BE ACTIVE. BE HEALTHY. BE HAPPY! A MESSAGE FROM OUR PARKS & OPEN SPACE PROJECT COORDINATOR Winter is here, which means my favorite season is right around the corner. Spring has always been my favorite time of year because of all the endless outdoor activities. It’s the perfect time to start fresh, get outdoors, and enjoy all of the programs, activities, and events that are coming up in our community. Over the last two years, I have seen truly remarkable moments take place at our events in American Canyon. I am honored to work for this City and to serve the members of this community. From Coastal Clean-Up Day to Ridge Trail Day, this Fall was packed with lots of fun outdoor volunteer events. If you missed them, don't worry! There are plenty more opportunities to get involved on the horizon. The City’s new River to Ridge Trail Conservation Program includes two wonderful projects. The first is the Newell Planting and Weeding Project. The second is the Pollinator Planting Project co-hosted with the American Canyon Community & Parks Foundation that will be held during our AmCan By The Bay event. AMCAN BY THE BAY: POLLINATOR PLANTING PROJECT All ages welcome (14 years and under with parent) Are you passionate about the outdoors and want to learn more about how you can help our pollinators? This is the project for you! This year, we are partnering up with the American Canyon Community & Parks Foundation to rally a great team of volunteers to lend a hand removing invasive plants and replacing them with pollinator-friendly vegetation at Wetlands Edge. Experience the tales of dedication and courage by taking a tour of the Rosie the Riveter Visitor Education Center and Memorial. Enjoy interactive exhibits and watch films about the war at home and home front heroes. Then, take time to reflect on the quotes from the “Rosies” and a World War II timeline that is inscribed into the memorial granite walkway in Marina Bay Park. The trip would not be complete without a drive by viewing of the Red Oak Victory Ship. Afterwards, you will enjoy your provided box lunch at a local park. This trip requires extensive walking. AMERICAN CANYON COMMUNITY & PARKS FOUNDATION SCHOLARSHIP FUND The City of American Canyon is proud to offer the American Canyon Community & Parks Foundation Scholarship Fund. The Scholarship Fund provides financial assistance to qualifying families living in American Canyon who have a financial need that prevents them from participating in our recreation programs.

CITY OF AMERICAN CANYON PARKS & RECREATION REGISTRATION FORM QUALITY ASSURANCE & REFUNDS Refunds, minus a $10 processing fee for each refunded class or program, will be given if the refund request is made at least seven (7) days prior to the start of the class or program. Refunds may be requested by fax, walk-in, e-mail, or by mail and must be made in writing. Nonattendance in a program is not grounds for a refund. If a class or program is canceled a full refund will be granted. For City-sponsored youth sports leagues, the start of the program shall be defined as the date teams are formed. Some programs or activities may be designated and advertised as non- refundable. Refunds will not be given for those programs and activities. We reserve the right to cancel or postpone any program or activity due to inclement weather, low enrollment, or City need. A $25 service fee will be imposed for all returned checks and could result in refusal of future participation.

WAIVER AND RELEASE FROM LIABILITY / ASSUMPTION OF RISK I, the undersigned, in consideration of being permitted by the City of American Canyon to participate in the indicated activity(ies), hereby waive, release, and discharge any and all claims for damages due to personal injury, death, or property damage which result from participation. I, the undersigned, hereby agree to indemnify and hold harmless any loss, liability, damage, cost, or expense of any of the City’s officers, employees, and agents arising out of or connected in any way with the above indicated persons’ participation in the indicated activity(ies), even though that liability may arise out of negligence or carelessness. It is understood that this waiver, release, and assumption of risk is to be binding on my heirs and assigns. I, the undersigned, hereby state that the persons listed under “Participant Information” may and are physically able to participate in the activity(ies) indicated. I also understand that any changes in the indicated participant’s health or conditions must be reported immediately on this page. I, the undersigned, hereby authorize the City of American Canyon Parks and Recreation Department, into whose care the indicated participants’ have been entrusted, to consent to any emergency x-ray examination, anesthetic, medical, or surgical diagnosis or treatment and hospital care to be rendered upon the advice of a physician, surgeon, or dentist licensed under the provisions of the Medical and/or Dental Practice Acts. The City of American Canyon does not provide medical insurance and any treatment given will be at the cost of the undersigned. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT BETWEEN THE CITY OF AMERICAN CANYON AND MYSELF. I HAVE CAREFULLY READ THE ABOVE AGREEMENT, WAIVER, AND RELEASE AND FULLY UNDERSTAND AND AGREE TO ITS CONTENTS.
PHOTOGRAPHY AND VIDEO RELEASE In addition to the forgoing, I give consent to the City of American Canyon to photograph or record video of me (or the minor on whose behalf I am signing this waiver), and to use such photographs and video footage in brochures, newspapers, digital media, website, and other forms of media describing the City of American Canyon activities. I agree to advise the City of American Canyon Parks and Recreation Department in writing if I do not agree to the forgoing.
