifies instruction so that young players can make an easy and effective transition onto the golf course. All equipment provided.

Baseball, basketball and soccer are taught in a safe, structured environment with lots of encouragement and a big focus on fun. Our MiniHawk games and activities were designed to allow campers to explore balance, movement, hand/eye coordination, and skill development at their own pace. Skyhawks staff are trained to handle the specific needs of young athletes.

Quickstart 4-8 Years June 10-14 • 476010-02 • $105 • Min/Max 8/16 • 10:00am-12:00pm Skyhawks Tennis with Quickstart is a new format to help kids ten and under learn to play tennis. To create a successful learning environment, we have made modifications to our traditional tennis program. The court is smaller and has shorter nets, the racquets are smaller, and the balls are foam. In addition to developing a love for tennis, athletes will learn the rules and etiquette that make tennis such an exciting game to play. All coaches are trained by USTA professionals. Due to limited court space, these programs fill up quickly. All participants receive a t-shirt and merit award. The participant-tocoach ratio is approximately 8:1. Participants should bring a tennis racquet, appropriate clothing, two snacks, a water bottle, running shoes and sunscreen.

Multi Sport 6-12 Years June 10-14 • 476010-03 and July 15-19 • 476010-08 $155 • Min/Max 12/24 • 9:00am-4:00pm Our multi-sport camp is designed to introduce young athletes to a variety of different sports in one setting. For this program we combine soccer, flag football, and baseball into one fun-filled week. Athletes will learn the rules and essentials of each sport through skill-based games and scrimmages. By the end of the week, your child will walk away with knowledge of multiple sports along with vital life lessons such as respect, teamwork, and self-discipline. All participants receive a t-shirt, sport ball, and a merit award. The participant-to-coach ratio is approximately 12:1. Participants should bring appropriate clothing, a lunch and snack (for programs over four hours) or two snacks (for programs four hours or less), a water bottle, running shoes, a baseball glove, and sunscreen.

Players learn proper grips, footwork, strokes, volleys, and serves during a series of drills designed to teach skills in a fun and dynamic way. Please list, in order of preference, all persons, (including yourself, and, if applicable, the child’s other parent/guardian) who are authorized to pick up your child. Your child will not be released to anyone not on this list. Any changes to this list may be made in person. Anyone on the list must be at least 16‐years‐old. Carmel Clay Parks & Recreation 2012-13 Extended School Enrichment & 2013 Summer Camp Series Registration Form PARENT AUTHORIZATION, WAIVER and RELEASE CHILD’S PHYSICAL CONDITION AND EXPECTED BEHAVIOR: I hereby declare my child to be physically sound, having medical approval to participate in the activities of the Carmel Clay Parks & Recreation’s Extended School Enrichment/Summer Camp Series. This health history is correct so far as I know, and my child has permission to engage in all prescribed program activities except as noted in the special needs section above. I understand that it is my responsibility to promptly notify my child’s site supervisor in writing of any changes in my child’s health. I certify that my child is amendable to discipline and free from habits or attitudes which would make him/her an undesirable participant. PHOTO & VIDEO PERMISSION: Photos and video footage are periodically taken of participants in the Extended School Enrichment/Summer Camp Series. Please be aware that these photos and video footage are the property of Carmel Clay Parks & Recreation and will be used in the department’s publications, website and video productions unless you elect not to give permission for the use of your child’s likeness. RELEASE AND WAIVER: In consideration of my child’s participation in the activities of the Carmel Clay Parks & Recreation’s Extended School Enrichment/Summer Camp Series and acknowledging that risk of injury exists, I HEREBY RELEASE, WAIVE, DISCHARGE, COVENANT NOT TO SUE AND AGREE TO HOLD HARMLESS THE CARMEL CLAY BOARD OF PARKS AND RECREATION, THE CARMEL CLAY PARKS & RECREATION DEPARTMENT, CITY OF CARMEL, CLAY TOWNSHIP, ITS AND THEIR OFFICIALS, OFFICERS, MEMBERS, INDEPENDENT CONTRACTORS, EMPLOYEES AND VOLUNTEERS from any and all claims or liability for personal injury or property damage my child or I may suffer directly or indirectly arising out of or relating in any respect to participating in Carmel Clay Parks & Recreation’s Extended School Enrichment/Summer Camp Series. This waiver and release of all claims, demands, action, and liability shall include, without limitations, any injury, damage or loss to person or property which may be (a) caused by any act, or failure to act, by Releasees even if said injury, damage, or loss results from the negligence of any or all of the above‐identified Releasees or (b) sustained by my child before, during or after Carmel Clay Parks & Recreation’s Extended School Enrichment Program. I agree to indemnify and hold harmless Releasees from all lawsuits, losses, damages, claims, and expenses, including attorney’s fees and costs arising from or relating in any respect to my child’s participation in Carmel Clay Parks & Recreation’s Extended School Enrichment Program or my breach of all terms and conditions contained in the Extended School Enrichment Program Registration Form. This provision will apply regardless of whether or not the lawsuit, losses, damages, claims, expenses, attorney’s fees and/or costs arises out of the negligence of any of the Releasees. The laws of the State of Indiana will govern any disputes or other matters relating to this Consent and Liability Release.

AGREEMENT • To abide by Carmel Clay Parks & Recreation’s Extended School Enrichment /Summer Camp Series staff/child relationships, which states that a Carmel Clay Parks & • Recreation Extended School Enrichment/Summer Camp Series employee may not provide care to children enrolled in Carmel Clay Parks & Recreation’s Extended School Enrichment/Summer Camp Series outside of the approved Carmel Clay Parks & Recreation’s Extended School Enrichment/Summer Camp Series activities that are conducted in locations operated by Carmel Clay Parks & Recreation. This would include babysitting, outings or trips. I understand that all Carmel Clay Parks & Recreation’s Extended School Enrichment/Summer Camp Series staff have been informed of this policy. • To read the Extended School Enrichment parent/guardian guide and the Summer Camp Series guide, so I am familiar with the policies and procedures of Carmel Clay Parks & Recreation’s Extended School Enrichment/Summer Camp Series. • That should my child’s behavior result in suspension (per the Behavior Consequence Rubric) or present a real threat of harm to him/herself or others, Carmel Clay Parks & Recreation may deny care and participation in Extended School Enrichment/Summer Camp Series programs, and this agreement may be terminated with notification. • That when my child is picked up or dropped off, by an authorized person, this person will make sure that a staff person is aware of my child’s arrival or departure, and will sign my child in and out. My child will not be admitted to the program unless this fee is paid. • To pay the non‐refundable Extended School Enrichment registration fee for each child and all applicable fees (e.g. monthly, weekly, punch card or drop‐in) by the • date and time designated by Carmel Clay Parks & Recreation.
