Chief's Message I want to extend my sincerest gratitude to each of you for your professionalism and dedication to service during these challenging times. Chandler employees, businesses, and residents are proud of the work that you do and appreciate your response to the COVID-19 virus. As things start to open up, and life begins to normalize, it is important to remember that the COVID-19 virus still exists within our community. I ask that each of you continue to protect yourself and your crew by following all internal guidelines and remain extremely aware of your surroundings when you are on scene or in the public. Again, thank you for everything you do to keep our community safe! -TD

EMS DIVISION

Story of the Quarter! We take lightly the words “you all are life savers!” but most certainly, it is a true statement! The following is one example of many that prove the point. Fantastic work 2810-B!!

On April 4th, E/M2810 were called to a 52 year old female that was unconscious. The following is a portion of the narrative from the chart explaining what happened: On scene patient care of her family. Her husband was kneeling next to her. The patient was breathing agonally. A carotid pulse was felt for, and no palpable pulse was present. With patient still breathing agonal, pads were placed and her rhythm was initially V fib. Compressions were started while the monitor was charged. After the monitor was charged a shock at 200 J was delivered, an iGel was placed and supplemental O2 was delivered via iGel. Compressions were started again immediately following the shock, an IO was placed in the left tibia. The first Epi was given and a bag was connected to flush in meds. After 200 compressions a rhythm analysis was v fib. A second shock was delivered. Second epi was delivered. After 200 compressions a rhythm analysis showed asystole. A third cycle of 200 compressions was started after approximately 100 compressions the patient’s breathing became more regular. ROSC was apparent. Palpable pulses were present and BP cuff was started. Pt rhythm was sinus tach. Patient was placed on a backboard and lifted onto the gurney. Patient was loaded into the back of medic 2810 and transferred to Chandler regional Hospital code 3. The patient had a blood pressure and a palpable pulse. Patient’s GCS was still 3. A 12 lead was placed on a patient. 1+mm of Depression was noted in V3,V4, II, III, and AVF. While rolling into the emergency room patient started to “Buck” the tube. It was removed by ER staff. Upon arrival to ER patient care was transferred to emergency staff.

CFD Pharmacy KETAMINE The roll-out of Ketamine training was a success and this new pharmaceutical tool should now be in all CFD drug boxes. Ketamine will be very helpful as part of the sedation and RSI offlines.

VERSED With CRH/MGMC experiencing a shortage on 10/2 Versed, we have been able to secure the medications via Bound Tree. Until we see Dignity get their shortage corrected, you will need to restock the Versed from the UCAPIT machine. PLEASE remember to put the incident number that pertains to the restock and do NOT use 888888 or 555555 or any other random numbers. Thanks for your help with this.

Routinely clean/disinfect common areas and frequently used surfaces – designate speciďŹ c personnel to this task and create a schedule Wash hands and take other necessary disinfecting actions after returning to workplace Keep appropriate PPE near public entry areas for use during interactions with the public (as needed)

Maintain distinct separation between response areas and living areas

Leave boots, brush pants or other potentially infectious clothing in bay before entering station

If a washer is used for potentially infected uniforms, disinfect the washer by running a subsequent load of bleach and hot water only

Wear civilian clothes to and from station to prevent transmission of virus to home

Shower and change into clean clothes before going home

Clean/disinfect all areas contacted by any public entry into station

If you begin to feel sick while on shift, put on surgical mask and report illness to supervisor. Periodically clean/disinfect personal work areas

Disinfect public entry areas after interactions with public and periodically

Do not bring EMS equipment into station for check off or training unless thorough disinfection precautions have been taken Adhere to periodic common area disinfection schedule

Routinely disinfect commonly contacted surfaces on the interior of apparatus throughout shift and after any possible exposure

TRAINING DIVISION

1st Quarter Fire Training

The first quarter of fire training was very productive for crews. We revisited some skill sets that were already in our toolbox but added some new data and techniques. As always, our crews did an amazing job, with all participants readily absorbing the latest information and using it. Our members made the most of the opportunity, as we had crews ask to repeat their stations so all participants could try different roles, striving to become masters in their craft.

Upcoming Fire Training Unfortunate recent events have changed how we are conducting fire training for the second quarter. Training has developed methods that will allow crews to learn and practice at their stations or within their districts. Third quarter fire training will focus on Fire Ground Survival and forcible entry. We will expose crews to traditional methods and some new aspects of fire ground survival, engaging crews with self-survival techniques and SCBA management in rescue situations.
