Be on the lookout! Case Review Premium Improving Compressions during CPR is really an individual matter since we each have our own strengths and weaknesses when providing compressions. We are seeking for each of you to know how you are doing when it comes to chest compressions.

CFD EMS is working towards getting mCPR (Mechanical CPR) devices in our system.

Zoll AutoPulse

Though there are several to choose from, two are front runners and will be field-tested for feedback during the next few months.

Even the best manual chest compression provides only 20% to 30% of normal cardiac output so learning how to be the best we can is imperative. For this purpose, we are excited to announce “Case Review Premium,” which is a detailed report card for our codes. You have seen these report cards that EMS has sent out before, however, Case Review Premium has lots of bells and whistles to see and track individual CPR performance. We will be giving access to all Company Officers in hopes that this can become a crew focus after each code you run.

E282 and E283 will each have a device for a period of time, then switch for another period of time so that we can get their feedback at the end as to which they preferred. Training for the devices will be happening for all three shifts January 13, 16, and 17. We look forward to getting feedback and selecting a device that is best for CFD. Note: Your feedback on EMS equipment we buy is a top priority for us prior to purchase. EMS does its best to know what you want before things are purchased.

The EMS division will be getting training on how to use this web-based program soon and then passing it on to the Captains for all of you to work on having the best compressions possible.

OUT with the old & in with the NEW

The former Ambu manual suction units came to their life's end and are now being replaced with a quick and improved set up known as SEADUC by SSCOR. These devices will be rolling out soon and you will get training on them during Q1 EMS training. 2

Give us our green NeoPro gloves back!

4th Quarter Overview

We hope everyone enjoyed Q4 PALS training with the implementation of Handtevy. We appreciate the positive comments on how the training went but also would love to know any suggestions for improvements. EMS assembles courses for you, so your feedback is always welcome. Keep us posted on any ideas or training topics you would like to see.

Yes, we understand there are those that miss the highly expensive NeoPro gloves but, unfortunately, they cost way too much.

However, we have 3 gloves we are testing with a few crews to soon have more options for you.

EtCO2 - PLEASE USE IT! A normal lactic acid (aka lactate level) is 0.5-1 mmol/L. When you run on a patient that has an EtCO2 of <25 then they have a lactate level of 6 mmol/L or more. I am glad we have many who like the current Apex gloves in use now but also look forward to getting another thinner option for those that want something different. Your A-shift EMS restock folks will soon be seeing the option for thinner gloves on the Bound Tree website in the next month or so.

On the horizon

Hyperlactatemia is defined as 2-4 mmol/L and

Did you Know ?

anything greater than 4 is considered lactic acidosis. Any lactate level >4 tips the acid-base balance, which may result in a serum pH <7.35 meaning metabolic acidosis. These are patients that are considered to be in severe sepsis and in septic shock. Please use your EtCO2 as it is a great tool to assist you.

OPS DIVISION

Time Truisms for Predicting Collapse:

The lighter the structural elements, the faster the structure comes down. The heavier the imposed load, the faster the structure comes down. Wet (cooled) steel buys time. Gravity and time are constant; resistance is not. The time opportunity window for interior operations in lightweight buildings has been reduced. There is no safe time window for interior operations when a building is under construction, being renovated, or being disassembled. This is a definite "no go" situation, and you should always establish a defensive attack. Brown or dark smoke coming from lightweight engineered wood products means that time is up. 5

Husqvarna Electric Cut-N-Break

TRT (L289) has a new saw that dramatically improves their ability to cut through thick slabs or walls of concrete. Last week, the crews were able to get their new Husqvarna Electric Cut-N-Break concrete saw in service. The saw had to be ordered from Europe to get the 220 volt version for improved torque. Once it arrived, Captain Barr and company worked to get the proper wiring on the saw to make it compatible with their generators. This saw is not only fast and powerful, with it's 2-blade design it has the capability to cut concrete up to 16 inches thick! We appreciate FF Tommy Curran and his Father for helping us obtain this saw and for providing us the concrete slabs for training!

TRAINING DIVISION

4th Quarter Overview

As always, Chandler Fire Department crews excelled when presented new training . Crews learned about NFPA requirements required of pressures and GPMs for successful outcomes in high-rise incidents, and tactics specific to standpipe systems. They put that new knowledge into application by stretching lines on higher floors of structures as part of a coordinated attack. Even with a lot of new information and moving parts, Chandler crews pushed forward and were able to demonstrate proficiency. Thank you to all the crews for their hard work, and to the battalion chiefs and Senior Staff that participated.

H I G H L I G H T S In November, the Chandler Fire Department hosted a training conference, with subjects taught by renowned national experts. 260 firefighters came from far away as Maine, Hawaii, and Alaska to learn hose management, rescue, ground ladders, forcible entry, leadership, wellness, as well as the science of fire behavior from Underwriter's Laboratories. Many of our members were excited to attend classes and got the opportunity to reinforce their professional skill sets.
